Effectiveness, safety, and drug sustainability of biologics in elderly patients with inflammatory bowel disease: A retrospective study

BACKGROUND Biologic therapy resulted in a significant positive impact on the management of inflammatory bowel disease(IBD) however data on the efficacy and side effects of these therapies in the elderly is *** To evaluate retrospectively the drug sustainability, effectiveness, and safety of the biologic therapies in the elderly IBD *** Consecutive elderly(≥ 60 years old) IBD patients, treated with biologics [infliximab(IFX), adalimumab(ADal), vedolizumab(VDZ), ustekinumab(UST)] followed at the McGill University Inflammatory Bowel Diseases Center were included between January 2000 and *** was measured by clinical scores at 3, 6-9 and 12-18 mo after initiation of the biologic therapy. Patients completing induction therapy were included. Adverse events(AEs) or serious AE were collected during and within three months of stopping of the biologic *** We identified a total of 147 elderly patients with IBD treated with biologicals during the study period, including 109 with Crohn’s disease and 38 with ulcerative colitis. Patients received the following biologicals: IFX(28.5%), ADal(38.7%), VDZ(15.6%), UST(17%). The mean duration of biologic treatment was 157.5(SD = 148) wk. Parallel steroid therapy was given in 34% at baseline,19% at 3 mo, 16.3% at 6-9 mo and 6.5% at 12-18 mo. The remission rates at 3, 6-9 and 12-18 mo were not significantly different among biological therapies. Kaplan-Meyer analysis did not show statistical difference for drug sustainability(P = 0.195), time to adverse event(P = 0.158) or infection rates(P = 0.973) between the four biologics studied. The most common AEs that led to drug discontinuation were loss of response, infusion/injection reaction and *** Current biologics were not different regarding drug sustainability, effectiveness, and safety in the elderly IBD population. Therefore, we are not able to suggest a preferred sequencing order among biologicals.

Modification of polypropylene separator with multifunctional layers to achieve highly stable sodium metal anode

Separator modification is an effective approach to suppress dendrite growth to realize high-energy sodium metal batteries（SMBs） in practical applications,however,its success is mainly subject to surface ***,a separator with multifunctional layers composed of N-doped mesoporous hollow carbon spheres（HCS） as the inner layer and sodium fluoride（NaF） as the outer layer on commercial polypropylene separator（PP） is proposed（PP@HCS-NaF） to achieve stable cycling in *** the molecular level,the inner HCS layer with a high content of pyrrolic-N induces the uniform Na+flux as a potential Na+redistributor for homogenous deposition,whereas its hollow mesoporous structure offers nanoporous buffers and ion channels to regulate Na+ion distribution and uniform *** outer layer（NaF） constructs the NaF-enriched robust solid electrolyte interphase layer,significantly lowering the Na+ions diffusion *** from these merits,higher electrochemical performances are achieved with multifunctional double-layered PP@HCS-NaF separators compared with single-layered separators（***@HCS or PP@NaF） in *** Na‖Cu half-cell with PP@HCS-NaF offers stable cycling（280 cycles）with a high CE（99.6%）,and Na‖Na symmetric cells demonstrate extended lifespans for over 6000 h at 1 mA cm-2with a progressively stable overpotential of 9 ***,in Na‖NVP full-cells,the PP@HCS-NaF separator grants a stable capacity of ～81 mA h g-1after 3500 cycles at 1 C and an impressive rate capability performance(～70 mA h g-1at 15 C).

Security and safety remain paramount concerns for both governments and individuals *** today’s context,the frequency of crimes and terrorist attacks is alarmingly increasing,becoming increasingly intolerable to ***,there is a pressing need for swift identification of potential threats to preemptively alert law enforcement and security forces,thereby preventing potential attacks or violent *** advancements in big data analytics and deep learning have significantly enhanced the capabilities of computer vision in object detection,particularly in identifying *** paper introduces a novel automatic firearm detection surveillance system,utilizing a one-stage detection approach named MARIE(Mechanism for Realtime Identification of Firearms).MARIE incorporates the Single Shot Multibox Detector(SSD)model,which has been specifically optimized to balance the speed-accuracy trade-off critical in firearm detection *** SSD model was further refined by integrating MobileNetV2 and InceptionV2 architectures for superior feature extraction *** experimental results demonstrate that this modified SSD configuration provides highly satisfactory performance,surpassing existing methods trained on the same dataset in terms of the critical speedaccuracy *** these innovations,MARIE sets a new standard in surveillance technology,offering a robust solution to enhance public safety effectively.

We use the Schrödinger–Newton equation to calculate the regularized self-energy of a particle using a regular self-gravitational and electrostatic potential derived in string *** particle mass M is no longer concentrated into a point but is diluted and described by a quantum-corrected smeared energy density resulting in corrections to the energy of the particle,which is interpreted as a regularized *** extend our results and find corrections to the relativistic particles using the Klein–Gordon,Proca and Dirac *** important finding is that we extract a form of the generalized uncertainty principle(GUP)from the corrected *** form of the GUP is shown to depend on the nature of particles;namely,for bosons(spin 0 and spin 1)we obtain a quadratic form of the GUP,while for fermions(spin 1/2)we obtain a linear *** correlation we find between spin and GUP may offer insights for investigating quantum gravity.

Effective spin dynamics of spin-orbit coupled matter-wave solitons in optical lattices

We consider matter-wave solitons in spin-orbit coupled Bose-Einstein condensates embedded in an optical lattice and study the dynamics of the soliton within the framework of Gross-Pitaevskii *** express spin components of the soliton pair in terms of nonlinear Bloch equations and investigate the effective spin *** is seen that the effective magnetic field that appears in the Bloch equation is affected by optical lattices,and thus the optical lattice influences the precessional frequency of the spin *** make use of numerical approaches to investigate the dynamical behavior of density profiles and center-of-mass of the soliton pair in the presence of the optical *** is shown that the spin density is periodically varying due to flipping of spinors between the two *** amplitude of spin-flipping oscillation increases with lattice *** find that the system can also exhibit interesting nonlinear behavior for chosen values of *** present a fixed point analysis to study the effects of optical lattices on the nonlinear dynamics of the spin *** is seen that the optical lattice can act as a control parameter to change the dynamical behavior of the spin components from periodic to chaotic.

A Graphon Mean Field Convex Duality Approach to Shaping the Terminal Probability Distribution of a Network of Nonlinear Multi-Agent Systems

In order to steer the population distribution of a large number of agents interacting over a large-scale complex network towards a set of desired probability distributions for each sub-population,an approximate control scheme is proposed and developed by the use of Graphon Mean Field theory and Convex Duality Optimal Control. For a general class of multi-agent nonlinear systems interacting over large networks, the original problem for a finite population over a finite network is reformulated as an optimal control problem for an infinite population over an infinite network by letting the number of nodes in the graph and the number of agents within each cluster approach infinity. Subsequently, the associated control problem for the graphon limit system is reformulated as a linear program over the space of Radon measures and is solved using the duality relationship between the space of measures and that of continuous functions. A numerical example of a network with randomly sampled weightings is presented to illustrate the effectiveness of the graphon control probability assignment methodology.
