Emergency department procedural sedation for primary electrical cardioversion — a comparison with procedural sedations for other reasons

BACKGROUND: Atrial fibrillation(AF) is the most common arrhythmia treated in the emergency department(ED), with primary electrical cardioversion(PEC) the preferred method of rhythm control. Anecdotally, patients undergoing ED procedural sedation(EDPS) for PEC differ from those requiring EDPS for other procedures: they are at higher risk of adverse events, and require fewer drugs and lower doses. We attempt to verify this using an EDPS registry at a Canadian, tertiary care teaching ***: This is a retrospective review of patients that underwent EDPS for the period of June 2006 to September 2014. We compared demographics, medication use and intra-procedural adverse events between those receiving EDPS for PEC for AF compared to that for other indications. We report the asssociation between AEs and predictors using logistic ***: A total of 4 867 patients were included, 714 for PEC for AF and 4 153 for other indications. PEC patients were more likely male(58.5% vs. 47.1%), older(59.5 years vs. 48.1 years), and less likely to be ASA I(46.6% vs. 69.0%). PEC patients received smaller doses of propofol and less likely to receive adjuvant analgesic therapy(11.5% vs. 78.2%). PEC patients were more likely to experience hypotension(27.6% vs. 16.5%) but respiratory AEs(apnea, hypoxia and airway intervention) were not ***: EDPS for PEC differs from that conducted for other purposes: patients tend to be less healthy, receive smaller doses of medication and more likely to suffer hypotension without an increase in respiratory AEs. These factors should be considered when performing EDPS.

Mercury source zone identification using soil vapor sampling and analysis

Development and demonstration of reliable measurement techniques that can detect and help quantify the nature and extent of elemental mercury （Hg（0）） in the subsurface are needed to reduce uncertainties in the decision-making process and increase the effectiveness of remedial actions. We conducted field tests at the Y-12 National Security Complex in Oak Ridge, Tennessee, USA, to determine if sampling and analysis of Hg（0） vapors in the shallow subsurface （〈0.3 m depth） can be used to as an indicator of the location and extent of Hg（0） releases in the subsurface. We constructed a rigid polyvinyl chloride push probe assembly, which was driven into the ground. Soil gas samples were collected through a sealed inner tube of the assembly and were analyzed immediately in the field with a Lumex and/or Jerome Hg（0） analyzer. Time-series sampling showed that Hg vapor concentrations were fairly stable over time, suggesting that the vapor phase Hg（0） was not being depleted and that sampling results were not sensitive to the soil gas purge volume. Hg（0） vapor data collected at over 200 push probe locations at 3 different release sites correlated very well to areas of known Hg（0） contamination. Vertical profiling of Hg（0） vapor concentrations conducted at two locations provided information on the vertical distribution of Hg（0） contamination in the subsurface. We conclude from our studies that soil gas sampling and analysis can be conducted rapidly and inexpensively at large scales to help identify areas contaminated with Hg（0）.

Estimating underground mine ventilation friction factors from low density 3D data acquired by a moving LiDAR

Ventilation system analysis for underground mines has remained mostly unchanged since the Atkinson method was made popular by Mc Elroy in 1935. Data available to ventilation technicians and engineers is typically limited to the quantity of air moving through any given heading. Because computer-aided modelling, simulation, and ventilation system design tools have improved, it is now important to ensure that developed models have the most accurate information possible. This paper presents a new technique for estimating underground drift friction factors that works by processing 3 D point cloud data obtained by using a mobile Li DAR. Presented are field results that compare the proposed approach with previously published algorithms, as well as with manually acquired measurements.

This comment will review and discuss recent results obtained with the use of calorimetry in assessing molecular information from complex phenomena such as association in surfactant/polymer mixtures. These examples were selected to support the view that it is possible to ally the great sensitivity of modem calorimeters with carefully planned experiments and, sometimes, ancillary techniques in order to derive detailed information on molecular interactions.

High resolution tracking of adult horseshoe crabs Limulus polyphemus in a New Hampshire estuary using fixed array ultrasonic telemetry

While several studies have documented the large-scale, seasonal movements of horseshoe crabs, little is known about their fine-scale, daily movement patterns. In this study we used a fixed array ultrasonic telemetry system to track the movements of 12 male and 16 female horseshoe crabs in the Great Bay estuary, New Hampshire. Data were obtained during the mating season, as well as during the remainder of the summer and fall, in the years 2005-2008. During the mating season animals were often, but not always, active during the high tides when they were approaching and leaving the spawning beaches. On average, both males and females approached mating beaches during 33% of the high tides they experienced and they most often made the tran- sition from being inactive to active during the last two hours of an incoming tide. From April-October horseshoe crabs were significantly more active during high tide periods vs low tide periods, with no clear preference for diurnal vs nocturnal activity. After the mating season ended horseshoe crabs continued to move into shallower water at high tide and then return to deeper water at low tide. Observations by SCUBA divers suggest that during these excursions into the mudflats horseshoe crabs were digging pits in the sediment while foraging for food. Thus, the tidal rhythm of activity that has been so well documented during the mating season probably persists into the fall, and primarily involves foraging activities

The concepts of conditional expectations, martingales and stopping times were extended to the Riesz space context by Kuo, Labuschagne and watson （Discrete time stochastic processes on Riesz spaces, Indag. Math.,15（2004）, 435-451）. Here we extend the definition of an asymptotic martingale （amart） to the Riesz spaces context, and prove that Riesz space amarts can be decomposed into the sum of a martingale and an adapted sequence convergent to zero. Consequently an amart convergence theorem is deduced.

Work has been ongoing in building a thermodynamic database for the prediction of phase equilibria in Pt-based superalloys. The alloys are being developed for high texture applications in aggressive environments. The database will aid the design of alloys by enabling the calculation of the composition and proportions of phases present in alloys of different compositions. In order to extend this database, a preliminary assessment of the Cr-Pt-Ru system has been undertaken, using a combination of Pandat and MTDATA software. As a first step, it was necessary to provide thermodynamic models for the three associated binary systems. Owing to a lack of thermodynamic information for these systems, the binary assessments were based on phase diagrams available in the literature. Using recent experimental phase equilibria data for the ternary system, a preliminary assessment of the Cr-Pt-Ru system has been produced. In this preliminary assessment, simplified models were employed for the L12 and sigma phases with a view to extending the descriptions as new experimental information becomes available.

Physico-mechanical characteristics of bast fibres of Sesamum indicum and Sesamum radiatum for bioprospecting

Natural fibres are renewable,inexpensive and biodegradable sources of sustainable materials from plants or *** fibres,as the notable natural fibres,are gathered from the outer cell layers of the stem of *** and young bast fibres from two species of Sesamum,namely Sesamum indicum *** *** Schumach.&*** extracted through traditional water retting technique and analyzed for their physical,mechanical and chemical *** studies revealed polygon shaped phloem(bast fibre)cells with small lumen were similar to jute,in terms of architecture,and bagasse,in terms of lumen *** extraction and subsequent drying,the young stems of *** were found to yield more bast fibres by mass compared with the rest of the Sesamum samples,that was,0.59 g(dry weight)of bast fibres from 100 g(fresh weight)of *** to stereo microscopic images,the mature *** fibres had rougher edges than the *** *** and *** fibre surfaces also seemed to have pores,according to scanning electron microscopy(SEM)*** porosity appeared to get more pronounced as the plants got *** Sesamum bast fibres were also found to be extremely hydrophilic with a high-water retention *** fibres obtained from *** were determined to be suitable for the textile industry due to their light colour,ideal diameter and length,and water-holding capacity that matched the requirements of fabric *** in the absence of a mordant,the mature fibres showed a significant degree and evenness of Alizarin dye attachment,which might be correlated to the increase in fibre surface roughness with fibre *** fibres were also discovered to be comparable with jute in terms of phloem cell shape(polygonal),diameter(13-15 m),tenacity(12.86-32.54 gf/(g·km-1)),and linear density(2.5-3.3 g/km).It suggested that they might find industrial applications if further research were to be done.
