BACKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

目的观察慢性间歇低氧（CIH）对大鼠肺脏局部炎症反应以及肺泡结构的影响,探讨药物诱导血红素加氧酶-1（HO-1）在此过程中的干预效果.方法 Sprague-Dawley雄性大鼠24只,按随机数字表法分为常氧对照组、CIH组、常氧＋hemin干预组（hemin组）和CIH＋hemin干预组（CIH＋ hemin组）,每组6只.CIH组大鼠予间歇低氧环境中（4 min10％＋2 min21％ 02,8 h/d,共6周）,对照组大鼠置于常氧环境中.利用腹腔注射hemin（4 mg/kg）诱导HO-1表达,IN组及CIH组腹腔注射等体积生理盐水.采用western blot检测大鼠肺脏HO-1及cleaved caspase3表达水平,ELISA检测大鼠肺脏局部炎症因子中性粒细胞化学趋化因子-1（CINC-1）、IL-6、肿瘤坏死因子-α（TNF-α）和单核细胞趋化蛋白-1（MCP-1）的表达水平,使用HE染色观察大鼠肺脏病理改变.结果为期6周的CIH可明显升高肺脏炎症因子TNF-α[（2.20 ±0.10） vs （1.80±0.08） ng/ml]、IL-6[（0.87 ±0.05） vs（0.52 ±0.05） ng/ml]、CINC-1[（66 ±6） vs （39 ±5） pg/ml]和MCP-1[（2.20土0.09）vs（1.40±0.10） ng/ml]以及cleaved caspase3表达（均P＜0.05）,hemin可明显抑制由于CIH诱导的肺脏炎症因子[（TNF-α：（1.60±0.20） ng/ml,IL-6：（0.60±0.07） ng/ml,CINC-1：（45±6） pg/ml,MCP-1：（1.80 ±0.10） ng/ml,均P＜0.05]水平增高以及cleaved caspase3表达（P＜0.01）,肺脏HO-1表达明显升高.hemin可进一步增加CIH下肺脏HO-1的表达水平.CIH导致肺泡结构损伤,而hemin诱导的HO-1升高可明显抑制由于CIH诱导的肺脏病理改变.结论 CIH可导致肺脏局部炎症因子表达升高,细胞凋亡及肺泡结构破坏,药物诱导HO-1升高可抑制CIH诱导的局部炎症反应和细胞凋亡,逆转肺泡结构损伤,表明CIH对肺脏的损伤作用及HO-1的保护功能.

We aim to evaluate prostate health index as an additional risk-stratification tool in patients with Prostate Imaging Reporting and Data System score 3 lesions on multiparametric magnetic resonance *** with biochemical or clinical suspicion of having prostate cancer who underwent multiparametric magnetic resonance imaging in two tertiary centers(Queen mary Hospital and Princess Margaret Hospital,Hong Kong,China)between January 2017 and June 2022 were ***-magnetic resonance imaging fusion biopsies were performed after prostate health index *** who only had Prostate Imaging Reporting and Data System score 3 lesions were further stratified into four prostate health index risk groups and the cancer detection rates were *** of the 747 patients,47.3%had Prostate Imaging Reporting and Data System score 3 lesions *** detection rate of clinically significant prostate cancer in this group was 15.0%.The cancer detection rates of clinically significant prostate cancer had statistically significant differences:5.3%in prostate health index<25.0,7.4%in prostate health index 25.0-34.9,17.9%in prostate health index 35.0-54.9,and 52.6%in prostate health index≥55.0(P<0.01).Among the patients,26.9%could have avoided a biopsy with a prostate health index<25.0,at the expense of a 5.3%risk of missing clinically significant prostate *** health index could be used as an additional risk stratification tool for patients with Prostate Imaging Reporting and Data System score 3 *** could be avoided in patients with low prostate health index,with a small risk of missing clinically significant prostate cancer.

Mothers’ health-related quality of life: Its relationship with children’s health-related quality of life and behavior in low-income families

Objective:To examine the association between mothers’health-related quality of life(HRQOL)and their children’s HRQOL and behavior in low-income ***:Mothers of 278 children aged 6-12 years from low-income families were invited to complete the Child Health Questionnaire Parent Form 50(CHQ-PF50)and the Strength and Dif-ficulties Questionnaire(SDQ)for their children as well as the 12-item Short-Form Health Survey version 2(SF-12v2)and the Patient Health Questionnaire-2(PHQ-2).Multiple linear regressions with mother-child pairs as the unit of analysis were performed to examine the associations between maternal and child variables with adjustment of mother-and child-level ***:Compared with the general population,low-income mothers had a lower mean SF-12v2 mental component summary score and their children also had lower mean CHQ-PF50 physi-cal and psychosocial summary scores and SDQ total difficulties *** of mothers with SF-12v2 scores below the population mean of 50 had significantly worse CHQ-PF50 scores and higher SDQ total difficulties *** mother’s PHQ-2 depression status had no association with the child’s CHQ-PF50 ***:Our findings suggest that more attention should be paid to reducing the negative impact of health problems on mothers’daily roles in childcare in low-income families.

AIM： To investigate the chemopreventive efficacy of the Indian medicinal plant Acanthus ilicifolius L Acanthaceae in a transplantable Ehrlich ascites carcinoma （EAC）- bearing murine model.METHODS： Male Swiss albino mice were divided into four groups： Group A was the untreated normal control; Group B was the EAC control mice group that received serial, intraperitoneal （ip） inoculations of rapidly proliferating 2 × 10^5 viable EAC cells in 0.2 mL of sterile phosphate buffered saline; Group C was the plant extract-treated group that received the aqueous leaf extract （ALE） of the plant at a dose of 2.5 mg/kg body weight by single ip injections, once daily for 10, 20 and 30 consecutive days following tumour inoculation （ALE control）; and Group D was the EAC ＋ ALE- treatment group. The chemopreventive potential of the ALE was evaluated in a murine model by studying various biological parameters and genotoxic markers, such as tumour cell count, mean survival of the animals, haematological indices, hepatocellular histology, immunohistochemical expression of liver metallothionein （MT） protein, sister-chromatid exchanges （SCEs）, and DNA alterations.RESULTS： Treatment of the EAC-bearing mice with the ALE significantly （P 〈 0.001） reduced viable tumour cell count by 68.34% （228.7 × 10^6 ± 0.53） when compared to EAC control mice （72.4 × 10^6 ± 0.49）, and restored body and organ weights almost to the normal values. ALE administration also increased （P 〈 0.001） mean survival of the hosts from 35 ± 3.46 d in EAC control mice to 83 ± 2.69 d in EAC ＋ ALE-treated mice. Haematological indices also showed marked improvement with administration of ALE in EAC-bearing animals. There was a significant increase in RBC count （P 〈 0.001）, hemoglobin percent （P 〈 0.001）, and haematocrit value （P 〈 0.001） from 4.3 ± 0.12, 6.4 ± 0.93, and 17.63 ± 0.72 respectively in EAC control mice to 7.1 ± 0.13, 12.1 ± 0.77, and 30.23 ± 0.57 respectively in EAC ＋ ALE-treated group, along with concurrent de
