Prognostic factors for a favorable outcome after varicocele repair in adolescents and adults

The effect of varicocele repair on male fertility remains controversial. It would be helpful to determined which men would benefit most from varicocele repair, and target repair efforts at those individuals. A detailed review of the literature on prognostic factors for varicocele repair was performed using the PubMed NLM database. We found that the best predictor of postvaricocelectomy semen parameters is the preoperative semen parameters. The greatest improvements in semen parameters were found in men with larger varicoceles. While there is controversy, higher testosterone, younger age and larger testis size, in some studies predict for improvements in semen parameters postvaricocelectomy. A nomogram has been developed to predict the postvaricocelectomy semen parameters based on the preoperative semen parameters, varicocele grade and the age of the man （***. com）. Limited data consistently demonstrates the greatest improvements in DNA fragmentation rates in men with higher baseline DNA fragmentation rates. With respect to reproductive outcomes, higher baseline sperm density consistently predicts for natural pregnancy or assisted reproductive technology （ART） pregnancy rates. In addition, varicocele repair does seem to reduce the need for more invasive modalities of ART. In conclusion, we can now start to use specific parameters such as baseline semen quality, varicocele grade and patient age to predict post-repair semen oualitv and fertility potential following varicocelectomy.

In this paper, gas control on EAST in open and closed loop is discussed and its implementation into EASTPCS （plasma control system for the experimental advanced supercon- ducting tokamak） is introduced. Using a model to describe the plasma density response to the gas puff command, a gas simulation server （simserver） using MATLAB simulink tools and real time workshop was built up. Proper operation of the gas control algorithm was verified using this simserver. The simulation results suggested that the gas control can be applied in the next EAST campaign.

The utilization of decision support systems which are flexible enough to handle information about cooperative behavior and stakeholder attitudes are useful for analyzing complex social conflicts. One such conflict which arose from the redevelopment of a private brownfield property in Kitchener, Ontario, Canada is examined using such a decision support tool. Specifically, a formal model referred to as COAT which allows for the examination of conflicts with both coalition and attitude properties is rigorously defined and then implemented within the framework of the Graph Model for Conflict Resolution in order that insights may be gained on how the decision makers can reach win-win resolutions.

Brain and spinal cord injuries initiate widespread temporal and spatial neurodegeneration, through both necrotic and programmed cell death mechanisms. Inflammation, reactive oxidation, excitotoxicity and cell-specific dysregulation of metabolic processes are instigated by traumatic insult and are main contributors to this cumulative damage. Successful treatments rely on prevention or reduction of the magnitude of disruption, and interfering with injurious cellular responses through modulation of signaling cascades is an effective approach. Two intracellular signaling pathways, the phosphatase and tensin homolog （PTEN）/phosphatidylinositol 3-kinase （PI3K） and mitogen- activated protein kinase （MAPK） signaling cascades play various cellular roles under normal and pathological conditions. Activation of both pathways can influence anatomical and functional outcomes in multiple CNS disorders. However, some mechanisms involve inhibiting or enhancing one pathway or the other, or both, in propagating specific downstream effects. Though many intraceHular mechanisms contribute to cell responses to insult, this review examines the evidence exploring PTEN/PI3K and MAPK signaling influence on pathology, neuroprotection, and repair and how these pathways may be targeted for advancing knowledge and improving neurological outcome after injury to the brain and spinal cord.

It is standard practice, whenever a researcher finds a new gene, to search databases for genes that have a similar sequence. It is not standard practice, whenever a researcher finds a new gene, to search for genes that have similar expression (co-expression). Failure to perform co-expression searches has lead to incorrect conclusions about the likely function of new genes, and has lead to wasted laboratory attempts to confirm functions incorrectly predicted. We present here the example of Glia Maturation Factor gamma (GMF-gamma). Despite its name, it has not been shown to participate in glia maturation. It is a gene of unknown function that is similar in sequence to GMF-beta. The sequence homology and chromosomal location led to an unsuccessful search for GMF-gamma mutations in glioma. We examined GMF-gamma expression in 1432 human cDNA libraries. Highest expression occurs in phagocytic, antigen-presenting and other hematopoietic cells. We found GMF-gamma mRNA in almost every tissue examined, with expression in nervous tissue no higher than in any other tissue. Our evidence indicates that GMF-gamma participates in phagocytosis in antigen presenting cells. Searches for genes with similar sequences should be supplemented with searches for genes with similar expression to avoid incorrect predictions.

Mobile Ad hoc Network(MANET)is decentralized wireless network and can communicate without existing infrastructure in many *** is vulnerable to various attacks that affect its performance such as blackhole *** attacker,inject fault routing information to persuade the source node to select the path with malicious node as the shortest *** eliminate malicious nodes from launching any collaborative attack.A cooperative Trapping Approach(CTA)was proposed based on modifying Ad-hoc On-demand Distance Vector(AODV)routing protocol and trapping the malicious nodes by responding to the trap request *** approach aims to eliminate and rule out both single and collaborative malicious blackhole nodes from any *** approach realizes a backward tracking mechanism to perform the elimination *** proposed algorithm(CTA)was executed using NS-2 network *** performance metrics that has been considered to evaluate the performance of the proposed algorithm such as throughput,end to end delay,packet delivery ratio,and consuming *** experimental results have shown the performance metrics of the proposed approach outperformed other state of at algorithms.

While an increasing number of therapeutic options are now available for the first-line treatment of locally advanced or metastatic pancreatic cancer,the optimal choice for treatment in the second-line setting and beyond is less well defined.A variety of cytotoxic agents,either alone or in combination,have been evaluated,although primarily in the context of small single-arm or retrospective *** regimens have been associated with median progression-free survival rates in the range of 2-4 mo and overall survival rates between 4-8mo,highlighting the very poor prognosis of patients who are candidates for such *** therapies studied in this chemotherapy-refractory setting,meanwhile,have produced even worse efficacy *** the current article,we review the clinical evidence for treatment of refractory disease,primarily in patients who have progressed on front-line gemcitabine-based *** the process,we highlight the limitations of the available data to date as well as some of the challenges in designing appropriate clinical trials in this salvage setting,including how to select an appropriate control arm given the absence of a wellestablished reference standard,and the importance of incorporating predictive biomarkers and quality of life measures whenever possible into study design.
