BACKGROUND AND OBjECTIVE: To report the results of trabeculectomy with adjunctive intraoperative mitomycin C in Chinese patients with glaucoma. PATIENTS AND METHODS: The medical records of Chinese patients who received trabeculectomy with intraoperative mitomycin C from 1992 to 1998 were retrospectively reviewed. A “ successful” trabeculectomy was defined as one after which the intraocular pressure (IOP) could be controlled to between 5 and 21 mm Hg,inclusive,with no more than 3 glaucoma medications. RESULTS: One hundred fourteen trabeculectomies in 105 eyes of 90 patients were retrospectively reviewed. Mean age ± standard deviation was 48.1 ± 21.9 years. Primary open-angle glaucoma (43.0% ) and angle-closure glaucoma (27.2% ) were the most common diagnoses leading to trabeculectomy. The mean follow-up ± standard deviation was 29.6 ± 18.6 mo-nths (range,6 to 92 months). The overall success rate at the last followup was 73.7% (84 of 114 trabeculectomies). CONCLUSIONS: Trabeculectomy with mitomycinC is a safe and effective procedure for the control of IOP in Chinese patients with glaucoma.

上肩盂唇前后(SLAP)撕裂:基于3TMR肩关节成像的诊断,三维TSE序列与传统2D序列的比较

目的本研究旨在探讨应用3D-TSE-SPACE(samp-lingperfectionwithapplication-optimisedcontrastsusingdiffer-entflipangleevolution)肩关节MR成像对上肩盂唇前后(SLAP)病变的诊断并与3.0TMR2DTSE序列对该病的诊断进行比较。材料与方法肩关节MR成像包括2DTSE序列与3DTSE-SPACE序列,由2名观察者回顾性分析87例接受肩关节成像病人的SLAP病变的存在及病变类型。使用McNemar’s检验对敏感性及特异性进行比较,Cohen’skappa法对观察者内部的一致性进行检验并分析受试者操作特征曲线。结果 2DTSE序列的平均敏感度、特异度、精确度分别为90%、85%、86%,3DTSE-SPACE序列为81%、86%、85%,两者之间差异没有统计学意义。2DTSE序列成像与3DTSE-SPACE的观察者内部的一致性均稳定(κ=0.76、0.68),2DTSE序列成像与3DTSE-SPACE在ROC曲线下的面积分别为0.92和0.90,差异没有统计学意义。结论 3DTSE-SPACE序列的MR肩关节成像对SLAP病变诊断具有较稳定的一致性并且具有较高的准确性。要点①MR关节成像被认为是肩关节成像的决定性方法。②多种不同的MR序列可以对SLAP病变进行检查。③与2DTSE序列相比,SLAP病变3DTSE-SPACE成像具有大致相同的精确性。④但是在3DTSE-SPACE成像中会出现更多的假阴性结果。⑤在使用3DTSE-SPACE成像代替2DTSE序列扫描之前,在技术上还需要进一步的提高。

Over the last several hundred years,donkeys have adapted to high-altitude conditions on the Tibetan ***,the kiang,a closely related equid species,also inhabits this *** reports have demonstrated the importance of specific genes and adaptive introgression in divergent lineages for adaptation to hypoxic conditions on the Tibetan ***,we assessed whether donkeys and kiangs adapted to the Tibetan Plateau via the same or different biological pathways and whether adaptive introgression has *** assembled a de novo genome from a kiang individual and analyzed the genomes of five kiangs and 93 donkeys(including 24 from the Tibetan Plateau).Our analyses suggested the existence of a strong hard selective sweep at the EPAS1 locus in *** Tibetan donkeys,however,another gene,i.e.,EGLN1,was likely involved in their adaptation to high *** addition,admixture analysis found no evidence for interspecific gene flow between kiangs and Tibetan *** findings indicate that despite the short evolutionary time scale since the arrival of donkeys on the Tibetan Plateau,as well as the existence of a closely related species already adapted to hypoxia,Tibetan donkeys did not acquire adaptation via admixture but instead evolved adaptations via a different biological pathway.

目的比较3DT2加权快速自旋回波(TSE)与多平面2DT2加权TSE对于侵袭性子宫颈癌的评估。方法 75个子宫颈癌病人进行了3.0TMRI盆腔检查,均使用了5mm层厚的多平面2D(总时间为12min25s)以及1mm层厚的冠状位3DT2加权TSE序列扫描(7min20s)。对信噪比(SNR)进行定量分析,对影像质量进行定性分析。对45例接受根治性子宫切除术的病人进行了局部区域的分期。结果 3D成像中子宫颈癌的预估SNR及肿瘤相关的对比度显著增高(P<0.0001)。3D序列肿瘤的显示更好,但2D序列肿瘤边缘的锐度更高。两个序列整体的影像质量没有显著差异(P=0.38)。对于诊断宫旁浸润、阴道浸润和淋巴结转移的准确性、敏感性及特异性,两者没有明显差异。结论除了更短时间的MR数据采集,3DT2加权多平面重组影像在整体影像质量和子宫颈癌的分期准确度上大体相当于2DT2加权成像,但是对于肿瘤边缘的锐度显示还是有局限性。要点①对于子宫颈癌分期,3DT2加权序列等同于2DT2加权序列。②冠状位3D数据采集可以减少检查时间。③3D序列SNR和肿瘤相关的显著性明显更高。④3DT2加权重组影像对于肿瘤边缘的锐度显示不佳。
