Maternal Fetal Conflict from Legal Point of View Comparing Health Law in the United States and Islamic Law

Objectives and Methodology: The purpose of this paper is to demonstrate how maternal and fetal interests can sometimes be conflicting, and a situation can place care providers in a difficult position when it comes to decisionmaking. The study also intends to cover the health laws that deal with such situations in the legal systems of the United States and Islamic Law. This study takes the form of a critical literature review. The literature search was conducted using electronic citation databases that included Pub Med, Medline, Google Scholar, and others. The keywords used in this research related to medical and legal terms concerned with fetal rights, such as embryo, fetus, abortion, fetal rights, health law in the United States, Islamic health law, and comparative studies. The search was conducted in English as well as Arabic. Data were collected from books, journals, government documents, and Internet websites. Results: Doctors sometimes have to make difficult decisions when caring for pregnant women, since one person’s care directly affects another’s. The fetus depends fully on its mother for survival in the uterus. Nevertheless, at times it so happens that the interests of the mother and fetus differ. In such cases, a conflict between the two arises. Due to advances in technology in the medical field, doctors can now specifically target the fetus during medical procedures. When treating two different patients, health practitioners must choose that which is best for each patient independently. Approach to the maternal-fetal conflict must consider medical concerns, as well as legal ones. While there is some consistency in the medical approach between different countries, the legal part varies considerably, even among different states in America, as fetal legal status, personhood, and rights differ greatly. Electron-phonon interaction and related self-energy are fundamental to both the equilibrium properties and non-equilibrium relaxation dynamics of *** ele ctron-phonon interaction has been suggested by various time-resolved measurements to be important for the relaxation dynamics of graphene,the lack of energy-and momentum-resolved self-energy dynamics prohibits direct identification of the role of specific phonon modes in the relaxation ***,by performing time-and angle-resolved photoemission spectroscopy measurements on Kekulé-ordered graphene with folded Dirac cones at the Γ point, we have succeeded in resolving the self-energy effect induced by the coupling of electrons to two phonons at Ω=177 meV and Ω=54 meV,and revealing its dynamical change in the time ***,these strongly coupled phonons define energy thresholds,which separate the hierarchical relaxation dynamics from ultrafast,fast to slow,thereby providing direct experimental evidence for the dominant role of mode-specific phonons in the relaxation dynamics.

Management of infected pancreatic necrosis in the setting of concomitant rectal cancer:A case report and review of literature

BACKGROUND Pancreatitis with infected necrosis is a severe complication of acute pancreatitis and carries with it high rates of morbidity and mortality. The management of infected pancreatic necrosis alongside concomitant colorectal cancer has never been described in *** SUMmary A 77 years old gentleman presented to the Emergency Department of our hospital complaining of ongoing abdominal pain for 8 h. The patient had clinical features of pancreatitis with a raised lipase of 3810 U/L, A computed tomography(CT) abdomen confirmed pancreatitis with extensive peri-pancreatic edema. During the course of his admission, the patient had persistent high fevers and delirium thought secondary to infected necrosis, prompting the commencement of broad-spectrum antibiotic therapy with Piperacillin/Tazobactam. Subsequent CT abdomen confirmed extensive pancreatic necrosis(over 70%). Patient was managed with supportive therapy,nutritional support and gut rest initially and improved over the course of his admission and was discharged 42 d post admission. He represented 24 d following his discharge with fever and chills and a repeat CT abdomen scan noted gas bubbles within the necrotic pancreatic tissue thereby confirming infected necrotic pancreatitis. This CT scan also revealed asymmetric thickening of the rectal wall suspicious for malignancy. A rectal cancer was confirmed on flexible sigmoidoscopy. The patient underwent two endoscopic necrosectomies and was treated with intravenous antibiotics and was discharged after 28 *** 1 wk post discharge, the patient commenced a course of neoadjuvant radiotherapy and subsequently underwent concomitant chemotherapy prior to undergoing a successful Hartmann's procedure for treatment of his colorectal *** This case highlights the efficacy of endoscopic necrosectomy, early enteral feeding and targeted antibiotic therapy for timely management of infected necrotic pancreatitis. 大量证据显示随机对照临床试验(randomised controlled trial,RCT)的报告质量不理想。报告不透明,则读者既不能评判试验结果是否真实可靠,也不能从中提取可用于系统综述的信息。最近的方法学分析表明,报告不充分和设计不合理与对治疗效果产生评价偏倚有关。这种系统误差对RCT损害严重,而RCT正是以其能减少或避免偏倚而被视为评价干预措施的金标准。为了提高RCT的报告质量,一个由专家和编辑组成的工作组制定了临床试验报告的统一标准(Consolidated Standards of Reporting Trials,CONSORT)声明。CONSORT声明于1996年首次发表,并于2001年更新。声明由对照检查清单和流程图组成,供作者在报告RCT时使用。许多核心医学期刊和主要国际性编辑组织都已认可CONSORT声明。该声明促进了对RCT的严格评价和解释。2001年,在对CONSORT进行修订时,人们就已经清楚地认识到,解释和说明制定CONSORT声明的原理,有助于研究人员等撰写或评价临床试验报告。一篇CONSORT说明与详述文章于2001年同2001版CONSORT声明一起发表。2007年1月的专家会议之后,对CONSORT声明作了进一步修订并已发表,即"CONSORT2010声明"。这次更新对原版对照检查清单作了文字上的修改,使其更为明晰,并收入了与一些新近才认识到的主题相关的建议,如选择性报告结局产生的偏倚。说明与详述文件旨在加强人们对CONSORT声明的理解、应用和传播,这次也作了大量修订,对每一项新增或更新的清单条目的含义和增改理由进行了解释,提供了优秀的报告实例,还尽可能地提供了相关的经验性研究的参考文献。文中收入了若干流程图实例。"CONSORT2010声明"、其说明与详述文件,以及相关网站(***),对于改进随机临床试验报告必将有所裨益。

当一个平滑肌肿瘤的生物学潜能不能确定时被称之为恶性潜能未定的平滑肌肿瘤(STUMP),是一种少见的诊断。由于该亚组肿瘤形态学上的异质性,其初始鉴别诊断非常广泛。最近有资料表明伴有ALK基因重排的子宫炎性肌纤维母细胞瘤(IMTs)可能被误诊为STUMP,但这种情况的发生程度并未得到检验。作者分析60例女性肿瘤患者,包括既往诊断为STUMP(48例)及前瞻性考虑诊断为STUMP(12例)。对每个病例均进行组织学复查及行ALK免疫组化检测,并对免疫组化阳性者行ALK断裂FISH法进行证实。14%(6/43)的子宫体和子宫颈肿瘤ALK免疫组化阳性,而其它部位肿瘤ALK免疫组化均呈阴性。虽然仅有数例伴黏液样特征,但所有6例ALK免疫组化阳性的肿瘤中均存在,占伴有伴黏液样特征的宫体及宫颈部位肿瘤的35%(6/17)。
