Background:Laparoscopic distal pancreatectomy(LDp)has become the preferred approach for surgical management of left sided pancreas *** institution previously published its experience with distal pancreatectomies using a clockwise technique with good *** now reexamine our outcomes across a longer time ***:From August 2008 to November 2020,364 patients underwent LDp by hepatobiliary surgeons(HA and JS).All procedures were performed using the same clockwise approach,which includes the stepwise slow compression *** descriptive analysis of patient demographic,clinical,operative,and pathologic data was ***:Of the 364 patients who underwent LDp using this technique,clinically significant postoperative pancreatic fistula(pOpF)was noted in 26(7.1%)patients,while major morbidity and mortality were reported in 9.9%and 0.3%,***-assisted method was required for 18(4.9%)patients and unplanned conversion in 20(5.5%)*** a subset analysis of patients with pancreatic adenocarcinoma(n¼90),pOpF was noted in 13(14.4%),with minor complications occurring in 34.4%and major morbidity in 14.4%.Conclusion:LDp with a clockwise approach for dissection,combined with the stepwise slow compression technique results in excellent outcomes,with even lower pOpF rates than originally *** analysis of patients with pancreatic adenocarcinoma shows acceptable perioperative outcomes with this technique.

Objective: In previous studies, laparoscopic pancreaticoduodenectomy (LpD) has demonstrated safety and potential benefits over open pancreaticoduodenectomy (OpD) for pancreatic adenocarcinoma (pDAC). After performing both LpD and OpD procedures interchangeably in routine practice for a decade, the outcomes of LpD versus OpD for pDAC were analyzed and compared at a single institution. Our primary aim was to compare features of LpD and OpD in pDAC patients so that the suitable surgical approach may be chosen for each individual. Methods: From January 2010 through December 2020, all patients undergoing pancreaticoduodenectomy (pD) were identified, and information was collected prospectively. At a single institution, pD was performed on 589 patients, of whom 347 were OpD patients and 242 were LpD patients. After excluding those who underwent pancreatectomy for indications other than pDAC, total pancreatectomy, major vascular or concomitant organ resection, there were 237 patients (OpD = 157, LpD = 80). Then propensity score matching was completed to analyze 77 OpD patients versus 77 LpD patients to create a similar group of patients who underwent either LpD or OpD for pDAC. A comparison of perioperative data and 90-day outcomes with subsequent statistical analysis was performed. Results: Operative time (491 min vs. 281 min, p pD than OpD. The rates of pancreatic fistula (11.7% vs. 0.0%, p ptying (15.6% vs. 3.9%, p = 0.027) were higher for LpD than OpD respectively but overall morbidity was similar. Blood loss, mortality and post-pancreatectomy hemorrhage were also similar for both groups, but total costs ($60,245 vs. $50,900, p = 0.002) were significantly higher for LpD than OpD. Recurrence and overall survival were similar for the two groups. Conclusion: In our experience, LpD does not offer any advantages over OpD for pDAC and is associated with a higher rate of complications and costs. © 2024 Zhejiang University

The objective of the current study is to investigate the importance of entropy generation and thermal radiation on the patterns of velocity,isentropic lines,and temperature contours within a thermal energy storage device filled with magnetic nanoencapsulated phase change materials(NEpCMs).The versatile finite element method(FEM)is implemented to numerically solve the governing *** effects of various parameters,including the viscosity parameter,ranging from 1 to 3,the thermal conductivity parameter,ranging from 1 to 3,the Rayleigh parameter,ranging from 102 to 3×10^(2),the radiation number,ranging from 0.1 to 0.5,the fusion temperature,ranging from 1.0 to 1.2,the volume fraction of NEpCMs,ranging from 2%to 6%,the Stefan number,ranging from 1 to 5,the magnetic number,ranging from 0.1 to 0.5,and the irreversibility parameter,ranging from 0.1 to 0.5,are examined in detail on the temperature contours,isentropic lines,heat capacity ratio,and velocity ***,the heat transfer rates at both the cold and hot walls are analyzed,and the findings are presented *** results indicate that the time taken by the NEpCMs to transition from solid to liquid is prolonged inside the chamber region as the fusion temperatureθf ***,the contours of the heat capacity ratio Cr decrease with the increase in the Stefan number Ste.

We investigated the parametric optimization on incremental sheet forming of stainless steel using Grey Relational Analysis(GRA) coupled with principal Component Analysis(pCA). AISI 316L stainless steel sheets were used to develop double wall angle pyramid with aid of tungsten carbide tool. GRA coupled with pCA was used to plan the experiment conditions. Control factors such as Tool Diameter(TD), Step Depth(SD), Bottom Wall Angle(BWA), Feed Rate(FR) and Spindle Speed(SS) on Top Wall Angle(TWA) and Top Wall Angle Surface Roughness(TWASR) have been studied. Wall angle increases with increasing tool diameter due to large contact area between tool and workpiece. As the step depth, feed rate and spindle speed increase,TWASR decreases with increasing tool diameter. As the step depth increasing, the hydrostatic stress is raised causing severe cracks in the deformed surface. Hence it was concluded that the proposed hybrid method was suitable for optimizing the factors and response.

In recent years,speculation of an increase in Internet Gaming Disorder(IGD)has surfaced with the growing popularity of internet gaming among Chinese children and *** detrimental impact of IGD on mental health cannot be denied,even though only a small portion of the screen-dependent population exhibits psychopathological and behavioral *** present study aimed to explore a latent proﬁle analysis(LpA)of Internet Gaming Disorder on the mental health of Chinese school *** data were collected from a sample of 1005 Chinese school students(49.8%male;age M=13.32,SD=1.34 years)using a paper-pencil survey through convenience *** explored three latent proﬁles of internet gamers:regular gamers(62.4%),moderate gamers(28.1%),and probable disordered gamers(9.4%).Results showed that the probable disordered gamers had signiﬁcantly higher levels of depression,anxiety,emotional and conduct problems,hyperactivity,and peer problem symptoms as well as lower life satisfaction,and pro-social symptoms compared to regular and moderate gamers(ppful to mental health professionals in designing interventions for gamers who present IGD *** longitudinal studies should also be undertaken to assess whether mental health worsens for probable disordered gamers.

Deep neural networks excel at image identification and computer vision applications such as visual product search, facial recognition, medical image analysis, object detection, semantic segmentation,instance segmentation, and many others. In image and video recognition applications, convolutional neural networks(CNNs) are widely employed. These networks provide better performance but at a higher cost of computation. With the advent of big data, the growing scale of datasets has made processing and model training a time-consuming operation, resulting in longer training times. Moreover, these large scale datasets contain redundant data points that have minimum impact on the final outcome of the model. To address these issues, an accelerated CNN system is proposed for speeding up training by eliminating the noncritical data points during training alongwith a model compression method. Furthermore, the identification of the critical input data is performed by aggregating the data points at two levels of granularity which are used for evaluating the impact on the model *** experiments are conducted using the proposed method on CIFAR-10 dataset on ResNet models giving a 40% reduction in number of FLOps with a degradation of just 0.11% accuracy.
