Comparing 11 early warning scores and three shock indices in early sepsis prediction in the emergency department

BACkGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

We aim to evaluate prostate health index as an additional risk-stratification tool in patients with Prostate Imaging Reporting and Data System score 3 lesions on multiparametric magnetic resonance *** with biochemical or clinical suspicion of having prostate cancer who underwent multiparametric magnetic resonance imaging in two tertiary centers(Queen mary Hospital and Princess Margaret Hospital,Hong kong,China)between January 2017 and June 2022 were ***-magnetic resonance imaging fusion biopsies were performed after prostate health index *** who only had Prostate Imaging Reporting and Data System score 3 lesions were further stratified into four prostate health index risk groups and the cancer detection rates were *** of the 747 patients,47.3%had Prostate Imaging Reporting and Data System score 3 lesions *** detection rate of clinically significant prostate cancer in this group was 15.0%.The cancer detection rates of clinically significant prostate cancer had statistically significant differences:5.3%in prostate health indexk of missing clinically significant prostate *** health index could be used as an additional risk stratification tool for patients with Prostate Imaging Reporting and Data System score 3 *** could be avoided in patients with low prostate health index,with a small risk of missing clinically significant prostate cancer.

Objective Sentinel lymph node in breast cancer is a predictor of the tumor's metastatic *** emerging from antitumor immune response might also target sentinel lymph node (SLN).In this study,we evaluated the cytokine profile including interleukin-8 (IL-8),interleukin-12 (IL-12),interferon-gamma (IFN-γ),and tumor necrosis factor alpha (TNF-α) for the T-cell *** From January 2008 to August 2010,35 patients with breast mass underwent SLN biopsy in UNIMED Medical *** specimens were formalin-fixed and paraffin-embedded for hematoxylin and eosin staining and immunohistochemistry with following markers:IL-8,IL-12,IFN-γ and TNF-α.The staining intensity was scored according to four grades:negative (-),weak expression (+),moderately positive expression (+ +),strong positive expression (+ + +).The correlation between the expression of cytokines and pathological characteristics were also analyzed using the Spearman Rank Correlation *** Out of 35 patients,SLN metastases were observed in 20 patients and among them,75% (15 /20) were strongly stained (+ + +) with IL-8 and moderately stained (+ +) with IL-12,but only 25% (5 /20) were strongly stained (+ + +) with IFN-γ and TNF-α.The expression of IL-8 was significantly associated with all tumor grades (r=0.639,P=0.00),estrogen receptor (r=0.778,P=0.01),progesterone receptor (r=0.759 P=0.00) and HER-2 /neu receptor (r=0.863 P=0.00).Conclusion The strong association between IL-8 expression and SLN metastases might highlight the prognostic significance of IL-8 in SLN.

BACkGROUND: Chemoembolization with doxorubucin-eluting beads (DEB) has been used to treat hepatocellular carcinoma (HCC) since 2007. This study compared the efifcacy and sur-vival between transarterial chemoembolization (TACE) with DEB and conventional approach (cTACE) in HCC treatment. METHODS: This retrospective case-control study compared the overall survival and tumor response of HCC patients to cTACE (n=190) and DEB (n=143) by the reassessment of com-puted tomography and serum alpha-fetoprotein (AFP). Mul-tivariate analysis was used to determine the factors affecting tumor response. RESULTS: The median post-treatment to pre-treatment AFP level was 0.8 for a DEB session (n=258) and 1.0 for a cTACE session (n=452), showing a signiifcantly greater decrease in AFP after DEB (P80 months after the ifrst TACE session in the DEB group. CONCLUSION: DEB is a safe alternative to cTACE in HCC pa-tients with better therapeutic efifcacy.
