In this study, the feasibility of using chitosan, a multifunctional environmentally friendly biopolymer, at different molecular weights to simultaneously adsorb various metal ions from aqueous solutions was evaluated. The experiments were carried out to estimate adsorptivity and selectivity of three molecular weights chitosan, 3.60 × 105 （Chl）, 5.97 × 105 （Ch2） and 9.47 × 105 （Ch3） g/mol, towards Cu（II）, Ni（II）, and Pb（II） ions in aqueous solutions. Experimental results indicated that chitosan of high molecular weight （Ch3） was the best at adsorption capacity than the others. The bidirectional Glenn procedure is an integral step in the optimal palliation for single ventricular physiology in many forms of complex congenital heart disease. An increasing number of women who have undergone this connection in childhood are now reaching childbearing years. Low pulmonary blood flow and volume over load on the single ventricle pose several problems during pregnancy. We are reporting a 33-year-old lady with congenital tricuspid atresia and mild pulmonary stenosis who had late Bidirectional Glenn procedure with pulmonary forward flow and later underwent six successful pregnancies, with delivery of six low birth weight babies with no reported complications.

AIM： To the look at the current strength of evidence and the potential application of anti-oxidants in this ***： Two electronic databases （PubMed and Web of Knowledge） were searched to January 2013 to find studies addressing serum bilirubin levels in non-alcoholic fatty liver disease （NAFLD）. The search used key word combinations in relation to NAFLD and serum bilirubin specifc to human adults only. After screening selected studies were reviewed in depth by two inde-pendent reviewers. Data synthesis with further metaanalysis was planned but not possible due to the het-erogeneity of the outcome measures in these ***： Out of 416 studies screened only seven studies were considered suitable for inclusion. All seven studies consistently reported an inverse association of bilirubin with NAFLD despite the heterogeneous sample of studies. Only two studies were prospective. No negative studies were ***： Most studies suggest a correlation between high bilirubin levels of any type are inversely correlated with NAFLD. But to date most of these stud-ies have been poorly designed to allow meaningful con-clusions, except one cohort study. There is a need for a large prospective cohort study in multiple populations to test this hypothesis fully before mechanistic associa-tions can be established and therapeutic options of the apparent anti-oxidant effect of bilirubin be explored in NAFLD. Furthermore these studies should include analysis of UGT1A1 gene to expound upon underlying cause of unconjugated hyperbilirubinaemia.

汉语动结式是汉语语法中比较复杂、比较难理解的语法。动结式一般由谓语动词与结果补语两个部分组成的,前面的动词表示动作的变化、行为等,后面的补语说明动作和行为的结果。在汉语作为第二语言的教学中,动结式是非常重要的语言点,也是留学生最难掌握的汉语语法点之一。由于阿拉伯语中没有“补语”系统,加上动结式本身的独特性、复杂性和多变性,对于阿拉伯语学生而言,动结式的习得比较困难。就目前关于汉语结果补语的习得研究看来,大部分都是针对欧美和亚洲国家的留学生,而很少有针对阿拉伯留学生或者非洲留学生的研究。因此针对阿拉伯汉语学习者习得汉语动结式的研究就非常有必要。本文使用的研究方法是实验方法,对母语为阿拉伯语的初级、中级和高级汉语学习者进行了动结式的测试。实验结果显示,汉语水平对阿拉伯语母语者习得汉语动结式有很显著影响,即随着汉语水平的提高,学习者的动结式习得正确率也逐渐提高。同时发现,因果事件框架也对动结式写的产生影响,在与汉语动结式对应的阿语结果中,如果阿语的谓语动词含有方式,则阿语中结果因子独立表达的动结式习得要好于结果因子融合的动结式,而阿语的谓语动词不含方式时,阿语中结果因子独立表达和融合表达的动结式没有显著差异。基于以上结论,汉语国际教学中动结式的教学提出了注重国别化、分类型、分阶段以及对比分析教法的教学建议,希望对动结式的教学有所启示。

Objectives: Diabetic retinopathy (DR) is one of the most common microvascular complications of type 2 diabetes mellitus (T2DM). It is multifactorial with the contribution of multiple genetic factors. We questioned the association of polymorphisms in the peroxisome proliferator-activated receptor ?2 (PPAR?2) gene (Pro12Ala and C161T) with DR in an Egyptian population. Methods: This case control study included one hundred healthy individuals and 252 T2DM among them 122 with DR and 130 without DR. Genotyping was done by polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP). Results: The Pro12Ala Ala allele was associated with decreased risk of DR with an odds ratio (OR) of 0.484, 95% confidence interval (CI) (0.254 - 0.920), and a p value = 0.024. The C161T T allele was associated with increased risk of DR with OR = 2.593, 95% CI (1.672 - 4.020), p 1c) in multivariate regression analysis only C161T was associated with increased risk of DR with OR = 3.479, 95% CI (1.907 - 6.346), p 1c was higher in Pro/Pro genotype when compared to those with Ala/Ala and Pro/Ala genotypes. Conclusion: We report that T allele of C161T increased risk of DR in the studied population. Further studies are warranted to investigate functional implications of polymorphisms of the PPAR-? gene in DR development.

Objective:To investigate the antifibrotic role of rosmarinic acid(RA),a natural polyphenolic compound,on HSCs activation/proliferation and apoptosis in vitro and in vivo. Methods:The impact of RA on stellate cell line(HSC-T6) proliferation,activation and apoptosis was assessed along with its safety on primary hepatocytes. In vivo,rats were divided into:(i) normal;(ii) thioacetamide(TAA)-intoxicated rats for 12 weeks;(iii) TAA+silymarin or(iv) TAA+RA. At the end of experiment,liver functions,oxidative stress,inflammatory and profibrogenic markers,tissue inhibitor metalloproteinases type-1(TIMP-1) and hydroxyproline(HP) levels were evaluated. Additionally,liver histopathology and immunohistochemical examinations of alpha-smooth muscle actin(α-SMA),caspase-3 and proliferation cellular nuclear antigen(PCNA) were determined. Results:RA exhibited anti-proliferative effects on cultured HSCs in a time and concentration dependent manner showing an IC50 of 276 μg/mL and 171 μg/mL for 24 h and 48 h,respectively,with morphological reversion of activated stellate cell morphology to quiescent form. It significantly improved ALT,AST,oxidative stress markers and reduced TIMP-1,HP levels,inflammatory markers and fibrosis score(S1 vs S4). Furthermore,reduction in α-SMA plus elevation in caspase-3 expressions of HSCs in vitro and in vivo associated with an inhibition in proliferation of damaged hepatocytes were recorded. Conclusions:RA impeded the progression of liver fibrosis through inhibition of HSCs activation/proliferation and induction of apoptosis with preservation of hepatic architecture.

Background: EEG is one of the most sensitive instrumental techniques for identifying sepsis-related neurological complications and a valuable tool in the ICU when clinical assessment is difficult. Aim of the Study: To detect EEG changes in neonates with evidence of sepsis. Patients and Methods: This was a prospective observational study for detecting EEG changes in cases of proven neonatal sepsis. This study was conducted at the NICU of Al Zahraa University Hospital for a period from October 2019 to March 2020. Our studied neonates were 50. This study included full-term newborns (37 to 42 weeks of gestational age) with clinical and laboratory evidence of sepsis (early and late onset). Neonatal birth asphyxia, jaundice, congenital malformations, suspected inborn error of metabolism and neonates electrolytes disturbances were excluded from the study. We record EEG changes during 20 min By an electroencephalogram (EEG) with abioelectric amplifier (Deltamed, Bourgogne), (Philips) Screen. Results: Among our studied neonates (neonates with evidence sepsis), 44.0% developed clinical seizures, 81% of the seizures group had abnormal EEG, 35.7% of No seizures group had abnormal EEG. Among our studied neonates, 56% had abnormal EEG EEG abnormalities were as follow, 26%/ had low voltage, 30% had spikes, 22% had sharp waves. Background: Anemia is reported by many studies as an important risk factor for poor locoregional disease control and survival in head and neck carcinoma. We aimed to study the frequency and prognostic effect of low hemoglobin (Hb) level in head and neck squamous cell carcinoma (HNSCC) before radiotherapy (RT) and/or chemoradiotherapy (CRT). Material and Methods: We retrospectively studied the charts of 86 patients diagnosed with laryngeal and hypopharyngeal SCC in a university hospital in Cairo, Egypt. Based on the World Health Organization (WHO), anemia was diagnosed in males at Hb levels 13 g/dl in 44/75 (58.7%). Tumor sites were larynx in 77 (89.5%) and hypopharynx in 9 (10.5%) patients. Disease stage 3, 4 was common in 59/86 (68.6%) patients. Thirty-six (41.8%) patients had surgery. 13/86 (15.1%) patients received induction chemotherapy, while 31/86 (36.0%) had concomitant chemoradiotherapy. Radiotherapy was given in 65/86 (75.58%) patients. The mean RT dose was 66 Gy. Non-anemic patients had significant higher inci-dence of complete response (CR) to treatment (p = 0.034). 25.8% of male pa-tients with Hb 13 g/dl (p = 0.403). Female patients whether anemic or non-anemic had no recurrence (p = 0.036 and p = 0.403 respectively). The median duration of DFS and OS was 6.52 and 9.33 months respectively. Background: incidence of Colorectal cancer (CRC) is increasing globally. In Egypt, CRC ranks the sixth most common cancer in males and the fifth in females. Aim: To assess the expression of estrogen receptors (alpha and beta) in pre-malignant (adenomatous polyps and IBD), malignant colorectal lesions and normal colonic mucosa in group of Egyptian patients. Methods: This prospective study was done on 45 patients presenting with colonic symptoms, patients were divided into four groups;15 CRC patients, 10 patients with adenomatous polyps, 10 IBD patients and 10 patients in the control group. Patients subjected to: Stool analysis, FOBT, CBC, CEA, Abdominal ultrasound & colonoscopy and biopsy (number = 80), Pathological, immunohistochemistry and RT- PCR quantification of ERα and ERβ were done. Results: Mean age: 39.2 (12 - 73), gender: M/F: 28/17. Bleeding per rectum was the commonest presentation;29/45 (64.4%). CEA was significantly elevated in the CRC group compared with other studied groups (1692 mg/L vs. 4.0, 4.0 and 4.4 mg/L). Ultrasonography of the studied patients showed that metastatic CRC: 3/15 (20%);Colonic wall thickening: 5/15 (33.3%), 1/10 showed colonic polypoidal lesions in adenomatous polyps groups, in IBD group: 4/10 (40%) showed colonic and ileocecal thicknening. All the studied patients showed negative results for estrogen receptors (alpha and beta) by the use of immunohistochemistry staining and RT-PCR technique. Conclusion: Role of estrogen receptors in the colonic mucosa, precancerous and colorectal cancer is doubtful, contradictory results with some literature data could be due to racial and genetic difference in the studied population.
