Background: EEG is one of the most sensitive instrumental techniques for identifying sepsis-related neurological complications and a valuable tool in the ICU when clinical assessment is difficult. Aim of the Study: To detect EEG changes in neonates with evidence of sepsis. Patients and Methods: This was a prospective observational study for detecting EEG changes in cases of proven neonatal sepsis. This study was conducted at the NICU of Al Zahraa University Hospital for a period from October 2019 to March 2020. Our studied neonates were 50. This study included full-term newborns (37 to 42 weeks of gestational age) with clinical and laboratory evidence of sepsis (early and late onset). Neonatal birth asphyxia, jaundice, congenital malformations, suspected inborn error of metabolism and neonates electrolytes disturbances were excluded from the study. We record EEG changes during 20 min By an electroencephalogram (EEG) with abioelectric amplifier (Deltamed, Bourgogne), (Philips) Screen. Results: Among our studied neonates (neonates with evidence sepsis), 44.0% developed clinical seizures, 81% of the seizures group had abnormal EEG, 35.7% of No seizures group had abnormal EEG. Background: Renal osteodystrophy (ROD) is a bone disorder resulting from chronic kidney disease (CKD) and related metabolic diseases. Dickkopf-related protein-1 (DKK-1) is critical in regulating bone biology. This study aimed to evaluate the serum DKK-1 level as a bone marker in children with CKD who undergo regular hemodialysis (HD). Subjects and Methods: This case-control study involved 40 children with CKD on HD and 40 healthy children as controls. The study measured serum DKK-1 levels and performed a dual-energy X-ray absorptiometry scan (DEXA) in line with routine laboratory investigations. Results: There was a significant increase in the serum level of DKK-1 in the patient group compared to the control group. The DKK-1 levels were 2540.65 (2215.4 - 2909.2) pg/ml and 1110.45 (885.45 - 1527.65) pg/ml, respectively, with a p-value of less than 0.001. In the hemodialysis group, 25 patients (62.5%) had low bone mineral density (BMD) with a Z-score of under -2.0. Eighteen of these patients had low BMD in both the neck of the femur and lumbar spines. Additionally, there was a significant increase in serum DKK-1 level in patients with low BMD (2567.35 (2303.8 - 3108.1) pg/ml) compared to patients with normal BMD (2454 (1859 - 2820) pg/ml) (p = 0.041). There was also a significant positive correlation between DKK1 level and phosphorus, alkaline phosphatase, and Parathormone serum levels. In conclusion, the study indicates a clear correlation between DKK-1 and BMD in children undergoing maintenance hemodialysis. DKK1 is a promising biomarker for CKD-MBD.

BACKGROUND Diffusion-weighted magnetic resonance imaging has shown promise in the detection and quantification of hepatic fibrosis. In addition, the liver has numerous endogenous micro-RNAs(miRs) that play important roles in the regulation of biological processes such as cell proliferation and hepatic *** To assess diffusion-weighted magnetic resonance imaging and miRs in diagnosing and staging hepatic fibrosis in patients with chronic hepatitis *** This prospective study included 208 patients and 82 age-and sex-matched controls who underwent diffusion-weighted magnetic resonance imaging of the abdomen, miR profiling, and liver biopsy. Pathological scoring was classified according to the METAVIR scoring system. The apparent diffusion coefficient (ADC) and miR were calculated and correlated with pathological *** The ADC value decreased significantly with the progression of fibrosis, from controls(F0) to patients with early fibrosis(F1 and F2) to those with late fibrosis(F3 and F4)(median 1.92, 1.53, and 1.25 × 10^(-3) mm^2/s, respectively)(P = 0.001).The cut-off ADC value used to differentiate patients from controls was 1.83 × 10^(-3) mm^2/s with an area under the curve(AUC) of 0.992. Combining ADC and miR-200 b revealed the highest AUC(0.995) for differentiating patients from controls with an accuracy of 96.9%. The cut-off ADC used to differentiate early fibrosis from late fibrosis was 1.54 × 10^(-3) mm^2/s with an AUC of 0.866. The combination of ADC and miR-200 b revealed the best AUC(0.925) for differentiating early fibrosis from late fibrosis with an accuracy of 80.2%. The ADC correlated with miR-200 b(r =-0.61, P = 0.001), miR-21(r =-0.62, P = 0.001), and miR-29(r = 0.52,P = 0.001).CONCLUSION Combining ADC and miRs offers an alternative surrogate non-invasive diagnostic tool for diagnosing and staging hepatic fibrosis in patients with chronic hepatitis C.

Novel CYP1B1 mutations and a possible prognostic use for surgical management of congenital glaucoma

AIM: To identify CYP1B1 gene mutations and evaluate their possible role as a prognostic factor for success rates in the surgical management of Egyptian congenital glaucoma ***: Totally 42 eyes of 29 primary congenital glaucoma patients were operated on with combined trabeculotomy/trabeculectomy with mitomycin-C and followed up at 1 d, 1 wk, 1, 6 and 12 mo postoperatively. Genomic DNA was extracted from peripheral blood leukocytes. Coding regions of CYP1B1 gene were amplified using 13 pairs of primers, screened for mutations using single-strand conformation polymorphism followed by sequencing of both strands. Efficacy of the operation was graded as either a success [maintaining intraocular pressure(IOP) less than 21 mm Hg with or without antiglaucoma medication], or a failure(IOP more than 21 mm Hg with topical antiglaucoma medications). RESULTS: Seven novel mutations out of a total of 15 different mutations were found in the CYP1B1 genes of 14 patients(48.2%). The presence of CYP1B1 gene mutations did not correlate with the failure of the surgery(P=0.156, odds ratio=3.611, 95%CI, 0.56 to 22.89); while the positive consanguinity strongly correlated with failure of the initial procedure(P=0.016, odds ratio=11.25, 95%CI, 1.57 to 80.30). However, the Kaplan-Meier survival analysis revealed a significantly lower time of IOP control in the subgroup with mutations in CYP1B1 versus the congenital primary glaucoma group without mutations(log rank test, P=0.015).CONCLUSION: Seven new CYP1B1 mutations are identified in Egyptian patients. Patients harboring confirmed mutations suffered from early failure of the initial surgery. CYP1B1 mutations could be considered as a prognostic factor for surgery in primary congenital glaucoma.

The purpose of this paper is to introduce second order (K, F)-pseudoconvex and second order strongly (K, F)- pseudoconvex functions which are a generalization of cone-pseudoconvex and strongly cone-pseudoconvex functions. A pair of second order symmetric dual multiobjective nonlinear programs is formulated by using the considered functions. Furthermore, the weak, strong and converse duality theorems for this pair are established. Finally, a self duality theorem is given.

Background:The increasing occurrence of diabetes mellitus(DM)noted worldwide has considerably elicited concern in the recent *** is associated with elevated vascular complications,morbidity,mortality,and poor quality of *** this context,mesenchymal stem cells(MSCs)have shown significant therapeutic potentialities in managing and curing type 1 DM owing to their self-renewable,immunosuppressive,and differentiation *** investigated the potential action of N,N′-diphenyl-1,4-phenylenediamine(DPPD),a well-known synthetic antioxidant to enhance the therapeutic ability of the adipose-derived stem cells(AD-MSCs)in alleviating kidney and liver complications in diabetic ***:Over the four weeks of experiments,albino male rats(n=36)were split into six test groups:control,DPPD(250 mg/kg,i.p.),STZ-diabetic(D),D+DPPD,D+AD-MSCs(1×10^(6)cell/rat,i.v.),and D+AD-MSCs+DPPD treated ***:Significant declines in the renal and hepatic oxidative stress markers(MDA,ROS,and AGEs)were observed coupled with a significant elevation in many antioxidant marker levels(GSH,SOD,CAT,GPx,HO-1,and TAC)in the diabetic rats treated with either DPPD or AD-MSCs or their co-administered injection compared to the diabetic untreated *** was suggested to be the leading cause of amelioration of the kidney functions(as measured by urea,uric acid,and creatine levels)and liver functions(as evidenced by the levels of AST,ALT,ALP,bilirubin,total proteins,albumin,and globulins).Conclusion:DPPD and AD-MSCs co-administration showed superior results in terms of the enhancement of the relative hepato-renal function,indicating the beneficial role of DPPD supplementation in increasing the therapeutic potential of AD-MSCs.

There are few numerical techniques available to solve the Bagley-Torvik equation which occurs considerably frequently in various offshoots of applied mathematics and mechanics. In this paper, we show that Chelyshkov-tau method is a very effective tool in numerically solving this equation. To show the accuracy and the efficiency of the method, several problems are implemented and the comparisons are given with other methods existing in the recent literature. The results of numerical tests confirm that Chelyshkov-tau method is superior to other existing ones and is highly accurate.

Background: Carcinoma of the male breast is relatively a rare disease accounting for about 1% of all cancer in men. Its treatments are based mainly on those of Female Breast Cancer (BCF). Materials and Methods: This is a single institution retrospective study including all presenting to the NCI, Cairo University with Breast Cancer in a Male (BCM) in the last 11 years between Jan. 2005 until Jan. 2016. Data were collected from patient’s files from the statistical department then analyzed. Aims: To study the clinico-pathological characteristics, the presentation, workup, surgical approaches and postoperative complications and outcome of management, with addressing similarities and difference from BCF. Results: 64 patients were included in this cohort. The mean age was 58.6 & the median age was 59 years (range: 31 - 87 years). The main presenting symptomatology was retroareolar breast lump (50 cases, 78.1%). Most of our cases were advanced;22 cases (34.3%) were stage III & 16 cases (25%) were stage IV. Surgery was performed for 50 patients;MRM for 26 cases (40.6%), RM for 18 cases (28.1%) & toilet mastectomy for 6 cases (9.3%). Primary closure was feasible in 34 patients (68%) while 16 cases (32%) required reconstruction by local or pedicled flaps. Tamoxifen is the most important non-surgical treatment. It was given to all our cases (64 cases, 100%) either postoperatively as adjuvant management (36 cases, 56.2%) or as palliation for metastatic disease (28 cases, 43.8%). Chemotherapy was given to 32 patients (100%) as an adjuvant for 24 cases (75%) and as a palliation for 8 cases (25%). Radiotherapy was given to 26 patients (100%) as adjuvant therapy for 20 cases (76.9%) and as a palliation for 6 cases (23.1%). The only significant factor determining the overall 5 years survival was the stage of the disease. In this work, a highly efficient algorithm is developed for solving the parabolic partial differential equation (PDE) with the nonlocal condition. For this purpose, we employ orthogonal Chelyshkov polynomials as the basis. The convergence analysis of the proposed scheme is derived. Numerical experiments are carried out to explain the efficiency and precision of the proposed scheme. Furthermore, the reliability of the scheme is verified by comparisons with assured existing methods.
