Clinical,laboratory,and therapeutic differences between immunoglobulin E-mediated and non-immunoglobulin E-mediated cow’s milk protein allergy in children

BACKGROUND Although breast milk is ideal for newborns,in some cases,it is replaced with cow’s milk,which contains proteins that increase the risk of cow’s milk protein allergy(CMPA).AIM To evaluate CMPA prevalence in Bahrain and compare clinical characteristics of children with immunoglobulin E(IgE)-and non-IgE-mediated *** This retrospective cohort study examined children with CMPA diagnosed at the pediatric gastroenterology outpatient clinic of the Salmaniya Medical Complex,Bahrain,between 2014 and 2022,and assessed CMPA *** presentations,laboratory findings,dietary modifications,and outcomes were compared between children with IgE-mediated and non-IgE-mediated *** variables were compared using Fisher’s exact test or Pearson’sχ2 test,whereas continuous variables were compared using Student’s t-test or the Mann-Whitney U *** Of 8332 patients,6022(72.3%)adhered to their *** them,618(10.3%)were suggested of having CMPA and 595(96.3%)were included;CMPA prevalence was 2%.Most patients were Bahraini(93.8%)and males(55.3%).Non-IgE-mediated type accounted for 77.3%cases and IgE-mediated type,22.7%.IgE-mediated CMPA presented more in boys(P=0.030),and later in life(5.1 months±4.5 months vs 4.2 months±4.2 months,P=0.016,95%CI:0.08-1.73),had more associated diseases(P<0.001);and presented with more cutaneous(P=0.024)and respiratory(P=0.003)manifestations,severe symptoms[rash/dry skin(P=0.031),facial swelling/angioedema(P=0.003),failure to thrive(P=0.013),apparent life-threatening event(P<0.001)],and positive physical findings(P=0.002)than non-IgE-mediated *** patients were exclusively fed cow milk formula(50.3%).The amino acid-based formula(AAF)was most prescribed(60.5%)with no difference between the two types(P=0.173).Although breastfeeding was recommended to 49.6%,only 8.2%were exclusively ***-mediated CMPA was associated with a longer follow-up duration than non-IgE-mediated CMPA(17.3 months±14.0 mo

In Vitro Study on Virulence Potentials of Burkholderia pseudomallei Isolated from Immunocompromised Patients

Eighty four throat swabs were obtained from Basrah General Hospital inpatients （N = 34）： 17 were suffering from renal failure and the other 17 were diabetics; and from outpatients （N = 50）. Throat swabs were cultured first in the selective media Ashdown＇s broth then subcultured on Ashdown＇s agar to isolate Burkholderia pseudomallei which was recovered from seven cases （8.33%）. Four isolates were from renal failure patients （23.53%）, two from diabetic patients （11.76%） and the seventh isolate was from an outpatient with tonsillitis. All isolates were able to produce capsules, form filament chains, exhibit swarming motility and were arabinose non assimilators （Ara-） indicative of their virulence. Additionally, isolated B. pseudomallei were found to produce protease, lipase, hemolysin, and lecithinase and were able to produce biofilm, the root of many troublesome persistent infections that resist antibiotic treatment. Susceptibility of the seven isolates of B. pseudomallei toward 11 antibiotics was assessed, isolates were found multiply resistant to all antibiotics apart from ciproflaxin. This study confirms for the first time isolation of B. pseudomallei from immunocompromised patients in Basrah city of Iraq and describes their virulence potentials.

A New Attempt of Using Crushed Plastic Solid Wastes in Water Filtration

The aim of this new study is to investigate the ability of using crushed plastic solid wastes in water filtration by using a pilot plant. Two sets of filters were used. The first set represents mono media filters. The first filter is a sand media with effective size of 0.65 mm and the others three are plastic media with different grain sizes. The second set represents dual media filters with different depths, the filters were made to operate with the same effective size （0.6-1.0 mm）. These filters were subjected to the same operating conditions of filtration rates and infiuent turbidity. The filters were operated at rates of 5, 7.5, 10, and 12.5 m/h in order to find the optimum filtration velocity with influent turbidity ranged between 7 and l0 NTU （nephelometric turbidity unit）. The results indicated that the single plastic filters and the dual filters produced water of the same high quality as the sand filter. Plastic filters were slower in the development of head losses by about of 8%-78% less and they have longer running time than the sand filter, while the dual filters were slower in the development of head losses by about of 14%-16% and they have longer running time by about of 12%-40% than the sand filter.

Background:Pediatric endoscopy has become an essential diagnostic and therapeutic tool for a range of gastrointestinal conditions in children,according to published *** study aimed to assess the indications,outcomes,and complications of therapeutic endoscopy in children at King Abdullah University Hospital(KAUH)in ***:This study conducted a retrospective chart review of therapeutic endoscopic procedures,including esophageal dilation,variceal and non-variceal hemostasis,foreign body retrieval,and percutaneous endoscopic gastrostomy(PEG)Tube insertion,performed between january 2014 and December 2020 at the Gastrointestinal Endoscopy Unit(a mixed adult and pediatric unit)at *** data,indications for intervention,comorbidities,procedural outcomes,postprocedural treatment,complications,readmission,and patient follow-up data were extracted from the patient's medical ***:During the study period,185 patients underwent 260 therapeutic endoscopic procedures.110(60%)of the patients were *** average age of the patients was 81.8±51.46(standard deviation)*** body retrieval was the most common procedure,with accidental ingestion being the primary cause and coins being the most commonly impacted foreign *** by esophageal dilatation and PEG tube *** were mild and ***:The success rates and post-procedural complications of pediatric endoscopy procedures are similar between low-and high-volume hospitals worldwide,and procedural volume is not the only factor contributing to procedural outcomes and complication rates.
