Quality of life following laparoscopic Nissen fundoplication: Assessing short-term and long-term outcomes

AIM: To investigate the quality of life following lapa-roscopic Nissen fundoplication by assessing short-term and long-term outcomes. METHODS: from 1992 to 2005, 249 patients under-went laparoscopic Nissen fundoplication. Short-term outcome data including symptom response, side effects of surgery, endoscopy, and patient's perception of over-all success were collected prospectively. Long-term out-comes were investigated retrospectively in patients witha median follow-up of 10 years by assessment of reflux symptoms, side effects of surgery, durability of antire-flux surgery, need for additional treatment, patient's perception of success, and quality of life. Antireflux sur-gery was considered a failure based on the following criteria: moderate to severe heartburn or regurgitation; moderate to severe dysphagia reported in combination with heartburn or regurgitation; regular proton pump inhibitor medication use; endoscopic evidence of erosive esophagitis Savary-Miller grade 1-4; pathological 24-h pH monitoring; or necessity to undergo an additional surgery. The main outcome measures were short-and long-term cure rates and quality of life, with patient sat-isfaction as a secondary outcome measure. RESULTS: Conversion from laparoscopy to open sur-gery was necessary in 2.4% of patients. Mortality was zero and the 30-d morbidity was 7.6% (95%CI: 4.7%-11.7%). The median postoperative hospital stay was 2 d [interquartile range (IQR) 2-3 d]. Two hundred and forty-seven patients were interviewed for short-term analysis following endoscopy. Gastro-esophageal reflux disease was cured in 98.4% (95%CI: 95.9%-99.6%) of patients three months after surgery. New-onset dysphagia was encountered postoperatively in 13 patients (6.7%); 95% reported that the outcome was better after antireflux surgery than with preopera-tive medical treatment. One hundred and thirty-nine patients with a median follow-up of 10.2 years (IQR 7.2-11.6 years) were available for a long-term evalu-ation. Cumulative long-term cure rates were 87.7% (81.0%-92.2%)

We present a method to improve the execution time used to build the roadmap in probabilistic roadmap planners. Our method intelligently deactivates some of the configurations during the learning phase and allows the planner to concentrate on those configurations that axe most likely going to be useful when building the roadmap. The method can be used with many of the existing sampling algorithms. We ran tests with four simulated robot problems typical in robotics literature. The sampling methods applied were purely random, using Halton numbers, Gaussian distribution, and bridge test technique. In our tests, the deactivation method clearly improved the execution times. Compared with pure random selections, the deactivation method also significantly decreased the size of the roadmap, which is a useful property to simplify roadmap planning tasks.

AIM To investigate markers for high-grade dysplasia for the optimal timing of liver transplantation in patients with primary sclerosing cholangitis(PSC).METHODS Earlier data support a dysplasia-carcinoma sequence,even low-to high-grade dysplasia,in PSC-associated cholangiocarcinoma(CCA). Surveillance using endoscopic retrograde cholangiography(ERC) and brush cytology aims to detect cases of biliary dysplasia,and liver transplantation is an option in cases with suspicion of malignancy in brushing. This study investigated markers to identify patients with highgrade biliary dysplasia for optimal timing in early liver transplantation. Patients undergoing surveillance using ERC and brush cytology during 2008-2014 and who were diagnosed with biliary dysplasia in explanted liver or CCA until february 2016 were included in the study. Demographic data,cholangiography findings,laboratory values,cytological morphology and DNA ploidy were analysed.RESULTS Thirty PSC patients had biliary neoplasia in the explanted liver during the study period. Sixteen of these patients had low-grade dysplasia,10 patients had high-grade dysplasia,and 4 patients had CCA. fifteen PSC patients diagnosed with CCA were not transplanted. Patients with low-grade dysplasia were younger. Alkaline phosphatase or carcinoembryonic antigen values did not differ between groups during surveillance,but carbohydrate antigen 19-9 was higher in CCA patients. No difference in PSC duration,ERC scores,suspicious cytology,or ploidy analysis was found between groups. No difference was observed between fibrosis stage in explanted livers. Low-and high-grade dysplasia could not be differentiated before liver transplantation based on liver enzymes,tumour markers,ERC scores,brush cytology or DNA ploidy.CONCLUSION Repeated suspicion of neoplasia in brush cytology should be an indication for evaluations of liver transplantation prior to the development of CCA.

Successful application of two-dimensional transition metal dichalcogenides in optoelectronic,catalytic,or sensing devices heavily relies on the materials’quality,that is,the thickness uniformity,presence of grain boundaries,and the types and concentrations of point defects.Raman spectroscopy is a powerful and nondestructive tool to probe these factors but the interpretation of the spectra,especially the separation of different contributions,is not straightforward.Comparison to simulated spectra is beneficial,but for defective systems first-principles simulations are often computationally too expensive due to the large sizes of the systems involved.Here,we present a combined first-principles and empirical potential method for simulating Raman spectra of defective materials and apply it to monolayer MoS_(2) with random distributions of Mo and S vacancies.We study to what extent the types of vacancies can be distinguished and provide insight into the origin of different evolutions of Raman spectra upon increasing defect concentration.We apply to our simulated spectra the phonon confinement model used in previous experiments to assess defect concentrations,and show that the simplest form of the model is insufficient to fully capture peak shapes,but a good match is obtained when the type of phonon confinement and the full phonon dispersion relation are accounted for.

Sinogram Interpolation Method for Sparse-Angle Tomography

In sparse-angle X-ray tomography reconstruction, where only a small number of projection images are taken around the object, appropriate sinogram interpolation has a significant impact on image quality. A novel sinogram interpolation method is introduced for extreme sparse tomographic reconstruction where only nine measured projection images are available. The sinogram is interpolated by solving characteristics of the so-called warps, which can be considered as approximation sine waves in a limited region. The numerical evidence suggests that this approach gives superior results over standard interpolation methods when the tomographic data are extremely sparse and noisy.

In exterior use, wood is subjected to weathering that causes checking and other deterioration in the appearance and technical properties. We studied quantitatively the surface checking of radially and tangentially sawn specimens of Scots pine (Pinus sylvestris L.) and Norway spruce (Picea abies Karst.) wood in a cyclic climate chamber test. The results strongly suggested that the sawing direction determines the checking performance of both Scots pine and Norway spruce wood. The radial surface of Scots pine specimens had 62% less checks than the tangential one, and the cumulative area of checks was 74% smaller. for Norway spruce, the respective figures were: 83% less in the check number and 91% less in the check area. Different from pine, in spruce specimens the checks of radial surface were significantly smaller. Thus, spruce timber gained clearly more about radial sawing pattern. The effect of annual ring width was similar for pine and spruce: the reduction in annual growth worsened the checking. The increase in density worsened the checking of spruce but did not change the performance of pine. In pine wood, the increase of heartwood proportion reduced the fluctuation of moisture content and the formation of checks.
