Aim: Purpose of the study was to investigate the efficacy of empiric arterial embolization in order to achieve hemostasis in patients with massive hemoptysis. Materials and Methods: A retrospective review of histories and interventional studies of 56 patients (40 male, 16 female, median age 57 years;range, 16 - 83 years) referred for endovascular treatment of massive hemoptysis over a period of 17 years. Arteries supposed to supply the bleeding bronchoalveolar sections were embolized with particles in all cases. Digital subtraction angiographical (DSA) studies were analyzed with respect to the morphology of the embolized arteries. Arteries were termed pathologic when they were either hypertrophic or supplied hypervascular lung sections as well as actively bleeding branches. Empiric embolization was defined as endovascular occlusion of arteries without visible contrast-material extravasation on DSA studies. Results: Continuing hemoptysis was encountered in one (25%) of 4 patients with active contrast extravasation and in 11 (21%) of 52 empirically embolized patients: Six (19%) of 32 patients with pathologic arteries visible on aortography, 3 (18%) of 17 with pathologic arteries visible by selective arteriography and 2 (67%) of 3 with no visible pathologic arteries. From 6 patients (11%, 5 male, 1 female) who died within 30 days after embolization, 3 suffered from tuberculosis while 3 had malignant tumors. Three had ongoing hemoptysis. One patient died of multiple organ failure caused by post-interventional paraplegia and consecutive pneumonia. Conclusion: In patients with hemoptysis, empirical embolization is effective when pathologic bronchial arteries can be identified by DSA.

128头母仔猪在其出生后的头28天,以两组不同的营养模式处理,确定它对母仔猪日后的生长和繁殖的影响。营养模式是将刚出生的母仔猪调整为每窝6头或12头;哺乳期为13日或28日。13日龄断奶的仔猪在这28天期间内,或任意采食基础日粮,或任意采食基础日粮的50％。28天后,所有的猪均喂以相同的日粮。仔猪在出生的早期遭受到不利条件而抑制增长的体重,到154天出售时得到恢复。出生后的营养模式不影响其初的情期。28日龄断奶的猪,窝养6头的比窝养12头的猪,多产卵2.4个(P<0.05);13日龄断奶的猪,窝养6头的比窝养12头的少产卵2.3个。对于断奶后限制饲料15天的仔猪,并不影响其排卵率。尽管仔猪在出生后的头28天遭受窝养头数、哺乳期限和饲料限制等强制处理,使其生长率和排卵率出现变化,然而并不影响母猪日后的产仔数。以上的资料表明,对仔猪在其出生后期进行限制窝仔猪数等抑制因素的处理,达性成熟后可使其卵子死亡;当这些影响繁殖性能的抑制因素排除,则其产仔数(出生率)不变。

Objective: This study focused on the antibacterial and anti-proliferative activity of extracts from Carica papaya and Cocos nucifera roots. Methodology: The minimum inhibitory concentration and the minimum bactericidal concentration of the extracts on Escherichia coli, Pseudomonas aeruginosa, Streptococcus mutans, and Staphylococcus aureus were deduced by the microdilution method. The anti-biofilm activity was determined on all four strains and anti-quorum sensing activity by inhibition of violacein production in Chromobacterium violaceum. Anti-proliferative activity on prostate cultured cancer cells was evaluated by MTT assay. Sterols and triterpenes were also assayed in this study. Results: The methanolic extract of Carica papaya showed the best anti-biofilm effect with a percentage inhibition of 66.10 ± 1.79. The methanolic extract of Cocos nucifera had the strongest inhibition on the production of quorum sensing (61.42 ± 0.28). In addition, the methanolic extract of Cocos nucifera roots showed the best cytotoxic effect on prostate cancer LNCaP cell lines (IC50 = 26.98 ± 2.6 μg/mL) and Carica papaya on the PC-3 lines (IC50 = 127.20 ± 5.99 μg/mL). The extracts were also rich in triterpenes and sterols. Conclusion: This study provides support for the ethnomedical use of Carica papaya and Cocos nucifera roots as an antimicrobial and anticancer.

We report fives sporadic cases of hyperekplexia or startle disease characterized by a highly exaggerated startle reflex and tonic attacks. Affected neonates suffer from prolonged periods of stiffness and are at risk for sudden death from apnea. An early diagnosis is needed. Sudden loud sounds, unexpected tactile stimuli or percussion at the base of the nose can also elicit excessive jerking or tonic attack. The diagnosis of hyperekplexia is a purely clinical one. A defect of the alpha1 subunit of inhibitory glycine receptor (GLRA1) has been observed in the dominant form with amutation in the chromosome 5. Clonazepam is effective and decreases the severity of the symptoms. The disease tends to improve after infancy and the psychomotor development is normal. The major form of “hyperekplexia”should be considered whenever one is confronted with neonatal hypertonicity associated with paroxysmal tonic manifestations (without electroencephalography anomalies). Conclusion: the diagnosis of hyperekplexia should be evaluated in any neonate with tonic attacks without evident cause.

研究目的研究青春晚期超重和吸烟影响成年后病死率的联合效应。研究设计用经过肌力、社会经济地位、年龄等因素校正的Cox比例风险模型进行连锁分析。研究地点瑞典军队征兵登记，死亡原因登记和普查数据。研究对象45920名瑞典人（平均年龄18．7岁，标准差为0．5），随访38年。主要观察指标体重指数（Body mass index，BMI）（低体重（BMI〈18．5），正常体重（18．5～24．9），超重（25～29．9），肥胖（≥30））；肌力，在1969-1970年间军队征兵的强制性测试中自报的吸烟状况（不吸烟者，轻度吸烟者（1～10支／天），重度吸烟者（〉10支／天）；全因死亡。结果在170万人年中，有2897人死亡。与正常体重的人群（病死率为17／10000，95％可信区间为16～18）相比，超重（风险比为1．33，1．15～1．53；病死率为23，20～26）和肥胖（风险比为2．14，1．61-2．85；病死率为38，27～48）的人群死亡的风险增高，同样，对吸烟者与非吸烟者的分析也得出类似的结论。尽管消瘦（BMI〈17）被认为与病死率增加有关（风险比为1．33，1．07～1．64；病死率为24，19～29），并没有发现低体重的人群中死亡风险增加（风险比为0．97，0．86～1．08；病死率为18，16～19）。源于体重指数和吸烟状况联合效应增加的相对危险度在任何一组人群中并不显著。同时，除了肥胖和重度吸烟（增加的相对危险度1．5，-1．7～3．7）以外，其他任何组合的联合效应与其单独存在时相比，差异均无统计学意义。与不吸烟者（病死率为14，13～15）相比，轻度（风险比为1．54，1．41—1．70；病死率为15，14～16）和重度（风险比为2．11，1．92—2．31；病死率为26，24—27）吸烟者的死亡风险均有所增加。结论无论吸烟状况如何，在青春晚期超重和肥胖均可致成年后的病死率增加。肥胖、超重的危害分别与重度吸烟、轻度吸烟相当，但是，体重指数和吸烟情况并没有交互影响。青少年中全球性的肥胖流行和吸烟仍然是促进公共卫生健康必须关注和解决的问题。

Blockchains have been around for more than ten years,and since 2015,a plethora of systems have been launched to target more flexible use *** recently,several enterprise blockchain systems,such as Consensys Quorum and Hyperledger Fabric,have been launched to make blockchain simpler to apply in complex organizational *** this paper,we identify a specific Department of Defense use case,extrapolate requirements,and perform a thorough assessment of the different layers of the blockchain stack to identify the existing state of the art and undertake a gap analysis of the technology for this *** describe a platform that meets many of these challenges and show how we architected,designed,and implemented a solution for this use case for deployment at *** solution connects transactions from two separate blockchain systems,Consensys Quorum and Hyperledger Fabric,by using a graph-based approach that preserves privacy while enabling full transparency across the military and supplier networks.
