Objective:To assess the clinical and microbiological characteristics as well as the risk factors of early readmission(n acute pyelonephritis(APn)patients discharged from emergency department(ED).Methods:The medical records of patients discharged with a diagnosis of APn between January 1,2014 and December 31,2015 were reviewed.Epidemiological,clinical,ultrasound results,and laboratory characteristics were collected,and the risk factors of prompt readmission and non-prompt readmission were assessed.Results:A total of 423 APn patients were included.The mean age was(44.3±19.0)years.The bulk of the patients were women(79.9%),and 20%of cases had episodes previously.Urine cultures were requested in 77.5%of cases,with Escherichia coli being the most frequently isolated bacterium(80.1%).Resistance to one or more antibiotics was found in 68.1%of the bacteria.The most widely used antibiotics were beta-lactam with beta-lactamases inhibitor(46%),and aminoglycosides in 24.4%of cases.Altogether,73.5%of the patients received antibiotic treatment in the ED,and 46.8%of the patients were discharged.Furthermore,6.1%of the patients discharged from the ED had re-admission without associated risk factors.Conclusions:Given the high rate of resistance,it is important to know the profile of antibiotics in different areas.Likewise,the administration of antibiotics in the ED is useful in the prevention of early readmissions of APn(<72 h).

Melon is as an alternative model to understand fruit ripening due to the coexistence of climacteric and non-climacteric varieties within the same species,allowing the study of the processes that regulate this complex trait with genetic approaches.We phenotyped a population of recombinant inbred lines(RILs),obtained by crossing a climacteric(Védrantais,cantalupensis type)and a non-climcteric variety(Piel de Sapo T111,inodorus type),for traits related to climacteric maturation and ethylene production.Individuals in the RIL population exhibited various combinations of phenotypes that differed in the amount of ethylene produced,the early onset of ethylene production,and other phenotypes associated with ripening.We characterized a major QTL on chromosome 8,ETHQV8.1,which is sufficient to activate climacteric ripening,and other minor QTLs that may modulate the climacteric response.The ETHQV8.1 allele was validated by using two reciprocal introgression line populations generated by crossing Védrantais and Piel de Sapo and analyzing the ETHQV8.1 region in each of the genetic backgrounds.A Genome-wide association study(GWAS)using 211 accessions of the ssp.melo further identified two regions on chromosome 8 associated with the production of aromas,one of these regions overlapping with the 154.1 kb interval containing ETHQV8.1.The ETHQV8.1 region contains several candidate genes that may be related to fruit ripening.This work sheds light into the regulation mechanisms of a complex trait such as fruit ripening.

Assessment of Climate Change in nicaragua: Analysis of Precipitation and Temperature by Dynamical Downscaling over a 30-Year Horizon

The present study has generated and analyzed Climate Change projections in nicaragua for the period 2010-2040. The obtained results are to be used for evaluating and planning more resilient transport infrastructures in the next decades. This study has focused its efforts to pay attention into the effect of Climate Change on precipitation and temperature from a mean and extreme event perspective. Dynamical Downscaling approach on a 4 km resolution grid has been chosen as the most appropriate methodology for the estimation of the projected climate, being able to account for local-scale factors like complex topography or local land uses properly. We selected MPI-ESM-MR as the global climate model with the best skill scores in terms of precipitation and temperature in nicaragua. MPI-ESM-MR was coupled to a mesoscale model. We chose WRF mesoescale model as the most appropriate regional model and we optimized their physical and dynamical options in order to minimize the model uncertainty in nicaragua. For this, model output against the available in-situ measurements from the national meteorological station network and satellite data were compared. Climate change signal was estimated by comparing the different climate statistics calculated from a model run over an historical period, 1980-2009, with a model run over a projected period, 2010-2040. The obtained results from the projected climate show an increase of the mean temperature between 0.6°C and 0.8°C and an increase of the number of days per year with maximum daily temperatures higher than 35°C. Regarding precipitation, annual projected amounts do not change remarkably with respect to the historical period. However, significant changes in the distribution of the precipitation within the wet period (May-October) were observed. Moreover, an increment between 5% and 10% of the number of days without precipitation is expected. Tissue engineering(TE)is a rapidly growing interdisciplinary field,which aims to restore or improve lost tissue function.Despite that TE was introduced more than 20 years ago,innovative and more sophisticated trends and technologies point to new challenges and development.Current challenges involve the demand for multifunctional bioscaffolds which can stimulate tissue regrowth by biochemical curves,biomimetic patterns,active agents and proper cell types.For those purposes especially promising are carefully chosen primary cells or stem cells due to its high proliferative and differentiation potential.This review summarized a variety of recently reported advanced bioscaffolds which present new functions by combining polymers,nanomaterials,bioactive agents and cells depending on its desired application.In particular necessity of study biomaterial-cell interactions with in vitro cell culture models,and studies using animals with in vivo systems were discuss to permit the analysis of full material biocompatibility.Although these bioscaffolds have shown a significant therapeutic effect in nervous,cardiovascular and muscle,tissue engineering,there are still many remaining unsolved challenges for scaffolds improvement.

Essential oils as solvents and core materials for the preparation of photo-responsive polymer nanocapsules

Light-triggered release of active ingredients from polymeric nanosized capsules can be employed in a wide range of applications, such as biomedicine, active packaging, and cosmetics. However, the preparation of core-shell polymeric nanocarriers typically involves the use of toxic organic solvents. To improve the sustainability and safety of nanocapsule applications, we demonstrate that natural essential oils can be used both as solvent and active material in light-responsive nanocapsules synthesized via miniemulsion polycondensation. The documented antimicrobial, anti-inflammatory, and antioxidant activity of essential oils enables the design of multipurpose light-responsive delivery platforms. The photo-responsive behavior of the capsules, achieved by means of photochromic azobenzene segments embedded in the capsule shell, is triggered by UV light irradiation （λmax= 360 nm）. Light-induced release kinetics of the essential oils and a fluorescent probe molecule, coumarin-6, is evaluated via UV-vis spectroscopy and spectrofluorimetry, respectively, demonstrating the efficiency and reliability of the release mechanism. Biological tests prove that the capsules are non-cytotoxic and readily internalized by cells, indicating the suitability of these smart nanocarriers for biological applications.
