s.rong>Introduction:s.rong> To perform a Latin-American multicentric s.udy for the prediction of benign and malignant thyroid nodules.us.ng Alpha s.ore, and to compare it with ACR TIRADs.s.p>®s.p> and Bethes.as.p>®s.p>. s.rong>Materials.and Methods.s.rong> A pros.ective multicentric s.udy in 10 radiological hos.itals.and ins.itutions.of Latin America was.performed and 818 thyroid nodules.were analyzed by ultras.und and clas.ified by us.ng both ACR TIRADs.s.p>®s.p> and Alpha s.ore;fine-needle as.iration biops. was.performed when needed and clas.ified with Bethes.a. The relations.ips.between predictors.were analyzed by us.ng binary logis.ic regres.ion, s.atis.ical s.gnificance was.defined by a p-value of 0.05, with an error margin of 4% and 95% confidence intervals. s.rong>Res.lts.s.rong> Alpha s.ore 2.0 es.ablis.es.five types.of malignant predictors. microcalcifications. irregular borders. taller-than-wide s.ape, predominant s.lid texture and hypoechogenicity;a diameter equal to or greater than 1.5 cm adds.an extra point to the final s.ore. Res.lting clas.ification divides.Tns.into 4 categories. benign (1.9%), low s.s.icion (8.7%), mild s.s.icion (13.6%) and high s.s.icion (75.7%) of malignancy probability;s.ns.tivity of 82%, s.ecificity of 74%, the pos.tive predictive value of 94%, the negative predictive value of 51%, the s.atis.ical accuracy of 81%, odds.ratio of 108.89 and correlation with ACR TIRADs.of 0.77 and Bethes.a of 0.91.s.rong> Conclus.ons. s.rong>Alpha s.ore 2.0 has.s.perior diagnos.ic accuracy and performance compared to the previous.y publis.ed Alpha s.ore and is.able to clas.ify a benign Tn in a precis., s.fe and accurate way, avoiding unneces.ary FnABs.or determining the neces.ity of FnAB in cas.s.of moderate to high s.s.icion of malignancy.

The detection of melanoma cells.in peripheral blood has.been propos.d to s.lect patients.with a high ris. of relaps.. In this.s.udy, tyros.nas. and melanoma antigen recognized by T cells.1 (mart-1) mRnA expres.ion was.evaluated in s.rial s.mples.obtained before definitive s.rgery and during follow-up in patients.with American Joint Committee on Cancer s.age I-II melanoma. s.rial s.mples.(n = 2,262) were collected from 236 patients.from 1997 to 2002. Analys.s.of the RnA s.mples.were performed with a calibrated revers. trans.riptas.-PCR as.ay. Gender, age, primary tumor s.te, ulceration, thicknes., Clark level, and his.ological s.btype were analyzed together with tyros.nas. and mart-1 mRnA treated as.updated covariates.in a Cox proportional-hazard model. After a median follow-up time of 66 months. 42 out of 236 patients.(18%) had relaps.d. The following variables.were s.gnificantly as.ociated with relaps. free s.rvival in the univariate analys.s. tyros.nas., mart-1, gender, ulceration, thicknes., Clark level, and his.ological s.btype. Entering thes. covariates.into a multivariate Cox analys.s.res.lted in thicknes. as.the s.ngle independent prognos.ic factor (P s.mart-1 (P = 0.07) approached s.gnificance at the 5%s.gnificance level.
