Pain and Fatigue Perceptions of Patients with systemic Autoimmune Myopathies before and during the COVID-19 Pandemic Period

Objective: To assess general pain and fatigue and their association with the disease status of patients with systemic autoimmune myopathies (sAMs), in terms of the impact of the COVID-19 pandemic on these parameters. Methods: This is a cross-sectional and prospective cohort study that included 72 patients with sAMs who were matched by age and gender with 67 healthy individuals. The patients engaged in an interview via a validity survey to measure disease status, and assess a visual analog scale (VAs) for pain and fatigue in two periods: before (phase I) and during (phase II) of the Brazilian COVID-19 pandemic. For cross-sectional analysis, patients’ data from phase I were compared to healthy individuals’ information, whereas for prospective analysis, data of the patients who were assessed in phase II were compared to the same patients’ data from phase I. Results: The patients had significantly more pain perception, comparable fatigue perception and fatigue severity, when compared to the healthy individuals. During the COVID-19 pandemic period, the disease activity, pain and fatigue perceptions, and fatigue severity remained unchanged. Despite this, the pain and fatigue perceptions correlated significantly with patients’ VAs, the patient health outcome, and fatigue severity. In addition, the pain perception correlated to creatine phosphokinase, whereas fatigue perceptions correlated to physicians' VAs. Conclusions: The study showed that patients with sAMs have significantly increased pain perceptions compared to healthy individuals. During the COVID-19 pandemic period, the pain and fatigue perceptions remained unchanged in patients with sAMs but they correlated to several disease status parameters.

Increased number of symptoms during the acute phase of sARs-CoV-2 infection in athletes is associated with prolonged time to return to full sports performance—AWAREⅧ

Purpose The aim of the study was to identify factors associated with prolonged time to return to full performance(RTFP)in athletes with recent severe acute respiratory syndrome coronavirus 2(sARs-CoV-2)*** Prospective cohort study with cross sectional analysis.A total of 84 athletes with confirmed sARs-CoV-2 infection assessed at a coronavirus disease 2019 recovery clinic gave a history of age,sex,type/level of sport,co-morbidities,pre-infection training hours,and 26 acute sARs-CoV-2 symptoms from 3 categories(“nose and throat”,“chest and neck”,and“whole body”/systemic).Data on days to RTFP were obtained by structured *** associated with RTFP were demographics,sport participation,history of co-morbidities,pre-infection training history,and acute symptoms(type,number).Outcomes were:(a)days to RTFP(median,interquartile range(IQR))in asymptomatic(n=7)and symptomatic athletes(n=77),and(b)hazard ratios(HRs;95%confidence interval)for symptomatic athletes with *** a factor(univariate,multiple models).HRs predictive of higher percentage chance of prolonged *** was ps to RTFP were 30 days(IQR:23–40)for asymptomatic and 64 days(IQR:42–91)for symptomatic participants(p>0.05).Factors associated with prolonged RTFP(univariate models)were:females(HR=0.57;p=0.014),endurance athletes(HR=0.41;pespiratory disease history(HR=0.54;p=0.026).In symptomatic athletes,prolonged RTFP(multiple models)was significantly associated with increased“chest and neck”(HR=0.85;p=0.017)and“nose and throat”(HR=0.84;p=0.013)symptoms,but the association was more profound between prolonged RFTP and increased total number of“all symptoms”(HR=0.91;p=0.001)and“whole body”/systemic(HR=0.82;p=0.007)*** A larger number of total symptoms and specifically“whole body”/systemic symptoms during the acute phase of sARs-CoV-2 infection in athletes is associated with prolonged RTFP.

BACKGROUND systemic autoimmune rheumatic diseases(sARDs)are a group of diseases with multiorgan involvement and a high prevalence of chronic pain and *** with sARDs and post-coronavirus disease 2019(COVID-19)syndrome experience aggravation of *** this context,it is essential to establish strategies to reduce chronic pain and fatigue and improve quality of *** To assess the efficacy of transcranial direct current stimulation(tDCs)for the treatment of fatigue and pain-associated post-COVID-19 syndrome in patients with *** This study included nine patients with different types of *** patients had reverse transcription-polymerase chain reaction(RT-PCR)test confirmed COVID-19 as well as significant,persistent fatigue and pain that began to worsen after *** tDCs was administered in five daily sessions(2mA,20 min).Concomitantly,patients were involved in aerobic exercise *** participants were evaluated using specific questionnaires and strength assessment by handgrip and physical function by timed-up-and-go test and sit-to-stand test at baseline(within one week before tDCs protocol),and one week after tDCs *** all procedures,the patients’treatments remained *** The sample comprised eight women and one man with a mean age of 48.7±9.6 *** the tDCs protocol,pain and fatigue significantly improved on the visual analog scale(Psical function also improved 9.5±2.7 vs 6.8±0.8(P=0.001)for timed-up-go-test and 10.3±3.7 vs 15.1±4.0(P=0.037)for sit-to-stand *** of the patients experienced any adverse *** The present study showed that tDCs in combination with aerobic exercise was effective in improving physical function,and reducing fatigue/pain in sARDs patients with post-COVID-19 syndrome.

Oral diseases related to dental biofilms continue to afflict the majority of the world＇s population. Among them, dental caries continues to be the single most prevalent and costly oral infectious disease （Marsh, 2003; Dye et al., 2007）. Dental caries results from the interaction of specific bacteria with constituents of the diet within a dental biofilm known as plaque （Bowen, 2002）. sucrose is considered to be the ＂arch criminal＂ from the dietary aspect because it serves as a substrate for synthesis of extracellular （EPs） and intracellular （IPs） polysaccharides in dental biofilm and is also fermentable （Bowen, 2002）.

Influences of trans-trans farnesol,a membrane-targeting sesquiterpenoid,on streptococcus mutans physiology and survival within mixed-species oral biofilms

Trans-trans farnesol （tt-farnesol） is a bioactive sesquiterpene alcohol commonly found in propolis （a beehive product） and citrus fruits, which disrupts the ability of streptococcus mutans （s. mutans） to form virulent biofilms. In this study, we investigated whether tt-farnesol affects cell-membrane function, acid production and/or acid tolerance by planktonic cells and biofilms of s. mutans UA159. Furthermore, the influence of the agent on s. mutans gene expression and ability to form biofilms in the presence of other oral bacteria （streptococcus oralis （s. oralis） 35037 and Actinomyces naeslundii （.4. naeslundil） 12104） was also examined. In general, tt-farnesol （1 mmol-L-1） significantly increased the membrane proton permeability and reduced glycolytie activity of s. mutans in the planktonic state and in biofilms （P〈0.05）. Moreover, topical applications of 1 mmol-L＂l tt-farnesol twice daily （1 min exposure/treatment） reduced biomass accumulation and prevented ecological shifts towards s. mutans dominance within mixed-species biofilms after introduction of 1% sucrose. s. oralis （a non-cariogenie organism） became the major species after treatments with tt-farnesol, whereas vehicle-treated biofilms contained mostly s. mutans （〉90% of total bacterial population）. However, the agent did not affect significantly the expression of s. mutans genes involved in acidogenicity, acid tolerance or polysaccharide synthesis in the treated biofilms. Our data indicate that tt-farnesoi may affect the competi- tiveness of s. mutans in a mixed-species environment by primarily disrupting the membrane function and physiology of this bacterium. This naturally occurring terpenoid could be a potentially useful adjunctive agent to the current anti-biofilm/anti-caries chemotherapeutic strategies.

A new friedelane-type triterpene named 3β-hydroxyfriedelane-7,12,22-trione,as well as nine known compounds were isolated from the whole stems of Drypetes laciniata Hutch.（Euphorbiaceae）.Their structures were established on the basis of spectroscopic *** new triterpene derivative and a known saponin were tested for antimicrobial and antifungal activities and they appeared to be moderate active.

Objective： To evaluate the antimicrobial activity of the methanol extract from the stem bark of Drypetes tessmanniana, fractions （DTB1-5） as well as compounds [friedelin （2）, 3,7-dioxofriedelane （3）, 3,15-dioxofriedelane （4）, 313 -O-（E）-3,5-dihydroxycinnamoyl-11 -oxo-olean-12-ene （6）, and 313,6 α -dihydroxylup- 20（29）-ene （7）. Methods： Agar disc diffusion was used to determine the sensitivity of the above samples, whilst the microdilution method was used for the determination of the minimal inhibitory concentration （MIC） and the minimal microbicidal concentrations （MMC）. Results： The diffusion test showed that the crude extract was able to prevent the growth of all tested organisms. All other samples showed selective activity. The inhibitory effect of the fraction DTB2 was noted on 63.7%, that of DTB1 and DBT3 on 54.6%, whilst DTB4 and DTB5 were active on 9.1% of the 11 tested organisms. The tested compounds prevented the growth of 81.8% of the tested microbial species for compounds 3 and 4, 36.7% for compound 6, and 18.2% for compound 7. The results of the MIC determinations indicated perceptible values for DTB and compound 4 on 81.8% of the tested organisms. For other samples, MICs were detected on 0-63.7%. The lowest MIC value （78.12 μg/mL） for the crude extract and fractions （DTB2） was observed on M. audouinii. The corresponding value for isolated compounds （156.25 μg/ mL） was noted with compounds 3 on s. faecalis and 4 on M. audouinii. The results of the MMC determination suggested that the microbicidal effect of most of the tested samples on the studied microorganisms could be expected. Conclusion： The methanol extract from the stem bark of Drypetes. tessmanniana （Euphorbiaceae） as well as some of the isolated compounds might be potential sources of new antimicrobial drues.
