Recurrence rates after endoscopic resection of large colorectal polyps:A systematic review and meta-analysis

BACKGROUNd Complete polyp resection is the main goal of endoscopic removal of large colonic *** techniques have evolved in recent years and endoscopic submucosal dissection(ESd),endoscopic mucosal resection(EMR)with margin ablation,cold snare polypectomy(CSP),cold EMR,and underwater EMR have been ***,efficacy of these techniques with regard to local recurrence rates(LRRs)vs traditional hot snare polypectomy and standard EMR remains *** To analyze LRR of large colonic polyps in a systematic review and *** MEdLINE,EMBASE,EBM Reviews,and CINAHL were searched for prospective studies reporting LRR or incomplete resection rate(IRR)after colonic polypectomy of polyps≥10 mm,published between January 2011 and July *** outcome was LRR for polyps≥10 *** Six thousand nine hundred and twenty-eight publications were identified,of which 34 prospective studies were *** for polyps≥10 mm at up to 12 mo’follow-up was 11.0%(95%CI,7.1%-14.8%;15 studies;4904 polyps).ESd(1.7%;95%CI,0%-3.4%;3 studies,221 polyps)and endoscopic mucosal resection with margin ablation(3.3%;95%CI,2.2%-4.5%;2 studies,947 polyps)significantly reduced LRR vs standard EMR without(15.2%;95%CI,12.5%-18.0%;4 studies,650 polyps)or with unsystematic margin ablation(16.5%;95%CI,15.2%-17.8%;6 studies,3031 polyps).CONCLUSION LRR is significantly lower after ESd or EMR with routine margin ablation;thus,these techniques should be considered standard for endoscopic removal of large colorectal *** techniques,such as CSP,cold EMR,and underwater EMR require further evaluation in prospective studies before their routine implementation in clinical practice can be recommended.

A revised Mississippian lithostratigraphy of County Galway(western Ireland) with an analysis of carbonate lithofacies, biostratigraphy, depositional environments and palaeogeographic reconstructions utilising new borehole data

An integrated study of borehole data and outcrop of Mississippian （late Tournaisian to late Visean） rocks in Co. （County） Galway, western Ireland has enabled a more detailed geological map and lithostratigraphy to be constructed for the region. Several carbonate formations have been distinguished by microfacies analysis and their precise ages established by micropalaeontological investigations using foraminifers and calcareous algae. In addition, palaeogeographic maps have been constructed for the late Tournaisian, and early to late Visoan intervals in the region. The oldest marine Mississippian （late Tournaisian） deposits are recorded in the south of the study region from the Loughrea/Tynagh area and further south in the Gort Borehole; they belong to the Limerick Province. They comprise the Lower Limestone Shale Group succeeded by the Ballysteen Group, Waulsortian Limestone and Kilbryan Limestone Formations. These rocks were deposited in increasing water depth associated with a transgression that moved northwards across Co. Galway. In the northwest and north of the region, marginal marine and non-marine Tournaisian rocks are developed, with a shoreline located NW of Galway City （Galway High）. The central region of Co. Galway has a standard Visdan marine succession that can be directly correlated with the Carrick-on- Shannon succession in counties Leitrim and Roscommon to the northeast and east as far as the River Shannon. It is dominated by shallow-water limestones （Oakport, Ballymore and Croghan Limestone Formations） that formed the Galway-Roscommon Shelf. This facies is lat- erally equivalent to the Tubber Formation to the south which developed on the Clare-Galway Shelf. In the southeast, basinal facies of the Lucan Formation accumulated in the Athenry Basin throughout much of the Visean. This basin formed during a phase of extensional tectonics in the early Visean and was probably connected to the Tynagh Basin to the east. In the late Visean, shallow-water limes

Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tNGCs). However, the use of tNGCs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tNGC would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tNGCs and compared the various features of these improved silk^(+) tNGCs with the tubes without holes(silk^(–) tNGCs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tNGCs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tNGCs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tNGCs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tNGCs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. The structural changes around a crack tip in a high density polyethylene were investigated by means of scanning synchrotron microfocus small-angle X-ray scattering technique. The scattering data confirm the process of craze structure development near a crack tip based on the evolution of voids. In addition, it was found that the main stress in the plastic zone near a crack tip exhibited a gradient distribution with respect to its strength and direction. The whole damaged area showed a strain distribution indicating a flow behavior toward the crack tip.

The influence of terrain age and altitude on the arthropod communities found on recently deglaciated terrain

Climate warming has been more pronounced in the Arctic than elsewhere, resulting in a recent rapid glacial retreat. Over 85% of the Almajallojekna glacier has disappeared over the last 115 years and it is one of the fastest retreating glaciers in Sweden. In 2011 and 2012, at 18 sites in the vicinity of the remaining glacier network, we sampled arthropods and related the species richness, abundance and proportion of herbivores to altitude （ranging from 824 to 1,524 m.a.s.1.） and the age of the site （ranging from 0 to 〉 115 years）. Temperature was measured at six sites and the average annual air temperature ranged from -3.7℃ to 1.1℃. In total, we recorded 3,705 arthropods from 117 species in nine different taxonomic groups. The most abundant and species-rich group was Coleoptera （1,381 individuals, 41 species） followed by Araneae （1,050 individuals, 15 species） and Macrolepidoptera （732 individuals, 17 species）. Only at lower altitudes did the abundance and species richness of arthropods increase with increasing age of the site. The results were consistent among the studied taxonomic groups and when controlling for sam- piing intensity using two different approaches. The proportion of herbivores decreased with increasing age of the site and with increasing altitude. Clearly, altitude appears more important than the age of the site and, at higher altitudes, abundance, species richness and herbivory are low [Current Zoology 60 （2）： 203-220, 2014].

Single-mode and tunable VCSELs in the near-to mid-infrared Invited Paper

Single-mode, long-wavelength vertical-cavity surface-emitting lasers （VCSELs） in the near- to mid-infrared covering the wavelength range from 1.3 to 2.3 μm are presented. This wide spectral emission range opens applications in gas sensing and optical interconnects. All these lasers are monolithically grown in the InGaAlAs-InP material system utilizing a buried tunnel junction （BTJ） as current aperture. Fabricated with a novel high-speed design with reduced parasitics, bandwidths in excess of 10 GHz at 1.3 and 1.55 μm have been achieved. Therefore, the coarse wavelength division multiplexing （CWdM） wavelength range of 1.3 to 1.6 μm at 10 Gb/s can be accomplished with one technology. Error-free data-transmission at 10 Gb/s over a fiber link of 20 km is demonstrated. One-dimensional arrays have been fabricated with emission wavelengths addressable by current tuning. Micro-electro-mechanical system （MEMS） tunable devices provide an extended tuning range in excess of 50 nm with high spectral purity. All these devices feature continuous-wave （CW） operation with typical single-mode output powers exceeding 1 mW. The operation voltage is around 1 - 1.5 V and power consumption is as low as 10 - 20 mW. Furthermore, we have also developed VCSELs based on GaSb, targeting functionality of tunable diode laser spectroscopy （TdLS） applying a 1.84-μm VCSEL. at the wavelength range from 2.3 to 3.0 μm. In a recent study published in Nature,York and coworkers identified the immunometabolic regulation of mono-unsaturated fatty acid(MUFA)pools and the subsequent increase in sphingolipid biosynthesis in TLR2-activated macrophages as an important link between impaired IL-10 signaling and the development of intestinal inflammation.1 Thus,targeted interference with the newly identified IL-10/MUFA/sphingolipid axis appears as a promising strategy to restore immunometabolic homeostasis in the intestine of patients with inflammatory bowel diseases(IBd).
