Endoclips vs large or small-volume epinephrine in peptic ulcer recurrent bleeding

AiM:To compare the recurrent bleeding after endoscopic injection of different epinephrine volumes with hemoclips in patients with bleeding peptic ***:Between January 2005 and December 2009,150 patients with gastric or duodenal bleeding ulcer with major stigmata of hemorrhage and nonbleeding visible vessel in an ulcer bed(Forrest Ⅱa) were included in the *** were randomized to receive a small-volume epinephrine group(15 to 25 mL injection group;Group 1,n = 50),a large-volume epinephrine group(30 to 40 mL injection group;Group 2,n = 50) and a hemoclip group(Group 3,n = 50).The rate of recurrent bleeding,as the primary outcome,was compared between the groups of patients included in the *** outcomes compared between the groups were primary hemostasis rate,permanent hemostasis,need for emergency surgery,30 d mortality,bleeding-related deaths,length of hospital stay and transfusion ***:initial hemostasis was obtained in all *** rate of early recurrent bleeding was 30%(15/50) in the small-volume epinephrine group(Group 1) and 16%(8/50) in the large-volume epinephrine group(Group 2)(P = 0.09).The rate of recurrent bleeding was 4%(2/50) in the hemoclip group(Group 3);the difference was statistically significant with regard to patients treated with either small-volume or large-volume epinephrine solution(P = 0.0005 and P = 0.045,respectively).Duration of hospital stay was significantly shorter among patients treated with hemoclips than among patients treated with epinephrine whereas there were no differences in transfusion requirement or even 30 d mortality between the ***:Endoclip is superior to both small and large volume injection of epinephrine in the prevention of recurrent bleeding in patients with peptic ulcer.

Seminomas are most commonly diagnosed in clinical stage i(CSi).After orchiectomy,approximately 15%of patients in this stage have subclinical *** radiotherapy(ART)delivered to the retroperitoneum and ipsilateral pelvic lymph nodes has been the mainstay of treatment for many *** highly efﬁcient,with long-term cancer-speciﬁc survival(CSS)rates approaching almost 100%,ART is associated with considerable long-term consequences,particularly cardiovascular toxicity and increased risk of secondary malignancies(SMN).Therefore,active surveillance(AS)and adjuvant chemotherapy(ACT)were developed as alternative treatment *** AS prevents patient overtreatment,it is associated with strict follow-up regimens and increased radiation exposure due to repeated *** to equivalent CSS rates to ART,and lower toxicity,one course of adjuvant carboplatin presents the cornerstone of chemotherapy for CSi *** is almost 100%for patients with CSi seminoma,regardless of the chosen treatment ***,a personalized approach in treatment selection is ***,routine radiotherapy for CSi seminoma patients is no longer ***,it should be reserved for patients who are unﬁt or unwilling for AS or ***ﬁcation of prognostic factors for disease relapse allowed for the development of risk-adapted treatment strategy and stratiﬁcation of patients in low-risk and high-risk *** risk-adapted policy needs further validation,surveillance is currently recommended in low-risk patients,while ACT is reserved for patients with a higher risk of relapse.

Since the initial coronavirus disease 2019(COViD-19)outbreak in China in December 2019,the infection has now become the biggest medical issue of modern *** major contributors that amplified the impact of the disease and subsequently increased the burden on health care systems were high mortality among patients with multiple co-morbidities and overcapacity of intensive care *** the gastroenterology-related community,particular concern was raised with respect to patients with inflammatory bowel disease(iBD),as those patients are prone to opportunistic infections mainly owing to their immunosuppressive-based ***,we sought to summarize current knowledge regarding COViD-19 infection in patients with ***,it seems that iBD is not a comorbidity that poses an increased risk for COViD-19 acquisition,except in patients treated with ***,outcomes of the infected patients are largely dependent on therapeutic modality by which they are treated,as some worsen the clinical course of COViD-19 infection,whereas others seem to dampen the detrimental effects of ***,we discussed the present and the future impact of COViD-19 pandemic and concomitantly increased health care burden on iBD-management.

investigating short- and long-term transfer effects of a Taiji beginner course in participants' daily life

in recent years research investigating various health benefits of Taiji practice has markedly increased. Despite this growing scientific interest, essential questions such as to what extent a Taiji course may exert noticeable effects in participants＇ everyday life, what these effects are, and how and where potential transfer effects occur, have hardly been considered. The aim of our study was to explore transfer effects from a Taiji course into participants＇ daily lives. METHODS： We conducted a longitudinal observational study in 45 healthy participants at the end of their three-month Taiji beginner course （tpl） and at two months （tp2） as well as one year after course completion （tp3）. Participants were asked to report their Taiji practice behavior at all time points, as well as to rate and describe perceived transfer effects of Taiji course contents on their daily life at tpl and tp3. RESULTS： Transfer effects were reported by 91.1% of all respondents after course completion （tpl） and persisted in 73.3% at the one-year follow-up assessment （tp3）, counting ＂increase of self-efficacy＂, ＂improvement of stress management＂, and ＂increase of body awareness＂ as the most frequently mentioned effects. Transfer effects predominantly occurred in participants＇ work and social environments, as well as during everyday activities in public areas. While self- reliant Taiji practice frequency significantly decreased from 82.2% at tpl to 55.6% at tp3 （P 〈 0.001）, the magnitude of self-reported transfer effects did not （P = 0.35）. As explorative analyses revealed, regular Taiji course attendance was highly correlated with stronger transfer effects at tpl （r = 0.51; P 〈 0.001） and tp3 （r = 0.35; P = 0.020）. Participants reporting high self-reliant Taiji practice frequency at tp2 were likely to maintain a regular practice routine at tp3 （r = 0.42; P 〈 0.004）, whereas self-reliant practice frequency and transfer effects at tpl were positively correlated with self-reliant practice freque

Applicability of alginate-based composite microspheres loaded with aqueous extract of Stevia rebaudiana Bertoni leaves in food and pharmaceutical products

To preserve its favorable health and nutritional qualities under processing conditions and oral ingestion,Stevia rebaudiana Bertoni leaf aqueous extract was encapsulated in composite microspheres based on calcium *** addition of other biopolymers to alginate(ALG)(xanthan gum(XG),casein(CA))influenced molecular interactions(mainly hydrogen bonding and electrostatic interactions)between microsphere constituents causing the change in microsphere structure and thus physicochemical properties(encapsulation effectiveness and capacity,surface morphology,swelling,stability)as well as the release of total polyphenols(TPC)in distilled water,NaCl solutions and in simulated gastrointestinal *** extent of crosslinking of calcium alginate was altered by blending with XG,CA,or XG+CA,but this did not affect the control mechanism of TPC release detected as *** revealed the unsuitability of calcium alginate microspheres for implementation in food products with high water content,while composite microspheres showed desired reduction in TPC *** best performance(the slowest release and the amount released)for implementation in functional food products containing NaCl exhibited composite microspheres with ALG+XG+*** desired prevention of microsphere degradation in simulated gastro fluids was achieved with ALG+XG+CA microspheres,while in simulated intestinal fluids all microspheres were decomposed and completely released TPC.

Stable gastric pentadecapeptide BPC 157 in the treatment of colitis and ischemia and reperfusion in rats: New insights

AiM To provide new insights in treatment of colitis and ischemia and reperfusion in rats using stable gastric pentadecapeptide BPC 157. METHODS Medication [BPC 157,L-NAME,L-arginine(alone/combined),saline] was bath at the blood deprived colon segment. During reperfusion,medication was BPC 157 or saline. We recorded(USB microscope camera) vessel presentation through next 15 min of ischemic colitis(iCrats) or reperfusion(removed ligations)(iC + RL-rats);oxidative stress as MDA(increased(iC-and iC + RLrats)) and NO levels(decreased(iC-rats);increased(iC + RL-rats)) in colon tissue. iC + OB-rats [iC-rats had additional colon obstruction(OB)] for 3 d(iC + OBrats),then received BPC 157 bath. RESULTS Commonly,in colon segment(25 mm,2 ligations on left colic artery and vein,3 arcade vessels within ligated segment),in iC-,iC + RL-,iC + OB-rats,BPC 157(10 μg/kg) bath(1 m L/rat) increased vessel presentation,inside/outside arcade interconnections quickly reappeared,mucosal folds were preserved and the pale areas were small and markedly reduced. BPC 157 counteracted worsening effects induced by L-NAME(5 mg) and L-arginine(100 mg). MDA-and NO-levels were normal in BPC 157 treated iC-rats and iC + RLrats. in addition,on day 10,BPC 157-treated iC + OBrats presented almost completely spared mucosa with very small pale areas and no gross mucosal defects;the treated colon segment was of normal diameter,and only small adhesions were *** BPC 157 is a fundamental treatment that quickly restores blood supply to the ischemically injured area and rapidly activates collaterals. This effect involves the NO system.

Relativistic Supernova Blast Waves Exhibit Properties of Gravitational Lenses and the Hubble Constant

Simulations based on Supernova (SN) observations predict several galactic SN explosions (SNe) can occur every century. Unlike SNes within the interstellar Medium (iSM) where ambient gas generally absorbs blast waves within a million years, SNes occurring in a rarified environment outside of the iSM generate blast waves which remain in a relativistic free expansion phase for more extended periods. The SN blast wave forms an expanding spherical shell and when multiple blast waves intersect, the overlapping region naturally takes the form of a ring, an arc, or an Einstein Cross structure. The analysis shows the relativistic plasma establishes a medium with permeability which drives the index of refraction greater than 1. As a result, when a shock discontinuity forms in the overlapping region, light is reflected from the host galaxy which exposes the intersecting blast wave regions. The expanding shells are shown to induce an achromatic redshift to the reflected light consistent with those measured for gravitational lenses. Further, it is shown that a Hubble equation for a blast wave around the Milky Way Galaxy can be parameterized to approximate measured redshifts over a wide range of distances.
