Accuracy of virtual chromoendoscopy in differentiating gastric antral vascular ectasia from portal hypertensive gastropathy:A proof of concept study

BACKGROUND Accurate detection of gastric antral vascular ectasia(GAVE)is critical for proper management of cirrhosis-related gastrointestinal ***,endoscopic diagnosis of GAVE can be challenging when GAVE overlaps with severe portal hypertensive gastropathy(pHG).AIM To determine the added diagnostic value of virtual chromoendoscopy to high definition white light for real-time endoscopic diagnosis of GAVE and *** We developed an I-scan virtual chromoendoscopy criteria for diagnosis of GAVE and *** tested our criteria in a cross-sectional cohort of cirrhotic adults with GAVE and pHG when high-definition white light endoscopy(HDWLE)diagnosis was in *** then compared the accuracy of I-scan vs HDWLE alone to *** Twenty-three patients were included in this study(65.2%Caucasians and 60.9%males).Chronic hepatitis C was the predominant cause of cirrhosis(43.5%)and seven adults(30.4%)had confirmed GAVE on histology.I-scan had higher sensitivity(100%vs 85.7%)and specificity(75%vs 62.5%)in diagnosing GAVE compared to *** translates into a higher,albeit not statistically significant,accuracy of I-scan in detecting GAVE compared to HDWLE alone(82%vs 70%).I-scan was less likely to lead to an accurate diagnosis of GAVE in patients on dialysis(ppatients with elevated creatinine(ppilot work supports that virtual chromoendoscopy may obviate the need for biopsies when the presence of GAVE is in *** studies are needed to assess the impact of virtual chromoendoscopy on success of endoscopic therapy for GAVE.

Negative consequences of positive feedbacks in US wild-fire management

Over the last two decades wildfire activity, damage, and management cost within the US have increased substantially. These increases have been associated with a number of factors including climate change and fuel accumulation due to a century of active fire suppression. The increased fire activity has occurred during a time of significant ex-urban development of the Wildland Urban Interface （WUI） along with increased demand on water resources originating on forested landscapes. These increased demands have put substantial pressure on federal agencies charged with wildfire management to continue and expand the century old policy of aggressive wildfire suppression. However, aggressive wildfire suppression is one of the major factors that drive the increased extent, intensity, and damage associated with the small number of large wildfires that are unable to be suppressed. In this paper we discuss the positive feedback loops that lead to demands for increasing suppression response while simultaneously increasing wildfire risk in the future. Despite a wealth of scientific research that demonstrates the limitations of the current management paradigm pressure to maintain the existing system are well entrenched and driven by the existing social systems that have evolved under our current management practice. Interestingly, US federal wildland fire policy provides considerable discretion for managers to pursue a range of management objectives however, societal expectations and existing management incentive structures result in policy implementation that is straining the resilience of fire adapted ecosystems and the communities that reside in and adjacent to them.

Investigation of the excluded stomach after Roux-en-Y gastric bypass: The role of percutaneous endoscopy

Accessing the bypassed portion of the stomach via conventional endoscopy is difficult following Roux-en-Y gastric bypass surgery. However, endoscopic examination of the stomach and small bowel is possible through percutaneous access into the bypassed stomach (BS) with a combined radiologic and endoscopic technique. We present a case of obscure overt gastrointestinal (GI) bleeding where the source of bleeding was thought to be from the BS. After conventional endoscopic methods failed to examine the BS, percutaneous endoscopy (pE) was used as an alternative to surgical exploration.

Application of DNA barcodes in Hedyotis L.(Spermacoceae,Rubiaceae)

The potential application of DNA barcodes of plastid(matK,trnH-psbA,petD,and rbcL) and nuclear(internal transcribed spacer(ITS) of rDNA) DNA regions was investigated for 25 Hedyotis *** ITS showed the best species discrimination by resolving 23 of the species as exclusive lineages with no shared alleles between any of the 24 distinct species(*** and *** are not supported as distinct species based on our molecular and morphological data).Conversely,rbcL performed the worst and only resolved 10 of the species as exclusive lineages,and 10 species with shared *** ITS has the advantage of high pCR amplification success and it provides good intra-and interspecific variation distribution *** most powerful plastid markers were petD and trnH-psbA,but we could amplify and sequence trnH-psbA for only 83% of the accessions *** of ITS and petD performed extremely well,with all 24 of the distinct species resolved as exclusive lineages and no shared alleles between any of the distinct *** therefore recommend ITS,or a combination of ITS and petD,as the standard DNA barcode in Hedyotis,but acknowledge that there are no shared alleles between distinct species for matK and rbcL combined.

A cost-effective method to enhance adenoviral transduction of primary murine osteoblasts and bone marrow stromal cells

We report here a method for the use of poly-L-lysine （pLL） to markedly improve the adenoviral transduction efficiency of primary murine osteoblasts and bone marrow stromal cells （BMSCs） in culture and in situ, which are typically difficult to transduce. We show by fluorescence microscopy and flow cytometry that the addition of pLL to the viral-containing medium significantly increases the number of green fluorescence protein （GFp）-positive osteoblasts and BMSCs transduced with an enhanced GFp-expressing adenovirus. We also demonstrate that pLL can greatly enhance the adenoviral transduction of osteoblasts and osteocytes in situ in ex vivo tibia and calvaria, as well as in long bone fragments. In addition, we validate that pLL can improve routine adenoviral transduction studies by permitting the use of low multiplicities of infection to obtain the desired biologic effect. Ultimately, the use of pLL to facilitate adenoviral gene transfer in osteogenic cells can provide a cost-effective means of performing efficient gene transfer studies in the context of bone research.

Dim artificial light at night reduces the cellular immune response of the black field cricket, Teleogryllus commodus

A functioning immune system is crucial for protection against disease and illness,yet increasing evidence suggests that species living in urban areas could be suffering from immune suppression,due to the presence of artificial light at night(ALAN).This study examined the effects of ecologically relevant levels of ALAN on three key measures of immune function(haemocyte concentration,lytic activity,and phenoloxidase activity)using a model invertebrate species,the Australian black field cricket,Teleogryllus *** reared crickets under an ecologically relevant daily light‐cycle consisting of 12 hr bright daylight(2600 lx)followed by either 12 h darkness(0 lx)or dim environmentally relevant ALAN(1,10,100 lx),and then assessed immune function at multiple time points throughout adult life using haemolymph *** found that the presence of ALAN had a clear negative effect on haemocytes,while the effects on lytic activity and phenoloxidase activity were more complex or largely unaffected by ***,the effects of lifelong exposure to ALAN of 1 lx were comparable to those of 10 and 100 *** data suggest that the effects of ALAN could be large and widespread,and such reductions in the core immune response of individuals will likely have greater consequences for fitness and survival under more malign conditions,such as those of the natural environment.
