Associations between Bodyweight and Clinical Outcome in Patients Post-Fontan Procedure: A Systematic Review

Background: Patients born with a single ventricle circulation commonly experience growth failure in early life,which is associated with adverse outcomes in infancy. However, associations between bodyweight or weight trajectoryand clinical outcome post-Fontan procedure are yet to be determined. Methods: On the 1st of July 2021, asystematic review was performed in MEDLINE, EMBASE, the Cochrane Library, and Scopus of studies of patientswith clinical outcome data post-Fontan procedure and association with bodyweight. Quality of studies was evaluatedby Newcastle–Ottawa scale for cohort studies and Joanna Briggs Institute tool for cross-sectional ***: Of 527 studies that underwent title and abstract screening, 15 were selected for final review. An increasedrisk of adverse post-Fontan outcomes was found for low weight patients, consistent with findings in *** there is some evidence to suggest increased mortality in overweight adult patients, studies are conflictingas to whether overweight status is associated with increased heart failure. Increased BMI is associated with diminishedexercise capacity and deceased physiological functioning. Negative weight trajectory is associated withadverse outcomes in the peri-Fontan period, whereas a positive weight trajectory is associated with increased Fontanfailure in adulthood. Abnormal BMI (high or low) is associated with increased heart failure and poorer performancein quality-of-life scores. Conclusions: Bodyweight is a modifiable risk factor for poor clinical outcomein patients with a single ventricle circulation. Recognizing associations between bodyweight and Fontan pathophysiologymay help to define patient-centered exercise and diet interventions that minimize patient morbidityand mortality.

Voltage gated calcium channel(VGCC) antibodies are generally associated with Lambert-Eaton myasthenic syndrome. However the presence of this antibody has been associated with paraneoplastic as well as nonparaneoplastic cerebellar degeneration. Most patients with VGCC-antibody-positivity have small cell lung cancer(SCLC). Lambert-Eaton myasthenic syndrome(LEMS)is an autoimmune disease of the presynaptic part of the neuromuscular junction. Its classical clinical triadis proximal muscle weakness, areflexia and autonomic dysfunction. Fifty to sixty percent of LEMS patients have a neoplasia, usually SCLC. The co-occurrence of SCLC and LEMS causes more severe and progressive disease and shorter survival than non-paraneoplastic LEMS. Treatment includes 3,4 diaminopyridine for symptomatic purposes and immunotherapy with prednisolone, azathioprine or intravenous immunoglobulin in patients unresponsive to 3,4 diaminopyridine. Paraneoplastic cerebellar degeneration(PCD) is a syndrome characterized with severe, subacute pancerebellar dysfunction. Serum is positive for VGCC antibody in 41%-44% of patients, usually with the co-occurrence of SCLC. Clinical and electrophysiological features of LEMS are also present in 20%-40% of these patients. Unfortunately, PCD symptoms do not improve with immunotherapy. The role of VGCC antibody in the immunopathogenesis of LEMS is well known whereas its role in PCD is still unclear. All patients presenting with LEMS or PCD must be investigated for SCLC.

In order to investigate the effects of Barvar-2 phosphate biofertilizer and nitroxin biofertilizers on agronomical characteristics of sesame, an experiment was carried out as factorial based on randomized complete block design with three replications and two factors in Khoy city, northwest of Iran, in 2014 growing seasons. The factors were as follows: phosphorus fertilizer in three levels (control, barvar-2 phosphate bio-fertilizer with 100 kg triple superphosphate per hectare and 200 kg triple superphosphate per hectare) and nitrogen fertilizer in four levels (control, nitroxin bio-fertilizer with urea at rate of 100 kg·ha−1, nitroxin biofertilizer with urea at rate of 150 kg·ha−1 and urea at rate of 200 kg·ha−1). The results showed that a significant effect of nitrogen and phosphate biofertilizer, on plant height, branches per plant, pods per plant, seed in pod, oil percent, seed yield and harvest index. Maximum seed yield of 1274.37 and 1232.5 kg·ha−1 was obtained from treatments of 200 kg·ha−1 triple superphosphate and barvar-2 phosphate biofertilizer besides 100 kg·ha−1 triple superphosphate. For nitrogen biofertilizer, maximum seed yield of 1264.83 and 1246.12 kg·ha−1 was obtained from treatments of 200 kg·ha−1?urea and nitroxin biofertilizer besides urea 150 kg·ha−1?respectively. According to the results of this experiment, consumption of chemical fertilizer of triple super phosphate was reduced equal to 50% using Barvar-2 phosphate biofertilizer and consumption of chemical fertilizer of urea was reduced equal to 25% using nitroxin biofertilizer.

Pregnancy and childbirth are major stressors for some women. They can be followed by deterioration in mental health status and cause mental illnesses during perinatal period. Undetected and untreated perinatal mental illnesses can have negative unexpected impacts on parenting skills of the women and children’s development. Mentally ill mothers may not effectively attend their children’s needs in a timely manner and may experience an unfavourable mother-child attachment affecting the child’s language, social, emotional and cognitive development. The rate of pregnancy and postnatal health complications and interventions is higher among mentally ill women with some certain risk factors. The mentally ill mothers along with their partners need comprehensive support and counselling to be able to care for their infants and establish strong parent-child bond and attachment. Mental health campaigns across the world have endeavoured to increase the knowledge and awareness of the public towards perinatal mental health illnesses. To this aim, a routine screening is recommended in order to identify the women who are at risk of mood or anxiety disorder during perinatal period. The development of knowledge on perinatal mental illnesses among public and the health professionals has resulted in timely recognition and treatment of perinatal mental illnesses. Although great volumes of research show high prevalence of perinatal mental illnesses and their impacts on parenting confdence and competence as well as child’s developmental process, there is still lack of research on various aspects of perinatal mental illnesses. To enable early prevention, diagnosis and intervention, it is crucial to identify families who are at an increased risk of perinatal mental illnesses and provide support and intervention to minimise the adverse outcomes. Proposed Models for Subcritical Water Extraction of Essential Oils

Mechanisms that control the extraction rate of essential oil from Zataria multiflora Boiss. (Z. multiflora) with subcritical water (SW) were studied. The extraction curves at different solvent flow rates were used to deter-mine whether the extractions were limited primarily by the near equilibrium partitioning of the analyte between the matrix and solvent (i.e. partitioning thermodynamics) or by the rates of analyte desorption from the matrix (i.e. ki-netics). Four simple models have been applied to describe the extraction profiles obtained with SW: (1) a model based solely on the thermodynamic distribution coefficient KD, which assumes that analyte desorption from the ma-trix is rapid compared to elution; (2) one-site kinetic model, which assumes that the extraction rate is limited by the analyte desorption rate from the matrix, and is not limited by the thermodynamic (KD) partitioning that occurs dur-ing elution; (3) two-site kinetic model and (4) external mass transfer resistance model. For SW extraction, the thermodynamic elution of analytes from the matrix was the prevailing mechanism as evidenced by the fact that ex-traction rates increased proportionally with the SW flow rate. This was also confirmed by the fact that simple re-moval calculations based on determined KD (for major essential oil compounds) gave good fits to experimental data for flow rates from 1 to 4 ml·min-1. The results suggested that the overall extraction mechanism was influenced by solute partitioning equilibrium with external mass transfer through liquid film.

The purpose of this study was to evaluate the physical properties and bioactivity potential of epoxy-based dental sealers modified with synthesized bioactive glass(BAG),hydroxyapatite(HA)and fluorine substituted hydroxyapatite(FHA)*** synthesized powders were incorporated at 10%and 20%into the epoxy-based dental *** setting time,flow and solubility and microhardness of the modified and unmodified samples were *** bioactivity was evaluated using FESEM-EDX and elemental mapping,ATR-FTIR and *** flow value of all of the experimental groups except the FHA modified samples,was greater than 20 *** solubility,no specimens exhibited more than 1%weight *** solubility value of the FHA groups was statistically significant lower than other groups(p≤0.001).The mean hardness values of all of the modified samples were significantly higher than the unmodified group(p≤0.001).Regarding bioactivity,in vitro study revealed that after 3 days immersion in SBF a compact and continuous calcium phosphate layer formed on the surface of epoxy sealers containing BAG and HA *** on these results,the addition of BAG and HA nanoparticles did not adversely alter the physical properties of epoxy ***,they improved the in vitro bioactivity of the epoxy sealer.

Automatic diagnosis tool helps physicians to evaluate capsule endoscopic examinations faster and more *** purpose of this study was to evaluate the validity and reliability of an automatic post-processing method for identifying and classifying wireless capsule endoscopic images, and investigate statistical measures to differentiate normal and abnormal images. The proposed technique consists of two main stages, namely, feature extraction and classification. Primarily, 32 features incorporating four statistical measures(contrast, correlation, homogeneity and energy) calculated from co-occurrence metrics were computed. Then, mutual information was used to select features with maximal dependence on the target class and with minimal redundancy between features. Finally, a trained classifier, adaptive neuro-fuzzy interface system was implemented to classify endoscopic images into tumor, healthy and unhealthy classes. Classification accuracy of 94.2% was obtained using the proposed pipeline. Such techniques are valuable for accurate detection characterization and interpretation of endoscopic images.
