Dengue fever,caused by the dengue virus(DENV),poses a significant public health challenge globally,with Nigeria experiencing sporadic outbreaks.A clear understanding of the dengue burden has not been achieved in Nigeria,just as in other African *** the epidemiology and burden of dengue fever is essential for effective prevention and control *** paper examines the recent dengue outbreaks in northern Nigeria,particularly in Sokoto state,and evaluates the recommended Takeda dengue vaccine(TDV)along with future prevention *** limited surveillance and underreporting,dengue fever is endemic in Nigeria(with over 5 million cases and 5000 dengue-related deaths in 2023),with recent outbreaks indicating a growing *** TDV,a live attenuated tetravalent vaccine,has shown promise in preventing dengue fever,but challenges such as vaccine acceptance and access-ibility need to be *** urbanization contributes to the disease's spread,which is influenced by factors such as population density,cultural beliefs,water storage practices,hygiene,and water supply *** prevention strategies must focus on government intervention,community practices,and innovative vector control measures to mitigate the spread of DENV in *** study will serve as a valuable reference for policy-makers,researchers,and clinicians in the management and control of DENV in Nigeria and Africa as a whole.

Efficient Resource Allocation with Improved Interference Mitigation in FFR-Aided OFDMA Heterogeneous Networks

Fractional frequency reuse(FFR) has recently emerged as an efficient inter-cell interference coordination technique for orthogonal frequency division multiple access(OFDMA) based multi-tier cellular networks due to its low complexity, minimal signaling over-head, and coverage improvement. In this work, an intermediary region(IR) at the border of the center region(CR) and edge region(ER) is defined, which prevents severe cross-tier interference and is usually ignored by other schemes. Furthermore, a strategic resource allocation scheme is proposed, which allows macro users in this new region to be served more resources due to their good channel conditions close to the serving base station(BS), while femto users are assigned resource blocks from sub-bands that receive the least net interference from a set of usable sub-bands in any region. We find by analysis and simulation the optimal threshold for IR, which minimizes the cross-tier interference, and show that the femto throughput is also maximized for this threshold. Numerical results show the proposed scheme outperforms other notable schemes in terms of throughput and outage performances.

Health Services Utilization and Health Status of Insured versus Uninsured Nigerian Children with Sickle Cell Disease

Introduction: Nigeria accounts for 150,000 infants born with the Sickle Cell Disease (SCD) every year. Children with SCD are affected by the deleterious effect of user fees which reduces affordability and utilization of health services. Evidences supported that institutionalized health insurance increases intensity of utilization. Methodology: The study was conducted at AKTH, Kano, and north-western, Nigeria. The study was retrospective comparative cross-sectional study. 100 patients were enrolled, 50 for each arm. Paediatric SCD clinic outpatients’ records and a specialty designed form containing the variables of interest were used. Data were analysed using Minitab 16. Proportions, percentages, tables, charts and chi squared test were used to compare the two groups. Result: The study found no association between the two groups in the likelihood of being insured based on age (χ2 = 1.478), gender (χ2 = 0.224) and dwellings (χ2 = 0.062). On health services utilization and insurance status, the study revealed that follow up clinic visits, unscheduled clinic visits and emergency room visits were more likely among the insured group compared to the uninsured group. While the health status and insurance status of the two groups over 12 months period showed a significant association with the insured more likely to have improved health status compared to uninsured (χ2 = 28.019, p = 0.0001). Similarly, health status and health services utilization were significantly associated with insurance status (χ2 = 12.191, p = 0.002). Conclusion: The insurance status of children with SCD is associated with their health services utilization and health status. However, age, gender and dwellings were not associated with insurance status of children with SCD. Urinary Retention in Adults Male Patients: Causes and Complications among Patients Managed in a Teaching Hospital in North Western Nigeria

Background: Urinary retention is one of the common urologic emergencies constituting a significant workload of urologists and non-urologists alike. Aims and Objectives: This study was undertaken to identify the causes and complications associated with urinary retention in adult male patients at AKTH, Kano. Patients and Methods: It was a prospective hospital-based study of 110 consecutive adult male patients who presented to Aminu Kano Teaching Hospital, Kano with urinary retention over 12 months. On presentation, a brief history was taken and rapid physical examination done;the urinary retention was relieved by urethral catheterization or suprapubic cystostomy. A detailed history and thorough physical examination findings were later obtained. Results: One hundred and ten patients were seen during the study period. There ages ranged from 17 to100 years with a mean of 56 ± 19.3 SD years. Patients within the age range 55 - 74 years accounted for the largest group, (25.5% + 23.6% = 49.1%). The most common cause of urinary retention in this study was benign prostatic hyperplasia (51.8% of the patients). This was followed by urethral stricture (20%), cancer of the prostate (7.3%), urethral injury (7.3%) and bladder tumour (6.4%). More than half of the patients (53.4%) presented with acute urinary retention, 30.5% presented with chronic urinary retention, and 16.1% were diagnosed to have acute-on-chronic urinary retention. The complications of urinary retention found were: urinary tract infection (in 24.5% of patients), renal impairment (14.5%), and anaemia (11.8%). Conclusion: Urinary retention is commoner among the middle aged and the elderly. Benign prostatic hyperplasia remains the leading cause of urinary retention. Acute urinary retention was the commonest type of urinary retention;however, complications due to the retention were associated with chronic and acute-on-chronic urinary retention.

分别在中性和酸性植酸溶液中制备植酸转化膜并研究其耐蚀性。结果表明,在中性植酸溶液中得到的植酸转化膜的耐蚀性优于在酸性溶液中得到的植酸转化膜,特别是在0.06 mol/L、pH值为7.0的植酸溶液中反应1 h所得到的植酸转化膜的保护效率可达到96.7%。虽然植酸转化膜与Q235碳钢电极表面通过物理吸附相结合,但是其仍然具有优良的界面结合力。XPS结果表明,植酸具有钝化碳钢表面的功能,且最终在碳钢表面得到的植酸转化膜实际上是一层由植酸转化膜和碳钢表面的Fe_2O_3钝化膜构成的复合膜,二者通过协同抑制作用来保护碳钢电极不受外界腐蚀介质的侵蚀。

A comparative study of Fungi (yeast and moulds) and their toxic metabolites was carried out on offal and muscles of freshly fried and stored Sallah meats from rams, bulls, goats and camels slaughtered for sacrifice in Danbatta Local Government Area, Kano State, Nigeria. The objective was to determine the effects of frying and four weeks storage at ambient temperatures on the mycological loads of the fried and stored meats. A total of seventy two (72) samples of muscles and offal were collected from six different and randomly selected households that slaughtered and fried any of the four animals during that period. The samples were analyzed immediately as freshly fried, and then stored for a period of four weeks during which analyses were conducted at the second and fourth weeks. An analysis of variance (ANOVA) was used to compare between the fungal loads and the toxin contents in the stored meat. A Comparison of Quality of Life, Medication Side-Effect and Adherence among Schizophrenia Patients on Conventional versus Atypical Antipsychotics

Quality of life (QoL) is becoming a widely accepted schizophrenia management outcome. But it is still not very clear if there are any significant differences between the conventional and atypical antipsychotics in terms of QoL improvement among people with schizophrenia (PWS). It is also imperative that antipsychotic drug-related factors, such as medication adherence and side-effect, which could directly or indirectly affect the QoL of PWS, are determined and compared among PWS on different classes of the drugs. Data were collected on Socio-demographic Characteristics, Quality of Life and Medication adherence using Socio-demographic and Schizophrenia Clinical Characteristics questionnaire, World Health Organization Quality of Life (WHOQoL)-Brief, and Morisky Medication Adherence Scale (MMAS) respectively from 250 respondents attending a tertiary health center’s Psychiatric clinic in Kano, Nigeria. Although PWS on the two classes of antipsychotic drugs showed inequalities in different aspects and domains of QoL, as well as in the levels of adherence and side-effects, the differences were all insignificant. However, presence of drug side effects was significantly associated with lower health-related QoL in the conventional antipsychotics group (p = 0.001), and lower score in the physical domain of QoL in the atypical antipsychotics group (p = 0.044). Medication adherence was found to be associated with better scores in different domains of QoL in both groups of PWS. There are no significant differences in terms of QoL, medication side-effect and adherence among PWS on the two classes of antipsychotics. However, drug side-effects and adherence were significantly and respectively associated with lower and higher scores in different domains of QoL in both groups.

A Prospective Study on the Incidence and Predictors of Postpartum Depression among Pregnant Women Attending an Antenatal Clinic in Kano, Northern Nigeria

Depressive disorder is a major contributor to years lived with disability (YLD) globally. It is estimated that 13% of all women experience the disorder in the first year postpartum. Postpartum depression (PPD) has significant implications to the physical and mental wellbeing of both the mother and her baby. Only non-depressed pregnant women (score of ≤ 6 on Edinburgh Postnatal Depression Scale) were recruited into this study. Data collection was done at 2 different stages;at recruitment stage (during the third trimester of pregnancy) and at the follow up stage (4th - 6th week postpartum). During the recruitment stage, intimate partner violence (IPV) and level of social support perception, among the women, were assessed using Hurt Insult Threaten Scream (HITS) and Multidimensional Scale of Perceived Social Support (MSPSS) questionnaires respectively. While at the follow up stage, the Obstetric questionnaire and Mini International Neuropsychiatric Interview (MINI-7) were used respectively to obtain obstetric-related data and to diagnose for occurrence of depression. The incidence of PPD was found to be 16.34%. Several factors such as level of education, husband’s socioeconomic status, stressful life events, low perception of social support, obstetric instrumentation and not having a baby of preferred gender were significantly associated with PPD. Positive predictors of PPD included not practicing breast feeding (OR = 186.72, 95% CI = 8.32 - 4188.35), family history of mental illness (OR = 4.95, 95% CI = 1.04 - 23.81) and pregnancy lasting beyond 40 weeks (OR = 7.26, 95% CI = 1.51 - 34.88). High incidence of PPD with numerous associated but largely modifiable risk factors call for more proactive measures, such as PPD screening among pregnant women.
