Abstract：Subclavian artery thrombosis is a rare complication of clavicle fractures. We reported a 20-year-old man who was admitted to the emergency room after a road traffic accident. He was a pedestrian who was initially hit by a bus and after he fell down on the road, he was run over by a car. On evaluation, he was found to have multiple facial and rib fractures, distal right humerus and right clavicle fracture. Significantly, right radial pulse was absent. After further evaluation including Doppler studies and an angiography which revealed complete obstruction of right subclavian artery just distal to its 1st portion, the patient was urgently taken to the operation room. A midclavicular fracture was adjacent to the injured vessel. We established proximal and distal control, removed damaged part. After mobilizing the subclavian artery, an end-to-end anastomosis was made. Then open reduction and internal fixation of right distal humerus was performed. The rest of the postoperative course was unremarkable. To prevent complications of subclavian artery thrombosis, different treatment modalities can be used, including anticoagulation therapy,angioplasty, stenting and bypass procedures。

化学物质的开发,特别是中环类化合物的发展一直受到医药、农药领域的青睐。其中,七元全碳环结构由于广泛存在于天然产物、药物和生物活性化合物中具有潜在应用价值。到目前为止,人们已经发展了许多策略用于构筑中环类骨架。然而,由于熵和空间效应,这些策略在很大程度上受到了限制。在不对称催化领域,通过不同的环加成策略是构筑七元环的主要方法,但经由[3+2]环加成的研究较少。由于七元环类化合物结构的新颖性以及潜在的生物活性,因此研究这些结构的新合成方法具有重要意义。氨基丙二酸二乙酯盐酸盐和原位生成的1,3偶极子通过分子内[3+2]环加成反应可以高效地构建环庚酮类化合物。此类反应条件温和、原子经济性高符合绿色有机合成的要求。基于此,本论文以氨基丙二酸二乙酯盐酸盐和原位生成的1,3偶极子为底物,通过分子内[3+2]环加成反应构建环庚酮骨架。通过对反应条件的筛选诸如催化剂、溶剂、碱和分子筛,确定了最优条件:二氯甲烷为溶剂(0.1 mol/L),催化剂为DMAP用量为10mol%,NaCO作为碱,加入3(?)分子筛,空气氛围下室温反应9小时,以96%的产率和大于20:1的dr值得到产物环加成产物。这一策略为构建具有三个立体中心的七元碳环和吡咯提供了一种新的合成思路。

Hypoglycemic effects were investigated in the ethanolic extract of fruits of Phyllanthus emblica Linn. with long evans rats (100-120 g). Residual gut sucrose content, gut perfusion and disaccharidase activity studies were carried out to assess these activities. The residual gut sucrose content study was carried out to reveal the amount of sucrose remaining in the gastro intestinal tract (GIT) at six different segments. The ethanolic fruit extract of Phyllanthus emblica Linn. significantly increases the residual sucrose content throughout the gut after sucrose ingestion. Study on intestinal enzyme activity showed that the extract caused a significant (p 0.05), dose dependent inhibition of intestinal disaccharidase enzyme activity. Evaluation of the effect of the plant extract on intestinal enzyme activity showed significant inhibition of intestinal disaccharidase activity, suggesting that reduction of sucrose absorption may be partly related to the inhibition of disaccharidase activity in the gut. The gut perfusion analysis showed that the extract significantly reduced intestinal glucose absorption. The results of the present study suggest that ethanolic extract of Phyllanthus emblica Linn. has significant antidiabetic effects in a dose dependent manner and this may be effective in the treatment of diabetes.

The present study was undertaken to evaluate the analgesic and anti-inflammatory activities of ethanolic extract of Terminalia chebula (commonly known as Haritaki) fruits in experimental animal models. The study was carried out using Swiss Albino mice (20-25 g) and Long Evans rats (100-150 g) of either sex. The analgesic activity of Terminalia chebula was assessed by using hot plate method. For the determination of analgesic effect, doses of ethanolic extract of Terminalia chebula used in the present study were 250 mg/kg and 500 mg/kg body weight (BW). Anti-inflammatory effect was analyzed by carrageenan induced paw edema method with the administration dose of 300 mg/kg BW of animals. The analysis of experimental data was performed by statistical process of ANOVA to determine the variability of sample, while Dunnet’s test was performed for evaluation of comparative analgesic and anti-inflammatory activity of Terminalia chebula with control and standard. The animals were divided into four treatment groups of six animals each and the “Mean ± SEM” is the statistical identifiable value of the data and P values <0.05 was considered statistically significant. Hot plate test showed a significant increase in the mean reaction time to heat stimuli in hot plate method at both 250 mg/kg and 500 mg/kg BW doses throughout the observation period in 30 minutes and 60 minutes after treatment, which was comparable to the standard ketorolac and control group. In carrageenan induced paw edema method, considerable results were found after determining the percentage change in paw volume in extract. In both cases of analgesic and anti-inflammatory study, % inhibition of pain and inflammation were evaluated. Comparing with control, largest inhibition was found in inhibiting inflammation 5 hours after treatment, while the largest inhibition of pain was obtained in 30 minutes and 60 minutes after treatment of doses. An RFID (Radio Frequency Identification) based security and access control system is proposed in this paper. Recently, a crime against children is increasing in higher rate and it is high time to offer a safety system for the school going children. In this work, our aim is to build a security system to keep the student secure. This system provides a unique identification number that is RFID card. Generally, the RFID card will be scanned in an RFID reader. RFID reader will send data to Arduino where the database program is previously set. Arduino programs are written in C language. If anyone remains absent after school starts, the GSM module will send an automated SMS to his or her parents. Students may need to leave school for an emergency case. Then he/she can take special permission from the administration. There is a button set for the administration to open the gate immediately for this reason. Parents will be provided RFID card from the school authority. Because student card won’t work for the second time in a day. So parents will have to scan their card after school to receive their children. And their card will only work after school. Hence, the risks of any occurrence are reduced and ensured the safety of the student.

Simvastatin Potentiates the Antihyperglycemic, Antidyslipidimic and Antioxidative Effect of Glibenclamide on Alloxan-Induced Diabetic Rats

The aim of the current study is to investigate the effect of combination of glibenclamide;an antidiabetic drug and simvastatin;a HMG-CoA reductase inhibitor on long-term (four weeks) alloxan-induced diabetes rats (ADRs). Methods: Alloxan (120 mg/kg body weight, BW) was injected intra-peritonially (i.p.) in rats. At first alloxan (120 mg/kg BW) induced diabetic rats were treated with single dose of glibenclamide (1.2 mg/70kg BW) and simvastatin (10 mg/70kg BW) for two weeks. Then fixed dose combinations of glibenclamide (0.6 mg/70kg BW) and simvastatin (5 mg/70kg BW) were injected along with those of two drugs for four weeks. Results: At first it was found that glibenclamide reduced significant amount of glucose in blood, but it had no significant effect on lipid profile on short term (two weeks) ADRs. In contrast, simvastatin had no effect on blood glucose level, whereas it significantly reduced total cholesterol (TC), triglycerides (TG) and low density lipoprotein cholesterol (LDL-C) and increased significant amount of high density lipoprotein cholesterol (HDL-C). However, pathological changes of pancreas’s Islets of Langerhans were observed only after long-term (four weeks) induction of alloxan in rats. The inhibitory effect of combination therapy on blood glucose, TC, TG and LDL-C level was higher than those of monotherapy alone on long term ADRs. In addition, treatment with combination therapy on long term ADRs showed higher amount of HDL-C level and super oxide dismutase and catalase enzyme activity than those with monotherapy. They also decreased serum glutamic pyruvic transaminase (SGPT) and Serum glutamic oxaloacetic transaminase (SGOT) level. Administration of simvastatin recovered Langerhans cells from shrinkage whereas glibenclamide displayed slight recovery. But the combination therapy showed complete recovery of Langerhans cells as compared with diabetic rats. In current decade, pharmaceutical industries of Bangladesh are giving much emphasize on the formulation of time release preparation to treat various chronic diseases in order to decrease the frequency of administration and to improve patient compliance. Objectives: The objective of this investigation is to design and evaluate sustained release matrix tablet of Gliclazide by direct compression method employing polymers of hydroxypropylmethyl cellulose (HPMC) derivatives (K15M CR and K4M CR) and to select the optimized formulations and compression process by performing a comparative release kinetic study with a reference product, Diamicron MR (one of the worldwide brand of Gliclazide sustain released tablet manufactured by Servier one of the French pharmaceutical company) tablet. Methods: Release kinetics of Gliclazide matrix tablets were determined using USP paddle method at Phosphate buffer (pH 7.4). The release mechanism was explored and explained with zero order, first order, Higuchi and Korsmeyer model. Result: It is found that formulation with lower polymeric concentration follows Higuchi release kinetics and that the formulation with higher concentration best fits with zero order release kinetics. Among the formulations, F1 and F6 show almost similar dissolution profile with Diamicron MR Tablet, which can be suitable candidates for further in-vivo bioequivalence study. Conclusion: Findings of this investigation suggest that F1 and F6 formulations are potential candidates for further bioequivalence study among other formulations.
