AIM To evaluate the real-world effectiveness of golimumab in ulcerative colitis(UC) and to identify predictors of *** We conducted an observational, prospective and multi-center study in UC patients treated with golimumab, from September 2014 to September 2015. Clinical activity was assessed at wk 0 and 14 with the physician's global clinical assessment(PGA) and the partial Mayo score. Colonoscopies and blood tests were performed, following daily-practice clinical criteria, and the results were recorded in an SPSS *** Thirty-three consecutive patients with moderately to severely active UC were included. Among them, 54.5% were female and 42 years was the average age. Thirty percent had left-sided UC(E2) and 70% had extensive UC(E3). All patients had an endoscopic Mayo score of 2 or 3 at baseline. Twenty-seven point three percent were anti-tumor necrosis factor(TNF) treatment na?ve, whereas 72.7% had previously received infliximab and/or adalimumab. Sixty-nine point seven percent showed clinical response and were steroid-free at wk 14(a decrease from baseline in the partial Mayo score of at least 3 points). Based on PGA, the clinical remission and clinical response rates were 24% and 55% respectively. Withdrawal of corticosteroids was observed in 70.8% of steroid-dependent patients at the end of the study. Three out of 10 clinical non-responders needed a colectomy. Mean fecal calprotectin value at baseline was 300 μg/g, and 170.5 μg/g at wk 14. Being anti-TNF treatment na?ve was a protection factor, which was related to better chances of reaching clinical remission. Twenty-seven point three percent of the patients required treatment intensification at 14 wk of followup. 背景:克罗恩病(CD)手术往往预后不佳,常需再次手术。本研究旨在比较初次手术与多次手术CD患者的术后治疗及临床结局。方法:本研究为观察性研究,纳入2007年1月至2010年12月间西班牙26家医院收治的行回结肠切除吻合术的成人CD患者。从医疗记录中回顾性收集数据。结果:314例患者纳入分析,其中262例(83%)为初次手术病例,52例(17%)既往有过CD手术史。除狭窄型病变的患者比例再手术组相对较高以外(P=0.03),其余基线资料两组患者的差异均无统计学意义。术后接受预防性药物治疗的患者比例再手术组高于初次手术组(P=0.04)。再手术患者临床复发的时间与是否接受预防性药物治疗无关;但对于初次手术患者,接受预防性药物治疗者无复发生存时间更长(P=0.03)。结论:与初次手术者相比,既往有过CD手术史的患者术后更多接受了预防性药物(免疫调节剂)治疗。尽管预防性药物治疗可以预防初次手术者的临床复发,但并没有降低再次手术患者的复发风险。提示术后预防性药物治疗仍应仅限于初次手术患者。

Small intestinal bacterial overgrowth in inactive Crohn's disease:Influence of thiopurine and biological treatment

AIM:To investigate the influence of thiopurines and biological drugs on the presence of small intestinal bacterial overgrowth(SIBO) in patients with inactive Crohn's disease(CD).METHODS:This was a prospective study in patients with CD in remission and without corticosteroid treatment,included consecutively from 2004 to *** was investigated using the hydrogen glucose breath ***:One hundred and seven patients with CD in remission were *** 58%of patients used maintenance immunosuppressant therapy and 19.6%used biological *** prevalence of SIBO was16.8%.No association was observed between SIBO and the use of thiopurine Immunosuppressant(12/62patients),administration of biological drugs(2/21 patients),or with double treatment with an anti-tumor necrosis factor drugs plus thiopurine(1/13 patients).Half of the patients had symptoms that were suggestive of SIBO,though meteorism was the only symptom that was significantly associated with the presence of SIBO on univariate analysis(P<0.05).Multivariate analysis revealed that the presence of meteorism and a fistulizing pattern were associated with the presence of SIBO(P<0.05).CONCLUSION:Immunosuppressants and/or biological drugs do not induce SIBO in inactive *** disease pattern and meteorism are associated with SIBO.

As the 5G ecosystem continues its consolidation,the testing and validation of the innovations achieved by integrators and verticals service providers is of preponderant *** this line,5GASP is a European H2020-funded project that aims at easing the idea-to-market process through the creation of an European testbed that is fully automated and self-service,in order to foster rapid development and testing of new and innovative 5G Network Applications(NetApps).The main objective of this paper is to present the 5GASP’s unified methodology to design,develop and onboard NetApps within the scope of different vertical services,letting them use specific 5G ***,we examine the whole 5GASP process in a tutorial fashion by adopting a specific use case focusing on the integration of a virtual On-Board Unit(vOBU)service that permits offloading processing from the attached vehicle and serving data-access *** demonstrated,the presented workflow permits the agile,rigorous,and safe development,testing and certification of NetApps,which will enable valuable in-network services for 5G and beyond infrastructures.

The expansion of the Internet of Moving Things(IoMT)leads to limitless and continuous working playgrounds exploited by highly dynamic end *** requires the adoption of multi-Radio Access Technologies(RATs)-based strategies to provide IoMT units with ubiquitous *** this end,the development of secure bootstrapping and authentication mechanisms is necessary to permit the secure operation of end *** the transmission and power limitations of these elements,current cryptographic solutions do not address these stringent *** that reason,in the study we present a Multi-Access Edge Computing(MEC)-based endto-end architecture that enables an efficient and secure authentication and key agreement between end devices and network servers over heterogeneous resource-limited networks such as the Low Power Wide Area Networks(LPWANs).Our proposal is based on the Authentication,Authorization,and Accounting(AAA)architecture and the recent Internet Engineering Task Force initiatives Static Context Header Compression and Low-Overhead *** results obtained from experimental tests reveal the validity of the proposal as it enables constrained IoMT devices to gain IPv6 connectivity as well as performs end-to-end secure authentication with notable reliability and controlled latency.

A new wave of electric vehicles for personal mobility is currently crowding public *** offer a sustainable and efficient way of getting around in urban environments,however,these devices bring additional safety issues,including serious accidents for ***,taking advantage of a connected personal mobility vehicle,we present a novel on-device Machine Learning(ML)-based fall detection system that analyzes data captured from a range of sensors integrated on an on-board unit(OBU)*** the typical processing limitations of these elements,we exploit the potential of the TinyML paradigm,which enables embedding powerful ML algorithms in constrained *** have generated and publicly released a large dataset,including real riding measurements and realistically simulated falling events,which has been employed to produce different TinyML *** attained results show the good operation of the system to detect falls efficiently using embedded *** considered algorithms have been successfully tested on mass-market low-power units,implying reduced energy consumption,flash footprints and running times,enabling new possibilities for this kind of vehicles.

Enzymatic and chemo-enzymatic strategies to produce highly valuable chiral amines from biomass with ω-transaminases on 2D zeolites

Amino transaminases(ATAs) have been supported on a 2D ITQ-2 zeolite through electrostatic interactions, resulting in a highly stable active biocatalyst to obtain a variety of valuable chiral amines starting from prochiral ketones derived from biomass. We have extended the biocatalyst applications by designing a chemo-enzymatic process that allows, as the first step, prochiral ketones to be obtained from biomass-derived compounds through an aldol condensation–reduction step using a bifunctional metal/base catalyst. The prochiral ketone is subsequently converted into the chiral amine using the immobilized ATA. We show that it is feasible to couple both steps in a semi-continuous process to produce industrially relevant chiral amines with yields of >95% and ～100% enantiomer excess.
