Endogenous endophthalmitis is a rare condition with a poor long-term visual prognosis and significant mortality, often associated with the hematogenous spread of intravitreal infections and subsequent disruption of the blood-ocular barrier. Its anatomical proximity to the central nervous system (CNS) poses a high risk of infection dissemination, although cases documented in the literature are rare, and endogenous endophthalmitis is typically described as secondary to neuroinfections. We report the case of an 82-year-old female patient with a history of hypertension who presented with fever, decreased visual acuity, severe headache, chemosis, and conjunctival injection. Endogenous endophthalmitis was diagnosed, and antimicrobial treatment was initiated alongside surgical intervention by the ophthalmology service. However, the patient’s condition worsened neurologically, and Streptococcus pneumoniae was identified in cerebrospinal fluid cultures. Despite intensive medical treatment, the patient’s clinical course was poor, leading to death.

Upscaling perovskite solar cell fabrication is one of the key challenges in the pathway for *** slow evaporation of frequently used solvents(DMF or DMSO) limits the fast perovskite layer crystallization,hindering their implementation in large scale deposition ***,methylamine-based precursors have demonstrated rapid crystallization,leading to uniform and specular ***,their application has been limited to MAPbI3 perovskites with limited efficiency and *** this work,we report the requirements for stabilizing α-phase of mixed cation perovskites with high amount of formamidinium by using a methylamine-based *** found that even though,there are many methods for incorporating the methylamine(MA) in precursors or films;the MA content determines stabilization of the α-phase and therefore the viscous-solution route is the only method to incorporate high amounts of *** low amounts of MA,perovskite tend to crystallize in 1D dimensional FA_(3)(MA)PbI5 phases due to the incomplete solvation of the *** contrast,high MA ratio induces a full solvation of the clusters,leading to a rapid crystallization and a full stabilization of the active 3D α-*** results open a window in the development and understanding of new precursors for the fabrication of high efficient,stable and scalable perovskite devices.

Eosinophilic esophagitis is an immune-allergic pathology of multifactorial etiology(genetic and environmental)that affects both pediatric and adult *** symptoms,which include heartburn,regurgitation,and esophageal stenosis(with dysphagia being more frequent in eosinophilic esophagitis in young adults and children),are similar to those of gastroesophageal reflux disease,causing delays in diagnosis and *** endoscopic findings such as furrows,esophageal mucosa trachealization,and whitish exudates may suggest its presence,this diagnosis should be confirmed histologically based on the presence of more than 15 eosinophils per high-power field and the exclusion of other causes of eosinophilia(parasitic infections,hypereosinophilic syndrome,inflammatory bowel disease,among others)for which treatment could be ***,the 3“D”s(“Drugs,Diet,and Dilation”)are considered the fundamental components of *** first 2 components,which involve the use of proton pump inhibitors,corticosteroids,immunosuppressants and empirical diets or guided food elimination based on allergy tests,are more useful in the initial phases,whereas endoscopic dilation is reserved for esophageal ***,the most important aspects of eosinophilic esophagitis pathophysiology will be reviewed,in addition to evidence for the various treatments.

The tunnel industry has considered that tunnels,especially tunnels in rock,are naturally resistant to earthquake action,including faulting,shaking,deflection and ground *** the number of case histories of tunnels subject to earthquake action has increased,the industry has started to recognize that,although tunnels in rock have good resistance against earthquakes generating peak ground accelerations(PGA)lower than 0.5 g,it is important to include the dynamic forces and displacements generated by seismic ground motions in the design process to obtain a more reliable *** additional earthquake forces impact the final design,potentially requiring changes to the ground support and additional reinforcement of the concrete lining,as illustrated by case histories presented in this paper.

Perovskite solar cells(PSCs) have revolutionized photovoltaic research. As a result, a certified power conversion efficiency(PCE) of 25.5% was recorded in late 2020. Although this efficiency is comparable with silicon solar cells;some issues remain partially unsolved, such as lead toxicity, instability of perovskite materials under continuous illumination, moisture and oxygen, and degradation of the metallic counter electrodes. As an alternative to tackle this last concern, carbon materials have been recently used, due to their good electrical and thermal conductivity, and chemical stability, which makes them one of the most promising materials to replace metallic counter electrodes in the fabrication of PSCs. This review highlights the recent advances of carbon-based PSCs, where the carbon electrode(CE) is the main *** have become very promising candidates for PSCs;they are mainly fabricated using a simple combination of graphite and carbon black powders embedded in a binder matrix, giving a paste that is then solution-processable, resulting in devices with improved quality stability, when compared to metallic electrodes. In this review, CE’s composition is emphasized, since it can give both, high and lowtemperature processed electrodes, compatible with different device configurations. Finally, the tendencies and opportunities to use CE in PSCs devices are presented.

Noise pollution is becoming a critical health risk for city *** 2020,the COVID-19 pandemic forced many cities to implement several mobility *** restrictions changed human activity patterns and decreased the noise levels and noise pollution that often affect urban *** the number of infections decreased,so did the outdoor activities,influencing the population’s perception of *** paper aims to evaluate the changes in noise levels associated with mobility restrictions between 2020 and 2021 in Guayaquil,*** study used crowdsourcing with the help of smartphones and mobile applications to collect geo-referenced environmental noise *** data was used to generate noise maps in different time ***,noise level maps were created using GIS-based tools to identify the urban areas that experienced the highest noise level variation during the study *** results show that the most significant noise increase occurred at ***,when analyzing noise level changes in different urban areas,the western area of Guayaquil was the one that experienced the most significant noise level *** inform the perception of noise pollution and could potentially serve as a reference for decision-makers during the proposal of public policies that ensure a better quality of life for its citizens.

Effectiveness of an Secondary Prevention Program in Chronic Kidney Disease

Background: There are many programs which focus on late-stage chronic kidney disease (CKD), and it is considered that further evidence needs to be generated regarding the effectiveness of the programs used before renal replacement therapy. Study Design: A cohort study. Settings & Participants: Patients over 15 years of age who had been diagnosed with CKD according to the KDOQI (Kidney Disease Outcomes Quality Initiative) guidelines and who had undergone conventional treatment (CT) or a renal protection program (RPP). These were patients of two Colombian health insurance companies. Predictors: Age, sex, marital status, comorbidities, CKD stage, and clinical indicators. Outcomes: First CKD progression, and need for renal replacement therapy (RRT). Measures: Clinical marker. Results: The RPP is structurally and functionally different from the CT. It offers the interdisciplinary management of patients, a greater number of medical appointments, and patients start to receive treatment at younger ages and at earlier stages of their condition. The clinical markers of the patients following the RPP are within adequate ranges, and their renal function is less impaired, despite the differences in basal conditions. Upon finishing the study, we found that patients who received CT had a higher risk of receiving nephrotoxic drugs and not receiving nephroprotective drugs. The explanatory variables for the first progression were age, stage, history of dyslipidemia, and hemoglobin, potassium, and albumin levels. These variables, together with glycemia levels were also valid for RRT, except for history of dyslipidemia, as it was not significant. Upon adjusting for the explanatory variables, it was found that belonging to the RPP and attending more appointments had a protective effect in the process of controlling renal damage.
