Endoscopic ultrasound guided fine needle aspiration and useful ancillary methods

The role of endoscopic ultrasound(EUS) in evaluating pancreatic pathology has been well documented from the beginning of its clinical use. High spatial resolution and the close proximity to the evaluated organs within the mediastinum and abdominal cavity allow detection of small focal lesions and precise tissue acquisition from suspected lesions within the reach of this method. Fine needle aspiration(FNA) is considered of additional value to EUS and is performed to obtain tissue diagnosis. Tissue acquisition from suspected lesions for cytological or histological analysis allows, not only the differentiation between malignant and non-malignant lesions, but, in most cases, also the accurate distinction between the various types of malignant lesions. It is well documented that the best results are achieved only if an adequate sample is obtained for further analysis, if the material is processed in an appropriate way, and if adequate ancillary methods are performed. This is a multi-step process and could be quite a challenge in some cases. In this article, we discuss the technical aspects of tissue acquisition by EUS-guided-FNA(EUS-FNA), as well as the role of an on-site cytopathologist, various means of specimen processing, and the selection of the appropriate ancillary method for providing an accurate tissue diagnosis and maximizing the yield of this method. The main goal of this review is to alert endosonographers, not only to the different possibilities of tissue acquisition, namely EUS-FNA, but also to bring to their attention the importance of proper sample processing in the evaluation of various lesions in the gastrointestinal tract and other accessible organs. All aspects of tissue acquisition(needles, suction, use of stylet, complications, etc.) have been well discussed lately. Adequate tissue samples enable comprehensive diagnoses, which answer the main clinical questions, thus enabling targeted therapy.

Management of obstructed defecation

The management of obstructed defecation syndrome(ODS) is mainly conservative and mainly consists of fiber diet, bulking laxatives, rectal irrigation or hydrocolontherapy, biofeedback, transanal electrostimulation, yoga and psychotherapy. According to our experience, nearly 20% of the patients need surgical treatment. If we consider ODS an "iceberg syndrome", with "emerging rocks", rectocele and rectal internal mucosal prolapse, that may benefit from surgery, at least two out of ten patients also has "underwater rocks" or occult disorders, such as anismus, rectal hyposensation and anxiety/depression, which mostly require conservative treatment. Rectal prolapse excision or obliterative suture, rectoceleand/or enterocele repair, retrograde Malone's enema and partial myotomy of the puborectalis muscle are effective in selected cases. Laparoscopic ventral sacral colporectopexy may be an effective surgical option. Stapled transanal rectal resection may lead to severe complications. The Transtar procedure seems to be safer, when dealing with recto-rectal intussusception. A multidisciplinary approach to ODS provides the best results.

An Architecture for Software Engineering Gamification

Gamification has been applied in software engineering to improve quality and results by increasing people’s motivation and engagement.A systematic mapping has identified research gaps in the field,one of them being the difficulty of creating an integrated gamified environment comprising all the tools of an organization,since most existing gamified tools are custom developments or *** this paper,we propose a gamification software architecture that allows us to transform the work environment of a software organization into an integrated gamified environment,i.e.,the organization can maintain its tools,and the rewards obtained by the users for their actions in different tools will mount *** developed a gamification engine based on our proposal,and we carried out a case study in which we applied it in a real software development *** case study shows that the gamification engine has allowed the company to create a gamified workplace by integrating custom-developed tools and off-the-shelf tools such as Redmine,TestLink,or JUnit,with the gamification *** main advantages can be highlighted:(i)our solution allows the organization to maintain its current tools,and(ii)the rewards for actions in any tool accumulate in a centralized gamified environment.

The long-term stability of calcium arsenates:Implications for phase transformation and arsenic mobilization

It is well known that calcium arsenates may not be a good choice for arsenic removal and immobilization in hydrometallurgical ***,they are still produced at some plants in the world due to various ***,calcium arsenates can also naturally precipitate under some specific ***,the transformation process of poorly crystalline calcium arsenates(PCCA)and the stability of these samples under atmospheric CO2 are not yet well *** work investigated the transformation process of PCCA produced by using different neutralization reagents(CaO ***)with various Ca/As molar ratios at pH 7-12 in the presence of atmospheric *** aging at room temperature for a period of time,for samples neutralized with NaOH and precipitated at pH 10 and 12,release of arsenic back into the liquid phase *** contrast,for the samples precipitated at pH 8,the aqueous concentration of arsenic was observed to ***,Raman,and SEM results suggested that the formation of various types of crystalline calcium carbonates and/or calcium arsenates controls the arsenic ***,the application of lime may enhance the stability of the generated ***,no matter what neutralization reagent is used,the stability of the generated PCCA is still of concern.

The authors study, by applying and extending the methods developed by Cazenave(2003), Dias and Figueira(2014), Dias et al.(2014), Glassey(1994–1997), Kato(1987), Ohta and Todorova(2009) and Tsutsumi(1984), the Cauchy problem for a damped coupled system of nonlinear Schrdinger equations and they obtain new results on the local and global existence of H^1-strong solutions and on their possible blowup in the supercritical case and in a special situation, in the critical or supercritical cases.

Local perceptions of climate change and adaptation needs in the Ifugao Rice Terraces (Northern Philippines)

The Ifugao Rice Terraces in the Philippines is recognized worldwide as a sustainable landscape where humans live in harmony with nature. The success of the Ifugao Rice Terraces largely depends on the attunement of local farmers to their environment and their ability to adapt to perceived changes, as manifested in their complex body of traditional ecological and climatic knowledge. This paper examines the local perceptions on climate change and other challenges to sustainability through focus group discussions with farmers and traditional knowledge holders. Our main findings can be summarized as follows:(i) Ifugao farmers were able to observe climatic changes in recent years, and these changes were intimately linked with broader environmental and socio-cultural changes in the Ifugao social-ecological system;(ii) The climatic changes qualitatively observed by the farmers were in agreement with trends in datasets commonly used in scientific assessments, although this agreement depends on the spatial and temporal resolution of the dataset, and the type of statistical analysis performed, and;(iii) The Ifugaos stressed the importance of traditional knowledge and culture in climate change adaptation, and preferred measures which could increase internal adaptive capacity while addressingbroader sources of community vulnerability. Our results support calls to recognize Indigenous and Western science as equally valid ways of knowing. Discussions with the farmers revealed that in the Ifugao context, climate change may be better framed in the context of multiple stressors on rural livelihoods, with adaptation integrated into broader development objectives. Our findings also emphasize the need for greater engagement of indigenous Ifugao people in planning processes in order to identify adaptation strategies that are culturally appropriate, equitable, and effective in responding to local needs.
