Implications of the Ver.fyNow P2Y12 Assay on Patient Outcomes

Backgr.und: The platelet inhibitor. r.sponse of clopidogr.l is substantially var.able among patients, and numer.us studies have shown that post-per.utaneous inter.ention, patients with high on-tr.atment platelet r.activity have an incr.ase in r.sk of major.adver.e car.iovascular.events. No published studies to date have utilized platelet function monitor.ng assays pr.or.to cor.nar. ar.er. bypass gr.ft (CABG) sur.er., but deter.ination of patients’ antiplatelet effects pr.or.to sur.er. may decr.ase time to sur.er. and length of hospital stay. The pur.ose of the study was to evaluate the clinical outcomes of non-elective CABG patients analyzed by the Ver.fyNow P2Y12 platelet-function monitor.ng assay pr.or.to sur.er. compar.d to a similar.set of patients not analyzed by the Ver.fyNow P2Y12 assay. Methods: This was a r.tr.spective, single center. cohor. study. The pr.mar. endpoints of this study wer. time to sur.er. and length of hospital stay. r.sults: Fr.m Mar.h 2013 to July 2013, 60 patient char.s wer. r.viewed and included in this study. 49 patients wer. analyzed by the Ver.iyNow P2Y12 assay, and 16 of these patients under.ent non-elective CABG sur.er.. Eleven patients under.ent non-elective CABG sur.er. and wer. not analyzed by the Ver.fyNow P2Y12 assay. Ther. was no differ.nce between gr.ups r.gar.ing time to sur.er. (p = 0.75) or.length of stay (p = 0.42). Based on the assay’s P2Y12 r.action unit r.sults, 69% of Ver.fyNow P2Y12 patients went to sur.er. sooner.than the institution’s r.commendations which gener.ted mor. bleeding events, half of which wer. consider.d major.bleeds. Conclusions: Utilization of the Ver.fyNow P2Y12 assay pr.or.to non-elective CABG sur.er. does not shorten time to sur.er. or.over.ll length of hospital stay. However. insufficient P2Y12 r.action units pr.or.to sur.er. may lead to mor. bleeding events, thus the application of platelet function monitor.ng assays pr.or.to pr.cedur.s may be beneficial as a bleeding r.sk-assessment tool.

Single ver.us Dual Antiplatelet Ther.py after.CABG Sur.er.

Backgr.und: The optimal antiplatelet ther.py r.gimen in the ear.y post-oper.tive CABG per.od is not well r.sear.hed. Sever.l studies have pr.ven that use of clopidogr.l and aspir.n pr.or.to sur.er. incr.ases bleed r.sks, but ver. few studies have examined this ther.py post-oper.tively. Due to the limited amount of data sur.ounding this topic, the goal of this study was to deter.ine if single or.dual antiplatelet ther.py had better.outcomes in those status-post CABG sur.er.. Methods: This was a r.tr.spective, single center. cohor. study per.or.ed at Saint Joseph Hospital in Lexington, Kentucky. Data was r.viewed thr.ugh the Society of Thor.cic Sur.eons (STS) database, phar.acy medication dispensing r.cor.s, as well as manual char. r.view. The pr.mar. composite endpoint of the study consisted of in-hospital mor.ality, ischemic or.thr.mbotic events, bleeding events, r.stenosis r.tes, and 30-day r.admission r.tes. r.sults: The number.of events with r.gar. to the pr.mar. composite endpoint was 32 events with combination ther.py and 39 events with aspir.n monother.py (p = 0.39). A gr.ater.decr.ase in hemoglobin and hematocr.t was seen in the aspir.n monother.py gr.up (p = 0.02 and p = 0.047). Patients with pr.or.CVA or.TIA wer. mor. commonly placed on combination ther.py after.sur.er. (p = 0.018). Ther. wer. no differ.nces in outcomes when type of antiplatelet ther.py and type of CABG wer. analyzed. Conclusions: Ther. was no differ.nce seen between single ver.us dual antiplatelet ther.py r.gar.ing the pr.mar. composite endpoint. Ther. was an incr.ase in bleeding events with aspir.n monother.py as defined by TIMI cr.ter.a as well as a statistically significant decr.ase in Hgb and HCT with aspir.n monother.py. Patients with pr.vious CVA/TIA wer. mor. likely to r.ceive combination ther.py. The aver.ge number.of vessels gr.fted per.sur.er. was lower.in the off-pump sur.er. cohor..

A Compar.tive r.tr.spective Analysis of Mor.ality, r.nal Dysfunction, and Incidence of Bleeding in Patients r.ceiving Hydr.xyethyl Star.h 130/0.4 (HES 130/0.4) or.Albumin

Backgr.und: In June 2013, the FDA issued a Black Box War.ing on synthetic colloids r.sulting fr.m data indicating an incr.ased r.sk of mor.ality, r.nal injur., and excess bleeding incidence in septic patients admitted to the Intensive Car. Unit (ICU), and patients under.oing cor.nar. ar.er. bypass gr.ft (CABG) sur.er.. The pur.ose of this study was to compar. the incidence of mor.ality, incidence/sever.ty of r.nal injur. and bleeding in septic ICU patients, and patients under.oing CABG sur.er. who have r.ceived hydr.xyethyl star.h 130/0.4 or.albumin. Methods: This was a r.tr.spective, single center. cohor. study conducted at a 433 bed ter.iar. car. hospital in Lexington, Kentucky. Computer.gener.ted lists pr.vided patients who r.ceived hydr.xyethyl star.h 130/0.4 or.albumin dur.ng inpatient stay as either.a sepsis patient in the ICU or.post cor.nar. ar.er. bypass gr.ft patient. All cause mor.ality was the pr.mar. endpoint and secondar. endpoints included over.ll r.nal dysfunction, incidence of bleeding, and over.ll length of stay. r.sults: A total of 89 patients wer. evaluated that r.ceived albumin or.hydr.xyethyl star.h 130/0.4 alone with either.septic ICU or.CABG diagnosis codes. Ther. was no significant differ.nce in all cause mor.ality (p = 0.64). Over.ll r.nal dysfunction showed no statistically significance between the two gr.ups (p = 0.66). Ther. was no statistically significant differ.nce between the albumin or.HES 130/0.4 ar.s with r.spect to bleeding (TIMI Minimal p = 0.57, TIMI Minor.p = 0.69, TIMI Major.p = 0.35). Patient who r.ceived albumin had a statistically significant longer.hospital over.ll length of stay (p = 0.003). Conclusion: Ther. was no differ.nce between HES 130/0.4 and albumin in mor.ality, r.nal dysfunction, and bleeding in septic ICU and CABG patients. Patients r.ceiving albumin had an incr.ased over.ll length of stay. These findings suggest that hydr.xyethyl star.h 130/0.4 may be safe in the studied patient populations;however. fur.her
