Projections of Precipitation Changes in Two Vulnerable Regions of São Paulo State, Brazil

Weather and climate extremes are part of the natural variability. However, the frequency and intensity of precipitation extremes have increased in the globe following the global warming. Extreme precipitation impacts such as landslides and flooding with implications to vulnerability and adaptation are discussed for two regions of the state of S?o Paulo: the Metropolitan Region of Campinas and the Metropolitan Region of the Baixada Santista, located in southeastern South America. Simulations and projections obtained from four integrations of the Regional Eta model are analyzed to investigate the model behavior during the period of 1961-1990 and the projections within the period of 2011-2100. Uncertainties are discussed based on the standard deviation among the model spread. The projections show precipitation increase in the Metropolitan Region of Campinas during DJF for the near and distant future, while there are more uncertainties in the other seasons. In the Metropolitan Region of Baixada Santista, the precipitation increase is projected to all seasons, except JJA, when there is higher uncertainty. Daily rainfall indices suggest an increase of precipitation during the rainy days, but a reduction in the number of rainy days in both locations. The projections show a reduction of light rains and an increase of heavy rains at both regions. The model identifies the South Atlantic Convergence Zone and frontal systems as precipitation patterns associated with extremes in the two locations. The results can be useful for adaptation actions, since the regions are highly populated and have high vulnerabilities.

Pneumo-computed tomography (PnCT) is a technique primarily developed and used to study stenotic lesions of the esophagus, gastroesophageal junction and stom-ach for pre-surgical planning. It helps to define both upper and lower borders of neoplasms located in the aforementioned areas. It achieves maximum lumen distension with CO2 highlighting thickened areas of the esophageal wall, thus allowing an accurate quantifica-tion of their extents. Although there are other alterna-tives for distension (oral contrast agents, water and ef-fervescent granules), they may be suboptimal. Patients with locally advanced esophageal cancer have a dismal prognosis despite surgical resection. Therefore, neo-adjuvant treatment strategies using radiation therapy and chemotherapy were developed to improve sur-vival. Neoadjuvant therapy improves esophageal tumor prognosis in a substantial proportion of patients, and the use of imaging techniques is mandatory to detect their response. PnCT combined with virtual endoscopy and multiplanar reconstruction enhances morphologic details in esophageal cancer, and thus would allow animproved assessment of response to neoadjuvant treat-ment. Therefore, more information could be provided to assess the efficacy of pre-surgical treatment. We de-scribe the potential use of PnCT to assess the response to neoadjuvant therapy in esophageal cancer with an imaging pathologic correlation.

AIM To study the effects of linagliptin on the structural signs of non-alcoholic fatty liver disease(NAFlD) in db/db mice. METHODS Male diabetic db /db mice(***-Dock7m+/+leprdb/J) aged 10 wk received the dipeptidyl peptidase 4(DPP4) inhibitor linagliptin(10 mg/kg) or saline as a placebo once per day by gavage for 8 wk. Intact db/db mice served as controls. Structural changes in the liver were analyzed from light and electron microscopic images of sections from intact, placebo-treated and linagliptin-treated animals. We estimated the changes in hepatocytes, sinusoidal cells, liver microvasculature and lymphatic roots. Hepatic staining for lymphatic vessel endothelial hyaluronan receptor-1(lYVE-1) was assessed by immunohistochemistry. RESUlTS In 18-wk-old diabetic mice, liver steatosis(predominantly microvesicular and mediovesicular steatosis) was accompanied by dilation of the roots of the lymphatic system, interlobular blood vessels and bile canaliculi. Compared to saline-treated mice, linagliptin-treated mice exhibited a reduction in the mean numeral densities of hepatocytes with lipid droplets(92.4% ± 1.7% vs 64.9% ± 5.8% per field of view, P = 0.0002) and a lower proportion of hepatocytes with a high density of lipid droplets(20.7% ± 3.6% vs 50.4% ± 3.1%, P = 0.0007). We observed heterogeneous hepatocytes and relatively preserved cell structures in the linagliptin group. Dilation of blood and lymphatic vessels, as well as ultrastructural changes in the hepatocyte endoplasmic reticulum and mitochondria, were alleviated by linagliptin treatment. In intact and placebo-treated mice, immunohistochemical staining for lYVE-1 was observed in the endothelial cells of interlobular lymphatic vessels and on the membranes of some endothelial sinusoidal cells. The methods of preparation, conservation and sale of Garba, the traditional Ivorian street meal, abundantly consumed in Côte d’Ivoire, can be exposed to various infections resulting in the poisoning of consumers. The aim of this study was to evaluate the hygienic quality of Garba through the analysis and the determination of certain toxic chemicals. In three hundred (300) samples of Garba collected in four districts in Abidjan, toxic metals (cadmium, mercury, lead), biogenic amine (histamine) and mycotoxins (aflatoxins and ochratoxin A) were detected and quantified using official standardized methods. Different toxics analyzed were present at various levels in the Garba. The mercury, lead, cadmium and histamine levels in the Garba were respectively 0.19 mg/kg, 0.19 mg/kg, 0.03 mg/kg and 32.69 mg/kg. The detected mycotoxins included aflatoxins B1, B2, G1, G2 and ochratoxin A, with respective average proportions of 3.44 μg/kg, 1.90 μg/kg, 8.07 μg/kg, 0.56 μg/kg and 0.42 μg/kg. The mycotoxins levels in the Garba are higher than the recommended toxic levels, particularly the ones in aflatoxin B1 and G1. This suggests a sanitary risk associated with the consumption of this meal. Consequently, awareness campaigns and training of the Garba sellers in hygiene and a better regulation of this sector by the competent authorities are required.

The localized forms of brucellosis, particularly osteoarticular, by their symptomatology usually nonspecific, still continue to be a diagnosis challenge, being fundamental to raise a high degree of suspicion based on a careful epidemiological history. The authors describe the case of a 69 year old farmer that was admitted due to an insidious lower back pain with irradiation to the left lower limb accompanied by nocturnal sweating, anorexia and weight loss. The imaging studies revealed a spondylodiscitis in l3-l4 with an associated epidural abscess. The diagnosis of brucellosis was brought on by the occupational exposure to sheep and was confirmed by a positive PCR to Brucella in the product gathered by aspiration from the epidural abscess and also by the serology (ElISA and Rose Bengal). Despite the initial therapy with doxycycline, rifampicin and gentamycin (the last one for 1 week), the patient had a clinical and imagiologic worsening, determining surgical decompression. After the surgery, antiobiotherapy with doxycyline and rifampicin was maintained and a cycle of gentamicin was started, this time for a month, with a favorable clinical evolution.

Spatio-temporal variations of harmful algal blooms along the coast of Guangdong,Southern China during 1980–2016

Harmful algal blooms (HABs) have become a recurring problem, posing severe impacts on marine ecosystems, fisheries, mariculture industry, and even public health. In this study, the geographic information system (GIS) was utilized to determine spatial and temporal characteristics of HAB events in the coastal waters of Guangdong from 1980–2016. We analyzed distribution patterns and characteristics of HABs by dividing the coast of Guangdong into well-known bays, estuary and coastal waters. Results showed that there were a total of 337 HABs recorded in Guangdong coastal waters. Spatial and temporal distributions varied among diff erent regions. Most HABs occurred in the Mirs Bay, followed by the west coast of Daya Bay, while a few occurred in the west and east coasts of Guangdong but with an increasing trend in the past two decades. HABs occurred mostly in warmer months of March to May in the western coast of Guangdong, March and April in Mirs Bay, April in Zhujiang (Pearl) River estuary, November in eastern coast of Guangdong. For Daya Bay, most HABs were reported between March and September. The most frequently occurring HABs species were Noctiluca scintillans , Phaeocystis globosa , Skeletonema costatum and Scrippsiella trochoidea , occurring mostly in Mirs Bay, western Guangdong coast area, eastern Guangdong coast area and Zhujiang River estuary and Daya Bay, respectively. Ichthyotoxic blooms were more common than toxic blooms, and Heterosigma akashiwo , Chattonella marina , Karenia mikimotoi and P haeocystis globosa were the most common ichthyotoxic species. Our results provide baseline information useful for policy making and management of HABs in the region.

Introduction: Nonalcoholic fatty liver disease (NAFlD) is the leading cause of chronic liver disease worldwide and its prevalence increases with that of metabolic syndrome and its components. NAFlD is associated with complications such as cirrhosis and hepatocellular carcinoma. Diagnosis is mainly based on liver biopsy, but there are validated non-invasive methods. The purpose of the study was to assess the impact of metabolic steatopathy in patients with metabolic syndrome in Cameroon. Methods: This was a cross-sectional and analytical study conducted over a 6-month period from January 1st, 2019, to August 31st, 2022. Included were patients with metabolic syndrome who had consulted in endocrinology or gastroenterology at Yaoundé Central Hospital, Douala General Hospital and Douala Gyneco-obstetric and Pediatric Hospital. The diagnosis of NAFlD was made on abdominal ultrasound in front of a homogeneous or heterogeneous hyperechogenic aspect of the hepatic parenchyma compared to that of the right renal cortex called “brilliant liver” and fibrosis evaluated through non-invasive scores (Fib4 and NAlFD Fibrosis score). logistic regression by a uni- and multivariate analysis made it possible to search for the associated factors. Results. We included 133 patients. The female sex represented 64.7%. The mean age was 55 ± 9 years. The prevalence of NAFlD was 48.9%. At the evaluation of fibrosis was significant according to FIB-4 and NAFlD fibrosis score respectively in 6.2% and 4.6% of cases. The independently associated factors were Triglyceridemia ≤ 1.5 g/l (OR = 0.33;95% CI [0.11 - 0.95];p = 0.04) and lDl hypercholesterolemia (OR = 2.94;95% CI [1.07 - 8.11];p = 0.036). Conclusion: NAFlD was present in almost half of patients with metabolic syndrome. We had very few patients with significant fibrosis, but it needs to be further evaluated. The associated factors are hypertriglyceridemia and lDl hypercholesterolemia.
