Many Asian countries are projected to age rapidly and will need to devote a larger proportion of their gDP to finance age-related expenditure including on *** therefore will have to create additional fiscal space to fund such expenditure to sustain the credibility of existing pension *** paper presents an exploratory framework for creating fiscal *** framework has three interrelated components:enhancing broadbased growth,improving revenue performance and better expenditure *** paper distinguishes between funding of pensions,*** share of gDP devoted to pensions,and financing of pensions,*** different methods and instruments used to finance *** focus of the paper is on funding,and the framework relies on both the income-expenditure flows and the government balance sheet to create fiscal *** examples of how potential fiscal space can be created in Asian economies of China,India and Indonesia to make their pension promises more credible are *** paper emphasises that the measures discussed for enhancing fiscal space should not be undertaken as a purely technical exercise,but should be combined with managing the political economy in a given context.

AIM To determine the short-term biochemical effects and clinical tolerability of intermittent fasting(IF) in adults with type 2 diabetes mellitus(T2DM).METHODS We describe a three-phase observational study(baseline 2 wk, intervention 2 wk, follow-up 2 wk) designed to determine the clinical, biochemical, and tolerability of IF in community-dwelling volunteer adults with T2DM. Biochemical, anthropometric, and physical activity measurements(using the Yale Physical Activity Survey) were taken at the end of each phase. Participants reported morning, afternoon and evening self-monitored blood glucose(SMBg) and fasting duration on a daily basis throughout all study stages, in addition to completing a remote food photography diary three times within each study phase. Fasting blood samples were collected on the final days of each study phase. RESULTS At baseline, the ten participants had a confirmed diagnosis of T2DM and were all taking metformin, and on average were obese [mean body mass index(BMI) 36.90 kg/m^2]. We report here that a short-term periodof IF in a small group of individuals with T2DM led to significant group decreases in weight(-1.395 kg, P = 0.009), BMI(-0.517, P = 0.013), and at-target morning glucose(SMBg). Although not a study requirement, all participants preferentially chose eating hours starting in the midafternoon. There was a significant increase(P gh few attained the 18-20 h fasting goal(mean 16.82 ± 1.18). The increased fasting duration improved at-goal(g SMBg to 34.1%, from a baseline of 13.8%. Ordinal Logistic Regression models revealed a positive relationship between the increase in hours fasted and fasting glucose reaching target values(χ~2 likelihood ratio = 8.36, P = 0.004) but not for afternoon or evening SMBg(all P > 0.1). Postprandial SMBgs were also improved during the IF phase, with 60.5% readings below 9.05 mmol/L, compared to 52.6% at baseline, and with

gastrointestinal stromal tumors (gIST) account for 2% of all tumors of the digestive tract. They are most frequently located in the stomach followed by the small intestine and most often present with mucosal ulceration and erosion associated with gastrointestinal bleeding. Bleeding from the small intestine can be difficult to diagnose and angiography becomes a useful diagnostic and therapeutic procedure in these patients. We present a case of a gIST located in the ileum which presented with a lower gastrointestinal bleeding (LgIB). The patient was stabilized by performing an arteriography and selective embolization of the bleeding point, followed by elective surgery.

Breast cancer is the leading cause for mortality among women *** of oncogenes and tumor suppressor genes is the major reason for the cause of *** these genes will provide clues and insights about their regulatory mechanism and their interplay in *** the present study,an attempt is made to compare the functional characteristics and interactions of oncogenes and tumor suppressor genes to understand their biological role.431 breast cancer samples from seven publicly available microarray datasets were collected and analysed using gEO2R *** identified 416 differentially expressed genes were classified into five gene sets as oncogenes(Og),tumor suppressor genes(TSg),druggable genes,essential genes and other *** gene sets were subjected to various analysis such as enrichment analysis(viz.,gO,Pathways,Diseases and Drugs),network analysis,calculation of mutation frequencies and guanine-Cytosine(gC)*** the results,it was observed that the Og were having high gC content as well as high interactions than ***,the Og are found to have frequent mutations than *** enrichment analysis results suggest that the oncogenes are involved in positive regulation of cellular protein metabolic process,macromolecule biosynthetic process and majorly in cell cycle and focal adhesion pathway in *** was also found that these oncogenes are involved in other diseases such as skin diseases and viral ***,paclitaxel and docetaxel are some of the drugs found to be enriched for oncogenes.
