Environment chemical pollution can be persistent, and even virtually irremovable. For some chemicals in the workplace environment reliably safe low exposure levels are technically unattainable or presumably nonexistent. As a supplement to decreasing harmful exposures to as low levels as possible, the “biological prophylaxis” aims at enhancing host’s protective mechanisms. During over 30 years in animal experiments modeling isolated or combined chronic or subchronic exposures to silica, asbestos, monazite, lead, chromium, arsenic, manganese, nickel, vanadium, nanosilver, nanocopper, formaldehyde, phenol, naphthalene, benzo(a)pyrene we tested so-called “bioprophylatic complexes” (bPCs) comprising innocuous substances with theoretically expected beneficial influence on the toxicokinetics and/or toxicodynamics of those toxics. The bPCs proved protectively effective in animal experiments were then subjected to controlled field trials on restricted groups of volunteers. Once the effectiveness and safety of a bPC was established, it was recommended for practical use, first of all, in the most vulnerable population groups (children, pregnant women) and in the most harmful occupations. At each stage of this work the effectiveness of the bioprophylactic approach to chemical risks management was successfully demonstrated. The bPCs tested up to now proved capable of mitigating systemic toxicity, cytotoxicity, fibrogenicity, and mutagenicity of the above-listed chemicals.

Adipokines levels are associated with the severity of liver disease in patients with alcoholic cirrhosis

AIM:To investigate the adipokine levels of leptin,adiponectin,resistin,visfatin,retinol-binding protein 4(RbP4),apelin in alcoholic liver cirrhosis(ALC).METHODS:Forty non-diabetic ALC patients[median age:59 years,males:35(87.5%),Child-Pugh(CP)score:median 7(5-12),CP A/b/C:18/10/12,Model for End-stage Liver Disease(MELD):median 10(6-25),follow-up:median 32.5 mo(10-43)]were prospectively *** serum adipokine levels were estimated in duplicate by *** characteristics were assessed with tetrapolar bioelectrical impedance ***’s rank correlation coefficient was used to assess possible associations with adipokine *** and multivariate Cox regression analysis was used to determine independent predictors for ***:body mass index:median 25.9(range:20.1-39.3),fat:23.4%(7.6-42.1),fat mass:17.8(5.49-45.4),free fat mass:56.1(39.6-74.4),total body water(TbW):40.6(29.8-58.8).Leptin and visfatin levels were positively associated with fat mass(PbP4 with TbW(P=0.025).Median adiponectin levels were significantly higher in CPC compared to CPA(CPA:7.99±14.07,CPb:7.66±3.48,CPC:25.73±26.8,P=0.04),whereas median RbP4 and apelin levels decreased across the spectrum of disease severity(P=0.006/P=0.034,respectively).Following adjustment for fat mass,visfatin and adiponectin levels were significantly increased from CPA to CPC(both Pbserved for both RbP4 and apelin(both P<0.001).In the multivariate Cox regression analysis,only MELD had an independent association with overall survival(HR=1.53,95%CI:1.05-2.32;P=0.029).CONCLUSION:Adipokines are associated with deteriorating liver function in a complex manner in patients with alcoholic liver cirrhosis.

为建立bMY牛的活体采卵(OPU)-体外胚胎生产(IVP)-胚胎移植(ET)快速扩繁技术体系,从人工草地全放牧牛群中选择健康的经产空怀bMY牛作为供体(n=14)集中OPU一次;卵母细胞经TCM-199体外成熟培养(IVM)、Fert-TALP液体外受精(IVF),受精卵用合成输卵管液(SOF)体外培养(IVC)生产胚胎;bMY牛受体(n=81)用CUE-MATETM孕酮阴道栓+Eb+FSH+PG方法同期发情处理,对黄体合格的移植OPU-IVP来源鲜胚。研究结果表明:①OPU头均获卵母细胞23.50枚,获10枚以上卵母细胞的供体占85.7%;IVP的平均囊胚数和可用胚数分别为5.21和4.00枚;OPU前8～9d卵巢上有黄体(CL)的供体,头均获卵母细胞数及IVP的囊胚数、可移植胚数、囊胚率和可移植胚率分别比对照组高7.28枚(P0.05)。结果显示,bMY母牛具有良好的OPU潜力,但个体间差异较大;可通过严格供体选择及改善IVP技术等措施,实现OPU-IVP效率最大化。CUE-MATETM+Eb+FSH+PG同期发情处理方法适于在牛规模化胚胎移植(ET)和人工授精(AI)中应用。综合应用OPU、IVP、同期发情和ET技术是加快优良bMY牛扩繁的重要途径之一。

目的探究特发性扩张型心肌病（DCM）患者的左心室心肌灌注和脂肪酸代谢状况,并确定核素显像对心脏再同步化治疗（CRT）疗效的预测价值。方法 63例DCM伴严重心力衰竭患者[纽约心脏协会（NYHA）分级ⅡⅢ级]进入本研究。CRT之前,所有患者均行99Tcm-MIbI和123I-β-甲基-碘苯基十五烷酸(123I-bMIPP)显像,以评价心肌灌注和脂肪酸代谢状况。CRT前和CRT后6个月,所有患者均行超声心动图检查,以了解心脏的血流动力学情况。结果 CRT后6个月,患者被分成2组。第1组患者治疗有效,其左心室收缩末期容积（LVESV）下降≥15%（n=39）;第2组患者治疗效果差,其LVESV下降<15%（n=24）。CRT前,2组患者的左心室泵血功能差异没有统计学意义,而心肌灌注显像放射性缺损区的占比[7.4%（5.9%,13.2%）和11.8%（8.8%,16.2%）,P<0.05]和代谢显像放射性缺损区的占比[7.4%（4.4%,14.7%）和8.8%（8.8%,17.6%）,P<0.05]差异有统计学意义。与心肌灌注显像相比,代谢显像在预测CRT疗效上有更好的诊断效能（ROC曲线下面积:代谢显像为0.722,灌注显像为0.612）。代谢显像中放射性缺损区占比的最佳阈值为7.35%时,其预测CRT疗效的灵敏度和特异性分别为77.8%和66.7%。结论心肌脂肪酸代谢显像在预测扩张型心肌病患者CRT疗效上有一定的价值。

The aim of the present review is to highlight the possible neuroregenerative potential ol adipose-derived stem cells. The key property of stem cells is plasticity including self-renewal, multilineage differentiation, and migration, whereas the required property is transplantability. For a long time, embryonic stem cells were thought to be the only source of pluripotency, a dogma that has been challenged during the last decade. Today, an alternative option might be adipose-derived stem cells, as easily accessible, ethical and autologous cellular source. Recent knowledge of adipobiology increasingly recognizes that adipose tissue is the major endo- and paracrine organ of the human body. Likewise, numerous neuropetides, neurotrophic factors, neurotransmitters, hypothalamic and steroid hormones and their receptors are shared by adipose tissue and brain. Accordingly, the regenerative potential of neuroprotective factor-secreting adipose-derived stem cells is outlined. Whether the possible benefits of adipose stem cell-based therapy may be mediated via cell transdifferentiation and/or paracrine mechanisms remains to further be evaluated.

Aims: The value of ECG in the perioperative risk stratification under modern treatment options is uncertain. The main objective of the present analysis was to determine the usefulness of a pre-operative ECG derivation for evaluating the risk of perioperative morbidity and mortality. Methods: We performed a secondary analysis of the prospective, international, multicenter, observational “No-Risk” Study (N-terminal b-type natriuretic peptide [NT-probNP] for the assessment of the perioperative cardiac risk after major noncardiac surgery) to determine the prognostic value of 12-lead ECG. Inclusion criteria were age >55 years and at least one of the following cardiovascular risk factors: arterial hypertension, diabetes mellitus, dyslipidemia, active smoking, and family history positive for coronary heart disease. The combined primary endpoint included total mortality, acute myocardial infarction (NSTEMI and STEMI), cardiopulmonary resuscitation, heart failure, and asystole or ventricular fibrillation during hospitalization. ECGs from 616 patients enrolled from 2006 to 2009 prior to noncardiac surgery in the No-Risk Study were analyzed. Results: The mean age was 67.6 (±8.1) years;300 (48.7%) patients were male. Fourteen (2.3%) patients suffered from the combined primary endpoint while in the hospital. In Kaplan-Meier analyses, a pathologic Q wave and QTc > 500 ms were significantly related to the incidence of the primary endpoint (p p = 0.042, respectively), whereas other ECG parameters such as Lbbb, Rbbb, PQ interval, QRS interval, and others were not related to worse in-hospital outcome. Conclusion: The 12-lead ECG is still an important diagnostic tool for perioperative risk assessment of cardiovascular events in noncardiac surgery in patients at risk.

Soil and Rhizosphere Associated Fungi in Gray Mangroves(Avicennia marina) from the Red Sea—A Metagenomic Approach

Covering a quarter of the world＇s tropical coastlines and being one of the most threat- ened ecosystems, mangroves are among the major sources of terrestrial organic matter to oceans and harbor a wide microbial diversity. In order to protect, restore, and better understand these ecosystems, researchers have extensively studied their microbiology, yet few surveys have focused on their fungal communities, Our lack of knowledge is even more pronounced for specific fungal populations, such as the ones associated with the rhizosphere. Likewise, the Red Sea gray man- groves （Avicennia marina） remain poorly characterized, and understanding of their fungal commu- nities still relies on cultivation-dependent methods. In this study, we analyzed metagenomic datasets from gray mangrove rhizosphere and bulk soil samples collected in the Red Sea coast, to obtain a snapshot of their fungal communities. Our data indicated that Ascomycota was the dominant phylum （76%-85%）, while basidiomycota was less abundant （14%-24~）, yet present in higher numbers than usually reported for such environments. Fungal communities were more stable within the rhizosphere than within the bulk soil, both at class and genus level. This finding is consistent with the intrinsic patchiness in soil sediments and with the selection of specific microbial commu- nities by plant roots. Our study indicates the presence of several species on this mycobiome that were not previously reported as mangrove-associated. In particular, we detected representatives of several commercially-used fungi, e.g., producers of secreted cellulases and anaerobic producers of cellulosomes. These results represent additional insights into the fungal community of the gray mangroves of the Red Sea, and show that they are significantly richer than previously reported.
