Purpose:To determine the trends with fatally or otherwise injured pedestrians lying on the road and the relationship to hit-and-run incidents in ***:We extracted data for 2012e2016 from the records of the Institute for Traffic Accident Research and Data Analysis,Japan,a nationwide traffic accident *** the injured and fatally injured pedestrians were *** examined the levels of pedestrian injury,vehicle speed immediately before the collision,whether or not the pedestrian was lying on the road,and hit-and-run ***-square test was employed to make a statistical comparison between the two ***:The database contained data on 286,383 pedestrian casualties and 7256 fatalities;8.3%of fatalities(602 persons)and 0.6%of casualties(1827 persons)involved pedestrians lying on the *** rates of fatalities and severe injuries were significantly higher for pedestrians who were lying on the road than for those who were ***-and-run incidents were evident in 4.0%of casualties and 7.3%of *** rate of hit-and-run cases was also significantly higher among pedestrians who were lying on the *** fatally injured pedestrians not lying on the road,the rates with speeds of30 km/h did not differ significantly between hit-and-run and other ***,when the pedestrians were lying on the road,the rate was significantly increased in hit-and-run ***:This is the first report to focus on pedestrians lying on the road and being involved in hitand-run *** addition to preventing hit-and-run incidents,prevention of pedestrians lying on the road could also decrease fatalities.

Nigrostriatal Degeneration in Parkinson Disease: Evaluation by Diffusion Tensor Tract-Specific Analysis

Diffusion tensor tractography was used to evaluate whether diffusion metrics in the nigrostriatal pathway could diagnose Parkinson disease. Diffusion tensor imaging was performed on 30 patients with Parkinson disease and 32 healthy controls by using a 3.0 Tesla magnetic resonance imaging system. Diffusion tensor tractography was used for both groups to visualize the nigrostriatal and corticospinal tracts. The fractional anisotropy (FA) and mean diffusivity (MD) of the tracts were evaluated. Receiver operating characteristic (ROC) analysis was used to determine whether diffusion metrics of the nigrostriatal pathway could be used to diagnose Parkinson disease. Mean FA values (±SD) of the nigrostriatal tract in Parkinson disease patients (0.41 ± 0.025) were significantly lower than those in the control group (0.43 ± 0.022;p = 0.00068) and showed a sensitivity of 66.7% and specificity of 60%. There were no significant differences in the MD values of the nigrostriatal tract or the FA and MD values of the corticospinal tract between Parkinson disease patients and the control group. FA values of the nigrostriatal pathway in Parkinson disease patients were significantly lower than those in normal, healthy individuals. Reduced FA was generally thought to reflect neuronal loss, gliosis, or demyelination of nerve fibers. This result might provide a useful measure for diagnosing Parkinson disease and evaluating patients’ clinical condition.

BACKGROUND： A recent study has found that ATP13A2 is the causative gene for PARK9-linked autosomal recessive early-onset parkinsonism, described previously in Jordanian and Chilean families （Kufor-Rakeb syndrome）. OBJECTIVE： To screen eastern Asian patients with early-onset parkinsonism for mutations in ATP13A2 and to describe positron emission tomography （PET） findings of PARK9-linked parkinsonism. DESIGN, TIME AND SETTING： In total, 117 patients were selected from the Department of Neurology, Juntendo University, from February 2003 to October 2006, for this molecular genetics and case-control study. PARTICIPANTS： The patients with parkinsonism consist of two cohorts. Ninety four patients with onset age of less than 30 years were selected for the first cohort. They included 49 males and 44 females, comprising 73 Japanese, 9 Korean, 8 Taiwan Residents, and 4 Mainland Chinese. Eleven patients had parkinsonism complicated with dementia, 15 patients had family histories of parkinsonism （including 2 families）, and 5 patients were from consanguineous parents （including one family）. The second cohort of 23 patients was composed of patients with consanguineous parents （n = 15） or who had affected siblings （n = 6） or both （n = 2）, but the age at onset ranged from 30 to 50 years. METHODS： In 117 patients with parkinsonism, direct sequencing of ATP13A2 exons 13, 16, and 26, in which mutations had been reported previously, were performed. Sequencing was also performed in all 29 exons, including splice sites, in 28 probands who showed homozygosity at the PARK9 locus by haplotype analysis. Mutation analysis was also performed in 150 normal people. Linkage analysis was performed on all 3 parkinsonism families using short tandem repeat markers flanking the PARK9 locus. Treatment with growth factors is an effective method to promote wound healing. Several growth factors have been used in wound treatment, including basic fibroblast growth factor (bFGF) discovered in the 1970s, platelet-derived growth factor (PDGF), epidermal growth factor (EGF), and bone morphogenetic protein (BMP). In 2001, Kaken Pharmaceutical Co., Ltd. launched Fiblast Spray in Japan. This product is a topical formulation containing recombinant human bFGF. It is used to treat decubitus ulcers and skin ulcers (burn ulcers and leg ulcers). There is increasing interest in its stimulatory effect on wound healing and its inhibitory effect on scar formation. Wound healing is a physiological phenomenon that involves a complex series of integrated cellular and biochemical responses, characterized by phases of hemostasis, inflammation, proliferation, and maturation. However, much of its detailed mechanism remains unknown. There have been many basic research studies on wound healing using cells and experimental animals. If findings can be used from such studies and clinical practice, development of even more effective treatments might be possible.

Pyelonephritis is an inflammation of the kidney tissue, calyces, and renal pelvis. It is commonly caused by bacterial infection that has spread up the urinary tract. Common bacteria are Escherichia coli and Enterococcus faecalis. Beta hemolytic streptococci also cause pyelonephritis. Beta hemolytic streptococcus such as Streptococcus agalactiae, and S. dysgalactiae subsp. equisimilis, when grown on blood-agar, produce complete hemolysis around each colony, usually cause severe infections including many cases of cellulitis, pneumonia, meningitis and sepsis. However the analysis of pyelonephritis causing beta hemolytic streptococcus has seldom been investigated. In this study, we tried to clarify the relationship between beta hemolytic streptococcus and pyelonephritis patient in Japan. We investigated beta hemolytic streptococcus isolated from pyelonephritis patient at two tertiary care hospitals in the central region of Japan by analyzing the background and antimicrobial susceptibility of beta hemolytic streptococcus. We recovered total sixteen beta hemolytic streptococcus in nine years. In pyelonephritis patients, beta hemolytic streptococcus was isolated from 9 S. agalactiae and 7 S. dysgalactiae subsp. equisimilis, respectively. The numbers of streptococci from female patients were more than those from male patients. Bacteria were not isolated from under 30 year patients. All S. dysgalactiae subsp. equisimilis were isolated from 60-year-age patients. About one fifth of beta hemolytic streptococcus had erythromycin and minocycline-resistant ability. Four beta hemolytic streptococci that were all S. agalactiae, had levofloxacin-resistant ability. Only one S. agalactiae had trimethoprim-sulfamethoxazole-resistant ability. Ampicillin was effective against all beta hemolytic streptococci. In our results, beta hemolytic streptococcus caused pyelonephritis in aged people and the quarter of these bacteria had some antibiotics resistance.

Posterior reversible encephalopathy syndrome induced after blood transfusion for severe anemia

Posterior reversible encephalopathy syndrome (PRES) is a clinicoradiological syndrome characterized by headache, confusion, seizures, and cortical visual loss, as well as subcortical edema without infarction on neuroimaging. We report a 56-year-old woman who developed typical manifestations of PRES, 6 days after blood transfusion for severe anemia. Acute volume overloads by transfusion may exceed the capacity of autoregulation of perfusion pressure, possibly resulting in vasogenic edema. We propose that it is clinically important to recognize that rapid correction of anemia by blood transfusion may carry the risk of inducing PRES.

Possibilities of Assessing Respiratory Muscle Strength and Trunk Muscle Mass in the Prevention of Sarcopenia in Older People Living in the Community

Objective: The purpose of this study was to clarify the relationship between respiratory muscle strength and skeletal muscle mass (trunk, upper limbs, and lower limbs) in older people who were certified as requiring nursing or supportive care. Methods: Thirty-five older people (65 years or older) who were certified as requiring nursing care or support were included in the study. The subjects were divided into a non-sarcopenic group (n = 12) and a sarcopenic group (n = 23) according to the sarcopenia diagnostic criteria proposed by the Asian Working Group for Sarcopenia. Maximum inspiratory pressure, maximum expiratory pressure, skeletal muscle mass (trunk, upper and lower limbs), and hand grip strength were measured. Pearson’s correlation coefficient and multiple regression analysis were used for statistical processing. Results: In the non-sarcopenic group, both expiratory muscle strength and hand grip strength were correlated with skeletal muscle mass. In the sarcopenia group, expiratory muscle strength was not correlated with skeletal muscle mass, and only hand grip strength was correlated with upper limb muscle mass. Multiple regression analysis revealed that, in the non-sarcopenic group, trunk muscle mass was the primary factor in expiratory muscle strength and upper limb muscle mass was the primary factor in hand grip strength. In the sarcopenia group, upper limb muscle mass was found to be the main factor in hand grip strength. Conclusion: Our results highlight the importance of assessing expiratory muscle strength and trunk muscle mass before sarcopenia develops in older people who require support and nursing care.
