Multiyear analyses of ground-level air contaminants over Paris metropolitan region using real-time observations and air mass backward trajectories

for the years 2008-2013, particles of diameter 〈 10 and 2.5 μm （PM10 and PM2.5, respectively）, NOx, SO2, and O3 concentrations at urban, suburban, rural, and traffic sites in the Paris metropolitan area were analyzed. Strong spatial variability at traffic and rural sites and relatively uniform profiles at urban and suburban sites for PM10, PM2.5, and O3 were observed. The O3 weekend effect was induced by lower NOx emissions during the weekend, and favored volatile organic compounds （VOCs）-limited atmospheric conditions. In conJunction with low ambient temperature, these conditions could also favor increased formation of secondary particulate nitrates in winter. Winter air pollution events were associated with multiple pollutants, whereas those observed in spring were caused by high PM10 and PM2.5 levels. Back- ward trajectory analyses showed the contribution of sources in Western and Central Europe on days with high PM10, PM2.5, and O3, and a local/national comvonent for NOx and SO2.

A 54-year-old female with Anderson-fabry disease(AfD)-R342 Q missense mutation on exon 7 in alphagalactosidase A(gLA) gene- presented with sustained ventricular tachycardia. Imaging confirmed the presence of a new left ventricular apical aneurysm(LVAA) and a significantly reduced intra-cavitary gradient compared to two years prior. AfDcv is an X-linked lysosomal storage disorder caused by gLA enzyme deficiency. The phenotypic expression of AfD in the heart is not well described. Cardiac involvement can include left ventricular hypertrophy(LVH), which is typically symmetric, but can also mimic hypertrophic cardiomyopathy(HCM). Left ventricular apical aneurysm is a rare finding in HCM. We suggest a shared mechanism of LVAA formation in AfD and HCM, independent of the underlying cardiomyopathy. Mechanisms of LVAA formation in HCM include genetic predisposition and long-standing left ventricular wall stress from elevated intra-cavitary systolic pressures due to mid-cavitary obstruction. Both mechanisms are supported in this patient(a brother with AfD also developed a small LVAA). Screening for AfD should be considered in cases of unexplained LVH, particularly in patients with the aneurysmal variant of HCM.

Pulmonary alveolar microlithiasis is a rare disease characterized by intra-alveolar presence of microliths. This study reports an interesting case of pulmonary alveolar microlithiasis and provides a systematic review of cases reported from India. A 23-year-old female presented with a history of cough, wheeze, chest pain, and episodic wheeze for five months. Pulmonary function tests demonstrated an obstructive pattern, and chest X- ray showed fine micronodular opacities predominantly involving the middle and lower zones of both lungs. Transbronchial lung biopsy revealed the diagnosis. She responded well to inhaled steroid therapy. A systematic review of literature was performed and identified 73 cases of pulmonary alveolar microlithiasis reported from India. The mean （SD） age of the patients was 28.8 （14.9） years, with an almost equal male：female ratio. Many patients were asymptomatic at presentation. Breathlessness and cough were the most common symptoms, and the disease progressed into respiratory failure associated with cor pulmonale. About one-third of the cases were initially misdiagnosed and treated as pulmonary tuberculosis. Extra-pulmonary manifestations and co- morbidities were also evident in our series. This systematic review helps to determine epidemiological and clinical characteristics of pulmonary alveolar microlithiasis. further research is needed to elucidate the etiopathogenesis, diagnosis, and therapeutic options, which are beneficial in developing and identifying cost- effective treatment for pulmonary alveolar microlithiasis.

Malnutrition is highly prevalent in liver cirrhosis and its presence carries important prognostic *** clinical conditions and pathophysiological mechanisms that cause malnutrition in cirrhosis are multiple and *** and liver decompensation symptoms lead to poor dietary intake;metabolic changes characterised by elevated energy expenditure,reduced glycogen storage,an accelerated starvation response and protein catabolism result in muscle and fat wasting;and,malabsorption renders the cirrhotic patient unable to fully absorb or utilise food that has been *** is therefore a considerable challenge to manage effectively,particularly as liver disease progresses.A high energy,high protein diet is recognised as standard of care,yet patients struggle to follow this recommendation and there is limited evidence to guide malnutrition interventions in cirrhosis and liver *** this review,we seek to detail the factors which contribute to poor nutritional status in liver disease,and highlight complexities far greater than“poor appetite”or“reduced oral intake”leading to *** also discuss management strategies to optimise nutritional status in this patient group,which target the inter-related mechanisms unique to advanced liver ***,future research requirements are suggested,to develop effective treatments for one of the most common and debilitating complications afflicting cirrhotic patients.

Iron is an essential element involved in various biological pathways. When present in excess within the cell, iron can be toxic due to its ability to catalyse the formation of damaging radicals, which promote cellular injury and cell death. Within the liver, iron related oxidative stress can lead to fibrosis and ultimately to cirrhosis. Here we review the role of excessive iron in the pathologies associated with various chronic diseases of the liver. We also describe the molecular mechanism by which iron contributes to the development of hepatic fibrosis.

Reduced graphene oxide （RgO） has been covalently functionalized with porphyrin moieties by two methods： A straightforward Prato reaction （i.e. a 1,3-dipolar cycloaddition） with sarcosine and a formyl-containing porphyrin, and a stepwise method that involves a 1,3-dipolar cycloaddition to the RgO surface using 4-hydroxybenzaldehyde, followed by nucleophilic substitution with an appropriate porphyrin. The chemical bonding of porphyrins to the RgO surface has been confirmed by ultraviolet/visible absorption, fluorescence, fourier-transform infrared, and Raman spectroscopies, X-ray powder diffraction and X-ray photoelectron specfroscop）~ transmission electron and atomic force microscopy, and thermogravimetric analysis; this chemical attachment assures efficient electron/energy transfer between RgO and the porphyrin, and affords improved optical nonlinearities compared to those of the RgO precursor and the pristine porphyrin.
