Background: Population ageing is a worldwide phenomenon. It is common for older adults to develop multiple age-related illnesses and the prevalence of multimorbidity increases substantially with age. Multimorbid adults are frequently treated with several concurrent medications and the regimen may be complex requiring multiple steps in the preparation of a medication prior to its administration. Polypharmacy is a concerning trend and older adults have a 100% risk of experiencing adverse drug events when taking ten or more medications concurrently. Discharge summaries communicating the number of medications, changes made to medication regimens during hospitalisations and the requirement for ongoing monitoring in the community are often incomplete. The aim of this study was to investigate contributing factors to medication-related hospitalisation, length of stay or readmission in older community-dwelling persons and examine the quality of discharge summaries. Methods: Descriptive and correlational analyses of demographic, clinical, admission, readmission, length of stay and medication variables were examined in Australia in 2016-2018. Discharge summaries were analysed for completeness, timeliness and interprofessional communication. Results: There were 295 participants, mean age 80 years, 55% were female, taking an average of 11 prescribed medications and with a mean Medication Regimen Complexity Index score of 34. Medication errors that were unrecognised at the time of hospitalisation were present in 19% of the sample. Factors associated with medication error were older age and a longer median length of stay. Fewer than 52% of these older patients had detailed discharge summaries. Conclusion: The prevalence of polypharmacy and medication regimen complexity at admission was high. ＜正＞ The purpose of this research is the online detection of ethnic conflicts. We have chosen a knowledge management approach. An ontology describing ethnic conflicts is designed by experts with the help of automatic tools. Then a recognition engine performs model-based inference in order to recognize instances of ethnic conflicts from events feeding the system. Ethnic conflict scenarios are described thanks to the template technique. This technique provides both a formalism to represent scenarios and mechanisms to evaluate the resemblance between a sequence of events happening in a monitored country and the scenarios provided to the system. It is possible to represent inaccuracies in the knowledge. In particular, imprecise temporal constraints between the pairs of events are represented by means of fuzzy sets. General domain knowledge is represented by means of an ontology close to the Description Logics formalism in which the templates are integrated. The domain ontology and the data are represented in XML files.

The contamination of heavy metals in various environmental matrices is a major concern to many researchers and requires attention due to their detrimental impacts on the environment and humans as well. This research was carried out to investigate heavy metal levels in soils of some selected sites along the Vaisigano river in Upolu, Samoa. The soil samples were also analysed for physicochemical parameters such as pH, Electrical Conductivity, Moisture Factor, Cation Exchange Capacity (CEC), Organic Carbon, Nitrogen, Phosphorus, and Soil texture. The mean concentrations of heavy metals at the Lelata site were 0.19 ± 0.05 mg/kg (Pb), 0.37 ± 0.45 mg/kg (Mn), 0.15 ± 0.02 mg/kg (Zn) and 0.56 ± 0.42 mg/kg (Cu). The mean concentrations of heavy metals at the Alaoa site were 0.13 ± 0.03 mg/kg (Pb), 0.42 ± 0.51 mg/kg (Mn), 0.16 ± 0.03 mg/kg (Zn) and 0.45 ± 0.43 mg/kg (Cu). The heavy metal concentrations from both sites were lower than the permissible levels given by the World Health Organization (WHO). The Geo-accumulation pollution index (I(geo)) of heavy metals in the Lelata site was Cu > Mn > Pb > Zn and from the Alaoa site was Cu > Pb > Zn > Mn.

Reishi mushroom, also known as Ganoderma lucidum, is a type of edible medicinal fungi that is found throughout Asia. Recent research has shown that it may also have antiplatelet activity that is similar to aspirin. Likewise simvastatin has been shown to inhibit platelet aggregation. In addition, montelukast has anecdotally related to one case of acquired thrombopathia and may interfere with platelet aggregation. We report a case of intraoperative hemorrhage with laparoscopic trocar placement and dissection in a patient concomitantly taking Ganoderma lucidum supplements, simvastatin and montelukast. The hemorrhage only responded to a transfusion of platelets. We believe this is the first report associating Ganoderma lucidum products with an intraoperative hemorrhage. We recommend that patients who are on simvastatin, montelukast or an anticoagulant should refrain from taking Ganoderma lucidum products for seven days prior to surgery.

Aims and objectives: To evaluate the effectiveness of a patient education tool (the ENAT) on self efficacy, patient knowledge, health outcomes and its usability in practice. Background: The effectiveness and value of patient education in rheumatoid arthritis have been questioned. The ENAT was developed to help the nurses to provide more focused, individualized and relevant education. The psychometric properties of the ENAT have been assessed. Design: This study is a single blind randomized controlled trial using both quantitative and qualitative methodology. The primary outcome is patient self efficacy. Secondary outcomes are physical function, psychological status, pain levels, social interaction and patient knowledge of disease. Methods: 130 RA patients will be randomized into an experimental (EG) or control group (CG). The EG patients will complete the ENAT prior to seeing a Clinical Nurse Specialist (CNS) at weeks 0, 16 and 32. The CNS will use the ENAT as a template to meet patients perceived educational needs in addition to the usual care. The CG will receive their usual care from the CNS without the aid of the ENAT. To determine the ENAT’s usability, semi-structured quailtative interviews will take place with the practitioners and a sample of patients once they have completed the quantitative arm of the study. Results: The study commenced in April 2011 and the results are expected to be available after April 2013. Conclusions: It is hypothesized that nurses’ use of the ENAT will lead to increased patient self efficacy, and improved patient knowledge and health outcomes among the patients who used it. Furthermore, it is hypothesized that the ENAT will be easy for staff and patients to use. Relevance to clinical practice: The ENAT was developed to assess the perceived educational needs of patients with rheumatoid arthritis and thus help the nurses to provide more focused, individualised and relevant education.
