Association of adverse effects with monoamine oxidase type B inhibitor and catechol-o-methyl transferase inhibitor combination therapy in Parkinson’s disease patients

Currently, levodopa is the most effective and commonly used medication to control motor symptoms in Parkinson’s disease (PD). However, its long-term use is associated with adverse effects (AEs). Combination therapy of a monoamine oxidase type B inhibitor (MAOBI) with levodopa or a catechol-O-methyl transferase inhibitor (COMTI) with levodopa provides benefits to PD patients. Direct comparison of efficacy and side effect profiles is complex. The aim of this study is to investigate the different AE profiles of MAOBI and COMTI combination therapies. Data used to analyze the AEs of different PD medications were retrieved from “The Boston University Medical Center’s Parkinson’s Disease and Movement Disorder Database”. Ten categories of AEs were compared between patients receiving MAOBI and COMTI combination treatment. In total, 87 subjects were included in the analysis. Out of ten AEs, the presence of dementia was signifi- cantly different between the MAOBI and COMTI groups with an OR of 6.9 (COMTI vs MAOBI, 95% CI 1.3 - 37.0). Motor fluctuations were also found to be differently distributed in the two medication groups with an OR of 3.1 (COMTI vs MAOBI, 95% CI 1.0 - 9.8). In this retrospective database analysis of patients treated with combination treatment for PD, combination therapy of a COMTI with levodopa was more likely to be associated with dementia and motor fluctuations than a MAOBI with levodopa.

Objective: To assess the prevalence of Parkinson’s disease (PD) among African- Americans compared to non-Hispanic Whites in a tertiary urban Movement Disorders center. Background: Contributing factors in the prevalence of Parkinson’s disease (PD) based on race may improve health disparities in the United States. According to the largest study analyzing prevalence and incidence of PD among Medicare beneficiaries, the ratio of African-Americans (AA) to Caucasians in the general Medicare population is 9%, while the prevalence ratio of PD among AA compared to Caucasians is approximately 5%. Methods: We performed a retrospective analysis of African-American patients with PD in the movement disorders database at Boston Medical Center, which is the largest safety net hospital in New England. The demographics of the patients seen in the general neurology clinic are 30.57% AA and 38.06% non-Hispanic Caucasian. Results: The Movement disorders database included 488 patients, 327 of whom had PD. Of these, 287 were Caucasians and 18 were of African descent. Based on our clinic population the expected PD case ratio among AA compared to Caucasians was approximately 49%. The actual race ratio in patients of African descent compared to Caucasian was 6%, which was substantially lower than expected at Boston Medical Center. Conclusion: The demographics of the PD patients in our movement disorders clinic do not reflect the population at Boston Medical Center, or the demographics of our neurology clinic as a whole.

Contribution to the Full 3D Finite Element Modelling of a Hybrid Stepping Motor with and without Current in the Coils

The paper presents our contribution to the full 3D finite element modelling of a hybrid stepping motor using COMSOL Multiphysics software. This type of four-phase motor has a permanent magnet interposed between the two identical and coaxial half stators. The calculation of the field with or without current in the windings (respectively with or without permanent magnet) is done using a mixed formulation with strong coupling. In addition, the local high saturation of the ferromagnetic material and the radial and axial components of the magnetic flux are taken into account. The results obtained make it possible to clearly observe, as a function of the intensity of the bus current or the remanent induction, the saturation zones, the lines, the orientations and the magnetic flux densities. 3D finite element modelling provide more accurate numerical data on the magnetic field through multiphysics analysis. This analysis considers the actual operating conditions and leads to the design of an optimized machine structure, with or without current in the windings and/or permanent magnet.

Evaluation of the Quality of Life of Patients Suffering from Chronic Rhinosinusitis in a Hospital Setting in Kinshasa

Background and objective: Chronic rhinosinusitis (CRS), a common disease worldwide, has a major impact on patients’ quality of life (QoL). In recent years, the measurement of health-related QoL has made it possible to assess the patient’s state of health, the severity of the CRS and the treatment. The aim of this study was to assess the QoL of patients with CRS in a hospital setting in Kinshasa. Methods: This was a cross-sectional analytical study conducted from June 2020 to May 2021 in the ENT service of the Kinshasa University Hospital and the Monkole Hospital Center. The study involved 113 patients aged at least 18 years, whose QOL was compared with that of a control group consisting of 100 non-patients. QOL was assessed using the Sino-Nasal Outcome Test (SNOT-22) Questionnaire. Results: The mean age of the patients was 41.0 years (18 - 74 years) and that of the controls was 39.1 years (19 - 77 years). More than half of the participants were university graduates. The median SNOT-22 score was 44 (14 - 78) in patients and 7 (1 - 40) in controls. Compared with subjects without CRS, patients with CRS had significantly higher scores in all four domains of the SNOT-22. Around 9 out of 10 patients had a severely impaired QoL compared with 2 out of 10 controls. Considering mildly impaired QoL, we found that in the otological and sleep domains, scores were not significantly different between patients and controls. In the case of severely impaired QoL, however, only the otological domain showed a non-significant difference in scores between the two groups. Conclusion: CRS is a real public health problem in our environment and significantly alters the QoL of patients suffering from it, with repercussions on their professional productivity;hence the need for better management.

The rate of progression of Parkinson’s Disease (PD) varies among individuals living with the disease. How- ever, there comes a time, later in the course of the disease, when most patients with PD develop balance impair- ments and falls. Unfortunately, these symptoms do not respond well to dopaminergic medications or deep brain stimulation.

This article on intentionality and temporality shows how Husserl remains indebted to Brentano for his conception of time.Since,it is through the Brentanian conception of the intimate consciousness of time that Husserl was able to discover how immanent time and transcendent time are distinguished.The article shows how Husserl,by taking advantage of this distinction between immanent and transcendent time established by Brentano,thought he had discovered time as an object that is within consciousness and not outside it.However,this article makes it clear that Husserl denies the power of the imagination,to which Brentano had recourse,to continually make possible the modifications that arise through the moments of transcendent time.This denial for Husserl shows how Brentano ignored both the intimate consciousness of time and the imagination.Consequently,the article underlines the fact that if for Husserl the intimate consciousness of time discovers time within oneself,the major stake of Merleau-Ponty’s criticism of Husserl consists rather in discovering time in the exteriority to oneself of the consciousness which cannot have a hold on time within oneself.Since it is in the movement that opens the consciousness on itself that time is discovered outside the consciousness and in the consciousness.This is how Merleau-Ponty understands the movement where the consciousness that accomplishes its passage towards itself accomplishes it by the retention that unceasingly opens on the protention.

This article shows,in Michel Henry,the relationship that Descartes’cogito maintains with phenomenology and Christianity.Thus,this article first shows to see how the phenomenology of life is only possible through a reading of Descartes’cogito.For,in it is in the cogito that one discovers how the life of the ego relates the ego to itself by excluding from itself what is outside of itself in the reduction in which this ego is constituted.The article then observes that it is from the way in which the ego is constituted in its most radical immanence that Michel shows how Husserl confused the immanence of the ego with the transcendence which lies outside of the ego.The article finally explains how the ego which discovers itself in pure immanence is the institute of a phenomenology of life which remains the mode of being of the philosophy of Christianity.

Sleep Disturbance in Parkinson’s Disease Varies with Age of Onset and Family History

Introduction: Parkinson’s disease (PD) is a progressive neurodegenerative disease more common in those over the age of 60. PD is classically characterized by motor features, although patients may also experience non-motor symptoms. Sleep disturbances, such as rapid eye movement (REM) behavior disorder (RBD), are common in patients with PD and may precede onset of PD. Methods: Data was collected on patients with PD (358 subjects)in a movement disorders clinic at a safety net hospital. In this retrospective database analysis, the association of PD complications with age of onset was evaluated using chi-square tests and logistic regression. Results: Of the PD complications analyzed, there was a significant difference in sleep disturbances by age. Among the 358 PD patients, 120 individuals (33.5%) had information regarding the presence or absence of sleep disturbances. There was a significant difference between the early (onset < 50) and later onset (≥50) groups (p = 0.03) with the odds of having a sleep disorder for the early group 1.6 times that of the late group. Those subjects with siblings who also had PD had 2.0 times the odds of having a sleep disorder compared those without (p = 0.02). Conclusion: Non-motor symptoms such as sleep disorders are a useful predictor of early onset PD. Genetic components of PD impact both motor and non-motor aspects of the disease.

Exploring Gender-Associated Socioeconomic Differences in Parkinson’s Disease

Background: Research on gender in Parkinson’s disease (PD) frequently focuses on clinical differences between men and women. Socioeconomic factors such as race, ethnicity, insurance type, and level of educational attainment, have not been extensively examined in relation to gender differences in PD. The goal of this study was to identify differences in PD presentation in men and women, and identify socioeconomic factors that may confound such differences. Methods: A movement disorder patient database containing 445 patients with idiopathic PD was analyzed for gender differences in motor symptoms and disease complications using linear or logistic regression. Socioeconomic variables were then evaluated as possible confounders. Results: A greater proportion of women were non-white (p < 0.05). Univariate analysis of gender, and multivariate analysis controlling for age at diagnosis and socioeconomic factors were concordant in demonstrating increased frequency of motor fluctuations and dyskinesia in women (p < 0.05). Controlled, multivariate analysis, when compared to univariate analysis, uniquely demonstrated that women were less likely to experience dementia and autonomic dysfunction, relative to men (p < 0.05). Conclusion: Women with PD are susceptible to motor fluctuations and dyskinesia, but may be relatively protected against dementia and autonomic dysfunction. Women and men with PD in our population had different socioeconomic profiles, which may have confounded some gender-associated differences.
