Outcomes of Hemodialysis Patients in Public Centres in Abidjan from September 2018 to October 2021: Data from the Renal Registry of Cote d’Ivoire

Background: Hemodialysis is the most widely used renal replacement therapy in developing countries such as Cote d’Ivoire. Objective: To study the outcome of chronic hemodialysis patients in public centres in Abidjan using data from the Renal Registry of Cote d’Ivoire (2RCI). Method: This was a prospective analytical cohort study of End Stage Kidney Disease (ESKD) patients undergoing iterative hemodialysis in the network of public hemodialysis centres in Abidjan and who had been registered in the 2RCI register by September 2018. Results: Our study involved 363 cases out of 379 patients registered in the 2RCI registry database in 2018. The mean age of the patients was 47.3 ± 12.1 years, with a male predominance (sex ratio 1.97). In 70.24% of cases, they had no health insurance. The average duration of dialysis for our patients was 6.94 ± 4.1 years, with extremes of 2 and 28 years. This duration was less than 5 years in 44.3% of cases, between 6 and 10 years in 42.9% and more than 10 years in 12.6% of cases. The comorbidities found were hypertension in 95.6% of cases, diabetes in 7.8% and HIV in 4.6%. Anemia was found in 68% of cases and stroke in 8.6%. During the study period, 161 deaths were observed, representing a mortality rate of 44.35%. Acute lung oedema (29.8%), stroke (6.8%) and other cardiovascular diseases (19.3%) were the main causes of death. In multivariate analysis, factors such as age ≥ 65 years (HR = 3.66;CI 95% = 1.55 - 8.67;p = 0.003), “married/coupled” status (HR = 2.02;CI 95% = 1.24 - 3.31;p = 0.005) and normal weight at the start of dialysis (OR = 9.59, CI95% = 4.19 - 21.95;p = 0.001) were associated with the risk of death. Conclusion: Hemodialysis is performed in Abidjan on young patients. The mortality rate after three years of dialysis is very high. Hence the need to pursue the policy of decentralizing public centres and optimizing access to dialysis in terms of quality and quantity in order to improve patient survival.

Epidemiological, Diagnostic, Therapeutic, and Prognostic Profile of Non-Tuberculous Community-Acquired Purulent Pleurisy in Children at the Bouaké University Hospital Center, 2017-2021

Introduction: In Côte d’Ivoire, there is a scarcity of data on children’s purulent pleurisies. Objective: This study aims to elucidate the epidemiological, diagnostic, therapeutic, and evolutionary facets of non-tuberculous purulent pleurisies in pediatric patients. Methods: A retrospective analysis was conducted using the medical records of children aged one month to fifteen years with purulent pleurisies at Bouaké University Hospital Center from January 2017 to December 2021. Results: The study identified 124 cases of purulent pleurisies, constituting 18% of lower respiratory tract infections and 0.8% of all hospitalizations. The majority of these cases (69%) were in children between 1 and 24 months of age. Prominent symptoms included dyspnea (85.5%), O2 saturation below 95% in room air (76.6%), respiratory distress (68.5%), cutaneous-mucosal pallor (63.7%), and fever (43.5%). Radiological findings predominantly showed right-sided pleurisy (62.1%). The pleurisy was often extensive (78.2%), accompanied by pneumothorax (37.1%), alveolo-interstitial opacities (8.1% of cases), and abscess formations (1.6%). Pleural fluid cultures were positive in 46.9% of cases, with Staphylococcus aureus (75%, methi-S) identified among 32 bacteria. Initial antibiotic treatment was empirical, favoring oxacillin (53.2%) or amoxicillin-clavulanic acid (53.2%) in dual (42%) or triple therapy (33%) with gentamicin (64.1%) and/or metronidazole (21.8%). Treatments also included pleural drainage (68.5%) or repeated evacuation punctures (33.1%), and blood transfusion (39%). The mortality rate was 18.8%. Conclusion: Non-tuberculous purulent pleurisy remains a significant concern in pediatric hospitalizations at the CHU of Bouaké, marked by high mortality rates.

Application of Sigma Metric Analysis to Evaluate the Performance of the Biochemistry Analytical System in a Medical Biology Laboratory in Côte d’Ivoire

Introduction: The Six Sigma methodology is an opportunity for a better understanding of the performance of analytical methods and for a better adaptation of the quality control management policy of the medical biology laboratory. Using the sigma metric, this study assessed the performance of the Biochemistry analytical system of a medical biology laboratory in Côte d'Ivoire. Methods: Six Sigma methodology was applied to 3 analytes (alanine aminotransferase, glucose and creatinine). Performance indicators such as measurement imprecision and bias were determined based on the results of internal and external quality controls. The sigma number was calculated using the total allowable error values proposed by Ricos et al. Results: For both control levels, ALT had a sigma number greater than 6 (7.6 for normal control and 7.9 for pathological control). However, low sigma numbers, less than or equal to 2 for creatinine (1.4 for normal control and 2 for pathological control) and less than 1 for glucose were found. Conclusion: This study revealed good analytical performance of ALT from the point of view of 6 sigma analysis. However, modifications to the overall quality control procedure for glucose and creatinine are needed to improve their analytical performance. The study should be extended to the entire laboratory’s analytes in order to modify the strategies of quality control procedures based on metric analysis for an overall improvement in analytical performance.

We analyse the Diophantine equation of Fermat xp yp = zp with p > 2 a prime, x, y, z positive nonzero integers. We consider the hypothetical solution (a, b, c) of previous equation. We use Fermat main divisors, Diophantine remainders of (a, b, c), an asymptotic approach based on Balzano Weierstrass Analysis Theorem as tools. We construct convergent infinite sequences and establish asymptotic results including the following surprising one. If z y = 1 then there exists a tight bound N such that, for all prime exponents p > N , we have xp yp zp.

Epidemiological, Diagnostic, Therapeutic and Progressive Aspects of Arterial Hypertension in Children at the University Hospital Center of Bouaké (Côte d’Ivoire)

Introduction: Arterial hypertension (AH) in children is under-diagnosed and often has a poor prognosis. The aim of this study was to describe the epidemiological, diagnostic, therapeutic and evolutionary aspects of hypertension in children at the University Hospital of Bouaké, with a view to improving the prognosis. Methods: This was a cross-sectional, analytical study carried out in the paediatrics department of Bouaké University Hospital. It concerned the medical records of children aged 4 to 15 hospitalised from 1 January 2017 to 31 December 2020 for hypertension. Diagnosis was based on the simplified blood pressure guidelines of the Expert Consensus of the French Society of Hypertension. The variables studied were epidemiological, diagnostic, therapeutic and evolutionary. Quantitative variables were compared at the significance level p ≤ 0.05. Results: The hospital incidence of hypertension was 0.32% (69/21,642). The sex ratio was 0.72. 97.1% of the children were over five years of age. Oedema (49.3%) and breathing difficulties (20.2%) were the main reasons for consultation. Hypertension was classified as threatening (56.5%), confirmed (31.9%) and borderline (11.6%). The cause was renal in 66.7%, dominated by impure nephrotic syndrome (24.6%). Treatment for hypertension consisted mainly of a diuretic (79.7%) and a calcium channel blocker (47.8%). Outcome was favourable in 50.7% of cases. Mortality was 20.3%. No factor was significantly associated with death. Conclusion: Hypertension in children at Bouaké University Hospital is serious. The aetiology is mainly renal. Early diagnosis and management are key to improving prognosis.

Epidemiological, Clinical Progress Aspects of Congenital Heart Disease with Neonatal Revelation at the Mother-Child Hospital of Bingerville (HME) Concerning 98 Cases from January 2021 to December 2022 (Côte d’Ivoire)

Introduction The distribution of congenital heart disease (CHD) in sub-Saharan Africa is highly imprecise and varies from one region to another due to the inequality of diagnostic facilities. The aim of this study was to determine the in-hospital prevalence of congenital heart disease in children at the Mother-Child hospital of Bingerville (HME) by specifying the diagnostic, therapeutic and evolutionary aspects. Materials and methods We conducted a retrospective, descriptive, cross-sectional study at HME of Bingerville from January 2021 to December 2022. All newborns with congenital heart disease confirmed by echocardiography were included in the study. Results Of 656 admissions to the neonatology department over the study period, congenital heart disease accounted for 14.9% (98/656) of cases. In our series, 76.7% were diagnosed before the 1st week of life, with a mean chronological age of 5.18 days and extremes of 0 and 46 days. There were as many male patients (50%) as female (50%), i.e. a sex ratio of 1. These newborns were premature in 60.2% of cases, with a mean and median gestational age of 34 weeks’ amenorrhea. Most were left-right shunts (90.8%). Persistent ductus arteriosus (PDA) (48.9%) predominated, followed by atrial septal defect (38.7%), ventricular septal defect (13.3%), common trunk artery (CTA) (3.1%) and open septal pulmonary atresia (OSPA) (1%) as the primary cyanogenic heart disease. Pulmonary arterial hypertension (PAH) (50%) was primary in 38.8% and secondary (61.2%). The mortality rate was 30.6%, and all CTA patients died (100%), with a significant statistical relationship (p = 0.027). Progression under treatment was marked by clinical stabilization (68/98) in 69.4% of cases. Conclusion: Congenital heart disease is relatively common at the Bingerville HME. Access to echocardiography should be facilitated in neonatology departments for rapid diagnosis and optimal management of congenital heart disease in newborns.

The development of artificial intelligence (AI), particularly deep learning, has made it possible to accelerate and improve the processing of data collected in different fields (commerce, medicine, surveillance or security, agriculture, etc.). Most related works use open source consistent image databases. This is the case for ImageNet reference data such as coco data, IP102, CIFAR-10, STL-10 and many others with variability representatives. The consistency of its images contributes to the spectacular results observed in its fields with deep learning. The application of deep learning which is making its debut in geology does not, to our knowledge, include a database of microscopic images of thin sections of open source rock minerals. In this paper, we evaluate three optimizers under the AlexNet architecture to check whether our acquired mineral images have object features or patterns that are clear and distinct to be extracted by a neural network. These are thin sections of magmatic rocks (biotite and 2-mica granite, granodiorite, simple granite, dolerite, charnokite and gabbros, etc.) which served as support. We use two hyper-parameters: the number of epochs to perform complete rounds on the entire data set and the “learning rate” to indicate how quickly the weights in the network will be modified during optimization. Using Transfer Learning, the three (3) optimizers all based on the gradient descent methods of Stochastic Momentum Gradient Descent (sgdm), Root Mean Square Propagation (RMSprop) algorithm and Adaptive Estimation of moment (Adam) achieved better performance. The recorded results indicate that the Momentum optimizer achieved the best scores respectively of 96.2% with a learning step set to 10−3 for a fixed choice of 350 epochs during this variation and 96, 7% over 300 epochs for the same value of the learning step. This performance is expected to provide excellent insight into image quality for future studies. Then they participate in the development of

In Côte d’Ivoire, the recurring and unregulated use of bushfires, which cause ecological damage, presents a pressing concern for the custodians of protected areas. This study aims to enhance our comprehension of the dynamics of burnt areas within the Abokouamékro Wildlife Reserve (AWR) by employing the analysis of spectral indices derived from satellite imagery. The research methodology began with the calculation of mean indices and their corresponding spectral sub-indices, including NDVI, SAVI, NDWI, NDMI, BAI, NBR, TCW, TCG, and TCB, utilizing data from the Sentinel-2A satellite image dated January 17, 2022. Subsequently, a fuzzy classification model was applied to these various indices and sub-indices, guided by the degree of membership α, with the goal of effectively distinguishing between burned and unburned areas. Following the classification, the accuracies of the classified indices and sub-indices were validated using the coordinates of 100 data points collected within the AWR through GPS technology. The results revealed that the overall accuracy of all indices and sub-indices declines as the degree of membership α decreases from 1 to 0. Among the mean spectral indices, NDVI-mean, SAVI-mean, NDMI-mean exhibited the highest overall accuracies, achieving 97%, 95%, and 90%, respectively. These results closely mirrored those obtained by sub-indices using band 8 (NDVI-B8, SAVI-B8, and NDMI-B8), which yield respective overall accuracies of 93%, 92%, and 89%. At a degree of membership α = 1, the estimated burned areas for the most effective indices encompassed 2144.38 hectares for NDVI-mean, 1932.14 hectares for mean SAVI-mean, and 4947.13 hectares for mean NDMI-mean. A prospective approach involving the amalgamation of these three indices could have the potential to yield improved outcomes. This study could be a substantial contribution to the discrimination of bushfires in Côte d’Ivoire.

In this paper we prove in a new way, the well known result, that Fermat’s equation a4 + b4 = c4, is not solvable in ℕ , when abc≠0 . To show this result, it suffices to prove that: ( F 0 ): a 1 4 + ( 2 s b 1 ) 4 = c 1 4 , is not solvable in ℕ , (where a 1 , b 1 , c 1 ∈2ℕ+1 , pairwise primes, with necessarly 2≤s∈ℕ ). The key idea of our proof is to show that if (F0) holds, then there exist α 2 , β 2 , γ 2 ∈2ℕ+1 , such that ( F 1 ): α 2 4 + ( 2 s−1 β 2 ) 4 = γ 2 4 , holds too. From where, one conclude that it is not possible, because if we choose the quantity 2 ≤ s, as minimal in value among all the solutions of ( F 0 ) , then ( α 2 ,2 s−1 β 2 , γ 2 ) is also a solution of Fermat’s type, but with 2≤s−10) is solvable in ( a 1 ,2 s b 1 , c 1 ) , s ≥ 2 like above;afterwards, proceeding with “Pythagorician divisors”, we creat the notions of “Fermat’s b-absolute divisors”: ( d b , d ′ b ) which it uses hereafter. In Côte d’Ivoire, the decline in soil fertility strongly impacts the productivity of maize (Zea mays L.) on heavily leached ferralitic soil. In this study, the general objective was therefore to improve the productivity of maize EV87-28 on the Ferralsols in pre-forested areas during different cropping seasons. Eight (8) micro-plots were set up according to a total randomization device with three repetitions. Two factors were studied: nitrogen fertilizer modalities (main factor) and crop season (secondary factor). Growth, flowering and yield parameters were measured and analyzed. The results showed that there was no interaction between the nitrogen fertilizer factor and the cropping season factor. In addition, this study showed the short rainy season had the most positive impact on growth, flowering and yield parameters than the long rainy season. The results also showed that the different nitrogen fertilizer modalities had no statistically different effects on growth, flowering and yield parameters. However, quantitative differences were reported, highlighting one nitrogen fertilizer modality, which is the combination of urea granule + farm manure (75% urea indorama granules and 25% farm manure). The combination of urea granule + farm manure (75% urea indorama granules and 25% farm manure) had the best effect on corn grain yield. So, the combination of urea (75%) and manure (25%), that resulted in yield gain, could be recommended for corn fertilization during the small rainy season.
