Survival of Antiretroviral Therapy Patients at the Outpatient Treatment Centre of the Community University Hospital of Bangui from 2015 to 2020

Background: The advent of antiretroviral treatment has considerably increased the life expectancy of people living with HIV in recent years. The Central African Republic, despite challenges related to the socio-political context, is committed to achieving the UNAIDS 90-90-90 targets and to the elimination of AIDS by 2030 advocated by the WHO. Objective: To analyze survival among HIV-positive adult patients on antiretroviral therapy from 2015 to 2020. Methods: This was a history-based cohort study of patients started on ART. The main variable was survival. The Kaplan-Meier method was used to describe the survival curve since inclusion in the cohort and a multivariate Cox model was used to investigate factors associated with mortality on ART. Results: A total of 145 naive patients started ART at the Outpatient Treatment Centre (OTC) in 2015. A female predominance was observed in our study with 78.08% of cases. The analysis of the patients’ fate at the time of point of treatment showed that 58.62% of them were still in active care and 13.10% died. The probability of survival at 5 years was 0.82 and mortality was significantly associated with very advanced disease (WHO stage IV) (p = 0.047) and anemia (p = 0.039). Conclusion: The majority of patients were still in care at the endpoint and mortality was significantly related to advanced disease and anemia. Early management of people living with HIV combined with better quality of care would improve their survival.

Delineation of Lineaments in South Cameroon (Central Africa) Using Gravity Data

Gravity method is among the most applied geophysical methods in mineral and oil exploration. It may help to identify fault networks which are of interest for mineral exploration. Potential field data can give valuable information on the location of faults in the basement. These faults may have propagated into the overlying sedimentary rocks and influenced fluid flow and distribution of hydrocarbon traps and mineralization zones. A study was therefore conducted in south Cameroon with the aim of highlighting the different lineaments of the region, which were completely or partially hidden by the sedimentary cover. Different gravity data processing techniques including horizontal gradient coupled with upward continuation and Euler deconvolution were used. The application of these methods has mapped out a number of lineaments depicting gravity density discontinuities whose directions are NS, NE-SW, EW and NW-SE. The predominant direction for major lineaments is NE-SW. The major lineaments associated to the faults are: the Kribi-Edea faults, Ambam faults, Ebolowa-south of Yaounde faults;Bipindi-Yaounde faults;Pouma-Yaounde fault and the fault system which crosses the east, north, west of Monatele city. Euler solutions indicate depths up to18 km for the roof of the faults. The main results worked out from this study provide with new elements that allow the improvement of the knowledge on the structure of the study area. The structural map obtained shows major tectonic events that are responsible of the structural layout of the study zone. In addition, information related to the dip and depth of the various structures was also obtained. The map of lineaments is a useful tool for the planning of hydrogeological and/or petroleum investigations.

Background:This study aimed to investigate renal replacement therapy (RRT) practices in a representative nationwide sample of French intensive care units (ICUs).Methods:From July 1 to October 5 2021, 67 French ICUs provided data regarding their ICU and RRT implementation. We used an online questionnaire to record general data about each participating ICU, including the type of hospital, number of beds, staff ratios, and RRT implementation. Each center then prospectively recorded RRT parameters from 5 consecutive acute kidney injury (AKI) patients, namely the indication, type of dialysis catheter used, type of catheter lock used, type of RRT (continuous or intermittent), the RRT parameters initially prescribed (dose, blood flow, and duration), and the anticoagulant agent used for the ***:A total of 303 patients from 67 ICUs were analyzed. Main indications for RRT were oligo-anuria (57.4%), metabolic acidosis (52.1%), and increased plasma urea levels (47.9%). The commonest insertion site was the right internal jugular (45.2%). In 71.0% of cases, the dialysis catheter was inserted by a resident. Ultrasound guidance was used in 97.0% and isovolumic connection in 90.1%. Citrate, unfractionated heparin, and saline were used as catheter locks in 46.9%, 24.1%, and 21.1% of cases, ***:Practices in French ICUs are largely compliant with current national guidelines and international literature. The findings should be interpreted in light of the limitations inherent to this type of study.

There is a considerable interest in producing and understanding the optical and spectroscopic properties of ordered nanoparticle assemblies. Herein, we describe and interpret the optical absorbance and Raman properties of 5.9 nm ± 0.3 nm diameter silver nanocrystals coated with dodecanethiol and organized in highly ordered 3D superlattices of different heights. Each superlattice was studied individually, which allowed to elaborate a model based on Maxwell-Garnett theory to reproduce qualitatively the height and wavelength dependence of the absorbance. Importantly, because of their small size compared to that of traditional nanoparticles used in Surface Enhanced Raman Spectroscopy （SERS）, the large 3D distribution of hot spots generated by the silver superlattices allowed to easily obtain SERS spectra of the surrounding ligands despite their intrinsic low Raman cross section. Accordingly, traces of thiophenol could be detected very easily.

The mammalian TOR（mTOR） and its orthologues are central regulators of cell growth, proliferation, and differentiation, and have been implicated in many diseases including cancers and diabetes. Amino acid levels are signaled to TORC1 via the EGO complex in yeast or the structurally orthologous ragulator complex in mammalian cells. The yeast EGO complex, consisting of Gtr1, Gtr2, Ego1, and Ego3, localizes to the endosomal and vacuolar membranes and plays a pivotal role in cell growth and autophagy regulation through relaying amino acid signals to activate TORC1. Depletion of any of the EGOC components leads to failure in recovery from rapamycin-induced growth arrest and causes low TORC1 activity, indicating that these proteins play an important role in cell growth regulation upstream of TORC1. In line with this assumption, GTP-bound Gtr1 combines with GDP-bound Gtr2 to interact with and activate TORC1 via a still elusive mechanism. Analogous to the role of p18 in the Ragulator complex in higher eukaryotes, the N-myristoylated Ego1 may serve to properly tether Gtr1-Gtr2 to the vacuolar/lysosomal membrane for proper TORC1 activation, while the role of Ego3 in this process is still poorly understood. Here we report the crystal structures of a wild-type and a mutant form of S. cerevisiae Ego3. Ego3 assumes a homodimeric structure similar to that of the mammalian MP1-p14 heterodimer and the C-terminal domains of the yeast Gtr1-Gtr2 heterodimer, both of which function in TORC1 signaling. Structural and genetic data demonstrate that the unique dimer conformation of Ego3 is essential for the integrity and function of the EGO complex. Structural and functional data also identify a potential binding site for Gtr1-Gtr2. These results suggest a structural conservation of the protein components involved in amino acid signaling to TORC1 and reveal structural insights into the molecular mechanism of Ego3 function in TORC1 signaling.

Evaluation of Oxidative Status of Patients with Gout Attack in a Cameroonian Urban Hospital

Background: Gout is the leading cause of microcrystalline arthritis worldwide and the most common cause of arthritis in adult men. The prevalence of cardiovascular disease, type II diabetes and metabolic syndrome is high among gout patients;one of the current hypotheses that may explain this relationship is based on oxidative stress;however, few studies have sought to investigate this relationship. Objective: The aim of this study was to determine the oxidative status of patients with gout attack. Methods: We conducted a case-control study over a 4 months period at the Rheumatology service of the Yaoundé Central Hospital. Cases were patients with an acute gout attack and controls were healthy subjects matched for sex and age. We evaluated uricemia, serum levels of Superoxide Dismutase (SOD), Glutathione Peroxidase (GPx), Catalase and Malondialdehyde (MDA). Oxidative stress was defined by an increase in MDA and/or a decrease in SOD, Catalase and GPx. Statistical analysis was performed by the S.P.S.S. 21.0 software. The Student’s T-test was used to compare means;the significance threshold was 0.05. Results: Sixty subjects were recruited of which 30 with an acute gout attack and 30 healthy subjects. The mean ages of the cases and controls were respectively 58 ± 8 years and 57.6 ± 8 years. Uricemia was statistically higher in cases (81 ± 20 mg/L) compared to controls (47 ± 11 mg/L) (p < 0.001). MDA levels were higher among cases (1.37 ± 0.46 mmol/L) compared to controls (1.14 ± 0.39 mmol/L) (p < 0.05). There was no significant difference in serum levels of catalase, SOD and GPx between cases and controls. Conclusion: Patients with gout attack have an elevated serum level of malondialdehyde but their oxidative status seems similar to that of normal individuals.

Outcome of delivery in nulliparous teenagers aged less than 17 years: the cameroon university centre hospital experience

In a previous study at the Yaoundé-Cameroon University Centre Hospital, we found that delivery at extreme ages of reproductive health was associated with several fetal and maternal risks. We conducted this study aimed at testing the hypothesis that deliveries by teenagers were associated with increased risk. Data collection was retrospective from delivery room registers at between 01/01/1996 and 31/12/2000. Data were collected in singleton pregnancies. The identification of each nulliparous woman aged less than 17 years was followed by that of 4 nulliparous women aged 20 - 29 years following her in the order of delivery. Since it was possible to have consecutive nulliparous teenage pregnancies before four deliveries in women in their twenties, we finally compared 65 cases of teenagers delivery with 218 cases of delivery in women aged 20 - 29 years. The difference was considered significant if p value was less than 0.05. Odd ratio with 95% confidence interval was used to measure the effect of age on the major outcomes. At univariate analysis, the risk of delivery by caesarean section for teenagers (21.5%) was significantly high compared to women in their twenties (11.5%), (OR: 2.1;95%CI: 1.0 - 4.1;p = 0.041). Similar observation was found for stillbirth delivery with 12.3% by teenagers compared to 5.0% by women in their twenties (OR: 2.7;95%CI: 1.0 - 7.1;p = 0.044). The risk of premature delivery by teenagers (30.8%) was also significantly increased compared to women in their twenties (12.4%) (OR: 3.6;95%CI: 1.8 - 7.1;p = 0.0002). After adjustment for different factors associated with the occurrence of several risks, only pre term delivery remained significantly increased at teenage (OR: 3.7;95%CI: 1.8 - 7.6;p = 0.003). These findings underscore the importance of public health programs in preventing the pregnancy in teenagers, and in developing age-appropriate intervention.

Impact of Ionic Liquid 1-Ethyl-3-Methylimidazolium Acetate Mediated Extraction on Lignin Features

This study aims at investigating the impact of ionic liquid extraction on lignin structure by studying the mechanism of lignin depolymerization in 1-ethyl-3-methylimidazolium acetate EMIM[OAc]) and comparing it with that of organosolv and milled wood methods. Ionic liquid mediated lignin (ILL) using EMIM[OAc]), ethanol organosolv lignin (EOL) and milled wood lignin (MWL) were isolated from Typha capensis (TC) and subjected to several analytical characterizations. Experimental data shows that ILL exhibited a relatively lower degree of condensation, lower aromatic C-C structures and a higher aliphatic OH with values of 0.42/Ar, 1.94/Ar and 1.33/Ar moieties compared with EOL values of 0.92/Ar, 2.22/Ar and 0.51/Ar moieties respectively. The ILL was depolymerized under mild conditions giving relatively higher β-aryl ether linkages content, higher molecular mass, and exhibited closer structures and reactivity to native lignin than EOL. These insights on TC lignin depolymerization in EMIM[OAc]) acetate may contribute to better value-addition of lignocellulosic biomass.
