Epidemiological, Diagnostic, Therapeutic, and Prognostic Profile of Non-Tuberculous Community-Acquired Purulent Pleurisy in Children at the Bouaké University Hospital Center, 2017-2021

Introduction: In Côte d’Ivoire, there is a scarcity of data on children’s purulent pleurisies. Objective: This study aims to elucidate the epidemiological, diagnostic, therapeutic, and evolutionary facets of non-tuberculous purulent pleurisies in pediatric patients. Methods: A retrospective analysis was conducted using the medical records of children aged one month to fifteen years with purulent pleurisies at Bouaké University Hospital Center from January 2017 to December 2021. Results: The study identified 124 cases of purulent pleurisies, constituting 18% of lower respiratory tract infections and 0.8% of all hospitalizations. The majority of these cases (69%) were in children between 1 and 24 months of age. Prominent symptoms included dyspnea (85.5%), O2 saturation below 95% in room air (76.6%), respiratory distress (68.5%), cutaneous-mucosal pallor (63.7%), and fever (43.5%). Radiological findings predominantly showed right-sided pleurisy (62.1%). The pleurisy was often extensive (78.2%), accompanied by pneumothorax (37.1%), alveolo-interstitial opacities (8.1% of cases), and abscess formations (1.6%). Pleural fluid cultures were positive in 46.9% of cases, with Staphylococcus aureus (75%, methi-S) identified among 32 bacteria. Initial antibiotic treatment was empirical, favoring oxacillin (53.2%) or amoxicillin-clavulanic acid (53.2%) in dual (42%) or triple therapy (33%) with gentamicin (64.1%) and/or metronidazole (21.8%). Treatments also included pleural drainage (68.5%) or repeated evacuation punctures (33.1%), and blood transfusion (39%). The mortality rate was 18.8%. Conclusion: Non-tuberculous purulent pleurisy remains a significant concern in pediatric hospitalizations at the CHU of Bouaké, marked by high mortality rates.

The search for low glycemic rice to manage type 2 diabetes is of utmost importance due to the increasing trend in diabetic cases and the importance of rice as a staple worldwide.This study aimed to investigate the effect of combinations of rice varieties and processing regimes on human glycemic index(GI),and elucidate the relationship between physicochemical properties of rice and human GI.Parboiled polished rice recorded the lowest human GI.Parboiled unpolished rice.

The genetic control of glabrous glume during African rice domestication

African cultivated rice,Oryza glaberrima,is characterized by its glabrous glumes.During domestication,the pubescent glumes of its wild ancestor,Oryza barthii,lost their trichomes,and in this study,we show that glabrous glume 5(GLAG5),a WUSCHEL-like homeobox transcription factor gene on chromosome 5,is required for trichome development.DNA methylation associated with an hATtransposable element inserted in the promoter region of GLAG5 is found to reduce its expression,leading to the formation of glabrous glumes and leaves in African cultivated rice.Among 82 African cultivated rice varieties investigated in this study,59(approximately 71%)lines exhibit glabrous glumes and harbor the hAT transposon;however,the other 23 varieties(approximately 29%),which exhibit pubescent glumes,lack the hAT transposon,indicating that glag5 had undergone strong artificial selection.Theπ;/π;ratios also show the hAT transposon insertions influence the genetic diversity of an approximately 150-kb interval encompassing the GLAG5 locus.The identification of the GLAG5 gene provides new insights into the domestication of cultivated rice in Africa.We speculate that the selection of varieties with mutations in their promoter regions is an important aspect of crop domestication.

Over the past 70 years,the world has witnessed extraordinary growth in crop productivity,enabled by a suite of technological advances,including higher yielding crop varieties,improved farm management,synthetic agrochemicals,and agricultural mechanization.While this"Green Revolution"intensified crop production,and is credited with reducing famine and malnutrition,its benefits were accompanied by several undesirable collateral effects(Pingali,2012).These include a narrowing of agricultural biodiversity,stemming from increased monoculture and greater reliance on a smaller number of crops and crop varieties for the majority of our calories.This reduction in diversity has created vulnerabilities to pest and disease epidemics,climate variation,and ultimately to human health(Harlan,1972).

1.Introduction Vascular stents are endo-prostheses that allow the restoration of blood flow in case of obstruction of a vessel.A stent usually consists of a corrosion-resistant metal such as Nitinol,316L steel or a CoCr alloy[1].Many studies have been undertaken for the production of polymer degradable stents[2,3]or metallic stents[4,5]and,among these,devices made of pure zinc or crystallized alloy based on zinc obtained by molding and extrusion are promising in terms of biodegradability,biocompatibility and mechanical properties[6],but also concerning the degradation rate of the stent which should to be effective until arterial remodeling and tissue healing are complete[5].The objective of this preliminary study is the comparison of the degradation in simulated biological medium and the endothelial cytotoxicity of amorphous and crystallized thin ZnZr films according to their composition and structure.In this work,the thin layers were deposited on silicon substrates by magnetron co-sputtering.This technology is already used to make metal stents[7,8].

Isolated Mediastinal Lymphangioma: Prenatal Diagnosis and Thoracoscopic Treatment

Isolated mediastinal lymphangiomas are uncommon. We report a case of a 14 ×8 mm right paracardiac cyst diagnosed at 20 weeks’ gestation. The prenatal evolution was uneventful and a magnetic resonance imaging at 31 weeks showed the limited extension of the cyst into the anterior mediastinum. At birth, the baby was asymptomatic, but the size of the lesion increased steadily (48 ×29 mm). At 7 months of life, he underwent a thoracoscopic resection of the cyst without intra or postoperative complications. Histological examination showed a lymphangioma. This case is remarkable for its prenatal diagnosis, the thoracoscopic treatment and the 8 years of followup without recurrence.

Gender development and hepatitis B and C infections among pregnant women in Africa:a systematic review and metaanalysis

Background:Although Africa is a region of hyper endemicity to viral hepatitis B(HBV)and C(HCV)infections,there is limited data on their related burden among pregnant women.The present systematic review and meta-analysis aimed to determine the magnitude of these infections among pregnant women living in Africa and investigate its association with gender-related human development indicators.Main text:We searched PubMed,Embase,Web of Science,Africa Journal Online,and Global Index Medicus,with no language restriction,to identify observational studies on HBV and HCV infections in pregnant women residing in Africa published from January 1,2000 until December 31;2017.Eligible studies reported the prevalence of HBV and/or HCV infection(s)(HBs antigen and HCV antibodies)and/or infectivity(HBe antigen or detectable HCV viral load).Each study was independently reviewed for methodological quality.We used a random-effects model metaanalysis to pool studies.In total,145 studies(258251 participants,30 countries)were included,of which 120(82.8%)had a low,24(16.5%)a moderate,and one(0.7%)had a high risk of bias.The prevalence of HBV and HG/infections was 6.8%(95%confidence interval[CI]:6.1-7.6,113 studies)and 3.4%(95%CI:2.6-A.2,58 studies),respectively.The prevalence of HBe antigen and HCV detectable viral load was 18.9%(95%CI:14.4-23.9)and 62.3%(95%CI:51.6-72.5)in HBV positive and HCV positive pregnant women,respectively.The multivariable meta-regression analysis showed that the prevalence of HBV infection increased with decreasing gender development index,males,level of education and females'expected years of schooling.Furthermore,this prevalence was higher in rural areas and in western and central Africa.The prevalence of HCV infection increased with decreasing proportion of seats held by women in parliament.Conclusions:To address the burden of HBV and HCV infections,beyond well-known risk factors at the individual-level,macro-level factors including gender-related human development indicators and dwelling in rural areas shoul

Patients with inflammatory bowel disease(IBD)have an increased risk of 10%-15%developing colorectal cancer(CRC)that is a common disease of high economic costs in developed countries.The CRC has been increasing in recent years and its mortality rates are very high.Multiple biological and biochemical factors are responsible for the onset and progression of this pathology.Moreover,it appears absolutely necessary to investigate the environmental factors favoring the onset of CRC and the promotion of colonic health.The gut microflora,or microbiota,has an extensive diversity both quantitatively and qualitatively.In utero,the intestine of the mammalian fetus is sterile.At birth,the intestinal microbiota is acquired by ingesting maternal anal or vaginal organisms,ultimately developing into a stable community,with marked variations in microbial composition between individuals.The development of IBD is often associated with qualitative and quantitative disorders of the intestinal microbial flora(dysbiosis).The healthy human gut harbours about10 different bacterial species distributed in colony forming units which colonize the gastrointestinal tract.The intestinal microbiota plays a fundamental role in health and in the progression of diseases such as IBD and CRC.In healthy subjects,the main control of intestinal bacterial colonization occurs through gastric acidity but other factors such as endoluminal temperature,competition between different bacterial strains,peristalsis and drugs can influence the intestinal microenvironment.The microbiota exerts diverse physiological functions to include:growth inhibition of pathogenic microorganisms,synthesis of compounds useful for the trophism of colonic mucosa,regulation of intestinal lymphoid tissue and synthesis of amino acids.Furthermore,mucus seems to play an important role in protecting the intestinal mucosa and maintaining its integrity.Changes in the microbiota composition are mainly influenced by diet and age,as well as genetic factors.Increasing evidence indicates that dys

An Institutional Innovation for Agricultural Technology Adaptation and Adoption： Rice in West and Central Africa

This paper traces the origins and achievements of the Réseau Ouest et Centre Africain du Riz—-ROCARIZ (West and Central Africa Rice Research and Development Network). The ROCARIZ institutional arrangement showed that rice science is approached from multi‐stakeholder perspectives that place end users and entrepreneurs at the center of research for development, and collaboration among rice‐based stakeholders can be enhanced and sustained. The ROCARIZ used competitive research grants through multi‐stakeholder task forces to generate, adapt, and facilitate the adoption of rice‐based technologies and approaches by resource poor households. The grants contributed to: (1) farmer adoption of high yielding and multiple stress resilient lowland NERICAs (New Rices for Africa) (NERICA‐L); (2) producer access to credit and services resulting in increased marketing of quality seed rice, paddy, and grain rice by farmer entrepreneurs and poor rural households; (3) improved skills of stakeholders on the use of impact assessment tools to demonstrate adoption of rice‐based technologies; (4) enhanced skills of rice breeders in molecular assisted marker selection to shorten the varietal development process; and (5) experiential learning and sharing to improve access to information and knowledge.
