Risk Factors Recurrence of spontaneous Ascitic Fluid Infection (slai) in Cirrhotic Patients

Introduction: After an episode of spontaneous infection of ascitic fluid (IsLA). The recurrence of IsLA at one year is greater than 70%. We studied the risk factors associated with the occurrence of recurrence. Patients and methods: this was a retrospective, descriptive and analytical study of patient files, hospitalized in the department for 12 months, the choice of the sample was of convenience. Results: We have 1347 patient files collected including 389 cases of cirrhosis. We had 37 files of cirrhotic patients with IsLA including 28 cures without recurrence of IsLA, 08 files of patients with recurrence of IsLA and 03 excluded, i.e. a hospital prevalence of recurrence of 0.6% and a prevalence in cirrhotic patients of 23.5%. The most common antecedents were: hospital contact recent (35.3%), the concept of iterative ascites punctures (32.3%), the presence of HCC (29.4%), hepatic encephalopathy (20.6%) and digestive hemorrhage (14.7%). In univariate analysis, recent digestive bleeding was associated with an increased risk of recurrence (OR 7.2, 95% CI 0.96 - 67.1). HBV (62.5%) is the main etiology of cirrhosis. The PNN rate at 250 - 499 mm3 (62.5%), the protein level 3 (75%). Patients on secondary prophylaxis with NORFLOXACIN were 25%. Recurrence of IsLA was treated with CEFTRIAXONE 2 g/24 hours. Conclusion: Recurrence of IsLA is serious, the predictive factors for recurrence are, hospital contact recent, the concept of iterative ascites punctures, the presence of HCC, the presence of hepatic encephalopathy and digestive bleeding.

Introduction: Quality of life in Parkinson disease is not necessary linked to motor symptoms. It’s correlated of overactive bladders and prostatic symptoms. Prognosis factors of urinary quality of life are unknown. Objectives: Our study aims to find prognosis factors of quality of life associated specifically of urinary disorders in PD. Patients and methods: We conducted a transversal, analytic and descriptive study in Physical Medicine, and Neurology departments, Fann Teaching Hospital, Dakar and included patients followed for treatment of PD. urinary disorders, quality of life and functional autonomy were assessed respectively by urinary symptoms Profile (usP), Qualiven short Form, schwab and England scale. Results: 38 patients presented PD, with a mean age of 60.89 ± 13.6 years and sex-ratio of 2.45. Mean duration of PD was 3.1 ± 2.9 years. urinary disorders were found in 47.36% and dominated by incontinence (88.88%) and overactive bladders (88.88%) which were minor in more than 55%. Quality of life was impaired in 88.88% of cases. Minor forms (43.75%) were predominant and constraint dimension (50%) was the most severely altered. Prognosis factors for quality of life of urinary disorders were PD stages (0.046) and functional autonomy (0.042). Discussion and conclusion: urinary disorders in PD are common, impaired quality of life, especially the constraint dimension, depends on stage of evolution of PD and functional autonomy of patients.

Premating reproductive isolation （RI） may reduce gene flow across populations that have differentiated in traits important for mate choice. Examining RI across genetic and phenotypic clines can inform the fundamental evolutionary processes that underlie population and lineage differentiation. We conducted female mate-choice studies across an intraspecific red-eyed treefrog cline in Costa Rica and Panama with 2 specific aims：（1） to characterize RI across the cline and examine the relationship between premating RI and genetic and phenotypic distance and （2） to evaluate our results within a broader evolutionary and taxonomic perspective through examination of other RI studies. We found that female red-eyed treefrogs prefer local males relative to non-local males, indicating that some premating RI has evolved in this system, but that preference strength is not associated with phenotypic or geographic distance. Our analysis of 65 other studies revealed no clear pattern between the strength of RI and geographic distribution （allopatry, parapatry, cline） or phenotypic distance, but revealed extreme variation and overlap in levels of intra- and interspecific levels of RI. This work contributes to a growing body of literature that examines intraspecific RI across a cline to understand the selective processes that shape evolutionary patterns at the earliest stages of divergence.

seroprevalence of Viral Hepatitis Markers B in secondary school in Abidjan: Advocacy for a Catch-up Vaccination

Aim: To determine the prevalence of HBs antigen in secondary schools, to appreciate the vaccination coverage of viral hepatitis B and to propose a vaccine catch-up strategy. Materials and Methods: This was a prospective cross-sectional study conducted over a two-month period from April 24 to June 24, 2006 among students aged 10 - 15, in two schools (one public and the other private) of the city of Abidjan selected at random. The assay of serum markers of viral hepatitis B of the samples collected after 5 ml peripheral venous sampling was carried out in the viral serology and bacteriology unit of the Pasteur institute of Ivory Coast using the ELIsA technique (Enzyme-Linked-Immunosorbant-Assay). The ELIsA technique is an enzyme immunoassay technique that can detect an antigen or an antibody by highlighting the antigen-antibody reaction. Results: Of the 282 students who participated in our study, 5.3% of students (n = 15) were infected with the hepatitis B virus. The majority of students (79.1%, n = 223) did not have no contact with the hepatitis B virus. Only 7.8% (n = 22) of the students were vaccinated against viral hepatitis B. Conclusion: The low immunization coverage and the high prevalence of seronegative students reflect the need for a vaccine catch-up policy for adolescents born before the introduction of vaccination against the viral hepatitis B virus in the Expanded Program on Immunization.

What Communication strategy Adopted for the Promotion of Cultural Heritage？ Example： The Case of Madagascar

Part of the article is to study the intersection of two areas： the study of conservation facts oral traditions, intercultural dimensions and social dynamics induced by ICT. On the one hand, oral traditions, their popular character, manifest themselves in public in well-defined circumstances. All the techniques of literary invention and social diffusion contribute to evoke a legendary past but also to think about the present. On the other hand, the latest ICT developments may cause contrasting effects： on the one hand, they give unexpected ways （＂a historic opportunity＂） to fight the inevitable decline of oral traditions and to bring the collection and transmission of these oral traditions to a level they had never met; on the other hand, ICTs are a part of a modernity that speeds to abandon ancient traditions and convenes content and modes of narration and representation based on a worldview defined. The set highlights a participatory strategy. This is part of an action research. In other words, it tends to work on the complexity of the situations, the dynamics of the process, the comprehensiveness, and the systemic, social, technological, and cultural forms.

The objective is to know and to exploit specificities of the local data because Madagascar needs to the media economic surveys deepened on its cultural and social diversities. The problems remain on the facts that the journalists are exceeded by the events and are often constrained to be unaware of the environment of proximity--policies, societies, and economics which could become inexhaustible sources. In this case, how and up to what point will they manage to collect all the data related there, to exploit them and to place them at the disposal of the public？ We chose a methodology of investigation according to the research methods developed for the study of the industrial and agricultural systems （Bear, Gibbon, Haggblade, ＆ Ritchie, 1993; Boomgard, Davies, Haggblade, ＆ Mead, 1992; Laurent, 1993; Montigaud, 1992） in order to establish the cartography of the media, the actors, the sets of themes to the menu. This enabled us to raise a whole proliferation of the media. The lnternet has also been diffused since 1997.

Inorganic Elements in Eggs of Two Cavity-Nesting Passerine species at and around Los Alamos National Laboratory, Los Alamos, New Mexico

Los Alamos National Laboratory (LANL) was established in 1943 as part of the Manhattan Project to build nuclear weapons, and currently operates as a national research laboratory. As part of an ongoing assessment of site-related ecological risk at LANL, western bluebird (sialia mexicana) and ash-throated flycatcher (Myiarchus cinerascens) eggs were collected from 1997 to 2012 and analyzed for 18 inorganic elements. Concentrations of many inorganic elements in eggs were below reporting limits. Between species comparisons revealed that western bluebird eggs had higher levels of barium while ash-throated flycatcher eggs had higher levels of mercury. No statistically significant differences were observed in concentrations of inorganic elements in western bluebird eggs collected from the study area (which consists of areas within the current and historic LANL boundary) and from a non-industrial reference site;nor were any statistically significant differences observed between two canyons of interest, known to have received effluents and storm water runoff from LANL facilities, and the non-industrial reference site. Inorganic element levels detected in western bluebirds were typically within the range measured in eggs of other passerine in the published literature. These data suggest that concentrations of inorganic elements in passerine eggs collected from the study area appear to be at levels causing negligible risks to local bird populations.

Prevalence of HBs Ag among blood donors in Transfusion Center, Abidjan (Ivory Coast)

Aim: To study the prevalence of HBsAg among persons without risk factors for hepatitis B. Patients and Methods: Cross-sectional study (28 January to 31 December 2010) about 4310 new volunteers blood donors consecutively recruited. Among all, a risk factor for hepatitis B was investigated (standardized questionnaire and clinical examination). Blood of all blood donors without risk factors of hepatitis B (n = 3823) were systematically collected for HBsAg, hepatitis C antibody, syphilis serology and human immunodeficiency virus serology. The blood was analyzed with a third generation ELIsA (Abbott Murex). Results: HBsAg was positive in 179 persons (4.68% [95% CI 4% - 5%]). It was associated with hepatitis C antibodies (0.28% (n = 11)), syphilis antibodies (0.68% (n = 26)) or anti-human immunodeficiency virus antibodies (0.2% (n = 8)). In multivariate analysis, age 30 to 39 years (1.545 [95% CI 1.074 to 2.222]), male sex (2.426 [95% CI 1.550 to 3.799]) and positive syphilis serology (14.344 [95% CI 5.161 to 39.865]) were associated with presence of HBsAg. Conclusion: The prevalence of HBsAg among adult without risk factors for hepatitis B was not negligible. Also, it is essential to maintain routine screening before any vaccination against hepatitis B in adult. Our study is not for an immunization against hepatitis B only targeting risk populations in adult.
