Introduction: Gastroduodenal lesions are common in chronic kidney disease (CKD). They are linked to various factors including Helicobacter pylori infection (H. pylori). Few data are available in Africa on H. pylori infection and chronic kidney disease. The aim of this study was to assess the impact of H. pylori infection and to describe the gastroduodenal lesions found in patients with chronic kidney disease. Patients and Methods: A cross-sectional study was conducted, February 1st to May 31st, 2021, at the Douala General Hospital in Cameroon. We included patients with CKD classified as stages 3 to 5 according KDIGO classification, on hemodialysis or not, who agreed to participate in the study. They were matched with a “control” population including patients with normal renal function according to sex and age (ratio 1:2). Patients on antibiotics and/or proton pump inhibitors were excluded. We collected data from CKD patients and from medical records for non-CKD group. Each patient underwent an upper digestive endoscopy and identification of H. pylori using a urease rapid test. Logistic regression was used to identify independent associations for a significance level set at p Results: We included 99 patients including 33 with CKD and 66 control patients. Among patients with CKD, the predominance was male (n = 18/33 or 54.5%). The mean age was 51.2 ± 12.8 years. Arterial hypertension was the first etiology of CKD (n = 13 or 39.4%). The prevalence of H. pylori in patients with CKD was 63.6% versus 37.9% in control patients (p-value = 0.015). The main endoscopic lesions were erosive gastropathy (n = 14 or 42.4%) and erythematous gastropathy (n = 7 or 21.2%). Patients with CKD were 5 times more likely to have H. pylori infection (OR = 5.69;CI 95% 0.14 - 0.82;p = 0.017). Factors associated with H. pylori infection were chronic kidney disease (aOR = 1.02;CI 95% 0.14 - 0.82;p = 0.017) and hemodialysis (aOR = 10;CI 95% 1.08 - 91.9;p = 0.042). Conclusion

POLDER多角度观测信息能够捕捉云虹的偏振反射特征,从而能够获取完整的云滴尺度分布信息,包括云滴有效半径和方差。反演算法要求水云具有较高的水平均一特性。然而为了弥补观测个数的不足,反演需要利用150 km的范围内所有观测。然而,自然界中的云团很难满足这一水平均匀假设且这一较低的分辨率难以满足很多应用的需求。本研究主要探讨提高POLDER反演分辨率的途径,以及进一步减少由于云的非均匀分布引入的反演误差。我们利用RT3模型和POLDER卫星数据分析了云的非均一性对反演的影响,以及反演所需最小的分辨率。进而尝试了用更高的分辨率进行反演。实验案例表明云滴半径的非均匀分布造成云滴半径（误差＜1μm）和方差的反演误差,云滴方差和云光学厚度的非均匀分布只会对云滴方差的反演造成可以忽略的影响。此外,我们发现当云虹散射角区间内包含一定数目（随波场变化）的多角度偏振反射率观测,即可满足云滴谱高精度的反演。我们也发现目前官方算法仅使用副虹（散射角范围:135-145°）的偏振观测,难以保证大云滴（有效半径＞15um）的反演精度。该研究对现有POLDER云滴反演进行了改进:1)加入主虹（散射角区间135-145°）的偏振观测,来提高算法对大云滴的反演精度。2)提升分辨率至40-60km。我们对POLDER2008年2、5、8、11月全球的数据进行了云滴有效半径的反演,分辨率为60km。通过与官方产品进行对比,发现官方产品中大云滴（有效半径＞15um）的云滴半径存在低估。

Pigs receiving daily tailored diets using precision-feeding techniques have different threonine requirements than pigs fed in conventional phase-feeding systems

Background: There is large variation in amino acids requirements among pigs,hence feeding pigs individually with daily tailored diets or in groups with a single feed may require different levels of ***,the response to different threonine levels(70%,85%,100%,115%,and 130% of the ideal threonine:lysine protein ratio of 0.65) was studied in growing pigs raised in a conventional group phase-feeding(GPF) system or fed individually using individual precision-feeding(IPF) *** a 21-day trial,110 barrows(25 ± 0.80 kg body weight) were housed in the same room and fed using electronic *** pigs per treatment were slaughtered at the end of the ***: Threonine intake increased linearly for the IPF and GPF pigs(P < 0.05).Lysine intake was similar across the *** daily gain,gain:feed ratio,and protein deposition were affected linearly by threonine level(P < 0.05)in both feeding *** deposition in the GPF pigs was maximized at 150 g/d and a 0.65 threonine:lysine ratio,whereas protein deposition increased linearly in the IPF *** Met and serine levels were 11 and 7% higher,respectively,in the IPF pigs than in the GPF pigs(P < 0.05).Dietary threonine increased(P < 0.05)threonine concentration in the longissimus dorsi in a quadratic manner in the IPF pigs,whereas there was no effect in the GPF *** dorsi collagen decreased as dietary threonine increased in the IPF and GPF pigs(P < 0.10).Carcass muscle crude protein was 2% higher in the GPF pigs than in the IPF pigs(P < 0.05).Conclusions: Individual pigs are able to modulate growth and the composition of growth according to threonine *** average amino acid ratio value that is currently used for GPF cannot be used for IPF.

MM is frequently associated with the development of osteolytic bone lesions, osteoporosis and pathological fractures. Bone destruction in MM is caused by osteoclasts recruited in areas adjacent to myeloma plasma cells;their contact triggers both cell types to secrete soluble factors sustaining one each other’s activation and proliferation. Osteoclasts differentiate and maturate upon binding of the receptor activator of NF-kappaB ligand (RANKL), secreted by bone marrow microenvironmental cells, to its receptor (RANK) on osteoclast progenitors, while osteoprotegerin (OPG), a natural decoy receptor, can block the aforementioned ligation. At the same time osteoblasts are inactivated by the Wnt/β-catenin signaling pathway inhibitor, Dickkopf-1 protein (DKK-1), secreted by malignant plasma cells. Furthermore, DKK-1 deregulates the OPG/RANKL equilibrium, promoting osteoclastogenesis. Myeloma bone disease (MBD) can be treated with myeloma-directed chemotherapy and agents inhibiting bone resorption such as aminobisphosphonates, although new promising biology driven monoclonal antibodies targeting osteoclastogenesis mechanisms are emerging. Palliative MBD treatment includes analgesics, orthotics, radiation therapy, vertebroplasty and kyphoplasty. In case of spinal cord compression, radiation therapy or surgical decompression, should be instantly performed, along with steroid administration. Surgery may also be an option especially in case of weight-bearing bone fractures. MBD is a morbid complication and should be carefully managed because it deteriorates patients’ quality of life and worsens disease outcome.

ON THE ALGORITHMIC COMPLEXITY OF STATIC STRUCTURES

This paper provides a first indication that this is true for a system comprised of a static structure described by hyperbolic partial differential equations and is subjected to an external random input force. The system deforms the randomness of an input force sequence in proportion to its algorithmic complexity. The authors demonstrate this by numerical analysis of a one-dimensional vibrating elastic solid (the system) on which we apply a maximally-random force sequence (input). The level of complexity of the system is controlled via external parameters. The output response is the field of displacements observed at several positions on the body. The algorithmic complexity and stochasticity of the resulting output displacement sequence is measured and compared against the complexity of the system. The results show that the higher the system complexity, the more random-deficient the output sequence.

Reducing the Prevalence of Catheter-Related Infections by Quality Improvement: Six-Year Follow-Up Study

Background: Peripheral venous catheter (PVC) insertion is a crucial nursing action during life support. Several factors that increase the risk of thrombophlebitis associated with PVCs have been reported. Objective: We wish to evaluate the impact of a quality improvement regarding PVC treatment for patients with coronary heart diseases. Method: A longitudinal, quantitative observational study was carried out in 2008 and 2013 in a hospital in southern Sweden with 360 consecutive patients suffering from acute chest pain. New routines for PVC treatment were included in the hospital with daily inspection according to a checklist. A structured observation protocol was used to survey the prevalence of thrombophlebitis between 2008 and 2013. Also, we examined the relationship between the location and luminal diameters of PVCs. Results: The student’s t-test showed significant differences between 2008 and 2013 with respect to luminal diameter of PVCs (p = 0.002), prevalence of thrombophlebitis (p = 0.003) and number of days with PVC left in situ (p < 0.001). Conclusion: These findings emphasize the value of using systematic daily inspections and checklists to achieve quality and safety in patients with acute chest pain having PVC-based treatment.

Cultivated potato is a clonally propagated autotetraploid species with a highly heterogeneous *** assemblies of six cultivars including two chromosome-scale phased genome assemblies revealed extensive allelic diversity,including altered coding and transcript sequences,preferential allele expression,and structural variation that collectively result in a highly complex transcriptome and predicted proteome,which are distributed across the homologous *** species contribute to the extensive allelic diversity in tetraploid cultivars,demonstrating ancestral introgressions predating modern breeding *** a clonally propagated autotetraploid that undergoes limited meiosis,dysfunctional and deleterious alleles are not purged in tetraploid *** a quarter of the loci bore mutations are predicted to have a high negative impact on protein function,complicating breeder’s efforts to reduce genetic *** StCDF1 locus controls maturity,and analysis of six tetraploid genomes revealed that 12 allelic variants of StCDF1 are correlated with maturity in a dosage-dependent *** of the complexity of the tetraploid potato genome with its rampant structural variation and embedded deleterious and dysfunctional alleles will be key not only to implementing precision breeding of tetraploid cultivars but also to the construction of homozygous,diploid potato germplasm containing favorable alleles to capitalize on heterosis in F1 hybrids.

The Role of <sup>18</sup>F-FDG-PET/CT in the Management of Patients with High-Risk Breast Cancer: Case Series and Guideline Comparison

Objectives: In grade III-IV breast cancer, dissemination of disease needs to be assessed. Until now this was done by conventional imaging (liver ultrasonography, chest X-ray and bone scintigraphy), but evidence favoring the use of FDG-PET/CT is accumulating. Methods: Patients with high-risk breast cancer, who had received conventional imaging and FDG-PET/CT, were included. Patients were staged and assigned a treatment after 1) conventional imaging and 2) FDG-PET/CT, both by a multidisciplinary oncology team. Equivocal FDG-PET/CT findings were histologically confirmed. Results: 16 patients were included (mean age 59 years). TNM-stage changed in 5 patients (31%) after FDG-PET/CT. In 3 patients (19%) unknown distant metastases were detected by FDG-PET/CT. An adjustment of treatment took place in 4 patients (25%). Conclusions: Our case series emphasizes the role of FDG-PET/CT in the staging of high-risk breast carcinoma, especially in the assessment of distant metastases. We suggest replacing conventional imaging with FDG-PET/CT.
