The efficacy and effectiveness of machine learning for weaning in mechanically ventilated patients at the intensive care unit: a systematic review

Weani ng from mechanical ventilation in the in tensive care unit (ICU) is a complex clinical problem and relevant for future organ engineering. Prolonged mechanical ventilation (MV) leads to a range of medical complications that increases length of stay and costs as well as contributes to morbidity and even mortality and long-term quality of life. The need to reduce MV is both clinical and economical. Artificial intelligence or machine learning (ML) methods are promising opportunities to positively influence patient outcomes. ML methods have been proposed to enhance clinical decisions processes by using the large amount of digital information generated in the ICU setting. There is a particular interest in empirical methods (such as ML) to improve management of "difficult-to-wean" patients, due to the associated costs and adverse events associated with this population. A systematic literature search was performed using the OVID, IEEEXplore, PubMed, and Web of Science databases. All publications that included (1) the application of ML to weaning from MV in the ICU and (2) a clinical outcome measurement were reviewed. A checklist to assess the study quality of medical ML publications was modified to suit the critical assessment of ML in MV weaning literature. The systematic search identified nine studies that used ML for weaning management from MV in critical care. The weaning management application areas included (1) prediction of successful spontaneous breathing trials (SBTs),(2) prediction of successful extubation,(3) prediction of arterial blood gases, and (4) ventilator setting and oxygenation-adjustment advisory systems. Seven of the nine studies scored seven out of eight on the quality index. The remaining two of the nine studies scored one out of eight on the quality index. Surveying the Oligomeric State of Arabidopsis thaliana Chloroplasts

Blue native-PAGE （BN-PAGE） resolves protein complexes in their native state. When combined with immu- noblotting, it can be used to identify the presence of high molecular weight complexes at high resolution for any protein, given a suitable antibody. To identify proteins in high molecular weight complexes on a large scale and to bypass the requirement for specific antibodies, we applied a tandem mass spectrometry （MS/MS） approach to BN-PAGE-resolved chloroplasts. Fractionation of the gel into six bands allowed iden- tification and label-free quantification of 1000 chloroplast proteins with native molecular weight separation. Significantly, this approach achieves a depth of identification comparable with traditional shotgun proteo- mic analyses of chloroplasts, indicating much of the known chloroplast proteome is amenable to MS/MS identification under our fractionation scheme. By limiting the number of fractionation bands to six, we facil- itate scaled-up comparative analyses, as we demonstrate with the reticulata chloroplast mutant displaying a reticulated leaf phenotype. Our comparative proteomics approach identified a candidate interacting protein of RETICULATA as well as effects on lipid remodeling proteins, amino acid metabolic enzymes, and plastid division machinery. We additionally highlight selected proteins from each sub-compartment of the chloroplast that provide novel insight on known or hypothesized protein complexes to further illus- trate the utility of this approach. Our results demonstrate the high sensitivity and reproducibility of this technique, which is anticipated to be widely adaptable to other sub-cellular compartments.

Portopulmonary hypertension and hepatopulmonary syndrome

Portopulmonary hypertension (POPH) and hepatopulmonary syndrome (HPS) are two frequent complications of liver disease, with prevalence among liver transplant candidates of 6% and 10%, respectively. Both conditions result from a lack of hepatic clearance of vasoactive substances produced in the splanchnic territory. Subsequently, these substances cause mainly pulmonary vascular remodeling and some degree of vasoconstriction in POPH with resulting elevated pulmonary pressure and right ventricular dysfunction. In HPS the vasoactive mediators cause intrapulmonary shunts with hypoxemia. Medical treatment is disappointing overall. Whereas liver transplantation (LT) results in the disappearance of HPS within six to twelve months, its effect on POPH is highly unpredictable. Modern strategies in managing HPS and POPH rely on a thorough screening and grading of the disease’s severity, in order to tailor the appropriate therapy and select only the patients who will benefit from LT. The anesthesiologist plays a central role in managing these high-risk patients. Indeed, the important hemodynamic and respiratory modifications of the perioperative period must be avoided through continuation of the preoperatively initiated drugs, appropriate intraoperative monitoring and proper hemodynamic and respiratory therapies.

Temperature dependence of parametric instabilities in the context of the shock-ignition approach to inertial confinement fusion

The role of the coronal electron plasma temperature for shock-ignition conditions is analysed with respect to the dominant parametric processes: stimulated Brillouin scattering, stimulated Raman scattering, two-plasmon decay(TPD), Langmuir decay instability(LDI) and cavitation. TPD instability and cavitation are sensitive to the electron temperature. At the same time the reflectivity and high-energy electron production are strongly affected. For low plasma temperatures the LDI plays a dominant role in the TPD saturation. An understanding of laser–plasma interaction in the context of shock ignition is an important issue due to the localization of energy deposition by collective effects and hot electron *** in turn can have consequences for the compression phase and the resulting gain factor of the implosion phase.

NUMERICAL SOLUTION OF A NONLINEAR REACTION-DIFFUSION EQUATION

A nonlinear reaction-diffusion equation is studied numerically by a Petrov-Galerkin finite element method, which has been proved to be 2nd-order accurate in time and 4th-order in space. The comparison between the exact and numerical solutions of progressive waves shows that this numerical scheme is quite accurate, stable andefflcient. It is also shown that any local disturbance will spread, have a full growth and finally form two progressive waves propagating in both directions. The shape and the speed of the long term progressive waves are determined by the system itself, and do not depend on the details of the initial values.

In Vivo Function of Tic22, a Protein Import Component of the Intermembrane Space of Chloroplasts

Preprotein import into chloroplasts depends on macromolecular machineries in the outer and inner chloroplast envelope membrane （TOC and TIC）. It was suggested that both machineries are interconnected by components of the intermembrane space （IMS）. That is, amongst others, Tic22, of which two closely related isoforms exist in Arabidopsis thaliana, namely atTic22-III and atTic22-IV. We investigated the function of Tic22 in vivo by analyzing T-DNA insertion lines of the corresponding genes. While the T-DNA insertion in the individual genes caused only slight defects, a double mutant of both isoforms showed retarded growth, a pale phenotype under high-light conditions, a reduced import rate, and a reduction in the photosynthetic performance of the plants. The latter is supported by changes in the metabolite content of mutant plants when compared to wild-type. Thus, our results support the notion that Tic22 is directly involved in chloroplast preprotein import and might point to a particular importance of Tic22 in chloroplast biogenesis at times of high import rates.

Tight focusing with very small f-numbers is necessary to achieve the highest at-focus ***,tight focusing imposes strong demands on precise target positioning in-focus to achieve the highest on-target irradiance We describe several near-infrared,visible,ultraviolet and soft and hard X-ray diagnostics employed in a~10^(22)W/cm^(2)laser±plasma *** used nearly 10 J total energy femtosecond laser pulses focused into an approximately1.3-μm focal spot on 5±20μm thick stainless-steel *** discuss the applicability of these diagnostics to determine the best in-focus target position with approximately 5μm accuracy(i.e.,around half of the short Rayleigh length)and show that several diagnostics(in particular,3ωreflection and on-axis hard X-rays)can ensure this *** demonstrated target positioning within several micrometers from the focus,ensuring over 80%of the ideal peak laser intensity *** approach is relatively fast(it requires 10±20 laser shots)and does not rely on the coincidence of low-power and high-power focal planes.
