Clinical Predictors for Recurrence after Curative Resection for Colorectal Cancer

Purpose: To identify clinical predictors of recurrence of colorectal cancer after curative surgical treatment. Methods: Retrospective follow-up-study of 925 consecutive patients treated with R0-resection for colorectal cancer Stage I, II and III from 1990 until 2000 with a mean follow-up of 60 ± 37 months. Predictors for cancer recurrence were identified in a pilot-sample of these patients, followed by analyses of the rest of the patients (test-sample), and finally with a concluding analyses of the entire patient group. Data were analyzed with Pearson Chi-square test (χ2), Cox regression analyses and log rank test. Results: Tumor stage (Stage I: HR 0.10 (0.05;0.19), Stage II: HR 0.31 (0.24;0.41)) and postoperative reoperations due to complications due to other causes than anastomotic leakage (HR 2.02 (1.21;3.36)) were significant predictors of cancer recurrence in the multivariate Cox regression model. The association between reoperations and recurrence was strongest for the patients with the best prognosis: Stage I and Stage II-cancers. Long duration of surgery, strongly associated with blood-loss and infusions of liquid and blood-products, reoperation due to anastomotic leakage as well as right colon/transversum localization were significant at a trend-level (10%). Conclusions: Tumor stage and reoperations due to postoperative complications other than anastomotic leakage are significant predictors for recurrence after curative surgery for colorectal cancer.

DNA-repair ERCC1 Gene Polymorphisms in Epithelial Ovarian Cancer and Relation to Platinum Resistance and Survival

Objectives: Excision repair cross-complementation group 1 (ERCC1) is a key DNA repair gene in the nucleotide excision repair pathway which is activated in the repair of intra- and interstrand DNA crosslink caused by platinum-based treatment. Two single nucleotide polymorphisms (SNPs) of the ERCC1 gene, codon 118 C/T and C8092A, have been reported to be functional, but the influence on platinum resistance and survival is not yet clear. The primary aim of the present study was to investigate whether the two SNPs were associated with resistance to standard combination carboplatin and paclitaxel chemotherapy and the potential prognostic impact in newly diagnosed ovarian cancer patients. Methods: Serum samples from 202 patients with newly diagnosed ovarian cancer were assessed for ERCC1 SNP genotyping using real time PCR. All patients were treated with first line carboplatin/paclitaxel chemotherapy. Results: There were no correlation between the ERCC1 118 C/T and C8092A genotypes and platinum resistance (P = 0.79 and P = 0.36, respectively). Furthermore, the results showed no association to progression free survival (P = 0.18 and P = 0.16, respectively) or overall survival (P = 0.89 and P = 0.78, respectively) for the two SNPs. Conclusions: The ERCC1 118 C/T and C8092A polymorphisms did not have significant influence on clinical outcome defined as platinum resistance, PFS and OS.

Purpose: To identify clinical predictors for reduced long-term survival and describe the cause of death after surgical treatment for rectal cancer. Methods: A retrospective follow-up study of 442 consecutive, unselected patients treated for rectal cancer at a tertiary centre from 1990 until 2000 and followed for 17 years or until death. Predictors for death were assessed by Cox regression analysis. The cause of death was obtained from the Norwegian Cause of Death Registry. Results: 254 men and 188 women with a median age of 71 years (21 - 95 years) were resected for rectal cancer with low anterior resection (n = 266), abdominoperineal resection (n = 125), Hartmann’s procedure (n = 19) or diverting stoma only (n = 32). Median follow-up was 5 years (0 - 17 years). The relative five-year survival rates for stages I, II, III and IV was 83.9%, 65.2%, 41.1% and 9.3%, respectively. The proportion of deaths due to recurrence from colorectal cancer in stages I, II, III and IV was 23.5%, 55.8%, 72.3% and 98.0%, respectively. Heart, lung and cerebrovascular disease and other malignancies were the cause of death in the other patients. The Prognostic Importance of EGFR and COX-2 Expression in Cervix Cancer Stages IIb-IVa

Objectives. Locally advanced cervix cancer represents a therapeutic challenge with a delicate balance between effect and toxicity. Consequently, there is an obvious need for new prognostic parameters with a perspective of a more individualized treatment. The aim of the present study was to evaluate the possible prognostic importance of epidermal growth factor receptor and cyclooxygenase-2 expression in locally advanced cervix cancer. Methods. The study included 91 patients with cervix cancer, FIGO stages IIb-IVa, and were treated with curative intent according to the Nordic Cervix Cancer protocol (NOCECA). The median observation time was 7 years. Epidermal growth factor receptor and cyclooxygenase-2 expression was evaluated by immunohistochemistry using commercially available antibodies. The tumor marker expression was evaluated according to a semi-quantitative scoring system with a positive score when more than 10% of the tumor cells were moderately or strongly stained. Results. Epidermal growth factor receptor and cyclooxygenase-2 over-expression was found in 22% and 18% of the patients, respectively. The survival differed according to epidermal growth factor receptor and cyclooxygenase-2 over-expression as calculated by Kaplan-Meier plots and log-rank test (p = 0.0002 and p = 0.0084 respectively). A multivariate Cox Regression analysis identified each tumor marker as an independent prognostic factor. Conclusion. The results clearly indicate epidermal growth factor receptor and cyclooxygenase-2 hold important prognostic information, markedly appearing with a long-term observation. We found that both parameters were independent prognostic factors, and to further clarify this matter our retrospective analysis should be confirmed in a prospective study with more patients.

Baseline effects of lysophosphatidylcholine and nerve growth factor in a rat model of sciatic nerve regeneration after crush injury

Schwann cells play a major role in helping heal injured nerves. They help clear debris, produce neurotrophins, upregulate neurotrophin receptors, and form bands of Büngner to guide the healing nerve. But nerves do not always produce enough neurotrophins and neurotrophin receptors to repair themselves. Nerve growth factor（NGF） is an important neurotrophin for promoting nerve healing and lysophosphatidylcholine（LPC） has been shown to stimulate NGF receptors（NGFR）. This study tested the administration of a single intraneural injection of LPC（1 mg/mL for single LPC injection and 10 mg/mL for multiple LPC injections） at day 0 and one（day 7）, two（days 5 and 7）, or three（days 5, 7, and 9） injections of NGF（160 ng/mL for single injections and 80 ng/mL for multiple injections） to determine baseline effects on crush ed sciatic nerves in rats. The rats were randomly divided into four groups： control, crush, crush-NGF, and crush-LPC-NGF. The healing of the nerves was measured weekly by monitoring gait; electrophysiological parameters： compound muscle action potential（CMAP） amplitudes; and morphological parameters： total fascicle areas, myelinated fiber counts, fiber densities, fiber packing, and mean g-ratio values at weeks 3 and 6. The crush, crush-NGF, and crush-LPC-NGF groups statistically differed from the control group for all six weeks for the electrophysiological parameters but only differed from the control group at week 3 for the morphological parameters. The crush, crush-NGF, and crush-LPC-NGF groups did not differ from each other over the course of the study. Single injections of LPC and NGF one week apart or multiple treatments of NGF at 5, 7 and 9 days post-injury did not alter the healing rate of the sciatic nerves during weeks 1-6 of the study. These findings are important to define the baseline effects of NGF and LPC injections, as part of a larger effort to determine the minimal dose regimen of NGF to regenerate peripheral nerves.

The Greenland ice core from NorthGRIP （NGRIP） contains a proxy climate record across the Pleistocene-Holocene boundary of unprecedented clarity and resolution. Analysis of an array of physical and chemical parameters within the ice enables the base of the Holocene, as reflected in the first signs of climatic warming at the end of the Younger Dryas/Greenland Stadial 1 cold phase, to be located with a high degree of precision.

Objective. There may be advantages in using magnetic resonance imaging (MRI) in small- bowel disease. The aims of this study were to optimize the MRI examination technique and to evaluate the capabilities of MRI compared with those of conventional enteroclysis (CE). Material and methods. MRI and CE were performed in 36 patients suspected of Crohn’ s disease. Based on 26 pilot studies optimal oral administration of plum juice and bulk fibre laxative was found. T2- weighted and gadolinium enhanced T1- weighted images were obtained using a breath- holding technique and butylscopolamine. Virtual endoscopy was performed. Conventional enteroclysis entailed duodenal intubation and administration of barium and air. Two radiologists evaluated the examinations independently. Finally, each patient scored the degree of discomfort, and preference for either MRI or CE was found. Results. The MRI technique ensured sufficient distension of the small bowel and small- bowel changes were found in 12 patients. In 3 patients this was not seen on conventional enteroclysis, which did not reveal any pathology that was not already seen on MRI. Pathological abdominal changes were found in 70% more patients during MRI than during conventional enteroclysis (p < 0.001). Endoscopic examination corresponded with the MRI findings. The examination quality decreased with increasing age (p = 0.002) and the interobserver agreement of the pathological changes was high (p < 0.001). Virtual endoscopy resulted in excellent demonstration of the mucosal surface. The examination discomfort scores obtained during the MRI were lower than those during conventional enteroclysis (p< 0.001). Conclusions. MRI using the current technique is preferable to conventional enteroclysis because of superior demonstration of the entire small- bowel pathology, low level of patient discomfort and absence of radiation exposure.

Major volcanic eruptions can impact on global climate by injecting large quantities of aerosols and ash into the atmosphere that alter the radiative balance and chemical equilibrium of the *** Millennium eruption of Tianchi(Paektu), China/North Korea, was one of the largest Late Holocene eruptions. Uncertainty about the precise timing of the eruption has hindered the recognition of its climate impact in palaeoclimate and historical records. Here we report the compelling identification of the eruption’s volcanic signal in Greenland ice cores through the association of geochemically characterized volcanic glass,represented in by bimodal populations that compare with proximal material from the source eruption. The eruption most probably occurred in the A.D. 940 s, 7 years after the Eldgjá eruption on Iceland. We examine the eruption’s potential for climate forcing using the sulfate records from the ice cores and conclude that it was unlikely to have had a global or extraregional impact.
