Screening pre-bariatric surgery patients for esophageal disease with esophageal capsule endoscopy

AIM:To determine if esophageal capsule endoscopy(ECE)is an adequate diagnostic alternative to esophagogastroduodenoscopy(EGD)in pre-bariatric surgery patients.METHODS:We conducted a prospective pilot study to assess the diagnostic accuracy of ECE(PillCam ESO2,Given Imaging)vs conventional EGD in pre-bariatric surgery patients.Patients who were scheduled for bariatric surgery and referred for pre-operative EGD were prospectively enrolled.All patients underwent ECE followed by standard EGD.Two experienced gastroenterologists blinded to the patient’s history and the findings of the EGD reviewed the ECE and documented their findings.The gold standard was the findings on EGD.RESULTS:Ten patients with an average body mass index of 50 kg/m2were enrolled and completed the study.ECE identified 11 of 14(79%)positive esophageal/gastroesophageal junction(GEJ)findings and 14of 17(82%)combined esophageal and gastric findings identified on EGD.Fisher’s exact test was used to compare the findings and no significant difference was found between ECE and EGD(P=0.64 for esophageal/GEJ and P=0.66 for combined esophageal and gastric findings respectively).Of the positive esophageal/GEJ findings,ECE failed to identify the following:hiatal hernia in two patients,mild esophagitis in two patients,and mild Schatzki ring in two patients.ECE was able to identify the entire esophagus in 100%,gastric cardia in0%,gastric body in 100%,gastric antrum in 70%,pylorus in 60%,and duodenum in 0%.CONCLUSION:There were no significant differences in the likelihood of identifying a positive finding using ECE compared with EGD in preoperative evaluation of bariatric patients.

Modified malleable prosthesis with a fixed mesh:case series for the Ghattas technique

Penile prosthetic implantation represents a cornerstone for patients with organic erectile dysfunction(ED)that is refractory,unsatisfactory,or contra-indicated for other approved medical or mechanical options.In this study,we introduce the“Ghattas technique,”wherein we constructed a polypropylene mesh sheath that surrounds and is fixed to a 13-mm malleable prosthesis cylinder,which can increase the cylinder diameter for cases that need a larger prosthesis.All patients underwent preoperative evaluation and completed the five-item International Index of Erectile Function questionnaire(IIEF-5).Postoperative outcomes were evaluated by IIEF-5 and Erectile Dysfunction Inventory of Treatment Satisfaction(EDITS)questionnaires at final follow-up.The mean age of the 23 included patients was 57.9(standard deviation[s.d.]:11.4)years and the mean duration of ED was 8.5(s.d.:7.9)years.Erection improvement was determined by comparing mean preoperative and postoperative IIEF-5 scores(8.3[s.d.:3.9]vs 24.6[s.d.:0.6],P<0.001).High treatment satisfaction was determined according to a mean EDITS score of 94.9(s.d.:9.9).The proposed Ghattas technique was safe and effective in our patients,and provides opportunity for cases that need a diameter larger than 13 mm.Further studies are needed to confirm these results.

This paper explores some design parameters of an interior permanent magnet synchronous motor that contribute to enhancing motor performance.Various geometry parameters such as magnet dimension,machine diameter,stator teeth height,and number of poles are analyzed to compare overall torque,power,and torque ripples in order to select the best design parameters and their ranges.Pyleecan,an open-source software,is used to design and optimize the motor for electric vehicle applications.Following optimization with Non-dominated Sorting Genetic Algorithm(NSGA-Ⅱ),two designs A and B were obtained for two objective functions and the corresponding torque ripples values of the design A and B were later reduced by 32%and 77%.Additionally,the impact of different magnet grades on the output performances is analyzed.

Adaptive Hybrid Bivariate Double Density Discrete and Complex Wavelet for Image Denoising

Image denoising is an important step in eliminating any noise impact in any image transmission process. Recently we presented two approaches for Bivariate based image denoising. They were Double Density Discrete Wavelet Transform (DD DWT) and Double Density Dual Tree Complex Wavelet Transform (DD CWT). In both techniques we decomposed noisy images with either DD DWT or DD CWT decompositions and then applied the Bivariate based denoising technique for noise removal. In this paper we propose an adaptive hybrid technique for Bivariate based image denoising that is based on the synthesis of DD-DWT bands or DD-CWT bands but with different weights, to deliver enhanced image features with less denoising impact especially around image edges, which is the most effected by noisy transmission channels. This proposed technique has been also enhanced by edge sharpening and Eigen analysis, as two separate stages. Simulation result comparisons have been performed between the proposed hybrid band adaptive DD-DWT and DD-CWT technique and the two primary techniques DD-DWT, DD- CWT, as well as other superior literature techniques such the original bivariate denoising technique with both original Complex Wavelet Transform and Double Density decompositions. This work in specific compares between Double Density DWT and Double Density CWT decompositions, proposes new filter design that suits each of them and proposes a hybrid technique between as will be shown.

A New BEM Modeling Algorithm for Size-Dependent Thermopiezoelectric Problems in Smart Nanostructures

The main objective of this paper is to introduce a new theory called size-dependent thermopiezoelectricity for smart nanostructures.The proposed theory includes the combination of thermoelastic and piezoelectric influences which enable us to describe the deformation and mechanical behaviors of smart nanostructures subjected to thermal,and piezoelectric loadings.Because of difficulty of experimental research problems associated with the proposed theory.Therefore,we propose a new boundary element method(BEM)formulation and algorithm for the solution of such problems,which involve temperatures,normal heat fluxes,displacements,couple-tractions,rotations,force-tractions,electric displacement,and normal electric displacement as primary variables within the BEM formulation.The computational performance of the proposed methodology has been demonstrated by using the generalized modified shift-splitting(GMSS)iteration method to solve the linear systems resulting from the BEM discretization.GMSS advantages are investigated and compared with other iterative methods.The numerical results are depicted graphically to show the size-dependent effects of thermopiezoelectricity,thermoelasticity,piezoelectricity,and elasticity theories of nanostructures.The numerical results also show the effects of the sizedependent and piezoelectric on the displacement components.The validity,efficiency and accuracy of the proposed BEM formulation and algorithm have been demonstrated.The findings of the current study contribute to the further development of technological and industrial applications of smart nanostructures.

The main aim of this paper is to propose a new memory dependent derivative(MDD)theory which called threetemperature nonlinear generalized anisotropic micropolar-thermoelasticity.The system of governing equations of the problems associated with the proposed theory is extremely difficult or impossible to solve analytically due to nonlinearity,MDD diffusion,multi-variable nature,multi-stage processing and anisotropic properties of the considered material.Therefore,we propose a novel boundary element method(BEM)formulation for modeling and simulation of such system.The computational performance of the proposed technique has been investigated.The numerical results illustrate the effects of time delays and kernel functions on the nonlinear three-temperature and nonlinear displacement components.The numerical results also demonstrate the validity,efficiency and accuracy of the proposed methodology.The findings and solutions of this study contribute to the further development of industrial applications and devices typically include micropolar-thermoelastic materials.

NUMERICAL MODELING OF TRANSIENT MAGNETO-THERMO-VISCOELASTIC WAVES IN A ROTATING NONHOMOGENEOUS ANISOTROPIC SOLID UNDER INITIAL STRESS

A numerical model based on the dual-reciprocity boundary element method(DRBEM)for studying the transient magneto-thermo-viscoelastic waves in a rotating nonhomogeneous anisotropic solid is presented.The formulation is tested through its application to the problem of an initially stressed solid placed in a constant primary magnetic field acting in the direction of the z-axis and rotating about this axis with a constant angular velocity.In the case of plane deformation,a numerical scheme for the implementation of the method is presented and the numerical computations are carried out for the temperature,displacement components and thermal stress components.The validity of DRBEM is examined by considering a magneto-thermo-viscoelastic solid occupies a rectangular region and good agreement is obtained with existent results.The results obtained are presented graphically to show the influence of initial stress on the displacement components and thermal stress components.

Objective MicroRNAs are fine regulators for gene expression during the post-transcriptional stage in many autoimmune diseases.HypoxamiRs(miR-210 and miR-21)play an important role in hypoxia and in inflammation-associated hypoxia.Systemic lupus erythematosus(SLE)is a chronic systemic autoimmune disease that would potentiate many pathological complications,including hemolytic anemia.This study aimed to investigate the role of hypoxamiRs in SLE/hemolytic anemia patients.Methods This work was designed to analyze the circulating levels of↱the miR-210 and miR-21 expressions and hypoxia-inducible factor-1α(HIF-α)in SLE/hemolytic anemia patients.SLE activity was evaluated for all patients by SLE Disease Activity Index(SLEDAI).Clinical manifestations/complications and serological/hematological investigations were reported.HIF-αconcentration was assayed by ELISA and expression of miR-21 and miR-210 was analyzed by qRT-PCR.Results The results indicated that the fold change of the miR-210/miR-21 expressions in plasma was significantly elevated in SLE/hemolytic anemia patients.A strong positive correlation between the miR-210 and miR-21 expression levels was also recorded.Among the associated-disease complications,hypertension,arthritis,oral ulcers,and serositis were associated with a high circulating miR-210 expression,while the occurrence of renal disorders was associated with the increased miR-21 expression.Furthermore,the HIF-αlevel was remarkably elevated in SLE/hemolytic anemia patients.A high positive correlation was recorded between the HIF-αconcentration and miR-210/miR-21 expression levels.The occurrence of oral ulcers,arthritis,and hypertension was associated with the increased HIF-αconcentration.On the other hand,SLEDAI and white blood cell count were positively correlated with miR-21/miR-210.The erythrocyte sedimentation rate was positively correlated with miR-21.Conclusion The dysregulation of the circulating miR-210/miR-210/HIF-1αlevels in SLE/hemolytic anemia patients advocated that the hypoxia pathway might h

A New BEM for Fractional Nonlinear Generalized Porothermoelastic Wave Propagation Problems

The main purpose of the current article is to develop a novel boundary element model for solving fractional-order nonlinear generalized porothermoelastic wave propagation problems in the context of temperaturedependent functionally graded anisotropic(FGA)structures.The system of governing equations of the considered problem is extremely very difficult or impossible to solve analytically due to nonlinearity,fractional order diffusion and strongly anisotropic mechanical and physical properties of considered porous structures.Therefore,an efficient boundary element method(BEM)has been proposed to overcome this difficulty,where,the nonlinear terms were treated using the Kirchhoff transformation and the domain integrals were treated using the Cartesian transformation method(CTM).The generalized modified shift-splitting(GMSS)iteration method was used to solve the linear systems resulting from BEM,also,GMSS reduces the iterations number and CPU execution time of computations.The numerical findings show the effects of fractional order parameter,anisotropy and functionally graded material on the nonlinear porothermoelastic stress waves.The numerical outcomes are in very good agreement with those from existing literature and demonstrate the validity and reliability of the proposed methodology.

Impact of COVID-19 infection during pregnancy on maternal and fetal outcomes

Objectives:The objective of this study is to assess the impact of coronavirus disease 2019(COVID-19)infection during pregnancy on maternal and fetal outcome in relation to gestational age.Methods:This retrospective study was conducted between May 2020 and July 2021.Sixty-five women infected with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and hospitalized at a quarantine hospital were included.Thirty-four women received prenatal care at the hospital until delivery,while 31 dropped out from the study due to COVID-19 recovery and discharge before delivery.Results:COVID-19 was found in 83%of the pregnant women after 20 weeks of pregnancy.The most common symptoms were cough and fever.Medical complications included severe pneumonia and thrombosis.The maternal mortality rate was 9.2%.Premature rupture of membranes and severe hypertension during labor affected nearly 9%of the pregnant women who completed prenatal care until delivery at the hospital.Preterm labor occurred at a rate of 38.2%.There were seven(20.8%)stillbirths and two cases of infant mortality.Conclusion:Hospitalized pregnant women who have coronavirus infection,lymphopenia,and a high C-reactive protein level were at a higher risk of developing severe illness,which can lead to maternal and neonatal complications.
