Premature Puberty Revealing an Ovarian Tumor in a Five-Year-Old Girl

Background: Ovarian tumors in the girl child are sometimes revealed by the development of secondary sexual characteristics. The authors report the case of a five-year-old girl in whom the disease was revealed by early puberty. Case presentation: A five-year-old girl with an enlarged abdomen for about four months. The onset of pain and the sensation of a mass prompted the consultation. The development of secondary sexual characteristics (SSC) noted by the family had not been mentioned. The patient was classified as pubertal stage 2 according to the Tanner classification. An abdominal ultrasound and a CT scan showed a large left ovarian mass, an enlarged uterus for the patient’s age and a normal right ovary. The hormonal workup was not contributive. The treatment consisted only of a left salpingo-ovarectomy, without complementary chemotherapy. Anatomic pathological examination of the surgical specimen concluded to a juvenile tumor of the granulosa. The evolution was good with a beginning of regression of the HSC one month after the ovarectomy. Discussion: Granulosa tumors are sometimes secretory cancers, generally with a low potential for malignancy and therefore a very good prognosis. Surgery based on total adnexectomy is the first-line treatment. The large size of the tumor, the presence of ascites and capsular rupture are factors of poor prognosis, hence the importance of early diagnosis. Conclusion: Routine comprehensive physical examination should be de rigueur for abdominal masses in girls, especially in the context of various beliefs that may impede early referral to care.

Risk of Waterborne Parasitic Infection among Vegetables Producers in the City of Ouagadougou, Burkina Faso: An Attempt to Quantify Them Using the Quantitative Microbiological Risk Assessment Method

The use of wastewater in urban agriculture has been promoted as an alternative to water scarcity and as a means to increase soil fertilizer. However, the use of wastewater in urban agriculture raises major public health concerns, mainly due to the often high concentration of pathogenic micro-organisms. Waterborne parasites are a major health concern in this regard, especially in endemic areas, mainly due to the high environmental resistance of eggs/cysts combined with a low infectious dose. In this study, the parasitological risk to vegetables producers in Ouagadougou using polluted water was evaluated through the quantitative microbiological risk analysis method. For this purpose, the search for and quantification of viable parasite eggs and cysts in irrigation water was coupled with epidemiological surveillance of vegetables producers in Ouagadougou. Protozoa and helminths belonging to 9 species were recorded and samples analyzed. These include Ancylostoma sp., Ascaris lumbricoides, tapeworm’s sp, Strongyloides stercoralis, Entamoeba histolytica, Giardia lamblia. Despite the variability of isolated parasitic species, Ancylostoma sp. eggs were common and severe to all types of water. Moreover, epidemiological approach showed that there were more parasitic species found in irrigation water and also other parasitic species circulating among vegetable farmers. In addition, there is no statistical significant association between the type of water used for irrigation and the carried parasites by vegetable gardeners. However, contamination of the groups using the raw wastewater is once higher than those using well water. When one is interested in the groups using treated wastewater as a source of irrigation, these risks of contaminations are halved. Background: Anastatica hierochuntica L. (Brassicaceae) has been claimed to treat various conditions, including complicated childbirth. The principal objective of this research was to provide scientific evidence on the contractile effect of the aqueous and hydroethanolic extracts of the whole plant from A. hierochuntica during labor. Method: Qualitative and quantitative methods helped identify and quantify the phytoconstituents in both extracts. The antioxidant activity was studied using four standard methods (ABTS, DPPH, FRAP, and LPO). Results: The inhibitory power of the extracts on pro-inflammatory enzymes such as 15-lipoxygenase and phospholipase A2 was determined. The contractile effects of the extracts were evaluated using isolated pregnant mice uteri. Steroids, triterpenoids, flavonoids, tannins, coumarins, and reduced compounds were identified in both extracts. The hydroethanolic extract exhibited higher content of hydrolyzable tannins (15.07 ± 0.24 mg tannic acid equivalent/g) than the aqueous decoction (11.8 ± 0.69 mg TAE/g). The extracts demonstrated the ability to scavenge DPPH and ABTS radicals, reduce ferric ions, and inhibit especially lipid peroxidation. No significant difference was noted in the phospholipase inhibition between the extracts and betamethasone. The hydroethanolic extract displayed the most significant anti-lipoxygenase activity with an IC50 value of 55.82 ± 1.15 μg/mL. The lethal dose (LD50) of extracts was estimated to be superior to 2000 mg/kg body weight (bw). The ability of the samples to produce one cm amplitude of contraction at 50% effective concentrations (EC50) was 5.33 × 10−8 ± 0.32 mg/mL for oxytocin, 1.51 ± 0.08 mg/mL for aqueous extract and 3.57 ± 0.61 mg/mL for hydroethanolic extract. Conclusion: These findings could provide evidence for using Anastatica hierochuntica to facilitate childbirth.

Background: Thyroid cancers commonly display slow evolution with local and or regional extension. The classic presentation is a painless nodule of the thyroid region in a euthyroid patient. Sometimes, the nodule is discovered only on ultrasonography. Cervical lymph node is often seen in papillary thyroid cancer due to their propensity to invade lymph node. This means that follicular thyroid cancers are more insidious. Observation: We report a painless slow-growing lesion of the scalp revealing a skull metastasis of thyroid cancer. Despite catastrophic intraoperative bleeding, a total removal was achieved. Lessons: Thus, in addition to local and regional control in the management of thyroid cancers, distant metastasis should be surgically removed to provide the best chance to prolong the patient’s survival. Moreover, neurosurgeon must be prepared to deal with massive bleeding in skull metastasis of thyroid cancer.

Anti-Biofilm, Anti-Quorum Sensing, and Anti-Proliferative Activities of Methanolic and Aqueous Roots Extracts of Carica papaya L. and Cocos nucifera L.

Objective: This study focused on the antibacterial and anti-proliferative activity of extracts from Carica papaya and Cocos nucifera roots. Methodology: The minimum inhibitory concentration and the minimum bactericidal concentration of the extracts on Escherichia coli, Pseudomonas aeruginosa, Streptococcus mutans, and Staphylococcus aureus were deduced by the microdilution method. The anti-biofilm activity was determined on all four strains and anti-quorum sensing activity by inhibition of violacein production in Chromobacterium violaceum. Anti-proliferative activity on prostate cultured cancer cells was evaluated by MTT assay. Sterols and triterpenes were also assayed in this study. Results: The methanolic extract of Carica papaya showed the best anti-biofilm effect with a percentage inhibition of 66.10 ± 1.79. The methanolic extract of Cocos nucifera had the strongest inhibition on the production of quorum sensing (61.42 ± 0.28). In addition, the methanolic extract of Cocos nucifera roots showed the best cytotoxic effect on prostate cancer LNCaP cell lines (IC50 = 26.98 ± 2.6 μg/mL) and Carica papaya on the PC-3 lines (IC50 = 127.20 ± 5.99 μg/mL). The extracts were also rich in triterpenes and sterols. Conclusion: This study provides support for the ethnomedical use of Carica papaya and Cocos nucifera roots as an antimicrobial and anticancer.

Anti-Fibrotic Effects of Calotropis procera (Ait.) *** Roots Barks against Diethylnitrosamine-Induced Hepatic Fibrosis in Rats

Background: Liver diseases including chronic hepatitis, steatosis, fibrosis, cirrhosis and liver cancer are now a public health problem. In 2002, cirrhosis accounted for 27.63% of hepatobiliary diseases in Burkina Faso. In Africa and more particularly in Burkina Faso, the majority of the population (about 80%) uses medicinal plants for their primary health care. Calotropis procera (Ait.) *** (Apocynaceae) is a medicinal plant used in Burkina Faso in the treatment of liver problems. This work aims to evaluate the anti-fibrotic properties of Calotropis procera roots barks. Methods: The anti-fibrotic activity of the ethanolic extract of Calotropis procera roots barks was evaluated using diethylnitrosamine (DEN) to induce liver fibrosis in male Wistar rats. Serum biomarkers, alanine transaminase (ALT), aspartate transaminase (AST), alkaline phosphatase (ALP), Total protein, Albumin, Υ-Glutamyl transferase (GGT) were evaluated and the activities of antioxidant enzymes (Superoxide dismutase and catalase) as well as the level of malonedialdehyde (MDA) and that of nitric oxide (NO) were determined in the liver homogenate. Results: The treatment of rats suffering from hepatic fibrosis with the ethanolic extract leads to a significant restoration of the biomarkers of the hepatic function in particular, AST, ALP, GGT, Albumin. The extract also causes a reduction in oxidative stress in the liver through a significant increase in the activity rate of the antioxidant enzymes Superoxide dismutase (SOD) and catalase accompanied by a significant drop in the rate of MDA and NO suggesting the anti-oxidant effect of extract. Conclusion: The results of the study show that the ethanolic extract of the roots barks of Calotropis procera has anti-fibrotic properties.

Comparative Study of the Thermal and Mechanical Properties of Foamed Concrete with Local Materials

Living in a habitat with comfort is requested by all. Cinder block bricks have poor thermal properties, leading people to use fan heaters and air conditioners to regain comfort. To overcome this problem of thermal discomfort in buildings, we used lightweight concrete such as foamed concrete which is a material that has improved thermal properties for thermal comfort. In addition, this material was compared with local materials used for the construction of buildings such as BTC, adobe and BLT mixed with binders. The results showed that foamed concrete is a material that has good thermal and mechanical properties compared to local materials mixed with binders. The foamed concrete having acceptable thermo-mechanical properties was that having a density of 930 kg/m3. It has a thermal resistance of 0.4 m2·K/W for a thickness of 20 cm. For sunshine on a daily cycle equal to 12 hours, the characteristic thickness achieved by this material is 7.29 cm. It also has a shallow depth of heat diffusion having a lower thickness than other materials. This shows that foamed concrete is a promising material for the construction of buildings.

Background: Urinary Tract Infection (UTI), a prevalent bacterial infection in adults, heavily relies on cytobacteriological examination of urine (CBEU) for diagnosis. However, in resource-limited countries, accessibility to CBEU remains hindered by cost and availability. This study aims to assess the utility of the Urinary Dipstick Test (UDT) in diagnosing UTIs among hospitalized patients in the context of limited resources. Methods: A cross-sectional study was conducted from February to May 2019, encompassing hospitalized patients who underwent CBEU at the bacteriology unit of Sour? Sanou University Hospital. UDT and CBEU were concurrently performed, and UDT’s analytical and diagnostic performance was evaluated against CBEU, considered the gold standard. Results: A total of 274 CBEU requests were registered, involving 274 patients (159 males) with a mean age of 45.8 ± 21.3 years (ranging from 1 to 90 years). UTI was confirmed in 90 patients, yielding a frequency of 32.85%. The UTI bacteriological profile was dominated by Enterobacteriaceae (75.23%), primarily Escherichia coli (60.55%). Nitrite and Leukocytes were positive in 54 (19.8%) and 157 (53.6%) of the samples tested. Among patients with confirmed UTI, Nitrite, and Leukocytes were positive in 30 (33%) and 71 (79%) patients respectively. UDT demonstrated variable performance based on nitrite and leukocyte combination: Sensitivity (57%-82%), Specificity (7%-98%), Positive Predictive Value (PPV) (43%-57%), Negative Predictive Value (NPV) (43%-67%). UDT performed slightly better in women (NPV = 88%) and inpatients without urinary catheters (NPV = 75% and PPV = 80%). Conclusion: This study underscores UDT’s potential utility in excluding UTIs among women, younger patients, and inpatients without urinary catheters, albeit with limited confidence. The UDT emerges as a complementary tool for UTI screening, particularly in resource-limited settings.

Opilia amentacea (Opiliaceae) is a woody plant with multiple medicinal claimed effects. The present study aimed to assess the anti-inflammatory and antioxidant activities of the decoction and macerate extracts from the leaves of Opilia amentacea. Moreover, acute toxicity and phytochemical analysis were performed. The acute toxicity was evaluated on NMRI mice at 2000 mg/kg bw. The anti-inflammatory activity was studied using the carrageenan-induced mouse paw edema and the lipoxygenase inhibition assay. The radical scavenging (DPPH and ABTS), ferric-reducing antioxidant power (FRAP), and lipid peroxidation (LPO) assays were used to measure the antioxidant capacity of the extracts. Qualitative and quantitative methods served for identifying and quantifying the extract’s phytoconstituents. The decoction demonstrated low acute toxicity;the lethal dose was therefore estimated to be superior to 2000 mg/kg bw. The extracts significantly reduced the mouse paw’s thickness at 600 mg/kg bw. The extracts developed weak radical scavenging and lipid peroxidation inhibitory effects. However, the macerate showed a high ability (664.90 ± 0.71 mol Ascorbic Acid Equivalent/g dry extract) to reduce the ferric ions. Saponins, sterols, triterpenes, and flavonoids were qualitatively detected in the two extracts. Total phenolics (TP) and total flavonoids (TF) were found abundant in the extracts, especially the decoction (TP content (TPC) = 94.03 ± 2.66 mg GAE/g;TF content (TFC) = 35.05 ± 0.32 mg QE/g). Strong positive correlations existed between ferric-reducing capacity and TPC (r = 0.959) for the macerate, while TFC was mainly involved in the DPPH radical scavenging of the two extracts. Instead, most correlations were negative between the polyphenol compounds and the anti-inflammatory assays. The results indicate potent in vivo anti-inflammatory and in vitro antioxidant effects of the aqueous extracts from the leaves of Opilia amentacea. From Traditional Management of Mental Illness to Self-Mutilation of the Penile: A Case Report

Introduction: Self-mutilation of the external genitalia is rare. It occurs most often on a psychotic ground. Case Report: Mr. P.O. 25-year-old, a dress maker, admitted to the urological emergency for self-section of the penile. The patient had cut his penile with a razor blade and the amputated distal extremity had been thrown in the water closet. The patient was admitted 19 hours after the act of self-harm. Clinical examination had found a stable hemodynamic status, a section of the penile at its root. Psychiatrically schizophrenia evolving for 08 months was diagnosed. An attempt of penile reconstruction was performed. The patient benefited serum and tetanus vaccine, antibiotic and neuroleptics. The parents asked to go home against medical advice the 5th day. Conclusion: The management of self-mutilation of external genitalia is multidisciplinary including surgery and psychiatry.
