Role of double-balloon enteroscopy in malignant small bowel tumors

AIM: To assess the double-balloon enteroscopy(DBE) role in malignant small bowel tumors(MSBT).METHODS: This is a retrospective descriptive study performed in a single center. All consecutive patients who underwent a DBE with final diagnosis of a malignant neoplasm from 2004 to 2014 in our referral center were included. Patient demographic and clinical pathological characteristics were recorded and reviewed. MSBT diagnosis was achieved either by DBE directed biopsy with multiple tissue sampling, endoscopic findings or histological analysis of surgical specimen. We have analyzed double-balloon enteroscopy impact in outcome and clinical course of these patients. RESULTS: Of 627 patients, 28(4.5%)(mean age = 60 ± 17.3 years) underwent 30 procedures(25 anterograde, 5 retrograde) and were diagnosed of a malignant tumor. Patients presented with obscure gastrointestinal bleeding(n = 19, 67.9%), occlusion syndrome(n = 7, 25%) and diarrhea(n = 1, 3.6%). They were diagnosed by DBE biopsy(n = 18, 64.3%), histological analysis of surgical specimen(n = 7, 25%) and unequivocal endoscopic findings(n = 2, 7.1%). Gastrointestinal stromal tumor(n = 8, 28.6%), adenocarcinoma(n = 7, 25%), lymphoma(n = 4, 14.3%), neuroendocrine tumor(n = 4, 14.3%), metastatic(n = 3, 10.7%) and Kaposi sarcoma(n = 1, 3.6%) were identified. DBE modified outcome in 7 cases(25%), delaying or avoiding emergency surgery(n = 3), modifying surgery approach(n = 2) and indicating emergency SB partial resection instead of elective approach(n = 2).CONCLUSION: DBE may be critical in the management of MSBT providing additional information that may be decisive in the clinical course of these patients.

Can DNA fragmentation of neat or swim-up spermatozoa be used to predict pregnancy following ICSI of fertile oocyte donors？

This study compared the potential of assessing sperm DNA fragmentation （SDF） from neat semen and the subsequent swim-up （SU） procedure to predict pregnancy when conducting ICSI of fertile donor oocytes. Infertile females （n=81） were transferred embryos resulting from intracytoplasmic sperm injection （ICSI） of their partner＇s spermatozoa and proven donor oocytes. This model normalized the impact of female factor in putative sperm DNA repair. Semen was blindly assessed for SDF using Halosperm immediately following ejaculation （NS） and after swim-up at the time of ICSI fertilisation. There was a decrease in SDF values of the ejaculated semen sample following the swim-up protocol （P=0.000）. Interestingly, pregnancy could be equally predicted from SDF values derived from either neat or swim-up semen samples. Receiver operator curves and the derived Youden＇s indices determined SDF cutoff values for NS and SU of 24.8% and 17.5%, respectively. Prediction of pregnancy from NS SDF had a sensitivity of 75% and a specificity of 69%, whereas for SU SDF was 78% and 73%, respectively. While increased levels of SDF negatively impact reproductive outcome, we have shown that a reduction in SDF following sperm selection using ICSI with proven donor oocytes is not mandatory for achieving pregnancy. This suggests that a certain level of DNA damage that is not detectable using current technologies could be impacting on the relative success of assisted reproductive technology （ART） procedures. Consequently, we propose a modification of the so called ＇iceberg model＇ as a possible rationale for understanding the role of SDF in reproductive outcome.

Ketamine anesthesia reduces intestinal ischemia/reperfusion injury in rats

AIM: To investigate the effects of ketamine anesthesia on the motility alterations and tissue injury caused by ischemia/reperfusion in rats. METHODS: Thirty male Wistar rats weighing 200-250 g were used. Ischemia was induced by ob-structing blood flow in 25% of the total small intesti-nal length (ileum) with a vascular clamp for 45 min, after which either 60 min or 24 h of reperfusion was allowed. Rats were either anesthetized with pento-barbital sodium (50 mg/kg) or ketamine (100 mg/kg). Control groups received sham surgery. After 60 min of reperfusion, the intestine was examined for mor-phological alterations, and after 24 h intestinal basic electrical rhythm (BER) frequency was calculated, and intestinal transit determined in all groups. RESULTS: The intestinal mucosa in rats that were anesthetized with ketamine showed moderate altera-tions such as epithelial lifting, while ulceration and hemorrhage was observed in rats that received pento-barbital sodium after 60 min of reperfusion. Quantita-tive analysis of structural damage using the Chiu scaleshowed significantly less injury in rats that received ketamine than in rats that did not (2.35 ± 1.14 vs 4.58 ± 0.50, P < 0.0001). The distance traveled by a mark-er, expressed as percentage of total intestinal length, in rats that received pentobarbital sodium was 20% ± 2% in comparison with 25.9% ± 1.64% in rats that re-ceived ketamine (P = 0.017). BER was not statistically different between groups. CONCLUSION: Our results show that ketamine anesthesia is associated with diminished intestinal injury and abolishes the intestinal transit delay induced by ischemia/reperfusion.

Tree-Based Revocation for Certificateless Authentication in Vehicular Ad-Hoc Networks

This work proposes authentication based on identity as a way to increase the efficiency and security of communications in vehicular ad-hoc networks. When using identity-based cryptography to achieve certificateless authentication, membership revocation is not a trivial problem. Thus, in order to improve the performance of revocation in such networks, the use of a dynamic authenticated data structure based on perfect k-ary hash trees combined with a duplex version of the new standard SHA-3 is here presented. Efficient algorithms in the used revocation trees allow reaching a refresh rate of at most simple updates per inserted node. Consequently, the proposal is especially useful for situations with frequent revocations, which are foreseeable when vehicular ad- hoc networks are widely deployed.

Highly graphitized carbon nanosheets with embedded Ni nanocrystals as anode for Li-ion batteries

A C/Ni composite was prepared via thermal decomposition of a nickel oleate complex at 700℃,yielding disperse Ni nanocrystals with an average size of 20 nm,encapsulated by carbon nanosheets as deduced from transmission electron microscopy(TEM)images and confirmed from X-ray photoelectron spectroscopy(XPS).Furthermore,the X-ray diffraction pattern revealed a good ordering of the carbon layers,forced by the Ni encapsulation to adopt a bending *** the close interaction between the graphitized framework and the metallic nanoparticles we have studied the properties of the composite as an anode for Li-ion *** with other nanostructured synthetic carbons,this carbon composite has a low voltage hysteresis and a modest irreversible capacity value,properties that play a significant role in its behaviour as electrodes in full cell *** moderate rate values,0.25 C,the electrode delivers an average capacity value around 723 mAh·g^−1 on cycling,among the highest values so far reported for this carbon *** higher rate values,1 C,the average capacity values delivered by the cell on cycling decrease,around 205 mAh·g^−1,but it maintains good capacity retention,a coulombic efficiency close to 100%after the first cycles and recovery of the capacity values when the rate is restored from 3 to 0.1 C.

Notwithstanding the emergence of FinTech startups in the financial services industry,most of these companies face significant difficulties in breaking even and *** study examines the main managerial, institutional, and financial drivers of FinTechprofitability and the time it takes for these firms to break even. The database includesrelevant qualitative factors, such as foundational characteristics, the technologicalprofile of the startup, and its funding structure. Using the full population of FinTechstartups operating in Spain from 2005 to 2017, we observe that most of these firms areunprofitable within three years of their inception. Combining panel data and survivalanalyses, we empirically find that large and solvent FinTech firms founded by singleentrepreneurs in an incubator or accelerator program are more likely to be profitableand prevail. FinTech firms reach their break-even points faster if they receive fundingthrough seed capital.

AIM:The pathogenesis of hypogonadism in liver cirrhosis is not well *** results from our laboratory showed that IGF-1 deficiency might play a pathogenetic role in hypogonadism of *** administration of IGF-1 for a short period of time reverted the testicular atrophy associated with advanced experimental cirrhosis. The aim of this study was to establish the historical progression of the described alterations in the testes, explore testicular morphology,histopathology,cellular proliferation,integrity of testicular barrier and hypophyso- gonadal axis in rats with no ascitic cirrhosis. METHODS:Male Wistar rats with histologically-proven cirrhosis induced with carbon tetrachloride(CCI_4)for 11 wk, were allocated into two groups(n=12,each)to receive recombinant IGF-1(2 μg/100 gd,sc)for two weeks or *** rats receiving vehicle were used as control group(n=12). RESULTS:Compared to controls,rats with compensated cirrhosis showed a normal testicular size and weight and very few histopathological testicular abnormalities. However,these animals showed a significant diminution of cellular proliferation and a reduction of testicular transferrin *** addition,pituitary-gonadal axis was altered,with significant higher levels of FSH(P<0.001 vscontrols)and increased levels of LH in untreated cirrhotic ***,IGF-1 treatment normalized testicular transferrin expression and cellular proliferation and reduced serum levels of LH(P=ns vs controls,and P<0.01 vs untreated cirrhotic group). CONCLUSION:The testicular barrier is altered from an early stage of cirrhosis,shown by a reduction of transferrin expression in Sertoli cells,a diminished cellular proliferation and an altered gonadal *** treatment with IGF-1 could be also useful in this initial stage of testicular disorder associated with compensated cirrhosis.
