Background: Pancreaticoduodenectomy is the standard treatment for resectable periampullary cancer. Surgical site infections(SSI) are common complications with increased morbidity. The study aimed to describe the prevalence, risk factors, microbiology, and outcomes of SSI among patients undergoing pancreaticoduodenectomy. Methods: We conducted a retrospective study in a referral cancer center between January 2015 and June 2021. We analyzed baseline patient characteristics and SSI occurrence. Culture results and susceptibility patterns were described. Multivariate logistic regression was used to determine risk factors, proportional hazards model to evaluate mortality, and Kaplan-Meier analysis to assess long-term survival. Results: A total of 219 patients were enrolled in the study;101(46%) developed SSI. Independent factors for SSI were diabetes mellitus, preoperative albumin level, biliary drainage, biliary prostheses, and clinically relevant postoperative pancreatic fistula. The main pathogens were Enterobacteria and Enterococci. Multidrug-resistance rate in SSI was high but not associated with increased mortality. Infected patients had higher odds of sepsis, longer hospital stay and intensive care unit stay, and readmission rate. Neither 30-day mortality nor long-term survival was significantly different between infected and non-infected patients. Conclusions: SSI prevalence among patients undergoing pancreaticoduodenectomy was high and largely caused by resistant microorganisms. Most risk factors were related to preoperative instrumentation of the biliary tree. SSI was associated with greater risk of unfavorable outcomes;however, survival was unaffected.

Contamination of Water for Domestic Use and Human Consumption by Heavy Metals Derived from Mining Activity in Parral, Chihuahua, Mexico

Research was done with the objective of evaluating the quality of water supplied to the potable water system in Hidalgo del Parral, which comes from two main supply sources: wells from "Valle del Verano", which provide water to the east side of the city, and the Water Treatment Plant, which receives water from different mines and supplies the west side of the city. When this plant is overcome on its capacity of treatment, a part of the water from the mines is fed directly to the northwest part of the city. The metals lead (Pb), chromium (Cr), cadmium (Cd), zinc (Zn), and arsenic (As) were analyzed by atomic absorption (AA) technique, based on the United States Environmental Protection Agency (EPA) Method 7000B (Revision 2, February 2007). The digestion process was done according to the guidelines established in Method NMX-AA-051-SCFI-2001, Analysis of Water-Determination of Metals by Atomic Absorption, developed by the National Standardization Technical Committee for Environmental Protection (Mexico). The samples were collected from the following points: 13 wells and 2 re-pumping stations of "Valle del Verano", the inlet and outlet of the Water Treatment Plant, and domestic faucets. The east side of the city, supplied by the wells of "Valle del Verano", is metal-pollution free. The water that comes from the mines contains high concentrations of As, Zn, Cd and Pb, with levels that can get to 0.12, 32.6, 0.26 and 3.5 mg/L, respectively. This water pollution generated by mining can be concluded from the high levels found in the domestic samples in which, for several points, the concentrations exceed the parameters set by the current guideline. According to the results, it is vital to set a plan of remediation for the water that comes from the mines, because there is a great risk for health in the consumption of this water.

The biological properties of therapeutic peptides,such as their pharmacokinetics and pharmacodynamics,are correlated with their structure and aggregation ***,we studied the aggregation properties of a therapeutic peptide(CIGB-814),currently in phase 2 clinical trial,for the treatment of rheumatoid arthritis over a wide range of concentrations(μM-mM).We applied spectroscopic techniques(fluorescence,circular dichro-ism,resonance,and dynamic light scattering),atomic force microscopy,and molecular dynamics simulations to determine the aggregation mechanism of *** found that the hierarchical aggregation of CIGB-814 at micromolar concentrations was initiated by the formation of peptide ***,the peptide oligomers trigger the nucleation and growth of peptide nanostructures(cac=123μM),ultimately leading to the fibrillization of CIGB-814(cac’=508μM).These results pave the way for a deeper understanding of the CIGB-814 therapeutic activity and may give important insights on its pharmacokinetics.

An Unusual Presentation of Aortic Dissection and Aneurysmatic Disease in Marfan Syndrome: A Case Report

Marfan’s syndrome (MFS) is an autosomal dominant disorder characterized by aortic root dilation, mitral valve prolapse and aortic regurgitation. The typical aneurysm location involves the thoracic aorta (60%). Thoracoabdominal aortic dissections are quite infrequent (10%). A type A aortic dissection can have fatal complications, such as aortic rupture and cardiac tamponade, and a prompt diagnosis is crucial. The present case involves a 46-year-old patient with Marfan’s syndrome who was admitted to the emergency department for dyspnea. The patient presented congestive signs and manifestations suggesting acute heart failure. An initial transthoracic echocardiogram (TTE) showed ventricular hypertrophy and diastolic dysfunction. Additionally, TTE showed plurivalvular regurgitation, pulmonary hypertension (42 mmHg), aortic dilatation, and a dissection flap. Coronary computed tomography angiography (CCTA) was performed, confirming the suspicion of ascending aortic dissection. The patient underwent a three-stage intervention for the repair of all the defects, with adequate recovery. This was a rare presentation of aortic dissection in MFS, which was a diagnostic challenge for the heart team.

Determination of Mercuric Ion in Water Samples with a LED Exciting and CCD Based Portable Spectrofluorimeter

The fluorescent characteristics of a fluorimetric chemosensor for mercuric ion, Hg2+, employing a synthesized Rhodamine 6G derivative, have been analyzed. For that, a portable spectrofluorimeter composed of a 515 nm LED as excitation source, two fiber-optics and a CCD camera as detector, has been used, intended for “in situ” analysis. A highly selective Rhodamine based probe for Hg2+, that is water soluble and gives a positive response upon analyte binding, is reported. The reagent is bearing a monothiospirolactone group in a Rhodamine 6G architecture and the thiol atom served for the direct attack of thiophilic Hg2+. The fluorescence enhancement is attributed to the spirolactone ring opening and the coordination of two sulphur atoms to Hg2+ giving a 2:1 reagent: Hg2+ stoichiometry complex.

As stated in the Declaration of the Rights of the Child, children have the right to go to school for free, to play, and to have the same opportunity to develop and to learn to be responsible and useful[1]. Young children are curious by nature and they have passion for learning [2]. An environment that encourages scientific thinking is what gives young children the time, space and materials to exercise their curiosity [3]. The aim of this paper is to present a methodology for the development of educational environments based on digital technology, internet and free programs available on the web, for the use and application of structural bioinformatics protein and DNA, for early stimulation in basic education at rural and indigenous communities in state of Oaxaca, conducted in On Line Learning & Training “OLL & T”. The course is divided into 4 sections: 1)Introduction to the topic of protein and DNA structure;2)Search in protein data bank;3)Download RasTop program and use it as bioinformatics tool;4)Emulation of building a viral molecule. Workshops were given to children in elementary, middle and high school at the Universidad del Papaloapan during the National Week of Science and Technology, every year in Oaxaca since 2007 to 2012 and in Monterrey 2010 at the Universidad Autonoma de Nuevo León. Conclusions: positive impact was achieved in basic education in the area of natural sciences by analyzing the molecular basis of protein and deoxyribonucleic acid, theoretically and practically, as a support for teachers.
