Hepatitis C virus(HCV)infection represents an important public health problem worldwide.Reduction of HCV morbidity and mortality is a current challenge owned to several viral and host factors.Virus molecular evolution plays an important role in HCV transmission,disease progression and therapy outcome.The high degree of genetic heterogeneity characteristic of HCV is a key element for the rapid adaptation of the intrahost viral population to different selection pressures(e.g.,host immune responses and antiviral therapy).HCV molecular evolution is shaped by different mechanisms including a high mutation rate,genetic bottlenecks,genetic drift,recombination,temporal variations and compartmentalization.These evolutionary processes constantly rearrange the composition of the HCV intrahost population in a staging manner.Remarkable advances in the understanding of the molecular mechanism controlling HCV replication have facilitated the development of a plethora of direct-acting antiviral agents against HCV.As a result,superior sustained viral responses have been attained.The rapidly evolving field of anti-HCV therapy is expected to broad its landscape even further with newer,more potent antivirals,bringing us one step closer to the interferon-free era.

Optimization of Exenatide plus Detemir Treatment for Morbid Obese Patients with Insulin Dependent Diabetes Resistant to Conventional Treatment. Effect of Exenatide on Patients with Mellitus Diabetes Type 2

The purpose of this study was to analyze the effects of the treatment with Exenatide joint to insulin detemir. The study was carried out with obese patients with type 2 diabetes mellitus (DM2) and deficient control, despite intensive insulin therapy combined with oral antidiabetics. A longitudinal study was made with 15 patients controlled by the Endocrinology and Nutrition Section of the Reina Sofía University Hospital, Murcia. These patients were treated with Exenatide joint to insulin during 3, 6 and 12 months. Statistically meaningful differences (p < 0.05) were observed in the reduction of HbA1c in the 3 and 6 months treatment groups (initial HbA1c: 9.55% ± 1.086%, weight: 119.94 kg, insulin requirement: 0.71 IU/kg/day;after 3 months: HbA1c: 7.77% ± 1.164%, weight: 98.13 kg, insulin requirement: 0.59 IU/kg/day;after 6 months: HbA1c: 7.21%, weight: 97.93 kg, insulin requirement: 0.55 IU/kg/day). It was observed that the association of Exenatide and insulin promotes an improvement in several indicators such as weight, body mass index (BMI), systolic pressure (SP), diastolic pressure (DP), triglycerides (TG), cLDL, cHDL and total cholesterol (TC), in patients with diabetes resistant to conventional therapy. However, these improvements were statistically no significant (p < 0.05). It is possible to talk of a reduction of cardiovascular risk factors in patients with Exenatide, being thus a suitable option for this kind of individuals. It is important to emphasize the fact that our study showed bigger body-weight reductions results after 6 and 12 months, as compared to the researches quoted.

Hypopigmented Mycosis Fungoides in a 7-Year-Old Boy

Hypopigmented mycosis fungoides (HMF) is an uncommon variant of cutaneous T-cell lymphoma. It is more frequent in dark-skinned people, particularly children. The HMF diagnose is difficult, especially in early stages because this condition resembles benign skin diseases. Thus is histopathological analysis very important for the diagnosis. We report a case of a 7-year-old child with widespread HMF confirmed by histopathology that showed cells tagging along the dermal/epidermal junction and extending into the epidermis in a pattern of epidermotropism and focal cell aggregates in the epidermis (Pautrier’s microabscess). We demonstrate the importance of clinical suspicion for this cutaneous neoplasia in patients with hypopigmentated lesions.

Prevalence of refractive errors in Villa maria,Córdoba,Argentina

Background:Refractive errors are among the most frequent reasons for demand of eye-care services.Publications on refractive errors prevalence in our country are few.This study has the purpose to assess the prevalence of refractive errors in an adult population of Villa maria,Córdoba,Argentina.Methods:The Villa maria Eye Study is a population-based cross-sectional study conducted in the city of Villa maria,Córdoba,Argentina from May 2008 to November 2009.Subject’s aged 40+received a demographic interview and complete ophthalmological exam.Visual acuity was obtained with an ETDRS chart.Cycloplegic auto refraction was performed.The spherical equivalent was highly correlated between right and left eyes,so only data of right eyes are presented.Myopia and hyperopia were defined with a±0.50 diopters(D)criterion and astigmatism>1 D.Results:This study included 646 subjects,aged 40 to 90(mean age:59.6±10.3 years old).Four hundred and sixty two(71.5%)were females.The mean spherical equivalent was+0.714±2.41 D(range,-22.00 to+8.25 D)and the power of the cylinder was,on average,-0.869±0.91 D(range,0 to-6.50 D).In this sample,61.6%subjects were hyperopic,and 13.5%were myopic.Myopia prevalence was lower in men(9.8%versus14.9%)but this difference among genders was not statistically significant.There were 141 subjects(21.8%)with anisometropia greater than 1 D,and 168 subjects(26.0%)with astigmatism greater than 1 D.Conclusions:The present study shows the prevalence of cycloplegic refractive errors in an adult population of Argentina.The prevalence of hyperopia was high,while myopia prevalence was very low.

Citizen scienee data have already been used to effectively address questions regarding migration,a fun dame ntal stage in the life history of birds.In this study,we use data from eBird and from 3 additional regional citizen scienee databases to describe the migration routes and timing of the red-footed falcon Falco vespertinus in the Mediterranean region across 8years(2010-2017).We further examine the seasonal and yearly variation in migration patterns and explore sites used during the species migration.Our results suggest that the autumn passage is spatially less variable and temporally more consistent among years than in spring and that birds migrate faster in spring than in autumn.The species seems to be more prevalent along the Central Mediterranean during spring migration,probably as a result of the clockwise loop migration that red-footed falcons perform.There was a high variation in annual median migration dates for both seasons as well as in migration routes across years and seasons.Higher variation was exhibited in the longitudinal component thus indicating flexibility in migration routes.In additi on,our results showed the species'preference for I owl a nds covered with cropla nd and mosaics of cropland and natural vegetation as stopover sites during migration.Stopover areas predicted from our distribution modeling highlight the importance of the Mediterranean islands as stopover sites for sea-crossing raptors,such as the red-footed falcon.This study is the first to provide a broad-scale spatiotemporal perspective on the species migration across seasons,years and flyways and dem on strates how citize n science data can inform future monitori ng and conservation strategies.

Cyclin-dependent kinases 4 and 6(CDK4/6)inhibitors demonstrated activity in terms of progression-free survival(PFS)in advanced dedifferentiated liposarcoma(DD-LPS),a sarcoma with CDK4 amplification.CDK4 overexpression is by far more common than amplification in sarcomas and it might be a rational target for CDK inhibitors.Preclinical investigators of this study found that CDK4 overexpression,while not of CDKN2A,was the most consistent predictive factor for palbociclib efficacy in sarcomas.Advanced adult-type soft-tissue sarcoma,excluding DD-LPS,or bone sarcoma patients,progressing after at least one systemic line,whose tumors overexpressed CDK4,but not CDKN2A at baseline biopsy,were accrued in this single-arm phase II trial(EudraCT number:2016-004039-19).With the main endpoint of a 6-month PFS rate,40%was considered promising in this population.Palbociclib was administered orally at 125 mg/day for 21 days in 28-day cycles.A total of 214 patients with 236 CDK4/CDKN2A determinations were assessed for prescreening,archival material(141),and screening,baseline biopsy(95).There were 28(29%)with favorable mRNA profiles from 95 screened patients at baseline.From 23 enrolled patients,21 evaluable,the 6-month PFS rate was 29%(95%CI 9–48),and there were 6 patients out of 21 with a PFS longer than 6 months.The median PFS and overall survival were 4.2(95%CI 3.6–4.8)and 12(95%CI 8.7–15.4)months,respectively.Translational research showed a significant correlation between CDK4 mRNA and protein expression.Palbociclib was active in a variety of sarcoma subtypes,selected by CDK4/CDKN2A,and deserves further investigation in the sarcoma context.

Synthetic Three-Dimensional Scaffold for Application in the Regeneration of Bone Tissue

Bone tissue engineering aims to use biodegrade able scaffolds to replace damaged tissue. This scaffold must be gradually degraded and replaced by tissue as similar as possible to the original one. In this work a hybrid porous scaffold containing chitosan, polyvinyl alcohol and bioactive glass was successfully obtained and subsequently characterized by scanning electron microscopy. The scaffold presented satisfactory pore size range and open interconnected pores, which are essential for tissue ingrowth. A cytotoxicity assay showed that this biomaterial allows adequate cell viability, so that it was considered suitable for an in vivo experiment. Promising results were obtained with the implant of the scaffold in an experimental model of a New Zealand rabbit femur bone lesion. Clinical and biochemical parameters measured such as complete blood count, total serum proteins, albumin, alanine aminotransferase and aspartate aminotransferase were similar between animals in the control group at all time periods studied. Histological and histometric studies showed that the scaffold was coated with a cement-like substance, exhibiting many areas of mineralized structures. Very few osteocyte-like cells or lining-like cells were found inside the amorphous mineralized deposit. In vivo results allow us to consider this scaffold as a promising biomaterial to be applied in bone tissue engineering.
