Diagnostic endoscopic ultrasonography:Assessment of safety and prevention of complications

Endoscopic ultrasonography (EUS) has gained wide acceptance as an important, minimally invasive diagnostic tool in gastroenterology, pulmonology, visceral surgery and oncology. This review focuses on data regarding risks and complications of non-interventional diagnostic EUS and EUS-guided fine-needle biopsy (EUS-FNB). Measures to improve the safety of EUS und EUS-FNB will be discussed. Due to the specific mechanical properties of echoendoscopes in EUS, there is a low but noteworthy risk of perforation. To minimize this risk, endoscopists should be familiar with the specific features of their equipment and their patients' specific anatomical situations (e.g., tumor stenosis, diverticula). Most diagnostic EUS complications occur during EUS-FNB. Pain, acute pancreatitis, infection and bleeding are the primary adverse effects, occurring in 1% to 2% ofpatients. Only a few cases of needle tract seeding and peritoneal dissemination have been reported. The mortality associated with EUS and EUS-FNB is 0.02%. The risks associated with EUS-FNB are affected by endoscopist experience and target lesion. EUS-FNB of cystic lesions is associated with an increased risk of infection and hemorrhage. Peri-interventional antibiotics are recommended to prevent cyst infection. Adequate education and training, as well consideration of contraindications, are essential to minimize the risks of EUS and EUS-FNB. Restricting EUS-FNB only to patients in whom the cytopathological results may be expected to change the course of management is the best way of reducing the number of complications.

中国与南美国家贸易具有长期的发展历史,近年来随着中国与南美国家经济发展,双边贸易越来越频繁,贸易结构也在发生改变,特别是随着“一带一路”倡议的实施,中国与南美国家的贸易快速发展。影响中国与南美国家贸易发展的因素有哪些?中国与南美国家贸易具有哪些合作潜力?在上述背景下,对这些问题的深入研究具有重要意义。本文首先对中国与南美贸易现状进行了分析,重点分析了中国与南美国家的贸易规模和产品结构,采用显性比较优势指数和产业内贸易指数,对中国与南美国家贸易的竞争性和互补性进行测算。其次,本文采用贸易引力模型,在原有变量基础上,引进了汇率、班轮运输连通性指数、是否同属一个贸易组织三个新增变量,进而对中国与南美国家贸易影响因素进行了实证分析。最后,本文采用贸易潜力模型对中国与南美国家的贸易潜力进行了实证分析。本文研究发现:中国与南美国家的贸易产品总体上以互补性为主,竞争性不强。中国与南美国家在信息和通讯技术产品、制造业产品、矿石和金属产品、燃料产品、食品、农业原材料方面都存在很强的互补性。中国与南美国家贸易影响因素的回归分析表明:GDP对双边的贸易额增长有正向影响,双方经济规模扩大有利于双边贸易发展;经济距离对双方贸易额增长有负向影响;人口数量对双边贸易额增长有正向影响;班轮运输连通性指数对双方贸易额增加有正向影响;南美国家货币兑换人民币的汇率对双边贸易额增长有负向影响;是否同属一个经济组织对双边贸易额具有正向影响。中国与南美贸易潜力巨大的国家有玻利维亚、厄瓜多尔、巴拉圭、哥伦比亚、圭亚那;具有贸易潜力再造市场的国家有巴西、秘鲁、乌拉圭、阿根廷;需要全力开发市场的国家是智利。论文在研究结论基础上提出对策建议,指出要提高中国与南美国家的贸易互补性,推进中国与南美的国际产能合作,促进自由贸易市场的合作和发展。

Phenotype analysis and rescue on female FVB.129-Fmr1 knockout mice

Fragile X syndrome （FXS） is the most common monogenic cause of intellectual disability and a cause for autism. FXS females report milder phenotypes and a lower rate of cognitive problems compared to males. This is most likely because most females are heterozygous, while males are hemizygous for the disease. Thus, most preclinical studies have been completed in males. As there is major interest in testing experimental drugs for FXS, it is imperative to determine whether females in animal models used for research, present behavioral alterations that might translate to humans in order to confirm that experimental drugs have an effect on both genders. In our study we describe behavioral phenotypes in homozygous FXS female mice developed on the FVB.129 background. We focused on detection of hippocampal-mediated cognitive abilities and other behaviors described for FXS. Our research shows that, while female FVB.129-Fmrl knockout mice present normal learning, they have impaired memory, as well as susceptibility to audiogenic seizures. In agreement with previous reports in rodents and humans, significant levels of the small GTPase Racl were found in FXS female mice. Because Racl is involved in neuronal development, plasticity and behavior, we additionally aimed to pharmacologically inhibit Racl and determine whether observed phenotypes are rescued. Treatment of female FVB.129-Fmrl knockout with a Racl inhibitor abolished behavioral deficits, bringing phenotypes to control levels. Our results suggest that female FVB.129-Fmrl knockout mice display behavioral impairments that resemble FXS in humans. Moreover, those behavioral shortfalls might be associated with alteration of plasticity involving excessive Racl function, since pharmacological reduction of Racl normalizes previously altered phenotypes to control levels.

Circulating monocytes （Mos） may continuously repopulate macrophage （MAC） or dendritic cell （DC） populations to maintain homeostasis. MACs and DCs are specialized cells that play different and complementary immunological functions. Accordingly, they present distinct migratory properties. Specifically, whereas MACs largely remain in tissues, DCs are capable of migrating from peripheral tissues to lymphoid organs. The aim of this work was to analyze the expression of the fractalkine receptor （CX3CR1） during the monocytic differentiation process. Freshly isolated Mos express high levels of both CX3CR1 mRNA and protein. During the Mo differentiation process, CX3CR1 is downregulated in both DCs and MACs. However, MACs showed significantly higher CX3CR1 expression levels than did DC. We also observed an antagonistic CX3CR1 regulation by interferon （IFN）-γ and interleukin （IL）-4 during MAC activation through the classical and alternative MAC pathways, respectively. IFN-γinhibited the loss of CX3CR1, but IL-4 induced it. Additionally, we demonstrated an association between CX3CR1 expression and apoptosis prevention by soluble fractalkine （sCX3CL1） in Mos, DCs and MACs. This is the first report demonstrating sequential and differential CX3CR1 modulation during Mo differentiation. Most importantly, we demonstrated a functional link between CX3CR1 expression and cell survival in the presence of sCX3CL1.

Effectiveness of dry needling on reducing pain intensity in patients with myofascial pain syndrome： a Meta-analysis

OBJECTIVE: To summarize the literature about the effectiveness of dry needling(DN) on relieving pain and increasing range of motion(ROM) in individuals with myofascial pain syndrome(MPS).METHODS: Papers published from January 2000 to January 2013 were identified through an electronic search in the databases MEDLINE, Dialnet, Cochrane Library Plus, Physiotherapy Evidence Database(PEDro) and Spanish Superior Council of Scientific Research(CSIC). The studies included were randomized controlled trials written in English and/or Spanish about the effectiveness of DN on pain and ROM in individuals with MPS.RESULTS: Out of 19 clinical trials that were potentially relevant, a total of 10 were included in the Meta-analysis. Regarding pain intensity reduction when measured before and immediately after the intervention, DN achieved improvement compared with the placebo treatment [d =-0.49; 95% CI(-3.21, 0.42)] and with the control group [d =-9.13;95% CI(- 14.70,- 3.56)]. However, other treatments achieved better results on the same variable compared with DN, considering the measurements for pre-treatment and immediately after [d = 2.54;95% CI(-0.40, 5.48)], as well as the pre-treatment and after 3-4 weeks [d = 4.23; 95% CI(0.78, 7.68)].DN showed a significantly increased ROM when measured before the intervention and immediately after, in comparison with the placebo [d = 2.00;95% CI(1.60, 2.41)]. However, other treatments achieved a significant better result regarding ROM when it was measured before the intervention and immediately after, as compared with DN [d =-1.42;95% CI(-1.84,-0.99)].CONCLUSION: DN was less effective on decreasing pain comparing to the placebo group. Other treatments were more effective than DN on reducing pain after 3-4 weeks. However, on increasing ROM,DN was more effective comparing to that of placebo group, but less than other treatments.

Robotic approach is already a new technological system in surgery and its potential advantages such as ergonomics,reduced tremor,3D view and improved instruments movements have been well described(1,2).Concerning the current literature,the studies addressing the robotic benefits on left pancreatectomies are few.However,it has been showed that robotic distal pancreatectomy is both feasible and safe as well as the laparoscopic and the standard open approach.Although,there is a lack of high-level economic studies comparing these techniques(3-6).

Patterns of body size variation along geographical gradients have long been searched for and generalized into eco-geographical rules.However,no rodent species has yet been analyzed in relation to the 3 dimensions of latitude,longitude and altitude.We analyzed geographical clines and dimorphism of body and skull size in the herb field mouse(Apodemus uralensis)across the species range,based on field data and on data from the literature.Sexual dimorphism in adult A.uralensis was not expressed at a large scale,while local patterns were inconsistent.Age-dependent size changes were most expressed in adult individuals:most characters of adults exceeded in size those of subadults,while subadult-juvenile size differences were only significant in body weight and length,zygomatic skull width,length of cranial diastema and breadth of braincase.Despite central morphological niches along the clines being separated,A.uralensis populations showed a high degree of size overlap in morphological space.We found the species to be characterized by high size variability,with the largest individuals inhabiting the eastern and southern edges of the distribution range.Tail,hind foot and ear lengths were largest in the southern part of the range,in agreement with Allen’s rule.The main measurements that we analyzed,namely body mass,zygomatic skull width and condylobasal skull length,show the presence of 3 clines in the size of adult A.uralensis:(i)a decreasing south-north cline,opposing Bergmann’s rule;(ii)an increasing west-east cline,in accordance with Murphy’s rule;and(iii)an increasing altitudinal cline.
