Percutaneous electrochemotherapy in the treatment of portal vein tumor thrombosis at hepatic hilum in patients with hepatocellular carcinoma in cirrhosis: A feasibility study

AIMTo treated with electrochemotherapy (ECT) a prospective case series of patients with liver cirrhosis and Vp3-Vp4- portal vein tumor thrombus (PVTT) from hepatocellular carcinoma (HCC), in order to evaluate the feasibility, safety and efficacy of this new non thermal ablative technique in those *** patients (5 males and 1 female), aged 61-85 years (mean age, 70 years), four in Child-Pugh A and two in Child-Pugh B class, entered our study series. All patients were studied with three-phase computed tomography (CT), contrast enhanced ultrasound (CEUS) and ultrasound-guided percutaneous biopsy of the thrombus before ECT. All patients underwent ECT treatment (Cliniporator Vitae®, IGEA SpA, Carpi, Modena, Italy) of Vp3-Vp4 PVTT in a single session. At the end of the procedure a post-treatment biopsy of the thrombus was performed. Scheduled follow-up in all patients entailed: CEUS within 24 h after treatment; triphasic contrast-enhanced CT and CEUS at 3 mo after treatment and every six months ***-treatment CEUS showed complete absence of enhancement of the treated thrombus in all cases. Post-treatment biopsy showed apoptosis and necrosis of tumor cells in all cases. The follow-up ranged from 9 to 20 mo (median, 14 mo). In 2 patients, the follow-up CT and CEUS demonstrated complete patency of the treated portal vein. Other 3 patients showed a persistent avascular non-tumoral shrinked thrombus at CEUS and CT during follow-up. No local recurrence was observed at follow-up CT and CEUS in 5/6 patients. One patient was lost to follow-up because of death from gastrointestinal hemorrage 5 wk after *** patients with cirrhosis, ECT seems effective and safe for curative treatment of Vp3-Vp4 PVTT from HCC.

Secure Messaging Implementation in OpenSC

Smartcards are used for a rapidly increasing number of applications including electronic identity, driving licenses, physical access, health care, digital signature, and electronic payments. The use of a specific smartcard in a 'closed' environment generally provides a high level of security. In a closed environment no other smartcards are employed and the card use is restricted to the smartcard's own firmware, approved software applications, and approved card reader. However, the same level of security cannot be claimed for open environments where smartcards from different manufacturers might interact with various smartcard applications. The reason is that despite a number of existing standards and certification protocols like Common Criteria and CWA 14169, secure and convenient smartcard interoperability has remained a challenge. Ideally, just one middleware would handle the interactions between various software applications and different smartcards securely and seamlessly. In our ongoing research we investigate the underlying interoperability and security problems specifically for digital signature processes. An important part of such a middleware is a set of utilities and libraries that support cryptographic applications including authentication and digital signatures for a significant number of smartcards. The open-source project OpenSC provides such utilities and libraries. Here we identify some security lacks of OpenSC used as such a middleware. By implementing a secure messaging function in OpenSC 0.12.0 that protects the PIN and data exchange between the SC and the middleware, we address one important security weakness. This enables the integration of digital signature functionality into the OpenSC environment.

The New Silk Road and the Impact on the Italian Production System

The present work analyzes the asymmetric relationship between China and Italy with a specific focus on the impact of the Belt and Road Initiative(BRI)on the Italian production *** this analysis,it comes to light the importance of China as a partner both for the industrial system and for the Italian production *** analysis of investments flows and trade to and from China in Italy evidence the potential benefits that the development of logistic infrastructure along the BRI,with specific reference to railway and maritime networks,could bring to Europe and Italy,as well as the quality of logistics infrastructure,comparing them also to others European Countries,using the performance index,such as Logistic Performance Index(LPI).In fact,it is evident that reducing transport costs and strengthening the infrastructure systems in several countries that are part of the BRI could gain ***,the issue is to understand and examine if,in an asymmetric structural economic and political situation,the result would be win-win,or instead the advantages would be redistributed unequally.
