Background: Agent targeting HER-2 pathway plus chemotherapy has represented a major progress in the management of patients with breast cancer. However, the role of late-line treatment in heavily pretreated patients is still largely unclear. In the last decade, nab-paclitaxel has shown significant activity and good toxicity profile in metastatic breast cancer. Case Presentation: We report the case of a 76-year-old Caucasian woman with metastatic HER-2 positive ductal infiltrating breast carcinoma treated with a combination of weekly nab-paclitaxel and trastuzumab as fifth-line therapy. She had previously received first-line paclitaxel and trastuzumab, second-line vinorelbine and trastuzumab, third-line TDM1 and fourth-line oral capecitabine and lapatinib. Clinical and radiological staging showed progression at bone, skin and soft-tissue. The patient received weekly nab-paclitaxel plus trastuzumab. Massive objective response was clinically and PET documented which lasted 8 months. Tolerance to treatment was fairly good as well as cardiac safety. Conclusion: To the best of our knowledge, this is the first reported case of efficacy of nab-paclitaxel in combination with trastuzumab as fifth-line of treatment in a patient with metastatic HER-2 positive breast cancer.

Qualitative Characterization and Differentiation of Digestates from Different Biowastes Using FTIR and Fluorescence Spectroscopies

Anaerobic digestion of biomasses originates different products, the most abundant of which are methane and carbon dioxide. During this process, a 60-70% organic matter reduction occurs and the final product, the digestate, is charac- terized by high biological stability and high contents of recalcitrant organic molecules and nutrients. In the present work digestates obtained by different mixture of biomasses in a full-scale co-digestion plant operating in Italy were characterized as whole samples without any pre-treatment or extraction by means of Fourier transform infrared spec- troscopy and fluorescence spectroscopy in the synchronous-scan mode and results were compared to those obtained on the single fresh substrates. Biomasses considered were: beef cattle slurry, maize or sorghum silage, agro-industrial residues, olive residues and olive mill wastewater. These substrates exhibited typical spectra related to their different chemical composition. Results obtained on digestates provided evidence of distinctive characteristic of the final product as a function of the different composition of the biomasses loaded into the digestion plant. We concluded that FTIR and fluorescence spectra of digestates produced in a real co-digestion plant “inherit” the main spectroscopic features of the organic wastes from which they are produced. Spectroscopic techniques used in this work succeeded in qualitatively characterizing and differentiating digestates obtained from biomasses of different chemical composition.

The role of lactic acid bacteria for a correct course of fermentation of table olives has been extensively reviewed and it is a common idea that the inoculum of selected strains could exert a strong benefit for product quality;however, the basic assumption of this research is that a not-correct preparation of starter could affect its performances in brines and delay the benefit of a starter inoculum. Thus, some selected strains of LAB (lactic acid bacteria) and yeasts were inoculated into brines of Bella di Cerignola olives (Spanish style), as a single starter (LAB or yeasts) or a combined preparation (LAB + yeasts) and compared to a natural fermentation, focusing on the performances of the process (acidification, production of lactic acid) and trying to verify the basic assumption. LAB and yeasts were grown in the opportune lab media and then directly inoculated in brine after the lye treatment. LAB exerts a kind of benefit (increased content of lactic acid) only for 2 - 3 weeks;then, the inoculated samples behave in the same way as not-inoculated ones. Thus, this research underlined that a starter culture, not prepared in the correct way, could not be useful to guide the fermentation.

BACKGROUND The coronavirus disease 2019(COVID-19) pandemic produced changes in intensive care units(ICUs) in patient care and health organizations. The pandemic event increased patients’ risk of developing psychological symptoms during and after hospitalisation. These consequences also affected those family members who could not access the hospital. In addition, the initial lack of knowledge about the virus and its management, the climate of fear and uncertainty, the increased workload and the risk of becoming infected and being contagious, had a strong impact on healthcare staff and organizations. Background: Patients coinfected with HBV, HDV, HCV and HIV are usually excluded from clinical trials. Data on pegylated interferon treatment in this setting are limited, with predictive factors for HDV virologic success being unknown. Objectives: In this study we analyzed the time course of HDV viral load and HBsAg in HBV-HDV-HCV-HIV patients, who underwent pegylated alfa-2a interferon (peg-IFN) therapy for HDV infection between 2005 and 2009, with different virologic outcomes (no response, relapse or sustained response). Methods: Three patients were selected for virologic analysis, since complete clinical and laboratory data and stored residual blood samples, collected before/during/after peg-IFN treatment were available. Plasma samples were retrospectively analyzed for HDV-RNA detection and quantitative HBsAg determination. Results: All patients were HCV-Ab positive, persistently HCV-RNA negative, and received a peg-IFN treatment curse (180 mcg/week) for 12 to 18 months. HIV and HBV viral loads remained undetectable due to underlying Tenofovir/Emtricitabine (TDF/FTC) treatment. Low baseline HDV-RNA and HBsAg levels were both observed in the patient with sustained viral response. A HDV-RNA decline greater than 2log10 at month 6 was observed in two of the three patients, both with compensated liver cirrhosis, achieving a viral clearance at the end of treatment. Conclusions: Although performed in few patients, this study suggests that a decline of HDV-RNA during treatment and low baseline quantitative HBsAg may be associated to HDV virologic response to peg-IFN in HIV-infected subjects, independently of fibrosis stage. If confirmed on larger patient number, these data may help to select those HDV-infected patients with a reliable chance to respond to prolonged peg-IFN treatment and suggest the importance of quantitative HBsAg monitoring in this setting.

Background: The autonomy of dental anxiety may be compared with other psychopathologies, however, it is classified as indicative of a specific phobia. It is interesting to know how dental anxiety operates within a wider context. Material and Method: The group was made by 514 subjects, recruited from several dental surgeries. The entire process, consisted in self-compilation of the Dental Anxiety Scale (DAS), and the Patient Health Questionnaire (PHQ). The software used for statistical calculations was the Statistical Package for Social Sciences (SPSS) version 16.0. The chi-square test was used for the distribution of the nominal variables. Results: The analysis of data revealed a significantly different distribution between high and low levels of dental anxiety and comorbidity in relation to only one diagnostic category, namely mood disorders. Dental fears and comorbidity were mostly in line with our expectations, except about fear of needles with eating disorders. Dental anxiety is to be considered as a dimension on a par with other psychopathologies. Conclusions: The study of comorbidity between mood disorders and dental anxiety, suggests that dental anxiety rather than belonging to the family of anxiety disorders would be better classified along with mood disorders. The findings regarding emotional inducers (fears) in relation to the distribution of psychopathology are worth noting. The above observations reaffirm the need to form links between dentistry and psychiatry.

Functional Starter Cultures for Meat: A Case Study on Technological and Probiotic Characterization

The selection of a starter is a complex process, involving different steps, like a preliminary characterization under laboratory conditions, the selection of the most promising strains, a lab validation and the final validation in a large-scale fermentation;the selection of a functional starter includes a further step, dealing with the assessment of the functional traits. This paper proposes a case study on how use a step-by-step approach, based on some rapid protocols, to select promising starter cultures with probiotic abilities. Nine strains, isolated from a commercial preparation, were studied in relation to some enzymatic activities (slime and acetoin production, reduction of nitrates, proteolytic activity and hydrolysis of tributyrin), technological (acidification and growth at different temperatures, pHs and salt amounts) and probiotic traits (hydrophobicity, co-aggregation towards Salmonella sp. and Escherichia coli, survival at pH 2.5 and in presence of bile salts, antibiotic resistance) and compared to some collection isolates. As a final step, a multivariate approach was used to select the most promising isolates. The isolates from the commercial preparation were able to perform the fermentation (e.g. acidification) at low temperatures with some interesting traits in terms of bioactivity towards E. coli O157:H7, whilst some interesting probiotic traits in terms of hydrophobicity and survival in conditions that mimicked the gut were found for the collection isolates.

Classical swine fever （CSF） is a highly contagious viral disease of Suidae animals, caused by a Pestivirus from Flaviviridae family, considered one major cause of economic losses to the swine producers. The CSF eradication and control program （CSFECP） established from 1992 evolved to 2001, divide the country in CSF Free Zone, without CSF vaccination and CSF infected area. The purpose of this work was to analyze the CSFECP efficacy in all Brazilian territory from 2000 to 2011 including CSF outbreaks in infected area of North and Northeast regions. CSF epidemiological data were obtained mainly from Federal Agriculture Defense Secretary and States Official Inspection Agencies. The CSF outbreaks occurred in CSF infected area were plotted and the tendency line analyzed by quadratic trend model （Minitab）. The results of Brazilian statutory for CSF point to a gradual CSF-eradication in all country since the number of Federal States including at CSF Free Zone, is increasing and reached 15 of 27 Brazilian States, in period from 2001 to 2009. In seven States of infected area, 49 CSF outbreaks occurred. The stamping out was the main sanitary measure but in two Federal States Amap~. and Rio Grande do Norte the emergency vaccination was also applied. In conclusion, the eradication of CSF of all Brazilian territory is in progress but depend on the continuity of official swine diseases surveillance jointed with a rigorous control of animal movement, serological and viral activities investigations until no more outbreaks could be cited in all country.
