Venous Doppler flow patterns,venous congestion,heart disease and renal dysfunction:A complex liaison

The World Journal of Cardiology published an article written by Kuwahara et al that we take the pleasure to comment on.We focused our attention on venous congestion.In intensive care settings,it is now widely accepted that venous congestion is an important clinical feature worthy of investigation.Evaluating venous Doppler profile abnormalities at multiple sites could suggest adequate treatment and monitor its efficacy.Renal dysfunction could trigger or worsen fluid overload in heart disease,and cardio-renal syndrome is a well-characterized spectrum of disorders describing the complex interactions between heart and kidney diseases.Fluid overload and venous congestion,including renal venous hypertension,are major determinants of acute and chronic renal dysfunction arising in heart disease.Organ congestion from venous hypertension could be involved in the development of organ injury in several clinical situations,such as critical diseases,congestive heart failure,and chronic kidney disease.Ultrasonography and abnormal Doppler flow patterns diagnose clinically significant systemic venous congestion.Cardiologists and nephrologists might use this valuable,noninvasive,bedside diagnostic tool to establish fluid status and guide clinical choices.

During the development of the nervous system,there is an overproduction of neurons and synapses.Hebbian competition between neighboring nerve endings and synapses performing different activity levels leads to their elimination or strengthening.We have extensively studied the involvement of the brain-derived neurotrophic factor-Tropomyosin-related kinase B receptor neurotrophic retrograde pathway,at the neuromuscular junction,in the axonal development and synapse elimination process versus the synapse consolidation.The purpose of this review is to describe the neurotrophic influence on developmental synapse elimination,in relation to other molecular pathways that we and others have found to regulate this process.In particular,we summarize our published results based on transmitter release analysis and axonal counts to show the different involvement of the presynaptic acetylcholine muscarinic autoreceptors,coupled to downstream serine-threonine protein kinases A and C(PKA and PKC)and voltage-gated calcium channels,at different nerve endings in developmental competition.The dynamic changes that occur simultaneously in several nerve terminals and synapses converge across a postsynaptic site,influence each other,and require careful studies to individualize the mechanisms of specific endings.We describe an activity-dependent balance(related to the extent of transmitter release)between the presynaptic muscarinic subtypes and the neurotrophin-mediated TrkB/p75NTR pathways that can influence the timing and fate of the competitive interactions between the different axon terminals.The downstream displacement of the PKA/PKC activity ratio to lower values,both in competing nerve terminals and at postsynaptic sites,plays a relevant role in controlling the elimination of supernumerary synapses.Finally,calcium entry through L-and P/Q-subtypes of voltage-gated calcium channels(both channels are present,together with the N-type channel in developing nerve terminals)contributes to reduce transmitter release and promote withdrawal o

The ability of mosquitoes to transmit a pathogen is affected,among other factors,by their survival rate,which is partly modulated by their microbiota.Mosquito microbiota is acquired during the larval phase and modified during their development and adult feeding behavior,being highly dependent on environmental factors.Pharmaceutical residues including antibiotics are widespread pollutants potentially being present in mosquito breeding waters likely affecting their microbiota.Here,we used Culex pipiens mosquitoes to assess the impact of antibiotic exposure during the larval and adult stages on the survival rate of adult mosquitoes.Wild-collected larvae were randomly assigned to two treatments:larvae maintained in water supplemented with antibiotics and control larvae.Emerged adults were subsequently assigned to each of two treatments,fed with sugar solution with antibiotics and fed only with sugar solution(controls).Larval exposure to antibiotics significantly increased the survival rate of adult females that received a control diet.In addition,the effect of adult exposure to antibiotics on the survival rate of both male and female mosquitoes depended on the number of days that larvae fed ad libitum in the laboratory before emergence.In particular,shorter larval ad libitum feeding periods reduced the survival rate of antibiotic-treated adult mosquitoes compared with those that emerged after a longer larval feeding period.These differences were not found in control adult mosquitoes.Our results extend the current understanding of the impact of antibiotic exposure of mosquitoes on a key component of vectorial capacity,that is the vector survival rate.

Symbiotic relationships in marine environments are not fixed and can change throughout the animal’s life.This study investigated the ontogeny of symbiosis of the spider crab Libinia ferreirae with the host medusa Lychnorhiza lucerna.We described the type of relationship,the temporal correlation among species,and food habits.More than 50%of the sampled crabs were symbionts,most in early life stages.The highest number of crabs found in a single medusa was 11.Symbiosis was observed throughout most of the year but was more evident in warm periods.The crab has many benefits in this relationship with a medusa.One is the use of food resources captured by the medusa,primarily copepods.Because the crab steals the medusa’s food,it is a kleptoparasitic relationship.There is a niche partition between symbiont and the free-living crabs as they occupy different habitats and use nonoverlapping food resources.Previous research reported that symbiosis first developed during the crab’s last larval phase(megalopa)when crab and medusa are in the same habitat.Observation of the crab’s behavior shows that symbiosis occurs when the crab can grab to the medusa when the host touches the sea bottom.The crab also took advantage of water currents,releasing itself from the substrate and then drifting toward the medusa.The symbiotic relationship that crabs have with the medusa provides then with a nursery,food resources,shelter,dispersion,and decreased competition with free-living adult crabs,all essential for the crab’s survival.

It has been recognized that physical and chemical properties of biomaterial surfaces mediate the quality of extracellular matrix(ECM)that may affect cell behaviors.In nature,ECM is a heterogeneous three-dimensional superstructure formed by three major components,glycosaminoglycan,glycoconjugate,and protein,that anchors cellular compartments in tissues and regulates the function and the behavior of cells.Changes in the biointerface alter the quality of ECM and morphology through cell surface receptors,which,in turn,enable it to trigger specific cell signaling and different cellular responses.In fact,a number of strategies have been used to improve the functionality of surfaces and direct cell behavior through precisely designed environments.Herein,we aimed to discuss,through a science-based viewpoint,the biomaterial surface features on cell behavior and analyze the impact of cell physical modification on dental implant development.

Surgical complications of oncological treatments: A narrative review

Gastrointestinal complications are common in patients undergoing various forms of cancer treatments,including chemotherapy,radiation therapy,and moleculartargeted therapies.Surgical complications of oncologic therapies can occur in the upper gastrointestinal tract,small bowel,colon,and rectum.The mechanisms of action of these therapies are different.Chemotherapy includes cytotoxic drugs,which block the activity of cancer cells by targeting intracellular DNA,RNA,or proteins.Gastrointestinal symptoms are very common during chemotherapy,due to a direct effect on the intestinal mucosa resulting in edema,inflammation,ulceration,and stricture.Serious adverse events have been described as complications of molecular targeted therapies,including bowel perforation,bleeding,and pneumatosis intestinalis,which may require surgical evaluation.Radiotherapy is a local anti-cancer therapy,which uses ionizing radiation to cause inhibition of cell division and ultimately lead to cell death.Complications related to radiotherapy can be both acute and chronic.Ablative therapies,including radiofrequency,laser,microwave,cryoablation,and chemical ablation with acetic acid or ethanol,can cause thermal or chemical injuries to the nearby structures.Treatment of the different gastrointestinal complications should be tailored to the individual patient and based on the underlying pathophysiology of the complication.Furthermore,it is important to know the stage and prognosis of the disease,and a multidisciplinary approach is necessary to personalize the surgical treatment.The purpose of this narrative review is to describe complications related to different oncologic therapies that may require surgical interventions.

Rabbit breeding has many critical aspects related to reproduction,production,and animal welfare,which reduce its profitability as well as consumer attractiveness.Dietary supplementation with n-3 polyunsaturated fatty acids(PUFA)seems to be a good nutritional strategy to improve several aspects of rabbit breeding,enhance animal welfare and produce a new functional food considered healthy for human consumption.For this reason,the main available scientific research regarding the physiological effects of n-3 PUFA rich products supplemented to the rabbit diet will be reviewed.In particular,consequences on the reproductive performances of both doe and buck,the productive parameters,and the meat quality will be analysed.

Background:Children with congenital heart disease(CHD),even after surgical approaches,and especially those who undergo staged procedures in theﬁrst months of life,remain vulnerable to readmissions and complications,requiring very close monitoring and differentiated intervention strategies.Methods:Descriptive and exploratory study,of the experience report type,which presents the process of building the high-risk outpatient clinic for complex congenital heart diseases(AAR)at the Instituto do Coração(InCor).Results:Report of the path taken to structure the AAR,demonstrating the organization,interface with the multidisciplinary team,admission and discharge criteria,training,and patient proﬁle.In theseﬁve years of care,275 patients were treated,59.65%with biventricular interstage physiology,followed by univentricular interstage physiology(34.55%),residual defects after surgical procedures(3.63%),tumors with risk of mechanical obstruction(1.45%)and patients with an intrauterine approach(0.72%).The signiﬁcant number of critical patients who were successfully discharged from hospital(44.72%),the low mortality rate due to sudden deaths at home and the high adherence to follow-up cor-roborate the impact of this specialized assistance.Conclusions:Despite the limitations of the study,this experi-ence report showed that with few resources,there is the possibility of organizing an AAR with differentiated care,with the objective of early detection and treatment of residual injuries,identiﬁcation of early interventions,edu-cation of parents for follow-up of their children,resulting in individualized treatment,promoting a better quality of life for this population.

Distribution and Prevalence of Serotypes of Group B Streptococcus Isolated from Pregnant Women in 30 Countries:A Systematic Review

Objective:This review aimed to compile scientific data on the distribution and prevalence of group BStreptococcus (GBS) serotypes isolated from pregnant women across 30 countries from 2010 to 2019.Methods:This was a systematic review that addresses the distribution and prevalence of GBS in pregnant women.The search included studies published between January 2010 and December 2019 in PubMed,Virtual Health Library (BVS),ScienceDirect,Scientific Electronic Library Online (SciELO),and LILACS databases.We also surveyed relevant articles published in English,Spanish,and Portuguese between February and April 2020.Original articles,communication,short report,theses,and dissertations were included.The prevalence of GBS colonization,method for capsular serotyping,antimicrobial resistance,distribution and prevalence of serotypes were extracted from each study.Results:In all,795 publications were identified.After applying the eligibility criteria,48 articles were included for the final systematic analysis;most articles were from Asia and were published during the years 2014 to 2017.For the identification of serotypes,most studies used the polymerase chain reaction technique.There were records of all 10 GBS serotypes,namely,Ia,Ib,and II–IX,among the countries analyzed.GBS susceptibility and resistance to antibiotics were addressed in 37.5% of the publications analysed.Conclusion:This review showed that GBS serotypes are distributed differently in the 30 analyzed countries,with serotypes Ia,Ib,and II to V being the most prevalent.Furthermore,our results highlighted the relationship of GBS with maternal colonization,implications for neonates,and antibiotic resistance.
