Background Studies investigating health-related quality of life(HRQoL)in youth with perinatally acquired HIV(PHIV+)are scarce.This study aimed to compare HRQoL of PHIV+to sociodemographic-matched youth not living with HIV(HIV-),Spanish general youth population,and to explore associations between sociodemographic variables,drug consumption,and HRQoL.Methods PHIV+youth were randomly selected from CoRISpe database(Cohort of the Spanish Pediatric HIV Network).HRQoL was evaluated by SF-12v2.Results Thirty-nine PHIV+youth(mean age:23.36 years,SD=3.83)and thirty-nine HIV-youth(mean age:22.97 years,SD=3.80)participated in this study.PHIV+obtained lower scores in SF-12 physical health subscale(PCS)than HIV-(P=0.001)and Spanish general youth population(P=0.006).PHIV+had lower scores on the mental health subscale(MCS)than the Spanish general youth population(P<0.001).PHIV+who were at school obtained better scores than those were not at school.PHIV+youth who had used cocaine and cannabis had lower scores in MCS(P=0.002).Conclusions There is a need for HRQoL management in the associated medical follow-up.

Occult hepatitis B virus(HBV) infection(OBI) is characterized by the persistence of HBV DNA in the liver tissue in individuals negative for the HBV surface antigen.The prevalence of OBI is quite variable depending on the level of endemic disease in different parts of the world,the different assays utilized in the studies,and the different populations studied.Many studies have been carried out on OBI prevalence in different areas of the world and categories of individuals.The studies show that OBI prevalence seems to be higher among subjects at high risk for HBV infection and with liver disease than among individuals at low risk of infection and without liver disease.

In 2008,the European Association for the study of the liver(EASL) defined occult hepatitis B virus infection (OBI) as the"presence of hepatitis B virus(HBV) DNA in the liver(with detectable or undetectable HBV DNA in the serum) of individuals testing hepatitis B surface antigen(HBsAg) negative by currently available assays".Several aspects of occult HBV infection are still poorly understood,including the definition itself and a standardized approach for laboratory-based detection,which is the purpose of this review.The clinical significance of OBI has not yet been established;however,in terms of public health,the clinical importance arises from the risk of HBV transmission.Consequently,it is important to detect high-risk groups for occult HBV infection to prevent transmission.The main issue is,perhaps,to identify the target population for screening OBI.Viremia is very low or undetectable in occult HBV infection,even when the most sensitive methods are used,and the detection of the viral DNA reservoir in hepatocytes would provide the best evaluation of occult HBV prevalence in a defined set of patients.However,this diagnostic approach is obviously unsuitable:blood detection of occult hepatitis B requires assays of the highest sensitivity and specificity with a lower limit of detection<10 IU/mL for HBV DNA and<0.1 ng/mL for HBsAg.

Non-technical approach to the challenges of ecological architecture： Learning from Van der Laan

Up to now, ecology has a strong influence on the development of technical and instrumental aspects of architecture, such as renewable and efficient of resources and energy, COz emissions, air quality, water reuse, some social and economical aspects. These concepts define the physical keys and codes of the current ＇sustainable＇ architecture, normally instrumental but rarely and insufficiently theorised. But is not there another way of bringing us to nature？ We need a theoretical referent. This is where we place the Van der Laan＇s thoughts： he considers that art completes nature and he builds his theoretical discourse on it, trying to better understand many aspects of architecture. From a conceptual point of view, we find in his works sense of timelessness, universality, special attention on the ＇locus＇ and a strict sense of proportions and use of materials according to nature. Could these concepts complement our current sustainable architecture？ How did Laan apply the current codes of ecology in his architecture？ His work may help us to get a theoretical interpretation of nature and not only physical. This paper develops this idea through the comparison of thoughts and works of Laan with the current technical approach to ＇sustainable＇ architecture.

Analysis of doping control test results in individual and team sports from 2003 to 2015

Background:Determining the prevalence of doping in sport might be useful for anti-doping authorities to gauge the effectiveness of anti-doping policies implemented to prevent positive attitudes toward doping.Using questionnaires and personal interviews,previous investigations have found that the prevalence of doping might be different among different sports disciplines;however,there is no sport-specific information about the proportion of adverse and atypical findings(AAF) in samples used for doping control.The aim of the present investigation was to assess the differences in the frequency of adverse analytical and atypical findings among sports using the data made available by the World Anti-Doping Agency.Methods:The data included in this investigation were gathered from the Testing Figures Reports made available annually from 2003 to 2015 by the World Anti-Doping Agency.These Testing Figures Reports include information about the number of samples analyzed,the number of AAFs reported,and the most commonly found drugs in the urine and blood samples analyzed.A total of 1,347,213 samples were analyzed from the individual sports selected for this investigation,and 698,371 samples were analyzed for disciplines catalogued as team sports.Results:In individual sports,the highest proportions of AAF were 3.3%±1.0% in cycling,3.0%±0.6% in weightlifting,and 2.9%±0.6% in boxing.In team sports,the highest proportions of AAF were 2.2%±0.5% in ice hockey,2.0%±0.5% in rugby,and 2.0%±0.5% in basketball.Gymnastics and skating had the lowest proportions at(≤1.0%) for individual sports,and field hockey,volleyball and football had the lowest proportions for team sports(≤1.4%).Conclusion:As suggested by the analysis,the incidence of AAF was not uniform across all sports disciplines,with the different proportions pointing to an uneven use of banned substances depending on the sport.This information might be useful for increasing the strength and efficacy of anti-doping policies in those sports with the highest prevalence in the use of

Concentrating solar power(CSP) has garnered considerable global attention as a reliable means of generating bulk electricity, effectively addressing the intermittent nature of solar resources.The integration of molten salt technology for thermal energy storage(TES) has further contributed to the growth of CSP plants;however, the corrosive nature of molten salts poses challenges to the durability of container materials, necessitating innovative corrosion mitigation strategies.This review summarizes scientific advancements in high-temperature anticorrosion coatings for molten nitrate salts, highlighting the key challenges and future trends.It also explores various coating types, including metallic, ceramic, and carbon-based coatings, and compares different coating deposition methods.This review emphasizes the need for durable coatings that meet long-term performance requirements and regulatory limitations, with an emphasis on carbon-based coatings and emerging nanomaterials.A combination of multiple coatings is required to achieve desirable anticorrosion properties while addressing material compatibility and cost considerations.The overall goal is to advance the manufacturing, assembly, and performance of CSP systems for increased efficiency, reliability, and durability in various applications.

Analysis of the postoperative hemostatic profile of colorectal cancer patients subjected to liver metastasis resection surgery

BACKGROUND Liver resection surgery has advanced greatly in recent years,and the adoption of fasttrack programs has yielded good results.Combination anesthesia (general anesthesia associated to epidural analgesia) is an anesthetic-analgesic strategy commonly used for the perioperative management of patients undergoing surgery of this kind,though there is controversy regarding the coagulation alterations it may cause and which can favor the development of spinal hematomas.AIM To study the postoperative course of liver resection surgery,an analysis was made of the outcomes of liver resection surgery due to colorectal cancer metastases in our centre in terms of morbiditymortality and hospital stay according to the anesthetic technique used (general vs combination anesthesia).METHODS A prospective study was made of 61 colorectal cancer patients undergoing surgery due to liver metastases under general and combination anesthesia between January 2014 and October 2015.The patient characteristics,intraoperative variables,postoperative complications,evolution of hemostatic parameters,and stay in intensive care and in hospital were analyzed.RESULTS A total of 61 patients were included in two homogeneous groups: general anesthesia (n = 30) and combination anesthesia (general anesthesia associated to epidural analgesia)(n = 31).All patients had normal coagulation values before surgery.The international normalized ratio (INR) in both the general and combination anesthesia groups reached maximum values at 2448 h (mean 1.37 and 1.45 vs 1.39 and 1.41,respectively),followed by a gradual decrease.There was less intraoperative bleeding in the combination anesthesia group (769 mL) than in the general anesthesia group (1200 mL)(P < 0.05).Of the 61 patients,38.8% in the general anesthesia group experienced some respiratory complication vs 6.6% in the combination anesthesia group (P < 0.001).The time to gastrointestinal tolerance was significantly correlated to the type of anesthesia,though not so the stay in critical care or the time to

Application of a biochemical and clinical model to predict individual survival in patients with end-stage liver disease

AIM:To investigate the capability of a biochemical and clinical model,BioCliM,in predicting the survival of cirrhotic patients.METHODS:We prospectively evaluated the survival of 172 cirrhotic patients.The model was constructed using clinical(ascites,encephalopathy and variceal bleeding) and biochemical(serum creatinine and serum total bilirubin) variables that were selected from a Cox proportional hazards model.It was applied to estimate 12-,52-and 104-wk survival.The model's calibration using the Hosmer-Lemeshow statistic was computed at 104 wk in a validation dataset.Finally,the model's validity was tested among an independent set of 85 patients who were stratified into 2 risk groups(low risk≤8 and high risk>8).RESULTS:In the validation cohort,all measures of fi t,discrimination and calibration were improved when the biochemical and clinical model was used.The proposed model had better predictive values(c-statistic:0.90,0.91,0.91) than the Model for End-stage Liver Disease(MELD) and Child-Pugh(CP) scores for 12-,52-and 104-wk mortality,respectively.In addition,the Hosmer-Lemeshow(H-L) statistic revealed that the biochemical and clinical model(H-L,4.69) is better calibrated than MELD(H-L,17.06) and CP(H-L,14.23).There were no significant differences between the observed and expected survival curves in the stratified risk groups(low risk,P=0.61;high risk,P=0.77).CONCLUSION:Our data suggest that the proposed model is able to accurately predict survival in cirrhotic patients.

Water,Nitrogen and Plant Density Affect the Response of Leaf Appearance of Direct Seeded Rice to Thermal Time

Field experiments were conducted in the Ebro Delta area （Spain）, from 2007 to 2009 with two rice varieties： Gleva and Tebre. The experimental treatments included a series of seed rates, two different water management systems and two different nitrogen fertilization times. The number of leaves on the main stems and their emergence time were periodically tagged. The results indicated that the final leaf number on the main stems in the two rice varieties was quite stable over a three-year period despite of the differences in their respective growth cycles. Interaction between nitrogen fertilization and water management influenced the final leaf number on the main stems. Plant density also had a significant influence on the rate of leaf appearance by extending the phyllochron and postponing the onset of intraspecific competition after the emergence of the 7th leaf on the main stems. Final leaf number on the main stems was negatively related to plant density. A relationship between leaf appearance and thermal time was established with a strong nonlinear function. In direct-seeded rice, the length of the phyllochron increases exponentially in line with the advance of plant development. A general model, derived from 2-year experimental data, was developed and satisfactorily validated; it had a root mean square error of 0.3 leaf. An exponential model can be used to predict leaf emergence in direct-seeded rice.

DORMANCY-ASSOCIATED MADS-BOX(DAM)genes have recently emerged as key potential regulators of the dormancy cycle and climate adaptation in perennial species.Particularly,PpeDAM6 has been proposed to act as a major repressor of bud dormancy release and bud break in peach(Prunus persica).PpeDAM6 expression is downregulated concomitantly with the perception of a given genotype-dependent accumulation of winter chilling time,and the coincident enrichment in H3K27me3 chromatin modification at a specific genomic region.We have identified three peach BASIC PENTACYSTEINE PROTEINs(PpeBPCs)interacting with two GA-repeat motifs present in this H3K27me3-enriched region.Moreover,PpeBPC1 represses PpeDAM6 promoter activity by transient expression experiments.On the other hand,the heterologous overexpression of PpeDAM6 in European plum(Prunus domestica)alters plant vegetative growth,resulting in dwarf plants tending toward shoot meristem collapse.These alterations in vegetative growth of transgenic lines associate with impaired hormone homeostasis due to the modulation of genes involved in jasmonic acid,cytokinin,abscisic acid,and gibberellin pathways,and the downregulation of shoot meristem factors,specifically in transgenic leaf and apical tissues.The expression of many of these genes is also modified in flower buds of peach concomitantly with PpeDAM6 downregulation,which suggests a role of hormone homeostasis mechanisms in PpeDAM6-dependent maintenance of floral bud dormancy and growth repression.
